뜻 (sense)과 지시체의 구별을 근간으로 하는 프레게류 의미론에 의하면, 같은 지시체를 갖는 이름들의 인지적 의미는 서로 다를 수 있다. 그 반면, 프레게의 뜻을 어떤 이름의 지시체를 결정함에 있어 매개적인 역할을 하는 존재자라 보고 단호히 거부하는 소위 직접 지칭론자들은 문맥에 관계없이 같은 지시체를 갖는 이름들은 동일한 인지적 의미를 갖는다고 주장한다. 그러나 일단 뜻을 배척하고나면, 명제 태도 문맥에서 왜 같은 지시체를 갖는 이름들이 대치될 수 없는지를 설명하기가 어렵고, 따라서 명제 태도의 퍼즐들은 직접 지칭론의 아킬레스건이 되어 왔다. 최근 Zalta는 다소의 수정을 통해 프레게와 직접 지칭론자 양자의 중요한 통찰들을 희생하지 않으면서 양자의 관점을 종합하는 것이 가능하며, 실제로 자신의 해결책은 명제 태도의 퍼즐들을 훌륭하게 해결해 준다고 주장한다. 본 논문은 Zalta의 절충안을 비판적으로 검토함으로써 선결되어야 할 쟁점들을 부각시키고자 한다.

Various minimally invasive repair techniques have been performed for acute Achilles tendon rupture. Despite this, it is difficult to use these techniques in common practice because of the necessity of special instruments. We propose a novel minimal invasive technique using sponge holding forceps, which are commonly used in the operating room for the acute Achilles tendon rupture.

Tendoachilles rupture has recently seen an increase in frequency, accounting for up to 40% of all tendon ruptures. However, an acute segmental rupture of the Achilles tendon is very rare with only one case caused by predisposing factors, such as steroid injection. In this report, we highlight an unusual clinical presentation of a segmental rupture of the Achilles tendon without any underling predisposing factor and without direct trauma. Herein, we discuss its mechanism. Twelve months after surgical repair with open technique, this patient became fully functional again in daily activity.

Ossification of the Achilles tendon is a rare condition to be distinguished from the more frequently occuring tendon calcification. It is characterized by the presence of an ossific mass contained within the fibrocartilaginous substance of the tendon and is usually associated with prior surgery or trauma to the tendon. We treated a 13 year old girl with ossification of the Achilles tendon after sprain-like injury. We excised it one year later and the microscopic finding of the extirpated bony mass revealed fully maturated lamellar bone.

Ossification of the Achilles tendon is a very rare condition. We report a case with ossification of Achilles tendon, recently treated surgically. The patient was a 44 year old male whose chief complaint was discomfort around the Achilles tendon. He didn't have a previous history of surgery or trauma. The roentgenography showed that the bony mass was $15\times3cm$ on the right leg. Ossification of Achilles tendon was found in the Achilles tendon and treated by surgical removal of a bony mass and suturing the tendon. Microscopic examination of the extirpated specimen revealed bone formation through enchondral and intramembranous ossification in the Achilles tendon.

Xanthoma is a relatively rare soft tissue lesion on the Achilles tendon and is usually associated with hyperlipidemia (lipid metabolism abnormality), mental retardation, cataract and atherosclerotic disease. We report on a case of normolipidemic bilateral Achilles tendon xanthoma without any notable cause. We herein describe the case where we achieved a satisfactory result by subtotal resection.

스포츠 활동 중 하퇴부 손상 및 동통은 과도한 훈련활동 및 강도 증가, 잘못된 훈련 방법, 훈련 환경의 적용 잘못으로 발생하는 경우가 많으며 주로 달리기, 축구, 롤러브레이드 등 하지를 많이 사용하는 종목에서 발생된다. 급성 손상으로는 급성 구획 증후군, 근육 손상 및 좌상, 근 경련 등이 있으며 만성 손상으로는 만성 활동성 구획 증후군, shin splints, 피로골절, 아킬레스 건염 등이 발생된다. 이러한 손상이 발생되는 원인과 치료 방법에 대해 알아보고 적절한 훈련 방법, 보조구 장착을 하여 하퇴부 손상을 예방할 수 있는 방법이 중요할 것으로 생각된다.

It is widely acknowledged that Achilles tendinopathy and resultant degeneration of the Achilles tendon could be either due to vigorous physical exertion or due to inflammation of the tendon associated with systemic disease. The overuse injuries are generally multifactorial in origin and are caused by repetitive strain of the affected tendon till the tendon can no longer endure the tensile stress. Various alignment and biomechanical faults are claimed to play a causative role. Only 2% of patients complaining of Achilles tendon pain are caused by systemic disease. However, to ensure the right approach to treatment, it is necessary to rule out inflammatory tendinitis caused by systemic diseases such as rheumatoid arthritis, gout, and seronegative spondyloarthrosis.

A 3rd degree burn on the heel including the Achilles tendon is vulnerable and requires active treatment to improve the functional outcomes. Previously, there have been a few treatments on severe burns, such as amputation, debridement or simple skin graft. The cooperative technique of an anterior lateral thigh flap with Achilles tendon reconstruction can be an innovative procedure that preserves the major arteries. The authors review a case and report the clinical outcome.

This review article attempts to describe several pathological conditions that can arise from the shortening of the Achilles tendon. The tension and tightening of the gastrocnemius-soleus-Achilles tendon complex (GSAC) can cause disharmony in the movement of the entire foot as well as the ankle joint when subjected to weight-bearing or walking. In addition, since these phenomena are observed in various lesions of the ankle joint, the dynamic shortening caused by the tension of GSAC may not be the primary cause of various ankle joint lesions, but is still considered to be a significant contributing factor.
