술중 심정지에 대한 심소생 치료 (Cardiac Resuscitation in the Uperating Room)

  • This is a report of 8 cases cardiac arrest developed in the operating room at Jeonbug university Hospital from January 1973 to October, 1975. Four patients of cardiac arrest developed during the elective operation, 3 during the emergency operation and the remaining one, bronchoscopy for foreign body removal under the general anesthesia. Immediate closed chest cardiac massage was performed in the 7 patients and the remaining one underwent open chest cardiac massage. Five of 7 patients with the closed chest cardiac massage regained consciousness and restored respiration, but 3 patients of these survived to be discharged. Two patients who underwent pneumonectomy for multiple lung abscess and open drainage for liver abscess, were resuscitated but did survived. The Latter died from bleeding due to rupture of the liver that developed during the closed chest cardiac massage. One patient who had open chest cardiac massage survived to be discharged without any sequele. Unsuccessful resuscitation was observed in two patients, one had a complication of malignant hyperthermia with muscle rigidity during gastrectomy for ulcer perforation and another had not firm support on the back during massage.

  • Rib fracture due to intense pain, may restrict patients from inadequate coughing. These conditions may produce varying degrees of complications such as atelectasis, pneumonia and arterial hypoxemia. Thoracic epidural analgesia has been used to treat pain associated multiple rib fractures because of its marked improvement in vital capacity and dynamic lung compliance. However, there are complications related to thoracic epidural analgesia which may include damage to spinal cord, perforation of dura, respiratory depression, decrease heart rate and arterial blood pressure. We experienced such a case of cardiac arrest during thoracic epidural analgesia while treating a patient for multiple rib fractures.

  • 심실조기수축(PVC: Premature Ventricular Contraction)은 성인에게서 가장 흔하게 발생되는 심장 부정맥 증상 중의 하나이다. 심실조기수축 부정맥이 자주 발현되는 사람의 경우 관상 동맥질환, 고혈압 등의 심혈관계 질환이 진행되고 있을 가능성이 많고, 심실빈맥이나 심실세동으로 전이되는 경우 심정지 등을 유발하여 사망에 이르기 때문에 지속적으로 관찰이 필요한 증상이다. 따라서 본 연구에서는 R-R 간격 정보를 이용하여 심실조기수축 부정맥 증상을 실시간으로 검출할 수 있는 신호처리 알고리즘을 구현하고자 하였다.

불화수소산 음독에 의한 심정지 1례 (Ingestion of Hydrofluoric acid: A rapid and fetal poisoning)

  • Hydrofluoric acid is a weak inorganic acid used for etching and as rust removals. Systemic toxicity after oral ingestion induces rapid development of hypocalcemia and hyperkalemia, leading to ventricular fibrillation and finally asystole. We report a case of intentional ingestion of hydrofluoric acid producing an altered mental state at the time of the patient's arrival in the emergency department. The patient died approximately 80 minutes after the exposure with asystol.

  • Cardiac arrest can occur in dental clinics. All dental professionals should be trained to deal with emergency situations that is threatening life. Dental professionals should be familiar with the protocol which include basic life support, advanced life support and specific drugs to be administered in emergencies. Emergency can occur in any dental office without any warnings. Further, it has been noted in recent times there is an increase in a number of medico-legal cases due to rise in a number of death in the dental chair. This review article aims at briefing the basic life support required to manage the medical emergency having life-threatening potential.

당뇨병이 없는 뇌경색 환자에서 발생한 저혈당과 동반된 심정지 (Cardiac Arrest in Conjunction with Hypoglycemia in a Non-Diabetic Patient with Cerebral Infarction)

  • Background: Hypoglycemia is uncommon in people without diabetes. There have been only a few reports of cardiac arrest in conjunction with hypoglycemia in non-diabetic patients. Case Report: A 66-year-old man visited the emergency room with dizziness. He was a chronic alcoholic. Laboratory test showed no evidence of diabetes mellitus. Brain magnetic resonance imaging revealed a left cerebellar infarction. Abdomen computed tomography demonstrated liver cirrhosis with minimal ascites. During his hospital stay, he consumed only a small amount of food because of nausea and headache. On hospital day 4, he had a cardiac arrest after two seizure episodes. His blood glucose was 10 mg/dL. The combination of liver cirrhosis, renal failure and poor oral intake was presumed to be the causes of the severe hypoglycemia. Conclusion: We report a rare case of cardiac arrest occurring in conjunction with severe hypoglycemia in a non-diabetic patient with cerebral infarction.

  • The causes of sudden death after medullary infarction involve arrhythmia, central respiratory failure, and dysautonomia. Sudden cardiac arrest in a medullary infarction is uncommon. Most of these cases experienced sudden cardiopulmonary arrest within 2 weeks from stroke onset as the extent of lesion increased. Here, we report two cases of medullary infarction presenting as sudden cardiac arrest. These cases indicate that medullary infarction could be one of the causes of sudden cardiac arrest.

  • 최근 우리나라의 인구구조가 표주박형태의 고령화 사회로 변화함에 따라 급사의 원인이 되는 심장질환의 발생이 고령이 될수록 증가하고 있다. 심정지나 호흡이 정지된 환자에게 사용되는 인공호흡기중 전 세계적으로 보급되고 있는 공급되는 산소의 압력만으로 자동으로 작동되는 휴대용 인공호흡기의 필요성이 증가하고 있다. 이러한 휴대용 인공호흡기의 구성품 및 인공호흡기에 중요시되는 성능요구사항에 따른 시험방법을 제시하고 그에 따른 인공호흡기 내부감압셋팅 변화조건에 따른 최적 성능요구조건에 적합한 내부 감압조건을 찾고자 한다.

  • 최근 심장 혈관 질환 등으로 인한 심정지 발생이 증대됨에 따라, 응급 시 수동 작동 장치나 전기적 구동 장치 없이, 공급 산소의 압력만으로 자동으로 작동되는 휴대용 인공호흡기가 전 세계적으로 보급되고 있다. 그러나 이러한 인공호흡기의 구체적인 압력강하 특성에 대해서는 잘 알려진 바가 없어 본 논문에서는 호흡저항과 관련하여 핵심부품인 유량조절디스크의 다양한 홀 크기에 대한 압력강하 특성을 CFD 시뮬레이션 해석을 통하여 규명하고 압력강하를 최소화할 수 있는 홀 형상의 최적설계 안을 제시하였다.
