Efficacy of Ecabet Sodium for Helicobacter pylori Eradication with Sequential Therapy (순차치료에 병합 투여된 Ecabet Sodium이 Helicobacter pylori 제균에 미치는 효과)

  • Background/Aims: We aimed to compare the outcomes of Helicobacter pylori eradication in patients receiving sequential therapy (ST) depending on the use of ecabet sodium (ES). Materials and Methods: Between January to December 2015, 176 patients randomly received either ST alone (n=72) or 10-day ES therapy combined with ST (n=104). After applying the exclusion criteria, 56 patients were finally assigned to the ST-only group and 84 to the ST with ES group. We retrospectively reviewed and analyzed the H. pylori eradication rate and adverse events between the two groups. Results: Among the 140 patients, 121 (86.4%) achieved successful H. pylori eradication and 24 (17.1%) had adverse events. Eradication was achieved in 50 patients (89.3%) in the ST-only group and in 71 patients (84.5%) in the ST with ES group (P=0.420). No significant difference in the incidence of adverse events was found between the ST-only and ST with ES groups (12.5% vs. 20.2%, respectively; P=0.234). However, the ST with ES group tended to have a higher prevalence of nausea or vomiting than the ST-only group (11.9% vs. 1.8%; P=0.050). Conclusions: ST showed a good H. pylori eradication rate without deteriorating the adverse events regardless of adding ES.

Validation of the Effectiveness of Education for Obtaining Consent in Clinical Study (임상연구 동의서 교육 시행의 효용성 검증)

  • Purpose: To validate the effectiveness of obtaining consent education on errors in the consent process and to develop the education program for researchers. Methods: From February 2019 to February 2022, a 30-minute, 1:1 face-to-face consent education developed using the ADDIE model was conducted on 78 nurses as principal investigators. An informed consent audit tool, which includes 6 items developed by Asan Medical Center Human Research Protection Center, was used to analyze errors in obtaining informed consent process. Data analysis was performed using the SPSS ver. 25.0, and the Mann-Whitney U-test and χ2-test were utilized to verify the difference in errors between the experimental and control groups. Results: The participants consisted of 42 in the experimental group and 36 in the control group, with no statistically significant difference between the 2 groups. Both 2 groups showed the highest frequency of documentation errors, followed by format errors, errors related to a suitability of investigator, participant, or participant's legally acceptable representative, witness and confidentiality issues. After education, there was a significant decrease in both format errors (p=0.002) and documentation errors (p<0.001) in the experimental group. The proportion of participants without any errors in all items was higher in the experimental group (35.7%) compared to the control group (5.6%), and this difference was statistically significant (p=0.001). Conclusion: The obtaining consent education program was found to be effective in reducing informed consent errors. This study emphasizes the importance of education, suggesting the need for its expansion and accessibility, as well as the necessity for all researchers conducting clinical studies to receive the obtaining consent education.

Regional Innovation System for Co-developing Neighboring Cities: Analysis on the Innovation Actors of Cheonan and Asan, Chungnam (인접지역 공동발전을 위한 지역혁신체계 구축의 과제: 충남 천안시와 아산시의 혁신주체 분석)

  • In a recognition of regional innovation system approach for a co-development effort of the Cheonan-Asan area, Chungnam, this research conducted a survey from the local leaders of these two cities. The results of the survey show that most local leaders strongly agree with the necessity of co-development efforts, that some suggest a merge of two cities, and that some suggest industrial development programs in the city border. The local leaders expect that public officials should play a major role in co-development of the two cities while local leaders are disappointed with the public officials activities. They also highly expect that civilian leaders (NGOs) and professors should play important roles in this matter. Upon these results, the research suggests to formulate a cooperative regional innovation system for the co-development of the two neighboring cities by three steps.

  • Autoimmune pancreatitis (AIP) is rarely associated with pancreatic pseudocysts. AIP-associated pseudocysts requiring drainage despite steroid therapy are rather rare. We report a case of AIP with an infected pseudocyst requiring drainage despite steroid therapy. A 68-year-old male was diagnosed with AIP via pancreatic imaging, a high serum immunoglobulin G4 level, and steroid responsiveness. The AIP was accompanied by a pancreatic pseudocyst. Steroid therapy was prescribed, but the pancreatic pseudocyst became aggravated during steroid tapering. Endoscopic ultrasonography-guided cyst drainage was required; the pseudocyst then became completely resolved.

Effects of Defecation Encouragement Program in Patients Undergoing total Knee Replacement Arthroplasty (배변 격려 프로그램이 인공 슬관절 전치환술 환자의 배변에 미치는 효과)

  • Purpose: The purpose of this study was to evaluate the efficacy of applying a defecation encouragement program for patients undergoing total knee replacement arthroplasty (TKRA). Methods: This program was based on the nursing best practice guideline: prevention of constipation in the older adult population by the National Guideline Clearing House (NGC) in 2011, which included fluid intake, bowel training, and abdominal and pelvic floor exercises. A team of one orthopaedic clinical nurse specialist and six orthopaedic nurses with a 10 years of clinical experience applied and assisted patients with the program. Formal counsel was provided by one orthopaedic doctor, one gastroenterologist, and one exercise therapist. Patients who defecated one day prior to or on the day of TKRA surgery were included. Data collected from 72 subjects were analyzed using SPSS/WIN 21.0. Results: Time until first defecation after surgery was 2.4±1.1 days in experimental group, which was significantly shorter than the 3.5±0.9 days in control group (t=4.28, p<.001). Constipation assessment scale showed significantly lower points (t=2.55, p=.013) in experimental group (1.3±1.2) compared to control group (2.6±2.6). The experimental group and control group were 17.3±7.67 and 23.7±14.43, respectively, and the experimental group used less laxatives (t=2.83, p=.021). Conclusion: A defecation encouragement program was proved to be an appropriate nursing intervention for patients undergoing TKRA. This study confirmed that constipation is a nursing problem that can be sufficiently prevented if nurses are interested and encourage defecation.

Assessment of the Health of Fish Species Collected in Agricultural Reservoirs, AsanHo and NamyangHo (농업용저수지 아산호와 남양호에 서식하는 어류의 건전성 평가)

  • The purpose of this study was to establish biomonitoring method for the assessment of the health of fish species in agricultural reservoirs polluted from various environmental contaminants. The ichthyofauna and fish community were investigated in the AsanHo and NamyangHo from September to November 2007. We measured abundance, size distribution, biomass, the number of fish with external and internal lesions, organosomatic index of the fishes collected in the AsanHo and NamyangHo. Dominant species were Lepomis macrochirus, Carassius auratus and the highest biomass were found in Cyprinus carpio, Carassius auratus in the AsanHo and NamyangHo, respectively. The composition of age in the population of Carassius auratus in the AsanHo and NamyangHo was stabled by size distribution analysis. The sex ratio of Carassius auratus were 1:0.36, 1:0.52 as female: male ratio in AsanHo and NamyangHo, respectively. The GSI of were 4.30, 1.69 of female and 1.45, 1.70 of male in AsanHo and NamyangHo, respectively. Most carp were 10-70 cm long in the AsanHo and 20-50 cm in the NamyangHo. The sex ratio of Cyprinus carpio were 1:0.42, 1:0.59 as female: male ratio in AsanHo and NamyangHo, respectively. The GSI of were 0.16, 3.46 of female and 0.93, 2.37 of male in AsanHo and NamyangHo, respectively. Of the 227 fish examined, 1.8% had some type of external lesion in AsanHo and 10% in NamyangHo. Overall, HAI scores ranged from 10-60 and HAI scores of AsanHo relatively lower than that of NamyangHo.
