Magnetocardiogram difference between pharmacologic and exercise stress test (운동과 약물에 의한 심장 부하의 심자도 양상 비교)

  • 심장의 박동수 자율신경계에 있어서 매우 중요한 감성 지표가 되어 왔다. 정상인 심장박동수의 개인간 차이는 유의하게 다르지 않다. 그러나 심장병 환자는 건강한 사람들과 유의한 차이를 보이게 된다. 특히 허혈 초기에는 증상이 잘 나타나지 않기 때문에 일부러 심장에 부하를 주어서 허혈 유무를 판정하기도 한다. 부하를 주는 방법은 운동에 의한 운동부하검사, 약물에 의한 약물부하검사로 나뉜다. 신체조건 및 안정상 운동부하 검사가 불가능한 경우에는 약물을 이용한다. 심전도를 이용한 심장부하 연구는 많이 수행되어 왔으나 심자도 연구는 초기단계이다. 본 연구에서는 20 대 정상인 20 명과 협심증 환자들 27명에게 약물 부하를 유도하여 나타난 심박수, 심자도의 차이를 비교하였다. 또한 8 명의 정상인을 대상으로 가혹한 운동부하를 일으켜 최대 부하상태에선 안정 상태까지 이르는 과정을 심자도로 관찰하였다. 결과로서 운동부하 검사에서 야기되는 스트레스의 정도는 약물로 얻어지는 부하에 비해 유의하게 큼을 알 수 있었다. 또한 약물 스트레스는 정상인의 심장 전기생리학적 변화는 미미하지만 일부 허혈 환자들에게서는 유의한 차이를 보였다. 미약한 약물 스트레스에 대해 변화되는 정도가 심자도에 관찰되어 허혈 검사에 사용 가능성을 보였다.

Effects of Cardiac Rehabilitation in Elderly Patients After Myocardial Infarction (노인 심근경색 환자에서의 심장재활의 효과)

  • To investigate the effects of cardiac rehabilitation (CR) on cardiopulmonary exercise capacity in both elderly and younger patients with myocardial infarction (MI). Of the MI patients who received hospital-based CR between February 2012 and December 2015, we retrospectively reviewed the medical records of patients who continued a follow-up through the outpatient clinic. A total of 46 patients (18 elderly patients (${\geq}60years$) and 28 younger patient (< 60)) were included in this study. The subjects visited the clinic to perform aerobic exercises with ECG monitoring based on their initial exercise tolerance test outcome. The exercise capacity was measured by symptom-limited exercise tests before and after hospital-based CR. Before CR, the elderly group had a significantly lower exercise capacity in peak VO2, METs, anaerobic threshold, exercise time, respiratory exchange ratio (RER) than the younger group. After CR, elderly groups showed a significantly improved exercise capacity in exercise time, HRmax, HRrest, peak VO2, METs, anaerobic threshold, and RER. Both elderly and younger groups showed similar improvement of cardiopulmonary exercise capacity after hospital-based CR. There is a very low cardiac rehabilitation participation and referral rates with MI patients in Korea. It is considered necessary to further expand the participation of cardiac rehabilitation in elderly MI patients.

The Effects of High-Intensity Aerobic Interval Training In The Elderly With Myocardial Infarction (노인 심근경색 환자에서 고강도 간헐적 유산소운동의 효과)

  • To investigate the effect of high-intensity interval training (HIIT) on the outcomes of cardiac rehabilitation(CR) in elderly and younger patients with myocardial infarction(MI). Of the MI patients who received hospital-based CR from 2015 to 2017, we retrospectively reviewed the medical records of the patients who continued follow-up through the outpatient clinic. A total of 39 patients (20 elderly patients (${\geq}60year$($64.1{\pm}3.6$)) and 19 younger patients (< 60 year($55.6{\pm}2.3$)) was included in this study. The HIIT program was composed of 10 minutes of warm-up, 30 minutes of interval aerobic exercise, and 10 minutes of cool down. The aerobic exercise based on their initial exercise tolerance test outcome consisted of 3-minute usual intensity phase (60-70% of heart rate/VO2 reserve) and 4-minute of high-intensity phase (70-90% of heart rate/VO2 reserve). Exercise capacity was measured by exercise tests before and after hospital- based CR. Before CR, elderly group had a significantly lower exercise capacity in Exercise time, peak VO2, METs, Anaerobic threshold, VEmax, 02pulse than younger group. Both group showed similar improvement of cardiopulmonary exercise capacity after CR. And HIIT is safe and effective in elderly patients. It is considered necessary to further expansion for the participation of high intensity interval training in elderly patients.

  • The estimation of the work of heart can be treated as one of the most important parameters for determining the amount of circulating blood needed for harmonious metabolism in the human body. By monitoring the work of heart, one can detect increased work load of heart and start the treatment at the early stage of CHF. Thus it is necessary to estimate the work of heart. The contractility of the left ventricle, the second important parameter for representing the motion of heart, can be estimated through information on the work of heart. In this study, the modified Windkessel model, which has been used for a measure of vascular hemodynamic impedance parameters, was adapted to estimate the work of heart.

  • This study was aimed to predicting the exercise load in mountain climbing related physical condition of trail, and to propose a capability of user management. It appeared that exercise load in mountain climbing was mainly influenced by the slope of trail and weight of load. It thought that the trails of steep area could be controled the use of person who have a low athletic power as a propose the exercise load index by age.

Effect of Rope-skipping Exercise on the Enhancement of Cardiopulmonary Function (줄넘기 운동 훈련이 심폐기능 항진에 미치는 효과)

  • In an attempt to observe the effects obtained by the regular physical training, nine soldiers performed regularly the rope-skipping for nine weeks. All subjects were healthy and did not experience any special military training Programs. During the course of the training, their cardiopulmonary functions were measured in the resting and the Post-exercise recovery periods, and the values were compared with ones of the pre-trained. The test exercises loaded to the subjects were rope-skipping and step-rising & falling. The results obtained were as follows: 1) By the training, heart rates decreased very significantly in the resting and post-exercise recovery periods. And the effects began to bring out at the early stage, about the 7th day. 2) As the duration of the training increased, the systolic blood pressures decreased meaningfully in the resting and recovery periods. 3) Only in the early recovery phase after the exercise of the rope·skipping, the respiration rates decreased significantly by the training. 4) The lighter the intensity of the test exercise loaded was, the more prominent the effect of the physical training on the cardiopulmonary functions was. The above results suggest that the 9 week training of the rope-skipping would bring about the enhancement of the cardiopulmonary functions.

Lung Uptake of $^{99m}Tc-sestamibi$ during Routine Gated Exercise SPECT Imaging : Comparison with Left Ventricular Ejection Fraction and Severity of Perfusion Defect (일상적인 운동 부하 게이트 심근 관류 SPECT에서 $^{99m}Tc-sestamibi$ 폐섭취 : 좌심실 구혈률과 관류 결손 정도와의 비교)

  • Background: Lung-to-heart uptake ratio (LHR) in $^{201}Tl-chloride$ myocardial perfusion scan is believed to be a reliable marker for left ventricular (LV) dysfunction, but the clinical value of LHR is controversial for $^{99m}Tc-MIBI$ imaging. Furthermore, most of results suggesting lung uptake of $^{99m}Tc-MIBI$ as a potential marker for LV dysfunction used immediate post-stress images, instead of routine images acquired 1 hour after tracer injection. The goal of our study was to investigate whether LHR evaluated with routine gated $^{99m}Tc-MIBI$ imaging can reflect the degree of perfusion defect or left ventricular performance. Subjects and Methods: 241 patients underwent exercise $^{99m}Tc-MIBI$ myocardial SPECT were classified into normal myocardial perfusion (NP, n=135) and abnormal myocardial perfusion (AP, n=106) group according to the presence of perfusion defect. LHR was calculated from anterior projection image taken at 1-hour after injection. Two legions of interest (ROIs) were placed on left lung above LV and on myocardium showing the highest radioactivity. Subjects were classified by left ventricular ejection fraction (LVEF), as Gr-I: >50%, Gr-II: 36-50%, Gr-III: <36% and by summed stress score (SSS), as Gr-A: <4, Gr-B: 4-8, Gr-C: 9-13, Gr-D: >13, LHR was compared among these groups. Results: In NP group(n=135), LHR, were higher in men than women ($men:\;0.311{\pm}0.03,\;women:\;0.296{\pm}0.03,\;p<0.05$). Significant difference, in LHR were found between NP and AP groups both for men and women ($men:\;0.311{\pm}0.03\;vs\;.\;0.331{\pm}0.06,\;women:\;0.296{\pm}0.03\;vs.\;0.321{\pm}0.07.\;p<0.05$). There were weak negative correlation between LHR and LVEF (r=-0.342, p<0.05) and weak positive correlation between LHR and SSS (r=0.478, p<0.05) in men, but not in women (LVEF: r=-0.279, p=0.100, SSS: r=0.276, p=0.103). Increased LHR was defined when for more than mean + 2SD value ($men{\geq}0.38,\;women{\geq}0.37$) of the LHR of the subject with normal perfusion. Increased LHR were observed more frequently in subjects with lower LVEF (Gr-I: 11.1%, Gr-II: 27.0%, Gr-III: 35.4%, p<0.05) and higher SSS(Gr-A: 14.0%, Gr-B: 5.7%, Gr-C: 18.2%, Gr-D: 40.7%, p<0.05). Conclusions: LHRs obtained from routine $^{99m}Tc-MIBI$ gated SPECT images were weakly correlated with LVEF and perfusion defect. Although significant overlaps were observed between normal and abnormal perfusion group, LHRs could be used as an indirect marker of severe perfusion defect or reduced left ventricular function.

Prognostic Value of Normal Exercise $^{99m}Tc$-MIBI Myocardial Perfusion SPECT: Comparison with Exercise Electrocardiography and Coronary Angiography ($^{99m}Tc$-MIBI 운동부하 심근관류 SPECT에서 정상소견을 보인 환자의 예후: 운동부하 심전도와 관동맥 조영소견과의 비교)

  • Purpose: Myocardial perfusion scintigraphy is a useful technique to diagnose and to predict prognosis in patients with suspected or known coronary artery disease. The purpose of the present study is to evaluate the prognostic value of normal exercise $^{99m}Tc$-MIBI myocardial perfusion single photon emission computed tomography (SPECT) and to analyze results with regard to those of exercise electrocardiography or coronary angiography. Materials and Methods: We evaluated 301 patients (mean age $52{\pm}10$ years, 166 males and 135 females) with normal exercise $^{99m}Tc$-MIBI myocardial perfusion SPECT performed for suspected coronary artery disease. Subject were evaluated for cardiac events and followed for 8-55 months (mean $19{\pm}10$ months) after imaging. Results: During the follow-up period, there was no cardiac death but only one non-fatal myocardial infarction (event rate 0.21% per year). In addition, only one patient underwent coronary revascularization. There was no significant difference in cardiac event rate between patients with positive (n=27) and negative (n=235) exercise electrocardiography (p:NS). There was no cardiac event in 17 patients who underwent coronary angiography (4 patients with >50% luminal narrowing, 2 patients with vasospasm and 11 patients with no significant lesion). Conclusion: Patients with normal exercise $^{99m}Tc$-MIBI myocardial perfusion SPECT has a very low risk for cardiac events regardless of exercise electrocardiographic and coronary angiographic findings.

  • 심박변이도는 자율신경계의 균형 상태를 나타내는 중요한 지표이다. 동적인 환경내에서 항상성을 유지할려는 인체가 각 신경계를 조율하는 수치로 심박의 변화를 시간영역과 주파수영역으로 나누어 그 값을 측정하여 분석하였다. 그 중 심전도의 측정방식에서 간략히 하고자 광학적 방법으로 심장의 박동을 측정하는 PPG 변화량이 심박변이도와 유사하다는 연구가 진행되어 왔다. 하지만, 심전도와 PPG 는 혈압의 영향을 받는다. 본 연구에서 운동부하 실험을 주어 혈압을 상승 시킨뒤, 심전도를 통해 산출한 심박변이도와 PPG를 통해 산출한 맥박변이도를 비교하여 두 상관성에 대하여 연구하였다.

The Effects of the Walking Exercise on ST/HR Slope and QRS Vector in the Middle-Aged Men (운동부하 심전도를 이용한 중년 남성들의 걷기 운동이 ST/HR 경사 및 QRS 벡터에 미치는 영향)

  • The purpose of this study was to investigate the changes of long term ECG response in a company with middle-aged male employees. Subjects were 60 men who were 40~55 years old. We enrolled 30 exercise group subjects into a 3-year exercise program. In measurement index, body composition was measured by % body fat and BMI. Exercise stress test analyses were measured using ST/HR slope and QRS vector. Statistical analysis was performed using analysis of repeated ANOVA. Results of this study were as follows: In ST/HR slope, the control group showed symptoms of ischemia after nine minutes of exercise. In the rest frontal axis of the QRS vector, the control group had a tendency towards right axis deviation. In the rest horizontal amplitude of the QRS vector, the control group had a tendency to show a significant decrease, but it was increased significantly in the exercise group. These findings suggest that inactive company workers was showed a decrease of exercise capacity, early diagnosis exercise-induced ST depression, and prolonged deviation of QRS vector, but that cardiac function could be elevated in active middle aged men through regular exercise program participation.
