Malignant Pericardial Mesothelioma Misdiagnosed as Constrictive Pericarditis (결핵성 심낭염으로 오인되어 치료한 악성 심낭 중피종)

  • We report aprimary malignant pericardial mesothelioma. Thirty-eight-year-old male patient complained of dyspnea and chest pain with left shoulder pain. At first, we thought it was because of tuberculous constrictive pericarditis and performed medical management for one and a half years. But, the above symptom recurred repeatedly; therefore we did pericardiectomy and diagnosed his case as malignant pericardial mesothelioma. Tumor was sticked to the myocardium and complete resection was impossible. He received postoperative chemoradiotherapy.

  • 석면은 여러 가지 장점에도 불구하고 석면을 흡입할 경우 폐암, 악성중피종 등과 같은 치명적인 폐질환을 유발한다. 몇 년 전까지 우리의 일상생활 속 건축물의 건축자재, 자동차 부품 등에 특별한 제약 없이 석면이 여러 가지 자재로 사용되어 우리 일상생활에 분포되어 있으며 석면을 안전하게 관리하기 위해 분야별 석면관리제도가 추진중에 있다. 석면해체 제거작업 및 건축물 석면조사의 법 사각지대가 발생하고 있어 제도적 보완이 필요한 시점이라 하겠다.

  • 중피종은 늑막의 중피세포에서 발생되는 매우 드문 종양이며, 저혈당은 비섬세포(Non-islet cell) 종양에서 발생되는 매우 드문 증상으로 알려져 있다. 본 저자들은 좌측 흉강을 차지하며 저혈당을 동반한 환자에서 종양의 수술적 제거직후 증상이 사라졌으며, 조직학적으로는 국소섬유종의 소견을 보일 경우를 치험하여 보고하는 바이다.

  • 석면은 자연적으로 발생되는 광물로 1887년 캐나다의 케베크 지방에서 채굴이 시작되어 흡음, 단열, 내부식성, 내약품성이 뛰어나고 값이 싸자는 이점 때문에 90%이상이 건축재료로 사용되었다. 석면은 장기간 노출될 경우 대략 15년에서 30년의 잠복기를 거쳐 석면폐증, 폐암 악성 중피종을 등을 유발하며 단 한번 진단되면 아직 이렇다할 치료방법이 없다. 대체적으로 석면폐의 경우 질병의 발생과 석면 섬유 사이에는 양-반응관계를 보이며(Beckla ke et al., 1980) 악성중피종과 폐암의 경우는 화학적 성질과 함께 섬유의 굵기, 길이, 모양 등의 물리적 성질이 질병의 발생과 밀접한 관계가 있는 것으로 보고되고 있다(Lippmann, 1988).   • Primary malignant pericardial mesothelioma(PMPM) is more rare than heart tumor, and the term of mcsothelioma was first used by Adami in 1910, although the lesion was Hrst descripted by Wagner in 1870. Most of 1:le reported 40 cascs have becn diagnosed on autopsy. Antemortem diagnosis are rarely reported with only 40 cases in the world. According to Cohen, its incidence in 500,000 autopsies were 2.2. An analysis of the recent review shows that an antemortem diagnosis was made in only 19∼25% of total cases. This report co sist of a case of our experience of PMPM.

Intracavitary Carboplatin Chemotherapy in a Malignant Mesothelioma Dog (개의 악성중피종의 흉강내 Carboplatin 투여 증례)

  • A 10-year-old castrated male Siba dog was presented for signs referable to pleural effusion associated with neoplasm of the thoracic cavity. The pleural effusion fluid consisted of blood and tumor cells by thoracocentesis. Histopathological examination of the sedimentary tumor cells revealed malignant mesothelioma. Intracavitary carboplatin was administered at 300 mg/$m^2$ every 5 weeks for 3 treatment and pleural effusion was disappeared after 3 treatments. The dog had recurrence of pleural effusion at 515 days but intracavitary carboplatin chemotherapy had no effect. It would be thought that the intracavitary carboplatin treatment was quite a useful method to control a canine malignant mesothelioma with minimal toxicity.

  • Malignant pleural mesothelioma (MPM) has a poor prognosis and a strong association with exposure to asbestos. Although there are not generally accepted guidelines for treatment of MPM, recent reports suggest that multi modality therapy combining chemotherapy, radiotherapy, and surgery can improve the survival of patients with MPM. Therefore exact staging is required to decide the best treatment option. However, it is well known that there are many difficulties in determining precise preoperative stage, predicting prognosis, and monitoring response to therapy with conventional imaging modalities such as CT and MRI in MPM. Recently PET with $^{18}F-FDG$ comes into the spotlight as an important staging method. There is increasing evidence that PET is superior to other conventional imaging modalities in diagnosis and staging of MPM. Particularly PET/CT improves the diagnostic and staging accuracy over PET or CT alone in MPM because it provides anatomic imaging data as well as functional information. PET and PET/CT are also useful for monitoring response to therapy and SUV is reported as a prognostic factor in MPM.

  • Primary pericardial malignant mesothelioma is a lethal and rare cardiac neoplasm of mesodermal origin. Most cases are associated with history of pericarditis with constriction and/or tamponade. Authors experienced a case of primary pericardial malignant mesothelioma in a 55-year old female who had suffered from dyspnea and chest pain. Pericardial nodules revealed intense uptake by FDG-PET scan and confirmed as primary pericardial malignant mesothelioma by thoracoscopic biopsy. Here we report this case with a brief review of the relevant literatures.

  • Pleural masses may be caused by various conditions, including benign and malignant neoplasms and non-neoplastic tumorlike conditions. Primary pleural neoplasms include solitary fibrous tumor, malignant mesothelioma, and primary pleural non-Hodgkin's lymphoma. Metastatic disease is the most common neoplasm of the pleura and may uncommonly occur in patients with hematologic malignancy, including lymphoma, leukemia, and multiple myeloma. Pleural effusion is usually associated with pleural malignancy. Rarely, pleural malignancy may arise from chronic empyema, and the most common cell type is non-Hodgkin's lymphoma (pyothorax-associated lymphoma). Non-neoplastic pleural masses may be observed in several benign conditions, including tuberculosis, pleural plaques caused by asbestos exposure, and pleural loose body. Herein, we present a review of benign and malignant pleural neoplasms and tumorlike conditions with illustrations of their computed tomographic images.
