Effect of Tourmaline-added Eyeglass Frames on Normalization of Intraocular Pressure (토르말린이 첨가된 안경테가 안압 정상화에 미치는 영향)

  • Purpose : Tourmaline, a natural ore material, was applied to the entire eyeglasses to observe changes in intraocular pressure (IOP), one of the factors related to human ocular metabolism. Methods : After making eyeglass frames by mixing TR-90, which is the main material of eyeglass frames and 7wt% of tourmaline, The changes of intraocular pressure before and after wearing of tourmaline spectacle frames were divided into low, middle and high groups according to the intraocular pressure in 90 normal subjects (46 men and 44 women) in their 20s. Results : Total intraocular pressure was a significantly decreased to -4.14% (p<0.000) in the right eye after wearing tourmaline frames, and significantly decreased to -6.39 % (p<0.000) and -4.64 % (p<0.017) in the High and Middle groups, respectively. Total intraocular pressure was a significantly decreased to -2.74 % (p<0.004) in the left eye, and -4.58 % (p<0.000) in the High group only showed statically significant value. Conclusion : In this study, the spectacle frame containing 7wt% tourmaline was used and it was confirmed that the intraocular pressure was significantly decreased after wearing the spectacle frame, and it became close to the average value of the normal intraocular pressure range. The results of this study showed that tourmaline, which has the effect of promoting the metabolism and blood circulation of the body, has an effect on the normalization of the intraocular pressure by attaching it to the spectacle frame.

A Comparison of Diclofenac versus Dexamethasone for the Treatment of Postcataract Inflammation

  • 백내장 수술 추 염증치료를 위해 일반적으로 부작용이 많은 국소 점안 스테로이드를 사용해왔다. 최근 NSAID계열의 점안액이 항염증 치료약물로 개발되면서 선택적인 약물치료가 가능하게 되었으나 단일치료약물로 사용하는 경우는 드물다. 본 연구는 NSAID계 diclofenac 점안액의 백내장 수술 후 염증치료 효과를 스테로이드체 dexamethasone 점안액과 비교하여 단일 치료약물로서 사용할 수 있는 지 연구하고자하였다. 백내장 수술을 받은 환자로서 안압이 22 mmHg 이상 당뇨환자, 수술대상 눈에 이미 질환 및 수술 경력이 있는 자를 제외하였다. 백내장 수술 후 항염종 약물로서 diclofenac 또는 dexamethasone을 28일 동안 투여하고 항염증효과의 평가를 위하여 세극등 검사로저 전방내 염증세포와 결막, 각막등의 전안부 관찰을 수술 전, 수술 후 1, 3, 7, 14, 28일에 시행하였고, 시력검사는 수술 추 7, 28일에 시행하였다. 안전성의 평가는 안압검사와 세극등 검사상 관찰된 이상소견으로 평가하였다. 총 73명의 연구대상 중 dexameasone군은 41명, diclofenac군은 32명이며 두 군간에 나이, 백내장의 심한 정도, 안구질환 등에 있어 유의한 차이가 없었다 전방내 항염증세포수의 감소에서 두 군간에 유의할 만한 차이가 없었고, 수술 후 최대 교정 시력에서도 동일한 효과를 보였다. 안전성에서 안압의 상승이 두군간에 통계적인 유의한 차이를 보이지 않았으나 dexamethasone군에서 1명의 환자가 45 mmHg 이상 증가하여 약물치료가 필요하였으나 diclofenac군에서는 안압이 상승한 환자가 없었다. 결론적으로 효능 및 안전성에서 두 약물간에 통제적, 임상적으로 유의한 차이가 없었으며 diclofenac 점안액은 백내장 수술 후 항염증치료제로서 충분한 효과가 있으며 안압상승, 감염 등 부작용이 우려되는 dexamethasone점안액의 대체약물로 사용될 수 있을 것으로 평가된다.

The Effect of Extracapsular Cataract Extraction and Posterior Chamber Lens Implantation on Intraocular Pressure (백내장적출술 및 인공수정체삽입술이 안압에 미치는 영향)

  • We studied the change in intraocular pressure (IOP) in 15 consecutive cataract patients who underwent extracapsular cataract extraction and posterior chamber lens implantation between Feb. 1993 and Apr. 1993 to evaluate the effect of this surgery on postoperative IOP. To evaluate the clinical usefulness of non-contact tonometer, the intraocular pressures were measured with Kowa non-contact tonometer (TM-2000, Japan) as well as Goldmann applanation tonometer. There was a decrease in IOP of $3.4{\pm}2.9$mmHg (p<0.00l) 3 months after this surgery and the intraocular pressure differences between pseudophakic eyes and contralateral phakic eyes at 3 months postoperatively were $2.4{\pm}3.8$mmHg (p<0.05). The correlation coefficient between non-contact tonometer and Goldmann tonometer was 0.8876 (p=0.001) in the postoperative 76 eyes. Therefore, our results suggest that extracapsular cataract extracion and posterior chamber lens implantation alone can be a useful surgical method in cataract patient with ocular hypertension, and non-contact tonometer was relatively accurate in measuring the postoperative intraocular pressure.

Predictors of Success of Selective Laser Trabeculoplasty Adjusted for Intraocular Pressure Variations (단안 선택적 레이저섬유주성형술에서 안압 변동을 보정한 성공예측인자의 분석)

  • Purpose: To investigate the efficacy, and identify predictors of success of selective laser trabeculoplasty (SLT) in open-angle glaucoma (OAG) patients after adjusting for intraocular pressure (IOP) changes in the untreated fellow eye. Methods: This retrospective chart review included 52 eyes of 52 OAG patients who underwent SLT in one eye and were followed-up for at least 1 year after the procedure. The IOP was measured before the treatment, at 1, 2, and 3 months posttreatment, and every 3 months thereafter. To account for the possible influence of IOP fluctuations on laser outcomes, post-laser IOP values of the treated eye of each patient were also analyzed, after adjusting for IOP changes in the untreated fellow eye. Success was defined as an IOP decrease ${\geq}20%$ of the pretreatment IOP. The success rate was determined based on Kaplan-Meier survival analysis and factors predictive of success were analyzed using the Cox proportional hazard model. Results: The mean pretreatment IOP was $23.17{\pm}6.96mmHg$. The mean IOP reduction was $5.59{\pm}4.78mmHg$ (29.7%) and the success rate was 65.4% at 1 year. The adjusted mean IOP reduction was $4.70{\pm}4.67mmHg$ (23.9%) and the adjusted success rate was 53.9%. Pretreatment IOP was associated with SLT success; the higher the pretreatment IOP, the greater the post-laser IOP reduction (p = 0.025). Age and mean deviation index did not show a significant association with SLT success (p = 0.066 and p = 0.464, respectively). Conclusions: SLT is a safe and effective alternative method of IOP reduction in OAG patients. Herein, pretreatment IOP was the only factor significantly associated with SLT success. IOP fluctuations of the untreated eye should be considered for a better understanding of the impact of treatment.

Clinical Experiences of Trabeculectomy with Mitomycin C (Mitomycin C를 사용한 섬유주절제술의 임상경험)

  • The use of intraoperative application of Mitomycin C at the filtration site has been known to improve the surgical outcome in glaucomatous eyes with high risk for failure of trabeculectomy. The author performed trabeculectomies with intraoperative Mitomycin C on 25 eyes of 20 patients with poor surgical prognosis to study the efficacy and safety of this technique in glaucomatous patients with high risk for failure of trabeculectomy. After the preparation of a scleral flap, 0.2mg/ml solution of Mitomycin C was applied between Tenon's capsule and the sclera for 2 minutes. The exposed area was then irrigated with 200 ml of balanced salt solution. The follow-up period was from 1 to 7 months (mean 3.8 months). The mean preoperative intraocular pressure (IOP) was $38.6{\pm}6.6mmHg$. The mean final postoperative IOP was $11.7{\pm}3.8mmHg$. Twenty three (88%) of the 25 eyes were successfully controlled with the IOP of less than or equal to 20 mmHg without glaucoma medication. There were early postoperative complications of hyphema in 5 eyes (20%), shallow anterior chamber in 4 eyes(16%), punctate keratopathy in 3 eyes (12%), aqueous leaking from conjunctival wound in 2 eyes (8%), encapsulated bleb in 1 eye (4%) and choroidal detachment in 1 eye (4%), and 4 eyes had long term hypotony lasting more than 2 months. Although Mitomycin C is simple to use and effective adjunct to trabeculectomy, further study will be needed to determine the mechanism of action, indication, dosage and optimal exposure time of Mitomycin C.

Sex Steroid Hormone and Ophthalmic Disease (성호르몬과 안질환)

  • Sex and its tropic hormones influence the lacrimal system, corneal anatomy and disease, aqueous humor dynamics and glaucoma, crystalline lens and cataract, and retinal disease. Dry eye occurs especially frequently during pregnancy, oral contraceptive use, and after menopause, during which androgen levels decrease. Androgen control development, differentiation, and lipid production of sebaceous glands throughout the body, and androgen deficiency also leads to meibomian gland dysfunction and evaporative dry eye. On the other hand, estrogen causes a reduction in size, activity, and lipid production of sebaceous glands. Sex and its tropic hormones also influence the corneal anatomy and disease, and corneal thickening occurred on the second day of the menstrual cycle and around the time of ovulation and appeared to be related to estrogen levels. Fuchs' dystrophy is more commonly seen in postmenopausal women than men and may be linked to hormonal changes that occur with aging. In addition, overexpression of estrogen and progesterone receptors in the conjunctiva of vernal keratoconjunctivitis patients. Serum progesterone levels also may be associated with intraocular pressure especially in pregnant women, and for the women. For women with cataracts, hormone levels were typical of menopause, and there was a significant negative correlation between estradiol and follicular stimulating hormone levels. In addition, serum testosterone levels are associated with the development of diabetic retinopathy. Although the role of sex hormones on the eye is largely unknown, and the results should be interpreted with caution until replicated, the functions of sex hormones in ocular disease remains to be investigated, because they may be involved in structure and function of the ocular components, which are important in the pathogenesis of ocular disease.
