Objectives : The aim of this study is to review domestic clinical studies about dizziness in Korean medicine. (Silsin)" were searched on five database systems(DBpia, KISS, KMbase, NDSL, RISS) from September, 13th to September, 15th, 2016. 60 clinical studies were collected and classified by published journal, year, etiologic disease, study design, dizziness assessment tools of studies, study results, evaluation of disease pattern and process, interventions(herbal medicine and acupuncture treatment points), and number of studies which chose that prescription. Results & Conclusions : Dizziness was researched constantly since 1998 and 7 clinical stuides were issued 2007, the most. The journal which the most studies were presented was Journal of korean oriental internal medicine(12 studies over 60, 20%). Original article were 12(20.0%) and case reports were 48(80.0%) among 60 studies. BPPV was reported as a etiologic disease on 9 studies(20.45%) among 44 studies. VAS was used mainly as dizziness assessment tools of studies(26 studies over 38, 68.42%). 9 studies(15.0%) got evaluation of disease pattern and process among 60. Banhabaekchulcheonma-tang was reported as a herbal medicine prescription on 9 over 44 studies(20.45%) and ST36 was chosen acupuncture point most(24 over 45 studies, 53.33%). It doesn't have enough representativenes but has meaning to apprehend present tendency to treat dizziness in Korean medicine clinical studies.
Background: Left ventricular rupture after acute myocardial infarction is a serious complication with high mortality. Emergency operation is usually the only available treatment. A 76-year-old female with persistent chest pain and syncopal attacks was admitted. Transthoracic echocardiography showed the pericardial effusion and generalized hypokinesia of the inferolateral wall of left ventricle. Coronary angiography revealed a total occlusion of the first diagonal branch. After percutaneous transluminal coronary angioplasty with coronary stent and insertion of intraaortic balloon pump, emergency operation was performed. Under cardiopulmonary bypass and cardiac arrest with cold blood cardioplegia, coronary artery bypass graft with saphenous vein, pericardial patch covering on the rupture area with 6-0 polypropylene running sutures, and fibrin glue compression under the patch were performed. We present a case of left ventricular (free wall) rupture after acute myocardial infarction.
A 3 years-old female mixed dog (weighing 5.3 kg) was referred to veterinary teaching hospital of Kangwon National University with primary complaints of syncope, severe exercise intolerance, depression and lethargy. Diagnostic studies revealed polycythemia, right sided cardiac enlargement on thoracic radiography and right-to left atrial septal defect, severe pulmonary stenosis (~5 m/s of peak velocity) and right ventricular hypertrophy. Based on diagnostic findings, the dog was diagnosed as trilogy of Fallot. To improve clinical condition of this dog, diltiazem and enalapril were prescribed with weekly phlebotomy. To author's best knowledge, this is the first case of trilogy of Fallot in Korea.
This paper describes an informatlon compression of electrically evoked myoelectric signal, M-wave. This wave shows a direct response m lato-response of nerve conductlQn study and has a characteristic with finite time support. M-wave is a useful factor for investing neurodi~ease and is often desirable to have a compact description of its shape and time evolution. The aim of this paper is to show that the AR modeling IS a effective method for compressing an information of M-wave. First, AR model parameters of real M-wave are estimated. And then. they are verified by approximatmg a M-wave using estimated AR parameters and by comparing to other melhod, Hermite tlansform[4]. To concretely evaluate the proposed method, the NMSE(normalized mean square error) of approximation curves are compared. As a result, AR modeling is effective for M-wave assessment because of its capability for the information compression.
Electronic commerce creates serious problems for tax administration because of difficulties associated with the identification of traders, coding of trade activities and so on. The core issue with respect to resolving tax-related problems in electronic commerce is first, the identification of individual transactions through internet and their contents as well as traders. Secondly, it is the ability of tax authorities to secure effectively such data and information as identifying taxpayers, taxable amount and tax evasion on time. The tax authorities are studying the way to resolve tax evasion associated with electronic commerce by using payment system. Above of all, it is imperative to establish a systematic international cooperation in collecting value added taxes for electronic commerce. In order to enhance the effectiveness of tax system, the authorities of different nations should make joint efforts to collect taxes by exchanging such information as identification and registration of business as well as details of transactions among nations of production and consumption.

Arrhythmia is a condition in which the heart beats abnormally or irregularly, early detection is very important because it can cause dangerous situations such as fainting or sudden cardiac death. However, performance degradation occurs due to personalized differences in ECG signals. In this paper, we propose arrhythmia classification using hybrid combination model of CNN-LSTM. For this purpose, the R wave is detected from noise removed signal and a single bit segment was extracted. It consisted of eight convolutional layers to extract the features of the arrhythmia in detail, used them as the input of the LSTM. The weights were learned through deep learning and the model was evaluated by the verification data. The performance was compared in terms of the accuracy, precision, recall, F1 score through MIT-BIH arrhythmia database. The achieved scores indicate 92.3%, 90.98%, 92.20%, 90.72% in terms of the accuracy, precision, recall, F1 score, respectively.
Purpose : We'd like to determine the incidence of congenital heart disease and arrhythmia in elementary school children in Busan, and to provide adequate prevention and treatment. Methods : A total of 23,802(male 12,909, female 10,893) 1st grade elementary school children living in Busan were studied. All children were 7-8 years old. We obtained their medical history by questionnaire and checked elecrocardiography(ECG). Subsequent screening tests including a 2nd ECG, chest X-ray, phonocardiogram and CBC for the students who had abnormal findings at the first screening test. The third screening test was done for students who had cardiac murmurs or abnormal ECG findings in the second screening test by echocardiogram, treadmill test and 24-hour Holter monitoring. Results : Among 23,802 children participants, 605(2.54%) had abnormal ECG findings at the first screening test. Q wave abnormality(0.58%) was observed most frequently, and complete right bundle branch block(RBBB)(0.26%), sinus tachycardia(0.24%), right axis deviation(0.22%) and ventricular premature contraction(VPC)(0.21%) followed in order. Four hundred and twenty four children participated in the second ECG screening test. Two hundred and two children(47.6%) had an abnormality such as sinus tachycardia(18.8%), VPC(17.8%), or complete RBBB(17.3%). After completing the third examination including echocardiogram, we couldn't find the students with ventricular tachycardia (VT) or SVT at the third arrhythmia screening test. Conclusion : A high incidence of arrhythmia was found in the 1st grade elementary school children in Busan despite their healthy appearances, although fatal heart diseases were not noted. Early diagnosis, adequate preventative measures and treatment will prevent and decrease the frequency of emergent situations like syncope and sudden death.
Platelet products are used to treat hemorrhagic or platelet dysfunction diseases. Plateletpheresis involves collecting the platelet components of blood using an apheresis blood-collection system. Various indicators are available for evaluating the qualities of the apheresis platelets. The productivity of platelet collection is evaluated through both the collection efficiency and collection rates. Platelet storage quality can be evaluated in vitro using several indicators, including visual appearance, metabolic activities, volume, platelet count, white blood cell count, microparticles, and various platelet activation markers. Platelet activation markers have been used as indicators of storage quality in various studies. Post-transfusion platelet quality can be evaluated based on the corrected count increment and the percentage of platelet recovery. Although various studies have investigated the aspects of plateletpheresis, no article has systemically presented assessments of the platelet products obtained from different plateletpheresis devices. The present study provides a review of plateletpheresis, including the specifics of the process, the types of devices employed, the platelet quality, the overall efficacy, and the evaluation indicator qualities. Furthermore, the differences in functionality among the different apheresis devices are discussed. Although adverse reactions to the citrate anti-coagulant have been reported, apheresis processing may provide a safer option for donors who are at a high risk for presyncopal or syncopal reactions related to whole blood collection.
Posterior canal benign paroxysmal positional vertigo (PCBPPV) is the most common disease among patients who visited the st. Carollo hospital dizziness center with the head suddenly feeling rolling or rotational dizziness caused by head and body movements. Most patients showed improvements in symptoms after a single treatment. On the other hand, there are more causes requiring multiple treatments. This study examined the factors affecting the canalithiasis repositioning maneuver for symptom recovery of dizziness, which is posterior BPPV. Dizziness patients who visited the dizziness center of PCBPPV were classified. From March 2008 to November 2010, the cure rate of 165 posterior BPPVs patients was investigated. The success rate after the first, second, third, and further treatments was 57.6%, 17.6%, 10.3%, and 14.5%, respectively. The factors affecting treatment of PCBPPVs included traumatic, medical disease, cerebral infarction, small vessel disease, vestbro basilar artery insufficiency (VBI), cerebrovascular disease, brain disease, and vestibuloplasty (P<0.05). Vomiting, nausea, and syncope can be treated easily by considering the factors affecting these treatments. If the related diseases are combined with the treatment of the canalithiasis repositioning maneuver for symptom recovery of dizziness, the results will show an improvement in daily life that is free from painful dizziness.
