The prostate cancer is the most common malignant tumor in males. Prostate cancer is the most common malignant tumor that occurs in the male in Korea in 2007 to an annual average of 5,292 cases and 3.3% of the total cancer incidence seventh occurred. Our study compared property for tomotherapy and proton therapy in radiotherapy of prostate cancer patients. We analyzed DVH(Dose Volume Histogram) and dose distribution for prostate, bladder and rectum for radiation treatment planning of prostate cancer with 11 patients in Ilsan K cancer hospital from June to November 2011. There was no differences between tomotherapy and proton therapy in the purpose of prostate cancer therapy for PTV. The adjacent organs of bladder and rectum of average dose-volume were 2port proton therapy that it was low dose treatment comparing with tomotherapy and 5port proton therapy. $H{\cdot}I$ of proton therapy was less than $H{\cdot}I$ of tomotherapy. Also, 5port was less than 2port in $H{\cdot}I$ of proton therapy. However, 2port proton therapy has more advantage over 5port proton therapy that the bladder and rectum of average dose-volume and control time of equipment in radiotherapy of prostate cancer.

Objectives : In order to find out the whitening effects of Leonuri Herba, this study was designed to identify the effects and the action mechanism of LHE(Leonuri Herba extract) on Malignant melanoma cell lines. Methods : After treating LHE on the B16F10 cell-Malignant melanoma cell line-, the cell survival rate, melanin biosynthesis rate, intra&extracellular tyrosinase activity rate, SOD-like activity, tyrosinase mRNA gene expression were investigated. The results were compared with control group without LHE treatment or with positive control group treated with whitening efficacy substance. Results : B16F10 cell survival rate, melanin biosynthesis rate, and intra&extracellular tyrosinase activity were significantly inhibited depending on the concentration of treated LHE. Melanin biosynthesis rate and tyrosinase activity rate were also decreased when ${\alpha}-MSH$ was combined with LHE. In addition, the SOD-like activity was increased in a concentration-dependent manner in the treatment with the LHE, indicating signigicant activity at high concentrations, and the tyrosinase mRNA gene expression was decreased in both the LHE-treated group, the LHE and ${\alpha}-MSH-treated$ group. Conclusions : LHE seems to inhibit melanin synthesis through inhibition of tyrosinase activity and inhibition of tyrosinase mRNA gene expression. It also has the effect of promoting SOD-like activity and may be used clinically as a skin whitening agent in the future.

Purpose: The goal of cancer surgery is complete removal of cancer tissue and prevention of recurrence. Surgeons can change the surgical instruments after total resection of the cancer mass. The purpose of this procedure is to prevent dissemination of the cancer cells attached to the surgical instruments. Authors hypothesize the possibility of local recurrence caused by the cancer cells attached to the surgical instruments in the skin cancer cases. Methods: Skin cancers were induced by using DMBA-TPA two-stage carcinogenesis model in 10 of Balb/c mice. In 2-weeks, skin cancer was developed in all 10 mice. cancer cell attached surgical instruments were made by pinching the removed cancer tissue using Adson tissue forcep 10, 20, 30 times each. To count number of cancer cells in each forcep with different number of pinching was done, the forceps were washed in 30 mL of the normal saline and Cytospin preparation was done. To make recurrence models from cancer cell attached surgical instrument, three incisions were made in normal skin of each mouse, and local seeding was done by pinching subcutaneous tissue in 10, 20, 30 times each by using Adson teeth forceps mentioned above as cancer cell attached surgical instrument. Results: All skin cancers were squamous cell carcinoma. Local recurrences were developed in 7 mice (3 in 10 times forceping site, 2 in 20 times forceping and 3 in 30 times forceping). In the cytospin test, the mean number of squamous cells in 100 microscope was 28.6 in 10 times, 47.2 in 20 times, 93.6 in 30 times, respectively. P value was 0.002 in Wilcoxon-Sign test. Conclusion: The number of cell count was significantly increased as number of pinching was increased. And these cells are able to induce local recurrence by local seeding. Considering this result, authors are able to confirm that the minimal handling in cancer surgery is important factor to prevent local recurrence.
Purpose: The rate of fistulas occuring followed by resection of oral cavity, oropharyngeal, hypopharyngeal, and laryngeal cancer are reported to be 9 ~ 23% according to various documents. Neglected treatment of the fistula can result in a setback in proper treatment with restrictions in oral intake leading to delayed return to daily life. Furthurmore, in severe cases, it may injure important vessels and adjacent structures of the neck area. The author reviewed previously reported cases of treatment methods for fistulas recurring after diverse head and neck operations and with sharing the treatment experiments of our patients, we tried to present a treatment algorism for different fistula types. Methods: Our study was based on retrograde analysis of 64 patients who were clinically diagnosed with fistula after operation for cancer of the head and neck from 1997 to 2008 at Severance Hospital. Their primary sites of cancer were 8 oral cavity, 22 oropharynx, 25 hypopharynx, and 9 larynx. The patients were aged 45 to 75 years and the male to female ratio was 11 to 1. The patient's operation records and progress notes were evaluated for determination of degree of fistula and treatment methods. Results: Most fistulas were clinically suspected after postoperative 5 days and symptoms noted for detection of the fistula were erythema, purulent discharge, edema, tenderness, and fluctuation. The fistula was definitely diagnosed at postoperative 2 weeks with barium test and treatment method ranging from conservative management to operative procedure were applied to each patients. Total 21 patients were managed with conservative protocol. In 15 cases, direct repair of the fistula was done and more stable repair of the fistula was possible with using of TachoComb$^{(R)}$. Pharyngostoma was performed in 14 patients. Among them, 4 patients healed spontaneously, 5 patients were taken direct closure, 4 patients were taken pectoralis major musculocutaneous flap, and one patient was taken esophageal transfer. The other 14 patients were taken 11 pectoralis major musculocutaneous flaps and 3 free flaps without pharyngostoma formation. Conclusion: Fistula is a troublesome complication resulting after resection of head and neck cancer. Early detection and adequate treatment according to the period and condition of the fistula may prevent further complications and reduce the pain of the patient.
63 patients who were irradiated with a goal of long term control among 101 patients with esophageal cancer seen during an 11 fears period between Jan, 1970 and Dec, 1980 at Yonsei Cancer Center in Seoul, Korea have retrospectively analysed. 52(82.5%) among the 63 Patients were confirmed to have epidermoid carcinoma in the histology. 46 cases(73.0%) except 17 cases of $T_1$, were locally or far advanced extension. Tumor dose of radical radiation in the management of esophageal cancer had delivered from 50 Gy to 74 Gys. 2 Gys daily fractions, between 5weeks and 8 weeks. After 1 month from the completion of radiation, 23 of the 63 patients had a repeat barium esophagogram for the assessment of tumor response, there had showed 7 cases of complete response and 15 cases of partial response. 45(71.4%) patients were followed up and the remaining 18 patients were lost to follow-up within 1 year after the completion of irradiation. Actuarial overall 3 and 5 years survival rate of all 63 Patients were 11.8% and 8.8%, respectively. The actuarial 3 and 5 years survival rates of 17 cases of $T_1$, esophageal cancer were 24.7% and 20.8%. To know the three questions about multiple primary cancers: 1) what are the characteristics of persons having multiple parimary cancer? 2) Does the presence of a single primary concer after the susceptability to multiple primary cancers? 3) Does the location of one multiple primary cancer influence the site of others?, we analysed 121 cases of multiple primary malignant neoplasms registered in Seoul National University Hospital during 8 years from July 1978 to August 1986. Of 121 cases, double primary malignant neoplasms were 119 cases and triples were 2 cases. The incidence of multiple primary malignant neoplasms was $0.7\%$. The metachronous tumor (>6 months) was found in 70 cases and the median time between the first and the second was 32 months. The most commonly associated tumors were stomach and primary liver carcinoma. Cervix and Lung cancer, Stomach and Rectal cancer, Stomach and Esophagus cancer were also commonly associated.
To assess the effect of postoperative radiotherapy on tumor recurrence and patient survival, 133 patients who received adjuvant postoperative radiotherapy for adenocarcinoma of the rectum were retrospectively analyzed. Sixty-one percent of the patients were in stage $C_2$ by Astler-Coller staging system. A significant statistical difference was noticed in failure rates for lymph node negative vs lymph node positive patients; $26\%(9/35)\;vs\;50\%(49/98)$. The incidence of local failure was found to be strongly dependent on the pathologic stages; with $9\%(3/35)$ of recurrence in stage B and $21\%(21/98)$ in stage C. Distant metastasis has occurred in $29\%(38/133)$ of the patients; $2\%(7/35)$ in stage B and $32\%(31/98)$ in stage C. The actuarial survival at 3 years for patients in stage $B_2$, stage $C_1$, and stage $C_2$were $78\%,\;47\%,\;and\;38\%$, respectively. In conclusion, the postoperative adjuvant radiotherapy for rectal carcinoma appears to reduce local recurrence significantly.
Carcinoembryonic antigen (CEA) has been studied in the field of gynecologic malignancy to determine whether it can be used as a tumor marker for early detection of recurrence or evaluation of therapeutic results. From January 1985 through December 1989, a total of 239 cervical cancer patients were entered for an analysis of plasma CEA level in the group with conical cancer compared to the control group consisting of 65 normal healthy women and 18 women with benign gynecologic disease. Plasma CEA levels appear to be directly related with the tumor extension and as stages advance, the incidence of patients with abnormal plasma CEA levels is increased. Also, there seems to be a little higher incidence of abnormal CEA levels in patients with adenocarcinomas or adenosquamous carcinoma but not statistically significant because of small number of patients. When the patients developed recurrence, plasma CEA levels are markedly elevated in the majority, particularly in patients with hepatic metastases, In conclusion, serial plasma CEA checks could be used to detect recurrence during follow-up after treatment of cervical cancer.

An 11-year-old intact female miniature poodle presented with a four-month history of hemorrhagic effusion. The patient was alert on physical examination, although muffled heart sounds were noted upon auscultation of the right hemithorax. The radiographic finding was pleural effusion. Ultrasonography revealed cystic changes in both ovaries and several nodules in the liver. A refractory opacity in the right lung field, as visualized with computer tomography (CT), was diagnosed as right middle lung lobe torsion with a collapsed bronchus. Five days after diagnosis, a right fifth intercostal thoracotomy was performed to remove the right middle lung lobe; the right middle lung lobe was grossly shrunken as a result of chronic lung lobe torsion. Ovariohysterectomy was also performed. Histopathologic examination revealed papillary adenocarcinoma in both ovaries and suspected metastasized ovarian adenocarcinoma cells in the lung lobe. The patient recovered favorably and had been doing well up to two months post-surgery. However, after four months, the dog presented with respiratory difficulty. The radiographic findings were pleural effusion and collapse of the right cranial and left caudal lung lobes. Malignant cells of epithelial origin were observed in the pleural effusion. The tumor cells were suspected to be metastasized cells from the previously resected lung lobe. Although cancer treatment was recommended, the suggestion was suspended and the dog was discharged from hospital. This was a case of lung lobe torsion that had occurred because of hemorrhagic effusion due to tumor. Although ovariohysterectomy and lobectomy were performed, there was a relapse of hemorrhagic effusion because of metastasized tumor from the previously resected lung lobe.
Social interest in breast cancer has increased. The most basic exams for diagnosis include breast X-ray and breast ultrasound. In particular, breast microcalcification requires histological diagnosis, and breast microcalcification biopsy is commonly performed. Therefore, this study aimed to analyze and assess X-ray guided biopsy (needle localized open biopsy) and ultrasound guided biopsy (sono guided core needle biopsy), which are basics in diagnosis of microcalcification. Targeting 241 cases in which magnification mammography was performed for patients who visited the hospital due to breast microcalcification, age distribution and the location of lesions were analyzed in X-ray guided biopsy and ultrasound guided biopsy. By classifying exams performed after magnification mammography, the frequencies of X-ray guided biopsy and ultrasound guided biopsy were analyzed, and malignant and benign results were confirmed. The results showed that 64 cases(26.6%) were X-ray guided biopsy, which was 5.4 times higher than 12 cases(4.9%) of ultrasound guided biopsy. Due to development of ultrasound equipments, stereotactic vacuum-assisted biopsy, etc. the methods of histological diagnosis of microcalcification have become diverse, but when considering characteristics and limitations of each exam, X-ray guided biopsy is thought to be most accurate and useful.
