Cephalometric study of the effect of cervical pull headgear based on facial growth patterns (안모의 성장유형에 따른 경부견인 헤드기어의 효과에 대한 두부방사선계측학적 연구)

  • The purpose of this study is to investigate the negative effects of cervical pull headgear and to compare the differences between the two groups of growers-vertical grower and horizontal grower group-which are classified by the posterior-anterior facial height ratio. Initial and final lateral cephalograms were taken for 26 patients including 15 vertical growers and 11 horizontal growers ; also, 3 angular measurements and 4 linear measurements were evaluated. The following results were found. 1. The palatal plane was tipped anteroinferiorly in the vertical grower group. 2. The posterior facial height/anterior facial height ratio was increased in the horizontal grower group. 3. The Mandibular plane angle remained stable on both groups. 4. There was no significant difference between the two groups in the amount of maxillary molar extrusion.

A STUDY ON CORRELATIONSHIP BETWEEN CRANIOFACIAL GROWTH PATTERN AND SYMPHYSIS MORPHOLOGY (악안면 성장양상에 따른 하악이부 헝태에 관한 연구)

  • Craniofacial growth pattern is an important diagnostic data in the course of orthodontic diagnosis and treatment planning ; it also has great influence in the establishment of occlusion as well as shaping and development of face. There have been many studies to classify different craniofacial growth patterns and attempts to predict growth patterns. This study aimed to correlate craniofacial growth pattern and symphysis morphology. 120 adult patients with age from 19 to 39 (mean age : 23.1) were chosen as subjects , using lateral cephalometric films. their anterior to posterior facial height ratios were calculated. They were divided into 3 groups - clockwise growth pattern with $56\%-62\%$(36subjects), counter-clockwise growth pattern group with $65\%$-80\%$(43subjects) and normal growth pattern group with $62\%-65\%$(41subjects). Symphysis morphology and Prominence evaluation in each subject were studied and the following conclusions were drawn : 1. In comparison of symphysis morphology between the sex groups, men showed large symphysis height and prominence. 2. Concerning the symphysis morphology, the clockwise growth pattern group showed larger height, H/D ratio and actual length but smaller depth, angle, effective length and E/A ratio compared to the counter -clockwise growth pattern group. 3. Those with smaller prominance of symphysis showed clockwise growth tendency and those with larger prominance showed counter-clockwise growth tendency.

  • The clinical cases presented here involve skeletal Class III malocclusion cases treated with maxillary protraction in a relatively short period of time with good results. When used on young patients, satisfactory results were obtained in a short period of time, but even for those with less growth potential remaining, skeletal enhancement was still evident. However, data on the criteria of diagnosis or relapse following maxillary protraction is limited despite the number of studies on the subject. The present study could not include the observations on retention and relapse, and further studies in the future nay include such observations.

Evaluation of factors influencing the change of vertical dimension fo face after orthodontic treatment (안모의 수직고경에 영향을 미치는 교정적 요인에 관한 연구)

  • The change of the vertical dimension is of fundamental importance to the orthodontist. However, the choice between the two methods of treatment, extraction versus nonextraction, is not clear. It is not verified that the extraction method decreases vertical dimension, or nonextraction methods result in an increase in vertical dimension. The purpose of this study was to evaluate the changes of vertical dimension of face after the orthodontic treatment with standard edgewise technique, and to compare them in relation to facial types and bicuspid extraction. The subjects consisted of 165 orthodontic patients (77 of adolescents, 88 of adults), and was divided into vertical nonextraction (VN) group, vertical extraction (VE) group, horizontal nonextraction (HN) group, horizontal extraction (HE) group. Pre-and Post-treatment cephalograms were taken with standard method, traced, and digitized for each subject. The comparison of the measurements were statistically executed with Student's t-test. The results were as follows : 1. The facial height and molar height were increased after orthodontic treatment in the all groups. 2. No significant difference was found in the facial height change between the vertical and horizontal groups. 3. No significant difference was found in the facial height change between the extraction and nonextraction groups. 4. As the upper molars were extruded in adolescents group and lower molars were extruded in adults group, lower anterior facial height (LAFH) was increased. 5. None of the pretreatment variables correlates to the change of lower anterior facial height (LAFH).

  • Osteopetrosis is characterized by impaired osteoclast function and increased bone density. Infantile osteopetrosis is a severe form of the disease and has characteristics such as diffusely sclerotic skeleton, pancytopenia, cranial nerve entrapment, infection susceptibility, and abnormal craniofacial appearance. Patients with infantile osteopetrosis often experience developmental delay, and may have a short life span. A 14-month-old girl with osteopetrosis presented to the department of pediatric dentistry. Incipient caries on deciduous incisors were observed. The patient revisited 4 years of age. Besides medical problems, oral complications such as growth retardation, narrow upper arch, crowding, dental caries, and abnormal tooth development were observed. After consultation with her pediatrician, dental treatments were performed on the deciduous molars under sedation after a prophylactic antibiotic injection. At a periodic follow-up, multiple deciduous teeth were treated and extracted, and oralrehabilitation with a removable partial denture was initiated. Patient with osteopetrosis are highly susceptible to infection because of their compromised immune system and problems associated with wound healing that lead to osteomyelitis or sepsis development. Active participation in dental care for sugar intake management and proper oral hygiene are obligatory.
