  • 본 연구에서는 손상된 심근의 재생을 위하여 골수단핵세포를 피브린 고분자와 함께 SD 래트에 이식하였고 8주 후에 신생혈관의 형성과 더불어 심장의 기능이 향상되었음을 확인할 수 있었다. 이 연구는 세포이식을 통한 조직 재생시 세포 이식용 매트릭스의 중요성을 보여준다. 환자 자신의 골수세포를 사용한다면 면역 문제가 없어서, 이 방법이 심부전 환자의 치료에 쉽게 이용될 수 있을 것이다.

산화적 손상에 의해 유발된 심근세포 독성에 대한 도홍사물탕의 방어효과 (Protective Effects of Dohongsamul-tang on Zinc-mediated Cytotoxicity in H9c2 Cardiomyoblast Cells)

  • The water extract of Dohongsamul-tang(DHSMT)has been traditionally used for treatment of ischemic heart in oriental medicine. However, little is known about the mechanism by which the water extract of DHSMT rescues cells from these damages. Therefore, this study was designed to evaluate the protective effects of DHSMT on zinc-mediated cytotoxicity in H9c2 cardiomyoblast cells. This study demonstrates that treatment of H9c2 cells with zinc caused a decrease in cell viability in a dose dependent manner and a chromatin condensation. Zinc induced the cleavage of poly(ADP-ribose) polymerase (PARP). In addition, zinc induced the decrease of Bcl-2, as well as increase of Bak expression and mitochondrial dysfunction. Zinc-induced H9c2 cell death was remarkably prevented by the pretreatment of DHSMT with consistent suppression of the cleavage of poly(ADP-ribose) polymerase (PARP), mitochondrial dysfunction and the expression of Bak and Bcl-2. Taken together, the results suggest that zinc induced severe cell death in H9c2 cardiomyoblast cells via intracellular GSH(reduced glutathione) depletion and the protective effects of DHSMT against oxidative injuries may be achieved through modulation of mitochondrial dysfunction and scavenging of ROS(reactive oxygen species).

  • 특발성 과호산구 증후군은 호산구 매개에 의한 조직 손상으로 인해 다발성 장기부전, 특히 심장을 침범하는 드문 전신성, 백혈구증식성 질환이다. 심장침범은 특발성 과호산구 증후군 환자의 75% 이상에서 일어난다. 심장증상은 심내막 하 섬유증, 제한성 심근병증, 판막부전, 그리고 말초동맥 혈전증을 유발하는 혈전이다. 이 질환은 남자에서 여자보다 9 : 1로 호발하고, $20{\sim}50$대에서 주로 발현하는 경향이 있으며 소아에서는 매우 드물다. 인공 대동맥판막부전을 나타낸 특발성 과호산구 증후군 환자(58세, 남자)를 인공판막 재치환 수술 후 부신피질호르몬제와 hydroxyurea 투여로 성공적으로 치료하였기에 보고하는 바이다.

심근세포의 산화적 손상에 대한 오미자탕의 효과 및 작용기전 연구 (Effects & Mechanism of Omija-tang on Oxidative Stress-Induced Death of H9c2 Cardiomyoblast Cell)

  • Objectives: The water extract of Omija-tang (OMIT) has traditionally been used for treatment of ischemic heart and brain damage in oriental medicine. However, little is known about the mechanism by which the water extract of OMJT rescues cells from these damages. Therefore, this study was designed to investigate the protective mechanisms of OMJT on oxidative stress-induced toxicity in H9c2 cardiomyoblast cells. Methods: Treatments of $H_2O_2$, or $ZnC_{12}$ markedly induced death of H9c2 cardiomyoblast cells in a dose-dependent manner. The characteristics of oxidative stress-induced death of H9c2 showed apparent apoptotic features such as DNA fragmentation. OMJT significantly reduced both ${H_2O_2}-induced$ cell death and chromatin fragmentation. The decrease of B치-XL expression by $H_2O_2$ were inhibited by OMJT. In addition, the increase of Bcl-XS expression was also inhibited by OMJT. In particular, Fas expression, which is generally recognized as cell death-inducing signal by Fas/FasL interaction, was markedly increased by H2O2 in a time-dependent manner. Also, the expression profile of proteins in Chang cells were screened by using two-dimensional (2-D) gel electrophoresis. Among 300 spots resolved in 2-D gels; the comparison of control versus apoptotis cells revealed that signal intensity of 6 spots decreased and 11 spots increased. Results and Conclusions: Taken together, this study suggests that the protective effects of the water extract of OMJT against oxidative damages may be mediated by the modulation of Bcl-XL/S Fas expression.

식이중의 Vitamin A와 E가 Adriamycin을 투여한 흰쥐의 심근손상의 원인인 과산화작용에 미치는 영향 (Effect of Dietary Vitamin A and E Peroxidative Deterioration of Heart in Adriamycin-Treated Rats)

  • The present study was designed to evaluate the protective effects of dietary vitamin A or E. and of combination of vitamins A and E on peroxidative deterioration of heart in adriamycin-treated rats. Male Sprague-Dawley rats were assigned to 5 groups according to the dietary supplementation of vitamin A or E Except control rats a dose of 2mg ADR/kg of B. W was injected to these animals intraperitoneally on the same day every week. Adriamycin treatment significantly decreased the weight gain of experimental rats compared with that of control rats, But this decrement was not modified by dietary supplementation of vitamin A or E. Lipid peroxide values of plasma were elevated by ADR treatment. The combined use of ADR and dietary vitamin A or E significantly reduced these values, The interaction between vitamins A and E seemed to be present in the lipid peroxide value of plasma. Catalase and superoxide dismutase(SOD) activities in rat heart were decrased by ADR treatment but glutathione peroxidase(GSH-Px) activity was elevated. Dietary supplmen-tation of vitamin A or E enhanced the heart catalase and SOD activities. except only vitamin A-supplemented group. GSH-Px activity of rat heart tended to be decreased by dietary supple-mentation of vitamin A or E. With ADR treatment polyunsaturated fatty acids such as archido-nic acid(20:4) and docosahexaenoic acid(22:6) were decreased in rat heart. However dietary supplementation of vitamins A and E reduced this decrease. The retinol and tocopherol contents of rat plasma were decreased by ADR treatment. Dietary vitamin A or E influence vitamin A or E content of plasma. The interaction between dietary vitamins A and E was observed in vitamin A or E level of rat plasma.

산화적 손상으로 유발된 심근세포 고사에 대한 경옥고의 방어효과 (Protective Effects of Gyungokgo on Oxidative Stress-Induced Apoptosis of H9c2 Cardiomyoblast Cells)

  • Backgrounds & Objectives : The water extract of Gyungokgo (GOG) has traditionally been used for treatment of general weakness and hemoptysis in oriental medicine. However, little is known about the mechanism by which the water extract of GOG rescues cells from these damages. This study was designed to investigate the protective mechanisms of GOG on H2O2­induced cell death in H9c2 cardiomyoblasts. Methods : In this study, we used H9c2 cells. Cells were treated with oxidative stress in the absence and presence of 1000㎍/ml GOG for 12hrs. Cells were treated with various concentrations of GOG for 12hrs. Cell viability was measured by MTT assay. Oxidative stress, which markedly decreased the viability of H9c2 cells, was characterized by apparent apoptotic features such as chromatin condensation as well as fragmentation of genomic DNA and nuclei. Results : GOG significantly reduced H₂O₂-induced cell death and apoptotic characteristics. The cotreatment of GOG and H₂O₂ in H9c2 cells also induced the phosphorylation of ERKs in a time-dependent manner. Moreover, PD098059, a MEK1 (upstream activator of ERK) inhibitor attenuated the protective effect of GOG on H₂O₂-induced cytotoxicity in H9c2 cardiomyoblast cells. Conclusions : These results suggest that MEK/ERK pathways play important roles in the protective effects of GOG in H9c2 cells. Taken together, they suggest that the protective effects of the water extracts of GOG against oxidative damages may be mediated by the regulation of HO-1, Fas/FasL and Bcl-XS proteins.

  • Determination and differentiation of cells in the skeletal muscle lineage is positively regulated by cell-cell contact. differentiation proteins proposed to mediate this effect include both classical MyoD and MEF members : potential interactions between the promyogenic activities of these classes of protein, however, are unknown. We show here that MyoD and MEF, two promyogenic family members that determine to each other in a cis fashion, form ineraction with MyoD- and MEF. These proteins contain myosin heavy chains and are enriched at sites of cell-cell contact between myoblasts, Therefore, In differentiation of MyoD MEF from CST (Chungsimyeonjatang) interact dependently, suggesting that the interactions occur in a cis fashio : consistent with this conclusion, MyoD-mediated differentiation is required for myoblast to occur by CST. Inhibition in myoblasts of a MyoD by STP in its ability to associate with MEF interferes with differentiation as assessed by morphological and transcription level, suggesting that this interaction is functionally important in myogenesis. Also, some of the differentiation-mediated proteins that are required for myogenesis seem to be based on interdependent activities of promyogenic classical SMAD-subfamilly.

XO/HX에 의하여 손상된 심근세포에 대한 과루해백백주탕 추출물의 방어효과 (Protective Effects of Guaruhaebaekbaekju-tang Extract in XO/HX-treated Rat Myocardial Cells)

  • To certify the protective effect of herbal medicine on myocardial damage against oxygen free radical-induced myocardiotoxicity, cytotoxicity was measured using by MTT assay, LDH activity and thiobarbituric acid reactive substances(TBARS) assay in the presence of Guaruhaebaekbaekju-tang(GHBT) extracts or single constituents of this prescription, Myocardial toxicity was evaluated in neonatal rat myocardiocytes in cultures. In the present study, xanthine oxidase/hypoxanthine (XO/HX) resulted in a decrease in cell viability, an increase in LDH activity in culture medium and lipid peroxidation in cultured myocardial cells, In the effect of GHBT extract, it showed the prevention from the XO/HX-induced cardiotoxicity such as the decrease of LDH activity and lipid peroxidation. In the protective effect of Fructus Trichosanthis (FT) and Bulbus Allii Macrostemi (BAM), all the extracts were significantly effective in the protection of XO/HX-induced cardiotoxocity in cultured myocardial cells. From these results, they show that XO/HX is cardiotoxic in cultured myocardial cells derived from neonatal rats, and it suggests that GHBT, FT and BAM extracts are positively effective in the blocking XO/HX-induced cardiotoxicity.

개심술시 Intra-aortic balloon pump (IABP)의 임상적 적용 (Clinical Experience with IABP in Cardiac Surgery)

  • 1994년 5월부터 1995년 12월까지 성인에서 심장수술을 시행한 122례중 18례(14.8%)에서 IABP를 순환보조장치로 사용하였다. 그중 술전에 IABP(intra-aortic balloon pump)를 시행한 경우는 9례이고, 술 중에는 7례, 그리고 술후에는 2례에서 시행하였다. 그 이유는 술전에는 저심박출, PTCA(percutaneous transluminal coronary angioplasty)의 실패였고, 술중에는 체외순환기 이탈을 위해서이며, 술후에는 술후 발생한 부정 맥 때문이었다. 환자들의 평균 나이는 61.8$\pm$6.9세(39세에서 75세)였다. 술전 및 술중에 IABP를 시행한 환자의 수술사망율은 각각 33.3, 42.9%였으며 술후 IABP를 시행한 환자는 모두 생존하였다. IABP이탈율은 술전의 경우 66.77%. 술중의 경우 85.7%였으며 술후에 시행한 IABP는 모두 이탈 가능하였다. 결론적으로 IABP는 심근 손상이 가역적인 시기에 적용할 경우, 수술전후 어느시기에나 안전하게 사용가능하며 수술전후의 혈류역학적 불안정, 체외순환기 이탈 및 저심박출증의 치료에 도움을 줄 수 있다고 판단된다.

말티즈 견에서 발생한 우심실양분증 (Double-chambered Right Ventricle with Intact Ventricular Septum in a Maltese Dog)

  • 임상증상이 없는 6년령의 3.1 kg 말티즈견이 심잡음의 원인에 대한 추가 검진을 위해서 내원하였다. 흉부 방사선에서는 우심비대가 확인되었다. 심장 초음파에서는 두드러진 우심실비대, 비정상적 근육 다발과 섬유성 결절이 우심실 유출로 아래 누두부에서 발견되었다. 이러한 소견은 우심실양분증의 전형적인 형태학적 소견이다. 비정상 근육다발에서 주폐동맥으로 향하는 와류의 속도는 4.6 m/sec, 삼첨판 역류는 4.4 m/sec, 주폐동맥 혈류는 1.1 m/sec로 각각 측정되었다. 본 환자는 비록 임상증상은 없지만 협착에 의한 과도한 후부하와 그로 인한 심장의 손상을 막기 위해서 현재 atenolol (0.5 mg/kg) 을 복용하고 있다. 본 증례는 한국에서 발생한 견종에서 매우 드문 우심실양분증에 대해서 기술하였습니다.
