Patient Exposure Dose Reduction in Coronary Angiography & Intervention (심혈관조영술 및 중재술 시 환자 선량 감소방안)

  • This study, the method of reducing the exposure dose by changing the geometrical requirements among the preceding studies and the method of directly wearing a protector on the patient were used to expose the patient. A comparative experiment was conducted on the method of reducing the dose and the most effective method for reducing the exposure dose was investigated. Using the phantom, the dose of the lens, thyroid gland, and gonad gland in the 5 views most used in coronary angiography and intervention accumulated 5 times for 10 seconds at 60~70 kV, 200~250 mA as an automatic controller of the angiography system, and measured by Optically Stimulated Luminescent Dosimeter(OSLD). SID 100 cm and Cine 15 f/s as a control group the experiment was conducted by dividing the experimental group into 3 groups: a group lowered to Cine 7.5 f/s, a phantom protector, and a group lowered to 95 cm SID. As a result of the experiment, showing decrease in exposure dose compared to the control group. Lowering the cine frame may be the simplest and most effective method to reduce the exposure dose, but there is a limit that it cannot be applied if the operator judges that the diagnostic value is small or feels uncomfortable with the procedure. Conclusion as fallow reducing the exposure dose by directly wearing protector is the next best solution, and it is hoped that the conclusions obtained through this study will help reduce the exposure dose to unnecessary organ.

The Relationship between Stress and pain in patient Undergoing Angiocardiography (심혈관조영술 환자의 스트레스와 통증과의 관계)

  • The purpose of this study was to provide basic data for nursing intervention in patient undergoing angiocardiography. Method: The subjects of this study were 60 patients undergoing angiocardiography. The data were analyzed using mean, percentage and pearson correlation Coefficient. Result 1) The mean score of the affective stress(anxiety) was $48.00{\pm}12.24$ and behavioral stress was $1.98{\pm}1.97$. The mean score of the physiologic stress(vital sign) was systolic pressure $124.10{\pm}17.72$, diastolic pressure $79.71{\pm}12.68$, pulse: $81.03{\pm}12.16$, respiration: $21.21{\pm}1.40$ 2) The mean score of the pain was $1.40{\pm}0.67.$ 3) There was a significant positive correlation between stress and pain. Conclusion : The result of this study show that stress is very important factor that effect on the pain therefore it will be considered stress in patient undergoing angiocardiography.

Radiation Exposure According to Radiation Technologist' Working Departments (방사선 종사자 근무 분야별 피폭에 관한 검토)

  • Radiation dose to radiologists working at three hospitals in Seoul was investigated from Jan 1, 2006 to Dec. 31, 2006. The results are as follows. First, radiation dose to radiologists at a cardiac angiography room was measured as 1.41mSv, the highest while radiation dose to radiologists at a department of radiation oncology was measured as 0.64 mSv, the lowest. Second, radiation dose proves to be in direct proportion to the number of X-ray treatment. Third, as for the radiation dose in X-ray treatments, radiologists in cardiac angiography room are exposed to the largest amount of radiation while radiologists in diagnostic radiology department are exposed to the smallest amount of radiation. Last, radiation dose at a cardiac angiography room is the largest and is followed by nuclear medicine, diagnostic radiology, and radiation oncology departments in order. According to ICRP, exposure less than 20mSv per year is highly recommended while radiation dose is allowed as long as it is ranged less than 50mSv per year or 100mSv within a 5-year period. Taking into account the results, radiation exposure does not do any harm to radiologists at any related departments in Korean hospitals because the dose per year is less than 1.60mSv.

Study on the Growth of the Aortic Aannulus. Root, and Anastomosis After Arterial Switch Operation in Infancy (영아기에 시행한 동맥전환술 후의 대동맥판륜, 근부, 및 문합부위 성장에 관한 연구)

  • We investigated changes of the size of neoaortic annulus, root, and aortic anastomosis after arterial switch operation for complete transposition of the great arteries performed in infancy. A total of 23 patients were included in this study. Age ranged from 6 to 153 days. Body weight averaged 3.9$\pm$0.8kg and 17 patients were male. The preoperative angiocardiographic dimensions of the pulmonary annulus, the pulmonary root, and the sinotubular junction, standardized to the diameter of descending aorta at the level of diaphragm, were compared to the size of postoperative measurements of the neoaortic annulus, the neoaortic root, and the aortic an stomosis at a mean interval of 17.2$\pm$ 9.4 months. Mean dimensions of the neoaortic annulus and the neoaortic root were significantly increased postoperatively(n=23, annulus; p<0.01, root; p<0.01), however, those of the aortic anastomosis did not reveal significant change(n=23, p=0.06). There were no significant differences in changes of diameters of the neoaortic annulus, the root, and the aortic anastomosis between patients with(n=8) and without(n=15) postoperative neoaortic regurgitation(annulus; p=0.32, root; p=0.29, anastomosis; p=0.86). Postoperative dimensions of the neoaortic root and annulus between patients with ventricular septal defect(n: 10) and without ventricular septal defect(ni 13) were not significantly changed compared to the preoperative measurements(annulus; p=0.09, root; p=0.07) but mean diameters of the aortic anastomosis decreased significantly after operation in patients with ventricular septal defect(p=0.04). This study revealed that the site of the aortic an stomosis grows in proportion to patient's somatic growth after arterial switch operation. Although we could not demonstrate the relation between the aortic root dilatation and the postoperative neoaortic regurgitation in this study, a continuous close follow-up might be necessary to detect a possible progression of the aortic root dilatation and the resulting significant aortic valve regurgitation.

Comparison of functional Images obtained by radionuclide angiocardiography and gated blood pool scan (방사성핵종 심혈관조영술의 기능적영상화에 대한 고찰)

  • Radionuclide cardiac studies lend themselves exceptionally well to functional imaging. This is especially true for gated blood pool scan (GBP). Making functional images is also possible in radionuclide angiocardiography (RNAC). In this study we tried to validate the functional images obtained from RNAC by comparing it with GBP. Twenty three patients (16 patients with coronary artery diseases, 5 with hypertensive heart diseases, and 2 with nonspecific chest pains) underwent simultaneous RNAC and GBP at the same position (LAO $45^{\circ}$). From both studies, global ejection fraction, regional ejection fraction, phase image, amplitude image, stroke image, paradox image, maximum ejection and maximum filling rates were obtained. Global ejection fraction are almost same in both studies. Regional ejection fractions of apex and inferior portion of left ventricle calculated from RNAC are well correlated with those of GBP. Phase and paradox image, maximum ejection and maximum filling rates were obtained. Global ejection fraction are almost same in both studies. Regional ejection fractions of apex and inferior portion of left ventricle calculated from RNAC are well correlated with those of GBP. Phase and paradox images of RNAC are very similar to those of GBP. However, amplitude and stroke images are different. Regional ejection fractions of the left ventricular base, maximum ejection and maximum filling rates obtained from RNAC are significantly different from those of GBP. In conclusion, albeit all of functional images of RNAC is not same as GBP, regional walt motions and global left ventricular function are expected to be successfully analyzed by phase and paradox image and ejection fraction.

Deducing Coronary Artery Disease Anxiety through Musical Therapy and Providing Information (정보제공과 음악요법이 심혈관조영술 환자의 불안에 미치는 영향)

  • This study was performed to evaluate the effectiveness of music therapy as one of the psychiatric nursing intervention tools, with addtional information in relieving anxiety during the procedure. Data were collected through nonequivalent pre-and post tests from July 1, 1998 to September 30 1998 in 90 patients (test group A: 28 patients, test group B: 27 patients, control group: 33 patients) who were hospitalized in DongSan Medical Center in order to have cardiac catheterization. The Subjects were informed by educational videos, which were modified according to the sensory information of the 10 study patients. They were based on the informative booklet by Kim keum-soon (1989). The procedural information was also modified according to the hospital`s customs. Provided the music for patients suitable to their tastes, and measured their blood pressure, heart rate, the degree of anxiety using the Spielberger`s measurement device of anxiety, and behavioral response of Finesilver`s. The statistical significance was analyzed using chi-square test and ANOVA. The results of this study were as follows : Hypothesis 1 : There are significant differences in the degree of anxiety among test group A, Test group A was provided only information, Test group B was provided information and the control group was provided neither. Hypothesis 2 : There are significant differences in systolic blood pressure among test group A, test group B, and control group.: non-significant. Hypothesis 3 : There are significant differences in diastolic blood pressure among test group A, test group B, and control group.: significant(F=1.31, p=.27, interaction; F=3.80, p=.00). Hypothesis 4 : There are significant differences in heart rate among test group A, test group B, and control group.: non-significant. Hypothesis 5 : There are significant differences in behavioral responses among test group A, test group B, and control group.: significant(F=10.05, p=.00). Further validation study is required with other subjects and other settings.

Stomach Cancer Surgery after Coronary Artery Bypass Surgery with in situ Right Gastroepiploic Artery Graft (정위 우위대망동맥을 이용하여 관상동맥우회술을 시행한 환자에서의 위암수술)

  • A 59 year-old male diagnosed as unstable angina underwent off-pump coronary artery bypass surgery using in situ left internal mammary and right gastroepiploic artery grafts. During harvesting the right gastroepiploic artery, there was no abnormal finding in intraabdominal organs including stomach and liver. He was discharged at the 3rd postoperative day without complication. In case of using in situ right gastroepiploic artery, we recommend gastrofberscopic study at regular follow-up, The patient underwent the gastrofiberscopic study at postoperative 3rd month and diagnosed as advanced gastric cancer on the posterior wall of gastric fundus. At 5th postoperative month, total gastrectomy without intraoperative injury of the right gastroepiploic artery was performed at the department of general surgery. He was discharged at the 9th postoperative day. Follow-up coronary angiography performed at the 1st postoperative year demonstrated patent grafts including right gastroepiploic artery.

  • A 57-year-old man with dyspnea was transferred to our institution. Echocardiography demonstrated grade III aortic valve regurgitation with a bicuspid aortic valve. The preoperative coronary angiography showed a left single nary artery. Replacement of the aortic valve was performed. Ventricular fibrillation developed during weaning patient of cardiopulmonary bypass after aortotomy repair. An anomalous origin of the coronary arteries is an incidental finding. However, in patients who are undergoing aortic valve replacement, a can significantly complicate the procedure. If this is present, then it is imperative that the clearly identified and the required procedure should be modified to avoid injury to these.

  • In detecting, localizing and quantitating cardiac shunts, radionulide angiocardiography has been known to be a simple and safe method compared with oxymetry method. To ascertain the availability of the results obtained by radionuclide angiocardiography for the evaluation of patients with cardiac shunt, author compared the Qp/Qs ratios(pulmonary to systemic flow ratios) obtained by radionuclide angiocardiography with the results of oximetry method in 40 patients with left to right shunt, and also compared the results of radionuclide angiocardiography examined before and after shunt operation in 8 patients. The results were as follows: 1) Of the 161 patients examined radionuclide angiocardiography, 98 were thought to have cardiac shunts: right to left shunt id 27, left to right shunt in 71. Of the 71 patients who had left to right shunt, 40 who were examined with both radionuclide angiocardiography and oxymetry had following congenital heart disease: VSD in 21, ASD in 9 and PDA in 10. 2) Comparison of Qp/Qs ratios obtained during radionuclide angiocardiography and oxymetry revealed good correlation (linear regression analysis yielded correlation coefficient of 0.80) in 32 patients whose Qp/Qs ratio obtained during oxymetry were below 3.0, but very poor correlation in 8 patients whose Qp/Qs ratios were above 3.0 3) Radionuclide angiocardiography is a relatively safe and simple method in postoperative evaluation of patients with cardiac shunt.

Transcatheter Closure of Patent Ductus Arteriosus with a Amplatzer$^{(R)}$ Vascular Plug in Two Dogs (개에서 Amplatzer$^{(R)}$ vascular plug를 이용한 동맥관개존증의 폐쇄 2 증례)

  • A 9-month-old, intact female Maltese dog and one-year-old, intact female beagle dog were presented with continuous heart murmur. These 2 dogs were diagnosed as patent ductus arteriosus (PDA) based on the two-dimensional echocardiography and angiography. Due to the large shunt size, commercially available ductal occlude device was used for transcatheter occlusion of PDA. After Amplatzer$^{(R)}$ vascular plug placement, cardiac murmur was abruptly disappeared in Maltese dog and mildly decreased in beagle dog. Complications and safety after the procedure were evaluated regularly in these 2 dogs. This is first clinical application of Amplatzer$^{(R)}$ vascular plug for transcatheter closure of PDA in two dogs in Korea.
