A STUDY ON THE EFFECT OF CHINCAP IN JUVENILE SKELETAL GLASS III MALOCCLUSION (유년기 III급 부정교합자에서 이모장치의 치료 효과에 관한 연구)

  • The purpose of this study was to investigate the changes in the craniofacial skeleton subsequent to chincap therapy in the juvenile skeletal Class III malocclusion with more appropriate control samples. The experimental group consisted of 29 Korean children(14 males, 15 females) who had skeletal Class III malocclusion with prognathic mandible and were undergone chincap thorny from the beginning of treatment. The control group was composed of 21 Korean children(10 males, 11 females) who had no orthodontic treatment, but with similar skeletal discrepancies to experimental group. Lateral cephalometric radiographs at the age of 7, and 2 years later were analyized and compared with student's t-test(p<0.05). The results of this study were as follows; 1. The control group without chincap therapy had not shown any improvement of the skeletal discrepancies, but had grown to be much severe. This means that the untreated Class III patient with prognathic mandible would not be corrected by growth. For the experimental group with chincap therapy, the anterior-posterior skeletal discrepancies and mandibular prognathism were both improved. 2. Neither significant restraint nor acceleration of growth was found in the cranial base and maxilla by chincap treatment. 3. The inhibition of mandibular growth could not be accepted, but the changes of the direction of growth and morphological changes were found. 4. Vertical growth tendency was increased with chincap therapy. 5. When Putting together the results of the analyses , it seems to be the rotation and displacement of the mandible that the major treatment effects of chincap we. The changes of the direction of growth and the morphological changes also seems to contibute to the treatment effect partly. In summary, the chincap doesn't restrain the mandibular growth. But, it is considered as a useful treatment modality for correction of skeletal discrepancies and reduction of mandibular prognathism in growing Class III patients with madibular prognathism.

  • Congenital aglossia is a very rare condition. The oral manifestations of an aglossia include micrognathia, high arched or cleft palate, defects of the lower lip, an absence of lateral incisors and a mandibular growth deficiency. Although the etiology of congenital aglossia is unclear, both genetic and teratogenic mechanisms have been proposed. Treatment of aglossia patients depends on the nature and severity of the condition which includes surgical rehabilitation of the tongue tip to some extent, orthopedic expansion of the mandible to guide mandibular growth, and mandibular expansion by a distraction osteogenesis. In the present case, a 6 year old female aglossia patient with situs inversus was treated. A bonded hyrax screw was used to increase her mandibular primary intercanine width and intermolar width. A second phase orthodontic and surgical treatment will be possible after some retention phase.

  • Postpubertal craniofacial growth changes were assessed from serial lateral cephalograms taken at which subjects were 16 and 18 years of age. The sample consisted of 19 male and 14 female subjects with no previous orthodontic treatment who had normal occlusion. The craniofacial growth changes of these samples during this study period might be summarized as follows 1. Significant increase in mean maxillary and mandibular length, cranial base length, posterior md anterior facial height were observed at the age period studied in male(P<0.05), however, in female significant increase was observed only mandibular length(Ar-Gn, Ar-Go) and anterior and posterior facial height(P<0.05). 2. Total growth increment of all measumenats were below 1mm in female, but in male, total mandibular(Ar-Gn) growth increment was 2.0mm and those of mandibular tody length, ram length, posterior facial height, and anteior facial height were 1 to 1.54mm and the others were below 1mm 3. A closing, forward rotation of the mandible occured with a significant decrease in mandibular plane angle of $0.61^{\circ}$ in male(P<0.01). 4. Summarized the findings, even though statistically significant growth changes in some dimensions of female were observed, the amount of growth change were very small, however, in male the growth changes of some dimensions associated with mandible were significant and considerable amount.

USEFULNESS OF CERVICAL VERTEBRAE MATURATION STAGE AS A MANDIBULAR MATURATION INDICATOR (하악골 성장 지표로서 경추골 성숙도의 유용성)

  • This study was to investigate if cervical vertebrae maturation stages are as useful as hand-wrist maturation stages in evaluating the mandibular growth. The subject consisted of 292 girls aged from 8 to 16 years with normal occlusion. They were classified according to diagnosis by using studycast, lateral cephalogram, and handwrist X-ray film. The results were as follow: 1. Cervical vertebrae and hand-wrist maturation stages increased with age. 2. All mandibular measurements (Ar-Go, Go-Me, N-Go, S-Gn, N-Me) increased linearly with cervical vertebrae maturation stages. 3. Ar-Go, Go-Me, N-Go, S-Gn increased linearly with hand-wrist maturation stages. 4. Ar-Go, Go-Me, N-Go, S-Gn increased relatively rapidly between cervical vertebrae maturation stages 3 and 4. Go-Me and S-Gn increased relatively rapidly between hand-wrist maturation stages 6 and 7. 5. Ar-Go, Go-Me, N-Go, S-Gn, N-Me had high correlations with cervical vertebrae maturation stages as well as hand-wrist maturation stages. These results suggest that cervical vertebrae maturation stages are reliable on evaluating the mandibular growth.

  • The purpose of this study was to assess the effect on mandibular growth of botulinum toxin type A (BTXA) injection into the unilateral massester muscle of growing rats at three different growing stages. Thirty six male Sprague-Dawley rats were divided into three groups according to the age (group 1: 4 week-old, group 2: 5week-old, group 3: 6week-old). Then each group was randomly divided into 3 subgroups (control group, unilateral injection group, bilateral injection group). Experimental animals were sacrificed after 4 weeks. Then the jaw measurements were evaluated. The results were as follows: 1. In the group 1, mandibular body length, condylar height and coronoid process height of the unilateral group(BTXA side) and the bilateral group were shorter than those of the control group (p<0.05). 2. In the group 2, anterior region height, condylar height, coronoid process height of the unilateral group(BTXA side) and the bilateral group were shorter than those of the control group (p<0.05). 3. In the group 3, mandibular body length, condylar height, coronoid process height of the unilateral group(BTXA side) and the bilateral group were shorter than those of the control group (p<0.05). 4. There was no significant difference in mandibular measurements between the control side and the injection side in the unilateral group in all age groups (p>0.05).

  • This study was to investigate the horizontal & vertical bone change pattern when using cervical headgear in Class II malocclusion of growing children and compared the skeletal features between the group with increased lower facial height and the group without increase in lower facial height. The results are as follows ; 1. Forward growth of maxilla was inhibited, downward tipping of anterior palatal plane could be seen and distal movement of maxillary first molar was observed. 2. There was relative forward movement of Mandible against the Maxillary cranial base, and relative forward movement of mandibular 1st molar against the Maxilla and vertical increase due to alveolar growth of Mandible. 3. There was significant increase in anterior and posterior facial heights but the ratio of facial height showed no significant difference. 4. The group with increased lower facial height has shorter ramus length, than the smaller palatal plane angle, and more distal movement of Maxillary 1st molar than the group without increase Ha-young Hyun

  • The purpose of this study was to provide the reference data evaluating the treatment effect of orthopedic appliances. The skeletal and dental growth increments were measured in 24 normal mixed dentition children (boys: 14, girls: 10) by three serial lateral cephalograms: initial mean age: 9${\pm}$1.3 years, mean observation period: 13${\pm}$1.3 months. Cephalometric changes were analysed. The results were as follows: 1. In boys, the maxilla showed forward and downward growth pattern and the mandible showed forward growth pattern (p<0.05). In girls, the maxilla and mandible showed forward and downward growth pattern(p<0.05). 2. Horizontal growth of both maxilla and mandible in girls was superior to those in boys (A point; girls: 2.39mm, boys: 1.26mm, with p<0.05), whereas vertical growth of both maxilla and mandible in boys was similar to those in girls. 3. The change in tooth axis showed labioversion of upper incisor (p<0.01) and comparatively stable lower incisor position.

  • The aims of this study were analyze the amount of stress condition when the traction force was applied to the condyle head from the chin area of mandible and amount of distortion condition when intended 0.5 mm distraction distance from surface or one point of dissected midline of mandible. For this study, 3D finite element analysis were performed. The following results were obtained : 1. When traction force of 500 g was applied to the condyle head from the chin area, condylar neck area showed the greatest amount of stress and coronoid process was the least amount of stress area. For the amount of distortion condition, infra dental area showed the greatest. 2. When 0.5 mm of intended surface distortion was applied after dissection of mid-mandible area, base anterior area showed the greatest amount of stress but the least stress area was coronoid process. For the amount of distortion, infra dental, menton area showed the greatest amount. 3. One point distortion was applied after dissection of mid-mandible area, ramus posterior area showed the greatest amount of stress and menton area were the least stress condition. For the amount of distortion, menton area showed the greatest amount of distortion condition.

  • Structures of jaw bones, gill rakers and pharyngeal bones of the three Pomadasys species collected from Arafura sea of Indonesia in May 1996 were described and illustrated to compare between species. The percentage of ascending process of premaxillary to head length was 21.6~23.3% in Pomadasys kaakan, 25.4~27.8% in P. maculatus and 15.3~21.0% in P. argyreus. The percentage of ascending process of premaxillary to premaxillary was 54.7~72.8% in P. argyreus, 107.8~114.9% in P. kaakan and 126.7~136.7% in P. maclllatlls. The ascending process of premaxillary of P. argyreus showed a tendency to increase as fish grow. Each jaw in all three grunters had a row of conical teeth exteriorly, but 1 to 5 rows of niddle like teeth interiorly. Among the three species examined, P. maculatus showed the most developed teeth. Pharyngeal bone, appeared to be stronger than jaw teeth, contained dense needle like teeth in P. argyreus, where as that in P. kaakan and P. maculatus had long and slender teeth exteriorly but thicker interiorly. Gill rakers of P. kaakan and P. maculatus are short, but those of P. argyreus are long.

  • Loss of mandibular continuity due to neoplasm, trauma, or infection results in major esthetic and biologic compromise. The use of costochondral grafts for reconstruction of temporomandibular joint, described first by Gillies in 1920, has been accepted as a suitable method for replacing the mandibular condyle, especially in growing children. Autogenous iliac bone graft has been a satisfactory source of mandibular reconstruction since Sykoffs report in 1900. Autogenous bone grafts from the posterior aspects of the ilium provide large amount of PMCB with acceptable donor site morbidity. In timing of reconstruction, initial disease, age, medical history, growth and development, esthetic and psychologic factors should be considered. We present a case of osteosarcoma in the mandible that was treated by a hemimandibulectomy and the defect was reconstructed 20 months later with composite method of costochondral and posterior iliac bone graft.
