  • Hemifacial spasm commonly occurs in muscles about the eye, but may also involve or spread to the entire side of the face. One hundred and seventy eight patients with hemifacial spasm visited our Neuro-Pain clinic from January 1992 to April 1996. There were 121 female and 57 male patients, a 2.1:1 ratio respectively. Largest percentages of patients were in the 50 year old range. Among them, 96 patients were treated by facial nerve block or O'Brien block. In most cases, induced facial palsy disappeared within one or two months. Among the 96 patients who received nerve block, 46 patients received a second block within 5 to 24 months. The average interval from first and second nerve block was 11.5 months. After nerve block, all patients were free from spasm for 1 to 21 months. We conclude that facial nerve block is a satisfactory and reliable method for the treatment of facial spasm.

  • Twenty seven patients with hemifacial spasm were treated by facial nerve block at the foramen stylomastoideum and O'Brien block from January 1992 to March 1994. There were 16 female and ll male patients, a 1.5:1 ratio respectively. Most patients were 40~60 years old. Among the 27 patients treated by nerve blocks, 26 responded well to the facial nerve block, including two cases of O'Brien block. One patient failed to respond to the facial nerve block. Induced facial palsy disappeared within one or two months within the majority of patients. Among the 26 patients who received nerve block, 6 patients required a second block within 3 to 9 months. After successful nerve block, all patients were free of spasm for 1 to 16 months. Although the follow-up period was short in duration, these results suggest that facial nerve block is a satisfactory and reliable method in the treatment of facial spasm.

  • Objectives : The aim of this study was to report that effect of Korean medicine treatment for patient with peripheral facial nerve palsy accompanied by hemifacial spasm through hospitalization. Methods : We treated a patient with facial nerve palsy by acupuncture, electroacupuncture, herbal medicine, pharmacopuncture and cupping. The effect of treatment was evaluated using the House-Brackmann scale, the Yanagihara grading system, and the visual analogue scale(VAS). Results : During the 18 days hospitalization period, the symptoms of hemifacial spasm disappeared and the facial palsy improved. The House-Brackmann scale changed from 4 grade to 2 grade, the Yanagihara grading system changed from 19 to 47, and the VAS changed from 3 to 0. Conclusions : This case report suggests that Korean medicine could be effective for facial palsy accompanied by hemifacial spasms.

  • Tic convulsif is a syndrome restricted to paroxysmal dysfunction of the fifth and seventh cranial nerves. It occurs predominantly in women over the age of 50 years and is usually associated with an ectatic vertebrobasilar artery - less frequently an arteriovenous malformation or cholesteatoma - which compresses the trigeminal and facial nerve roots in the postetior fossa. In rare instances this syndrome may be caused by brain tumor. Because of the high incidence of posterior fossa lesions in painful tic convulsif, a complete neurological evaluation including computerised transaxial tomography should be performed in every case. We experienced a case of trigeminal neuralgia(mandibular division)and contralateral hemifacial spasm.

  • Objectives : This study investigated the relationship between presymtoms and stroke subtype in early state of stroke patients. Methods : From Oct. 2005 to May 2007, 623 early state stroke patients were included. Patients were hospitalized within 30 days after the onset of stroke at DongGuk University International Hospital, Kyungwon University In-cheon Oriental Medical Hospital, or Kyung Hee University Oriental Hospital. We investigated general characteristics, stroke subtype, and presymtoms (facial spasm, neck stiffness). Results : Hemorrhagic stroke had more facial spasm than ischemic stroke(odds ratio 2.80). Small vessel occlusion had more neck stiffness than others(odds ratio 1.59). Conclusions : Facial spasm and neck stiffness would be valuable as stroke presymtoms.

Results : Before therapy, the grades of spasm intensity classified by Scott's description in three cases were 3. And after Dong-Si acupuncture therapy, the grade of one case was 0, and two cases were 1. • In the treatment of hemifacial spasm, the operative methods, such as neurovascular decompression, rhizotomy etc. are the most efficacious therapy on its control. But, due to some side effects and complication, pharmacotherapy has been the first attempted by first intention of hemifacial spasm. The carbamazepine is the most commonly used agent, however it is often inefficacious in some cases, and attenuates as the time passes, engages in some symptoms, such as dizziness, nausea, vomiting, and causes an aplastic anemia, it has many deficiencies in becoming the first intention. Acupuncture is a simple, effective and conservative spasm control modality. It is so simple to use in hemifacial spasm. We had observed 3 cases of hemifacial spasm which were treated by acupuncture in Oriental clinic recently. We reported these cases and reviewed related literatures briefly. The results of treatment are as follows; The clinical conditions of spasm intensity was improved after the therapy. These results suggest that acupuncture and negative therapy be effective as the first intention in healing hemifacial spasm.

  • Objectives: This study is designed to investigate the difference of gender, triggering factors and Heart rate variability(HRV) between left and right hemifacial spasm patients. Methods: Among hemifacial spasm patients who visited the Acupuncture and Moxibustion medicine clinic of Kyung Hee University Korean Medicine Hospital from January 1, 2017, to April 18, 2022, we reviewed the clinical records of subjects with right hemifacial spasm(n=55) and left hemifacial spasm(n=76). We investigated the difference of gender, triggering factors and HRV between left and right hemifacial spasm patients groups by student's t-test, chi-square test and fisher's exact test using SPSS for windows(version 28.0.0.0.). Results: There was no difference in gender, triggering factors and HRV between left and right hemifacial spasm groups. Conclusion: According to this study, there were few specificity of lesions of left and right in hemifacial spasms.
