In a patient with single ventricle associated with complex systemic and/or pulmonary venous drainage, intraatrial Fontan procedure is sometimes technically difficult due to the complex spatial relationship between their orifices in the atrium. We report a case of the modified extracardiac conduit Fontan procedure in a patient with a single ventricle in which the inferior vena cava and the hepatic vein drained separately into the atrium and the intraatrial orifice of the hepatic vein was abut to the orifice of the left lower pulmonary vein.

Virtual testing devices are required due to rapid changes in the health care industry and the increase of the medical or nursing workforce. The importance of devices such as the simulator, blood vessels, and lab equipment for modeling blood flow to the heart is increasing too. In this study, we made heart pump by using a step motor and developed device which simulates arterial, venous blood pressure, and blood flow. We finally evaluated the function of proposed device. The proposed system is composed of the pump for simulating, the valve device to describe the resistance of the artery and vein, and a reducing device showing the characteristics of the venous system. We used BOXER pump for heart simulator and silicon tube for arterial and venous vessels, and designed a reducing device. We also used the pressure sensor to measure arterial blood pressure. For the evaluation of the proposed system, we selected a range of 50~100mmHg of the blood circuit 60 per minute and then compared the blood pressure of a person and the measured blood pressure.

배경: 하지 정맥률의 치료법중의 하나인 압박경화요법의 치료효과를 알아보고자 하였다. 대상 및 방법: 영동세브란스병원에서 1997년 8월부터 1999년 8월까지 복대-대퇴정맥판막 부전이 없는 하지정맥류 환자 중 압박경화용법을 시행 받은 94명을 대상으로 하였다. 병역 및 이화학적 검사결과, 압박경화용법의 치료효과와 합병증 등을 살펴보았다. 결과: 환자의 연령은 20대에서부터 70대까지 있었으며, 평균 43.4세였고 성별은 남자가 28명 여자가 66명 이었다, 정맥의 돌출 이외에 다른 증상은 없는 환자가 87명이었으며, 다리가 무거운 증사, 피로, 통증도 각각 1예, 2예, 2예가 있었다. 대상 환자 중에서는 소복재정맥에 이환된 사람이 83예이었다. 50명환자에서 1회의 시술만으로 만족할 만한 결과를 보였으며, 시술 후의 합병증으로 부분적인 열감, 정맥염, 괴사가 있었다. 결론: 복재-대퇴정맥판막 부전이 없는 하지정맥류의 치료에 있어 경화요법은 미용적으로 우수한 효과적인 치료법으로 생각된다.

Considering the similarity between fluid vortex and arrhythmogenic reentrant waves in heart, we applied the non-dimensionalization method in fluid dynamics to arrhythmia analysis and discovered a new non-dimensional simulation results, there was a threshold value of the number that resulted in the induction of a reentrant wave.

Intracardiac hemangiomas are very rare primary cardiac tumor and there have been at least 37 reports of surgically resected cardiac hemangiomas. Most cardiac hemangiomas are asymptomatic. In symptomatic patients, symptoms are related to the location of tumor and outflow tract obstruction or obstruction of inferior and/or superior vena cava. Sudden death may occur due to conduction disturbances. The principle of treatment is surgical resection, and the prognosis is dependent upon the size, location and multiplicity of the tumor. A 40 year old man was admitted due to chest contusion and was found to have an intracardiac mass during echocardiographic examination. The mass was successfully removed and pathologic examination showed benign hemangioma. The patient was recovered uneventfully in postoperative period and was followed up for 1 year without evidence of recurrence.
