Applying Method of Arch Bar in Maxillofacial Surgery without Orthodontic Treatment (교정치료 비적용 악안면수술에서 치열활봉 거치법)

  • Temporomandibular joint(TMJ) and dental occlusion are important in functional aspect as well as in cosmetic aspect for craniofacial surgery such as orthognathic and traumatic surgery. During these operations, appropriate maxillomandibular fixation(MMF) is especially necessary to conserve or reconstruct, dynamic TMJ and functional occlusion. Arch bar is one of the most popular method to gain proper MMF. Seventeen patients including 5 patients with mandibular fracture, 12 patients with orthognathic surgery(6 patients had relatively normal occlusion, however 6 patients had facial anomalies such as hemifacial microsomia with irregular occlusal plane.) were joined in this study. Arch bar was contoured on the dental cast, which was prepared for model surgery, prior to apply it on the facial anomaly patient. On using pre-contoured arch bar, patients felt better during the procedure and surgeons saved time. Moreover, well-contoured arch bar promises precise transmission of force on the bone, which implies surgeons do not need to worry about the occlusion while fixing bony structures. Authors suggest how to apply arch bar to satisfy basic theories about dental row and occlusion based on the experience. In addition, it is proposed to contour arch bar on the dental impression prior to apply it on the patient who has irregular and complex occlusal plane.

CLINICAL STUDY ON SURGICAL MANAGEMENT OF MANDIBULAR CONDYLAR FRACTURES (하악 과두 골절의 외과적 처치에 관한 임상적 연구)

  • 79 surgically managed mandibular condylar fracture patients included the 25 patients treated with Dr, Nam's method were analysed the postoperative resluts in Oral and Maxillofcial Surgery, School of Dentistry, Wonkwang University since 1993 to 1995. Mean patient's age is 32.5 years (range, 8 to 65 years), and follow-up periods were a minimum of 3 months to 28 months. 19% condylar fractures were associated with mostly symphysis portion. According to the patient's age, severity of condylar fractures, clinical signs and symptoms, radiographic findings, treatmenet plans had been performed. Rigid fixation have performed greatly, and then fragment removal of fractured mesial pole of proximal segment of the condylar and little cases of reshaping and eminoplasty and lag screw have been applied. Two cases of the both condylar resorption and deviated condyle posteriorly in Dr. Nam's method. None of infection or necrosis signs of treated condyle surgically. In my opinion, whenever possible, displaced condylar fracture can be managed surgically with rigid fixation, but not Dr. Nam's method. Usually if perform the surgical management of condylar fractures you should maintain maxillomandibular fixation for 2 weeks, or more and has to follow-up functional mandibular exercise should be kept continuously.

A HISTOPATHOLOGIC STUDY OF THE EFFECTS OF ARTHROGRAPHY ON THE TISSUES OF TEMPOROMANDIBULAR JOINT (악관절조영술이 악관절 조직에 미치는 영향에 대한 병리조직학적 연구)

  • This study was performed to observe the effects of arthrography on the tissues of temporomandibular joint histopathologically. Among 17 rabbits (34 joints), 2 (4 joints) were used as normal control group, others (30 joints) were experimentally arthrographed using 0.03㎖ diatrizoate meglumine (Hypaque meglumine 60, Wintrop, U.S.A). The rabbits were sacrificed at 2-hour, 1-, 2-, 4- and 7-day after experiment and the tissues of temporomandibular joint were prepared according to the usual method for light microscopic examination. The results were as follows: 1. There were no changes of the fibrous connective tissues of the condylar fossa, the fibrous connective tissues and hyaline cartilages of the condyle, and the articular dislc 2. In 2-hour experimental group, the proliferation of the surface synovial membrane cells, the congestion and hemorrage of the vessels were observed. 3. In 1 and 2-day experimental group, minimal lymphocytes infiltration was observed with the proliferation of the surface synovial membrane cells, the congestion and hemorrage of the vessels. 4. In 4-day experimental group, the diminution of the proliferation of the surface synovial membrane cells was seen, but there was no vascular changes. 5. In 7-day experimental group, the tissues showed similar appearance to the normal control group.

Surgical treatment of recurrent mandibular dislocation by augmentation of the articular eminence (악관절 융기 증강술을 통한 습관성 악관절 탈구환자의 치험례)

  • Recurrent mandibular dislocation is frequent morbidity of temporomandibular joint relatively. There are many etiologic causes in TMJ disorders but, difficult to find obvious one. Various treatment methods have been utilized for a mandibular dislocation. It is categorized into two groups broadly - nonsurgical or surgical methods. The basic rationale of the surgical method is to allow free movement of the condyle by reducing height of articular eminence or to limit anterior excessive movement of the condyle by increasing height of articular eminence or soft tissue anchoring procedure. In this case, 69 year-old woman was treated by augmentation of the articular eminence with mandibular symphysial bone graft leading to osteosynthesis without difficulty. As a result, favorable postoperative outcome was obtained functionally without any complication or recurrence.

CHANGES OF REFERRED SYMPTOMS FOLLOWING SURGICAL TREATMENT IN PATIENTS DIAGNOSED AS TMJ INTERNAL DERANGEMENT (악관절 내장증의 증상 중 연관통의 악관절 수술 후 변화)

  • Purpose: The aim of this study was to investigate the improvement of referred symptoms of TMJ internal derangement after open surgery of TMJ. Materials and Methods: The present study focused referred symptoms such as headache, neck pain, shoulder pain and evaluated the incidences and changes in their improvement at 1, 3, 6, 12 months after surgery in 118 patients who had TMJ internal derangement and received meniscoplasty(or meniscorrhaphy) and eminectomy at TMJ clinic of Yonsei University, Yongdong Severance Hospital, between 1992 and 1997. Results: One hundred eighteen patients(102 females, 16 males) with mean age of 29.6 years(range, 14 to 74) were included in this study and the mean follow-up period was 19.5 months(range, 12 to 60). The incidences of headache, neck pain, shoulder pain among the surgically treated TMJ internal derangement patients were 66.9%, 28.8%, 28.0% before surgery. Thirty-two preoperative severe(+++) headache patients were all improved their headache at 1, 3, 6 months after surgery, but slight relapse(9.4%) was found at 12 months after surgery. The clearance rate of preoperative neck and shoulder pain were 91.2% and 90.9% at 1 month after surgery and all of these groups had no pain at 12 months after surgery. Conclusions: Referred symptoms of TMJ internal derangement such as headache, neck pain, shoulder pain were dramatically improved after surgical treatment of TMJ.
