Autogenous Transplantation of Third Molar to Replace Hopeless Tooth (예후 불량한 치아를 대체하는 제3대구치를 이용한 자가치아이식술)

  • Autotransplantation is the surgical repositioning of an autogenous erupted or unerupted tooth from one site to another in the same individual. This treatment is indicated in traumatic tooth loss, teeth with severe caries, congenitally missing teeth, teeth with bad prognosis and in case of developmental anomalies of teeth. The following 2 cases describe the potential to utilize autotransplantation to replace hopeless teeth with sound wisdom teeth.

  • Forced orthodontic eruption(FOE) is a non-surgical treatment approach that allows augmenting both soft- and hard-tissue profiles of potential implant sites, by forced orthodontic extrusion of "hopeless" teeth and their periodontal apparatus. By stretching the gingival and periodontal ligament fibers during extrusion, tension is imparted to the entire alveolar socket, stimulating osseous apposition at the alveolar crest. FOE increases the width of the attached gingiva, and the mucogingival junction remains stable when the gingival margin migrates coronally. Based on these effects, FOE of non-restorable teeth prior to implant placement is a viable alternative to conventional surgical augmentative procedures in implant site development. The aim of this case report is to describes coronal soft-tissue augmentation around fractured teeth, which was achieved by FOE before implant placement.

- Aesthetic Restoration of the Gingival Recession with All-ceramic Partial Crown - (전치부 ALL-CERAMIC PARTIAL CROWN 의 임상)

  • The black triangle is a common clinical finding in aged people with gingival recessions. Among other prosthetic treatment molalities such as composite resin filling and laminate venners, the all ceramic restoration procedure can lead to most successful result. With improved bonding strength and ceramic properties, the tooth preparation design for all-ceramic crown can be modified to minimize the reduction of sound tooth structure without loosing properties of conventional preparation design. Case selection is an important factor in acheiving succesful prosthesis. In this case report, the leucite reinforced pressable ceramic, $Authentic^{TM}$ [Ceramay, Germany] was used to fabricate the prosthesis.

  • In everyday dental practice, one of the most important procedures is fixed prosthodontics which includes gold and ceramic restorative treatments. This procedure can be divided into tooth preparation, impression taking, laboratory work, occlusal adjustment and cementation. The first step is tooth preparation and it needs not only good techniques but also deep knowledge and understanding of oral biology. Also, there must be good knowledge of the principles and materials of the procedure. The patient's satisfaction can be achieved from natural contour, good shade, and precise margin fit, especially in ceramic restorations on anterior regions. It is essential to fastidiously prepare the tooth to make aesthetic restorations with a good margin fit. Tooth preparation techniques and three case reports of ceramic restorations on the anterior region are presented and discussed in this paper.

  • The Schwachman-Diamond syndrome is an autosomal recessive syndrome(1/20,000 births), consisting of pancreatic insufficiency, neutopenia, which may be intermittent, neutrophil chemotaxis defects, metaphyseal dysostosis, failure to thrive and short stature. Patients present in infancy with poor growth and grease, foul-smelling stools that are characteristic of malabsorption. These children can be readily differentiated from those with cystic fibrosis by their normal sweat chloride levels, lack of the cystic fibrosis gene, and characteristic metaphyseal lesions. Pathologically, the pancreatic acini are replaced by fat with little fibrosis. The neutropenia may be cyclic. Recurrent pyogenic infections otitis media, pneumonia, dermatitis(fig 1), sepsis are common and a frequent cause of death. In dental examination, these patients had a poor oral hygine and moderate generalized marginal gingivitis, also show delayed primary tooth exfoliation and oral development. This report illustrates a case that pancreatic agenesis 6 yeas-old boy with various esthetic dental problems has been served the esthetic dental restoration of 6 years.

A MORPHOLOGIC STUDY ON MOLAR ABUTMENT DIES (금관 가공의치에서의 대구치 지대치에 관한 형태학적 연구)

  • Abutment dies which resemble the actual size and shape of crown abutment is essential in most of the research area of fixed prosthodontics like marginal accuracy, crown seating, behavior of luting agent and so on. Seeing the large portion of research is done with round shaped dies in different size and cone angles, the necessity of research on the crown abutment is self-evident. 500 molar abutments were collected randomly through the commercial dental laboratoy, regrdless of their position in the dental arch, sex, and age. The measurements of 22 points of a die were done, and the results were as fogbows : 1. The height of the molar dies was $3.9{\pm}1.2mm$ 2. The bucco-lingual width was $8.9{\pm}1.2mm$ at the base, and $7.4{\pm}1.2mm$ at the occlusal. 3. The desio-sistal width was $8.2{\pm}1.2mm$ at the base, and $7.0{\pm}1.3mm$ at the occlusal.

Considerations in Porcelain Fused Metal Restorations (Porcelain 제작시 고려 사항)

  • Although various kinds of porcelain restorations, including full porcelain jacked crowns and laminate crowns, are used for Esthetic Dentistry nowadays, the most widely used one so far is porcelain metal restoration. It goes without saying that shade matching in porcelain-metal restoration is much more difficult than in full ceramic veneer restoration because of the metal substructure limitation of the porcelain to metal restoration and then let it go even when shade matching is a little bit unsatisfactory. I think a more satisfactory shade matching can be achieved by developing more elaborate techniques, by selecting proper metal and porcelain and by enabling more detailed communication between the dentists and the dental technicians. Some considerations will be made in an attempt to achieve more successful outputs.

  • The use of fluoride and other preventive methods caused a dramatic decline in dental caries and the need for restorative dentistry. In our modern society that places a high value on appearance, emphasis is now shifting towards esthetic dentistry which has experienced a high popularity with the development of bonding techniques and materials including porcelain and composite. To satisfy patients wanting whiter teeth and a better looking smile, there are many treatment options ranging from full crown coverage to conservative procedures such as bleaching. Whether bleaching should be combined with other esthetic procedures or used alone depends on the type of discoloration and the patient's willingness to accept the various treatment options. Therefore, in order to obtain a successful esthetic outcome, careful diagnosis and treatment planning followed by consultation with the patient is of utmost importance.

Hybrid Prosthesis Supported by Osseointegrated Implants in Maxilla (골유착성 임프란트에 의해 지지되는 새로운 설계의 Hybrid Prosthesis)

  • A fixed bridge is preferred as a prosthetic option supported by oral implants. However, it is very difficult to re tore edentulous maxilla with fixed prosthesis in cases with improper position and angulation of fixtures, abnormal jaw relation, and need for proper lip support. Six Br${\aa}$nemark implants were installed in edentulous maxilla opposing mandible with natural dentition. A removable hybrid prosthesis attached to a bar milled by 6 degrees was therefore designed to overcome such disadvantages of fixed prosthesis. Lateral stabilization of removable prosthesis was obtained by framework closely fitting the milled bar. Support for the prosthesis was ensured by three elevated areas on the bar. Two precision attachments(CEKA REVAX) provided appropriate direct and indirect retention without influencing support. A clinical and laboratory procedure was presented.

  • By the concerns of esthetic restoration were increased recently. many all ceramic crowns were developed. But they except In-Ceram Alumina were used only single crown. In-Ceram Alumina, developed by Dr. Sadon, was revealed to have high flexural Strength(450MPa). So it could be used not only anterior bridges but also posterior bridges. But In-Ceram Alumina was seen to be opaque, a little green color in transillumination light by high content of alumina oxide(85%). So new all ceramics with high strength and high translucence were needed. Spinell($MgAl_2O_3$) have a high melting point, high flexural strength, low heat conductivity, high light conductivity. In-Ceram Spinell offers glasslike light transmission by using the spinell cores instead of the alumina cores. And they have a high translucency like to natural tooth, an excellent margin integrity and a high strength(350MPa). The purposes of this study are 1) to know about the construction method of In-Ceram Spinell System, 2) to investigate the its clinical possibiliy through patients and literature reviews.
