Effectiveness of a Post-operative Exercise Program on Pain and Disability Activities in Patients with Lumbar Spinal Fusion (운동 프로그램이 요추유합술 환자의 통증과 일상생활활동장애에 미치는 효과)

  • Purpose: This study was to develop a post-operative exercise program, apply it to patients undergone lumbar spinal fusion surgery, and evaluate the effectiveness of the program on pain and disability activities of daily living. Methods: Fifty six patients who had lumbar spinal fusion were enrolled in this study. The patients were divided into two groups; 28 patients in the intervention group completed post-operative lumbar exercise program including walking for four weeks and 28 patients in the control group only did walking exercises. The degrees of pain on low back and leg were evaluated using visual analog scale (VAS) and the functional outcome was evaluated using the Korean version of Oswestry Disability Index (KODI) before surgery and 5 weeks after surgery. The data were analyzed using descriptive statistics, Chi-square test, t-test with SPSS 18.0 program. Results: Low back and leg pain of the participants in both experimental and control groups were improved after surgery compared to pre-surgery pain. However, there was no statistically significant difference between the groups. KODI score in the intervention group was significantly lower than that of the control group (p=.014). Conclusion: The developed post-operative exercise program in patients with lumbar spinal fusion surgery seems to be a useful intervention to reduce disability in activities of daily living.

Incidence of Peripheral Cyanosis in Patients with Therapeutic Hypothermia after Cardiac Arrest in the Acute Care Unit of a Tertiary General Hospital (일개 상급종합병원 응급중환자실 내 심정지 후 저체온요법을 적용한 환자의 말초 청색증 발생 현황)

  • Purpose : This study aimed to identify the incidence of peripheral cyanosis and the characteristics and clinical results of patients with therapeutic hypothermia after a cardiac arrest. Methods : Data were collected from April to June 2021 via the electrical medical records of 95 patients with therapeutic hypothermia for 72 hours after a cardiac arrest admitted to an acute care unit at a tertiary hospital between January 1, 2016, and December 31, 2019. The data were analyzed using descriptive statistics and a t-test, Mann-Whitney U test, Chi-squared test, Fisher's exact test, and logistic regression using SPSS/WIN. Results : The incidence of peripheral cyanosis was 20%. In the peripheral cyanosis group, peripheral vascular disease, fibrinogen, vasopressor, infection, disseminated intravascular coagulation, acute physiology, chronic health evaluation II score on the second hospital day, nursing intervention, and mortality on the seventh hospital day were higher. The level of fibrinogen and use of vasopressors affected the occurrence of peripheral cyanosis. Conclusion : Considering the influencing variables, careful observation is necessary for patients with high fibrinogen levels and vasopressor use. These results provide basic data to recognize the need for nursing intervention for peripheral cyanosis and encourage nurses to deliver them during therapeutic hypothermia.

Factors Influencing Distress in Premenopausal Patients with Breast Cancer Undergoing Endocrine Therapy (내분비요법 중인 폐경 전 유방암 환자의 디스트레스 영향 요인)

  • Purpose: The purpose of this study was to investigate relationships among menopausal symptoms, functional status, and distress and to identify factors influencing distress in premenopausal breast cancer patients who had been on endocrine therapy. Methods: A descriptive correlational study was conducted. Data were collected using questionnaires from 140 patients with breast cancer undergoing endocrine therapy at a general hospital. Data were analyzed using descriptive statistics, t-test, ANOVA, Tukey HSD test, Pearson's correlation analysis and hierarchical regression analysis. Results: The mean scores for menopausal symptom, functional status, and distress were $19.65{\pm}7.86$, $2.67{\pm}0.33$ and $3.69{\pm}2.19$, respectively. The menopausal symptoms and distress were positively correlated (r=.76, p<.001). The menopausal symptoms and functional status (r=-.43, p<.001) and functional status and distress (r=-.31, p<.001) were negatively correlated. The most influential factor for distress was menopausal symptoms (${\beta}=.79$, p<.001). Conclusion: Based on the findings of this study, developing nursing intervention programs focusing on decreasing menopausal symptoms and distress are recommended.

The Effects of an Early Knee Joint Exercise Education Program on Pain, Knee Range of Motion, and Satisfaction in Patients after Undergoing Total Knee Replacement Arthroplasty (슬관절 전치환술 후 조기 슬관절 운동 교육 프로그램이 통증, 슬관절가동범위, 환자만족도에 미치는 영향)

  • Purpose: This study was conducted to evaluate the effects of early knee joint exercise education program on pain, knee ROM, and satisfaction among patients with a total knee replacement arthroplasty. Methods: An experimental study with non-equivalent groups was conducted using 32 patients for an experimental group and 32 patients for a control group. The experimental group received knee joint exercise education including systemic continuous passive motion (CPM) exercise and knee exercise. The control group received conventional CPM exercise. Pain, flexion and flexion contracture range of motion, and patient satisfaction were evaluated by $x^2$ test, Fisher's exact test, t-test, and repeated measures ANOVA using the SPSS 23.0 Windows program. Results: There were a significant improvement in patients' knee flexion and satisfaction in the experimental group compared to the control group. There was no significant difference in pain and flexion contracture between the two groups. Conclusion: The findings suggest that the proposed education program is efficient and effective when providing nursing care after a total knee replacement arthroplasty.

Usefulness of Manual Analysis of Bone Mineral Density Using Dual Energy X-ray Absorptiometry (이중에너지엑스선흡수기(Dual energy X-ray Absorptiometry: DXA)를 이용한 골밀도검사 결과분석에서 수동분석법의 유용성 평가)

  • The results of bone mineral density analysis using DXA were compared between automatic and manual methods. The purpose of this paper is to verify the range of errors of each analysis method in the same patient and select a proper method to minimize errors. Comparisons between automatic and manual analysis methods were made using BMD, BMC and AREA. Basal and follow up examinations were performed with the patients of normal, osteopenia and osteoporosis. In the basal examinations, the precision errors between automatic and manual method showed 1.9% in normal, 3.1% in osteopenia and 3.8% in osteoporosis. In case of follow up studies, the precision errors between automatic and manual method showed 2.3% in normal, 3.2% in osteopenia and 3.5% in osteoporosis. BMC and AREA also showed a tendency to increase precision errors on osteopenia and osteoporosis. Therefore, a manual method would be a better option to minimize errors in patients with osteopenia and osteoporosis.

Comparison of Topics Related to Nurse on the Internet Portals and Social Media Before and During the COVID-19 era Using Topic Modeling (토픽 모델링을 활용한 COVID-19 발생 전후 간호사 관련 토픽 비교: 인터넷 포털과 소셜미디어를 중심으로)

  • Purpose: The purpose of this study is to compare topics through keywords related to nurses in internet portals and social media Pre coronavirus disease (COVID-19) era and during the COVID-19 era. Methods: For six months before and during the outbreak of COVID-19 in Korea, "nurse" was searched on the internet. For data collection, we implemented web crawlers in programming languages such as Python and collected keywords. The keywords collected were classified into three domains of topic Modeling. Results: The keyword 'nurse' increased by 15% during COVID-19 era. Keywords that ranked high in Term Frequency - Inverse Document Frequency (TF-IDF) values were before COVID-19, such as "nurse" and "C-section". during COVID-19, however, they were not only "nurse" but also "emergency" and "gown" related to pandemics. Conclusion: Various topics were being uploaded into the internet media. Nursing professionals should be interested in the text that is revealed in the internet media and try to continuously identify and improve problems.

Differences in performance and importance of handover items between 119 paramedics and emergency department staff (119구급대원과 응급실 의료진의 인수인계 항목 수행도 및 중요도의 차이)

  • Purpose: This study aims to identify the present handover status and the difference in performance and importance of handover between 119 paramedics and emergency department (ED) staff to improve the quality of handover. Methods: The study involved a questionnaire survey of 171 paramedics and 108 ED staff in four regions from August 28 to September 23, 2020. Subsequently, 279 questionnaires were collected and 277 copies were used for this study. Data were analyzed by SPSS 26.0 version. Results: The results showed that most of the paramedics and the ED staff rely on memory and verbally handover, without formal education about the handover process. And there were significant differences in performance in important information of prehospital. Some of the handover items were a difference in performance and importance between the paramedics and the ED staff. Conclusion: The unified education based on required and optional items is necessary to reduce the loss of information between the paramedics and ED staff, and standardized tools need to be developed in the handover process.

Effects of Hip Joint Mobilization on Pain, Balance, and Gait in Patients with Patellofemoral Pain Syndrome (고관절 가동술이 슬개대퇴통증증후군 환자의 통증, 균형 및 보행에 미치는 영향)

  • Background: Patellofemoral pain syndrome (PFPS) is common knee disorder encountered in clinical: notably, altered hip biomechanic may contribute to PFPS. In this study, We investigated the effects of hip joint mobilization on pain, balance, and gait in patients with PFPS. Methods: Patients were randomly assigned to a control group (n=18) or an experimental group (n=20). Both groups received exercise therapy thrice a week for 4 weeks. The experimental group performed additional hip joint mobilization thrice a week for 4 weeks. Measurement were obtained in each patient pre-intervention and post-intervention (after 4 weeks). Results: The assessed items included the visual analog scale (VAS), one leg standing test (OLS), timed up and go test (TUG), and the 10m walk test (10MWT). Post-intervention assessment showed significantly improved results in both groups (p<.01). A significant intergroup difference was observed only in the results of the 10MWT (p<.05). Conclusion: Our results indicate that hip joint mobilization with exercise therapy may be useful to improve PFPS.

The Effect of Perceived Patient- and Family-Centered Care on Nurses' Caring Behavior in Intensive Care Units (중환자실 간호사의 환자-가족 중심 간호에 대한 인식이 돌봄 행위 수행에 미치는 영향)

  • Purpose: The purpose of this study was to identify the factors influencing nurses' caring behavior. Methods: The descriptive study included 178 intensive care unit (ICU) nurses who completed a self-report structured questionnaire. The data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, and Pearson's correlation and multiple regression analysis using the IBM SPSS/WIN 23.0 program. Results: Significant correlation existed between perception of Patient- and Family-Centered Care (PFCC) and nurses' caring behaviors (r=.36, p<.001). Multiple regression analysis showed that perception of support (β=.36, p<.001), total career length (β=.33, p<.001), and familiarity with PFCC (β=.15, p=.018) affected the caring behavior of ICU nurses. These variables explained 30.0% of the variance in caring behavior. Conclusion: Effective strategies are needed to improve perceptions of support as well as to promote PFCC for increasig the frequency of caring behavior among ICU nurses.

Risk Factors for Cardiac Implantable Electronic Device-Related Infections (이식형 심장 모니터링 장치 관련 감염의 위험요인)

  • Purpose: This study aimed to investigate the risk factors for cardiac implantable electronic device (CIED)-related infections within the first post-procedural year after CIED insertion. Methods: This study included 509 adult patients undergoing CIED implantation procedures between January 1, 2011 and December 31, 2015. The data were analyzed by t-test, chi-square test, Fisher's exact test, and logistic regression analysis using SPSS/WIN 23.0. Results: Fifteen infections and 494 non-infections were examined. The CIED-related infection rate was 2.9%; patients with 14 pocket infections and one bacteremia were included in the CIED-related infection. The risk factors of CIED-related infections were the estimated glomerular filtration rate (eGFR) of ≤ 45 mL/min/1.73 m2 (Odds ratio [OR]= 4.03, 95% confidence interval [CI],1.15-14.10) and taking a new oral anticoagulant (NOAC) (OR = 4.50, 95% CI 1.09-18.55). Conclusion: These results identified the CIED infection rate and risk factors of CIED-related infection. It is necessary to consider these risk factors before the CIED implantation procedure and to establish the relevant nursing interventions.
