Objectives Despite considerable advances in technique, experience and skill, the precise place of surgery in the treatment of facial nerve injury remains uncertain. We designed a facial nerve crush injury model in rats and evaluated the recovery of crushed nerve which is the most common injury type of facial nerve using adenovirus vector mediated in vivo gene transfer of Brain derived neurotrophic factor(BDNF). Materials and methods In 48 Sprague Dawley rats, we made a facial nerve crush injury model to main trunk before the furcation, and injected a $10^{11}$pfu adenoviral BDNF in experimental group(BDNF adenoviral injection group; ad-BDNF) and $3{\mu}l$ saline in control group(Saline injection group; saline). After a period of regeneration from 10 to 40 days, nerve regeneration was evaluated with functioinal test (vibrissae and ocular movement), electrophysiologic study(threshold, peak voltage, conduction velocity) and histomorphometric study of axon density. Results Vibrissae and ocular movement, threshold and conduction velocity improved as time elapse in both group, however axon density was increased significantly only in experimental group. Functional test in 10 days and 20 days showed no difference between experimental group and control group. Vibrissae movement, threshold, conduction velocity and axon density in 30 days revealed that the regeneration in quality of experimental group was significantly superior to that of control group. Conclusion In general, there is tendency for nerve regeneration in experimental group (BDNF-adenovirus injection group) during 40 days, functional recovery was detected successfully after facial nerve crush in 30 days postoperatively.
구강악안면 영역의 외상은 치아파절, 탈구, 골절, 연조직 손상을 일으킬 수 있으며, 뇌진탕의 원인이 되기도 한다. 미국과 일본에서는 contact sports 일부에서 마우스가드의 장착이 의무화되어 있으나, 현재 한국에서는 운동 시 마우스가드의 장착을 거의 볼 수 없는 실정이다. 마우스가드는 크게 ready-made type과 custom-made type으로 나눌 수 있다. Ready made type의 마우스가드에는 형태가 일정하고 조정이 곤란한 stock mouthguard와 다소 조정이 가능한 mouth formed mouthguard가 있다. Custommade type의 마우스가드는 적합성이 뛰어나며 정확한 교합, 하악위를 재현할 수 있다는 장점이 있다. 마우스가드는 입술과 구강연조직을 치아로부터 보호하고, 치아에 가해지는 충격을 분산시켜 파절이나 탈구가능성을 줄여준다. 또한, 악골의 골절과 변위를 방지하고 충격을 흡수한다. 성장기 아동의 경우 치아손상의 30% 정도가 스포츠나 레포츠에 의한 것이라는 보고가 있으며, 이 경우 치아나 잇몸은 물론 성장 중인 치조골에 악영향을 주게 된다. 이 때 외상처치는 어려우며, 그 예후 또한 불명확하다. 따라서 성장기 아동의 마우스가드 장착은 외상 예방차원에서 중요하다. 이에 저자는 단국대학교 치과병원에 내원한 환아에게 ERKODENT$^{(R)}$사의 ERKOFORM-RVE를 이용한 흡입성형 마우스가드(vacuum type mouthguard)를 적용한 임상경험을 보고하는 바이다.
헛개나무 추출물은 알코올 분해, 간 손상 회복과 항산화 기능을 가지고 있다. 본 연구에서 헛개나무 추출물이 formalin으로 유도된 염증통증 동물모델에서 염증성 안면통증의 개선하는 능력여부에 대하여 조사하였다. 동물모델은 각각 4개의 그룹으로 나누었다; 그룹1 (대조군), 그룹2 (우측 안면부에 5% fomalin을 피하 주입하여 염증성 안면통증 유발군), 그룹3 (5% fomalin+D.W. 투여군), 그룹4 (5% fomalin+ 4.5 ml/kg 헛개나무 추출물 투여군). 긁거나 문지르는 행위에 대한 계수시험은 3그룹 사이의 다른 점을 평가하기 위해 적용되었다. 뇌와 연수에서의 p38 MAPK, iNOS, Nrf2의 발현은 통증조절, 염증, 항산화와 NO생성과 관련된다. 이것들을 단백정량분석(western blot)으로 분석하였다. 안면통증의 정도는 그룹4가 그룹 1,2,3에 비하여 유의하게 낮았다. 뇌와 연수에서의 p38 MAPK, iNOS, Nrf2의 발현도 또한 그룹4이 다른 그룹에 비하여 낮았다. 이러한 결과는 헛개나무 추출물이 p38 MAPK, iNOS와 Nrf2의 발현을 억제함으로서 염증성 안면통증을 감소시킬 수 있다는 것을 시사한다.
With the exception of gun shot wound, the incidence of penetrating injury of face and neck areas nonorganic foreign bodies is relative low. But the diagnostic evaluation and therapeutic management of penetrating facial wounds need careful decision, when the anatomic proximity of the major vessels and nerve is considered. Penetrating facial trauma with concomitant vascular injury present challenging problems, the immediate complication of this vascular injury are severe bleeding, hematoma formation, shock, obstruction of airway. The vascular injury is conformed by angiography. In this report, a industrial tool(long tack) fired by explosive air is penetrated into face and to neck. In angiograms penetrating injury of the vertebral artery is detected. We performed the embolization of the vertebral artery with coils and manual removal of the foreign body without any complication was followed.
As carotid-cavernous fistula is the major complication that can be occurred uncommonly after maxillofacial injuries, it is abnormal arteriovenous communication between cavernous sinus and internal carotid artery. Such an arteriovenous communication is most often the result of injuries, but need not be associated with bony fracture. It usually begins soon after an injury, but it may be delayed for as long as several months. It begins undramatically with eye pain, headache and slow protrusion of eye ball. A bruit may be heard above the eye with stethoscope. Close examination will reveal dilatation of superficial veins of the eyelid and forehead and periorbital edema. There will be complete or partial ophthalmoplesia of the affected eye. Compression of the common carotid artery on the ipsilateral side will reduce or obliterate the bruit. The lesion in the cavernous sinus is them demonstrable by angiography.
Optic nerve injury serious enough to result in blindness had been reported to occur in 3% of facial fractures. When blindness is immediate and complete, the prognosis for even partial recovery is poor. Progressive or incomplete visual loss may be ameliorated either by large dosage of steroid or by emergency optic nerve decompression, depending on the mechanism of injury, the degree of trauma to the optic canal, and the period of time that elapses between injury and medical intervention. We often miss initial assessment of visual function in management of facial fracture patients due to loss of consciousness, periorbital swelling and emergency situations. Delayed treatment of injuried optic nerve cause permanent blindness due to irreversible change of optic nerve. But by treating posttraumatic optic nerve injuries aggressively, usable vision can preserved in a number of patients. The following report concerns three who suffered visual loss due to optic nerve injury with no improvement after steroid therapy and/or optic nerve decompression surgery.

Gunshot wounds to the face are often dramatic at the time of presentation. Although care must be individualized, protocol approach helps the clinician rapidly evaluate patient and plan treatment. Early X-ray films help to determine the precise trajectory. Rapid neurologic assessment and early CT scanning based on physical examination ot trajectory will allow for identification of even occult injuries early. Lead poisoning arising from bullet lead in the synovial cavity of the hip, synovial cavity of the chest and pleural space have been reported. A combination of surgical debridement and chelation therapy with oral succimer produce a satisfactory outcome. We have managed a patient with Lt facial gunshot wound and Lt mandible angle fracture by open reduction of angle fracture and surgical removal of remnant bullet and medially advanced cheek skin flap. We have obtained good result and report this case with review of literatures.

구강악안면 영역에 발생한 foreign body impaction은 흔하지 않지만, 일단 발생하면 즉시 제거되어야 하며 술후 후유증을 충분히 방지하기위해 주의깊은 병력청취, 임상 검사, 방사선 사진을 이용한 localization이 꼭 필요하다. 그리고 수술시에는 조심스럽게 조직을 박리하고 전체 손상부위를 철저히 탐진해서 foreign body를 제거해야 한다. 저자등은 침구용 바늘, nasolacrimal ductoplasty용 silicone tube, 산탄 총알등 다양한 구강 악안면영역에서 발견된 이물질을 제거한 경험후 문헌고찰과 함께 보고하는 바이다.
Our faces can express a remarkable range of subtle emotions and silent messages. Because the face is so essential for complex social interactions that are part of our everyday lives, aesthetic repair and restoration of function are an important tasks that we must not take lightly. Soft-tissue defects occur in trauma patients and require thorough evaluation, planning, and surgical treatment to achieve optimal functional and aesthetic outcomes, while minimizing the risk of complications. Recognizing the full nature of the injury and developing a logical treatment plan help determine whether there will be future aesthetic or functional deformities. Proper classification of the wound enables appropriate treatment, and helps predict the postoperative appearance and function. Comprehensive care of trauma patients requires a diverse breadth of skills, beginning with an initial evaluation, followed by resuscitation. Traditionally, facial defects have been managed with closure or grafting, and prosthetic obturators. Sometimes, however, large defects cannot be closed using simple methods. Such cases, which involve exposure of critical structures, bone, joint spaces, and neurovascular structures, requires more complex treatment. We reviewed and classified causes of significant trauma resulting in facial injuries that were reconstructed by microsurgical techniques without simple sutures or coverage with partial flaps. A local flap is a good choice for reconstruction, but large defects are hard to cover with a local flap alone. Early microsurgical reconstruction of a large facial defect is an excellent choice for aesthetic and functional outcomes.
목적: 본교에 재학중인 태권도 선수들이 태권도와 관련하여 과거의 경험하였던 운동 손상의 유형, 치료 경과 및 운동 복귀 등에 대해서 분석 하고자 하였다. 대상 및 방법: 태권도를 전공으로 하는 본교 대학생 선 수 49명 중 과거 태권도와 관련하여 사지, 두부 및 척추 부위 의 손상으로 치료를 위해 병원을 방문한 경험이 있는 47명을 대상으로 하였다. 남자가 39명, 여자가 8명이었으며, 평균 연령은 18.8세이었다 태권도 운동 경력은 평균 9.6(4-13)년 이었으며, 수상 시 태권도 경력은 평균 6.7(5-9)년 이었다. 운동 손상의 양상과 치료경과 등에 대해서 후향적으로 분석하였다. 결과: 47명이 과거에 태권도와 관련하여 손상 빈도는 1회 손상이 26명 (55.3%), 2회가 11명(23.4%), 3회가 4명(0.5%), 4회가 5명(10.6%), 5회가 1명(2.2%)으로 총 85예 이었다. 연습 중 수상한 빈도는 50예(58.8%), 시합 중 수상한 빈도는 35예(41.2%)로 연습 중 손상이 더 빈번하였다. 공격 시 수상 빈도는 26예 (31.7%), 방어 시 수상 빈도는 49예 (57.6%)이었다. 손상의 유형별 분류상 골절이 49예(57.5%) 및 인대손상 21예(24.7%)로 가장 큰 비중을 차지하였다. 부위별 손상으로는 상지 손상이 32예 (37.7%), 하지 손상이 44예 (51.8%), 몸통 및 안면부 손상이 9예 (10.5%)였다. 치료는 수술이 15예 (17.7%), 석고 고정이 21예 (24.7%), 부목 고정이 33예 (38.8%), 물리치료가 15예 (17.7%), 침술이 1예 (1.1%)였다. 결론: 대학 태권도 선 수들의 대부분(47/49, 96%)에서 병원을 방문할 정도의 심한 운동 손상을 과거에 경험하였고, 유형별로는 골절이 가장 많았으며, 손상이 시합 때보다는 연습 중에 더 빈번하였다는 것을 확인 할 수 있었다.
