Effect of Multifaceted Intervention Program on Multi-Dimensional Psychologic Condition, Empowerment, Work Readiness, and Functional Capacity in Industrially Injured Workers (다면재활프로그램이 산업재해근로자의 다차원 심리상태, 역량강화, 직업복귀준비 및 작업능력에 미치는 효과)

  • This study examined the effects of a multifaceted intervention program on the psychological condition, empowerment, work readiness, and functional capacity for job performance of industrially injured workers. Twelve injured workers in C hospital were included in this study. The subjects participated in a social rehabilitation program 2 hours a day, twice a week, for 8 weeks in total, as well as a work hardening program 3 to 4 hours a day, 5 times a week, for 8 weeks in total. The study was conducted from March to September, 2017. A multi-dimensional psychological examination, empowerment scale, work readiness interview and functional capacity evaluation were conducted and the test scores compared before and after the program with the Wilcoxon signed rank test. In the multi-dimensional psychological examination, there were significant differences in the levels of anxiety, depression, lack of social support, and somatization symptoms (p<.05) except anger. The participants also showed significant differences in the empowerment scale, work readiness scale, and functional capacity evaluation. This study suggests that a multifaceted intervention program can be effective in improving the psychologic condition, empowerment, work readiness, and functional capacity of industrially injured workers and, in turn, may improve their rate of returning to work.

A Comparison of Psychological Characteristics between Sexually Assaulted and Molested Victims Using the Minnesota Multiphasic Personality Inventory-2 (다면적 인성검사 II를 이용한 성폭행과 성추행 피해자의 심리적 특성 비교)

  • Objectives : This study analyzed psychological aftereffects and made comparisons between victims who were either sexually assaulted or molested using the Minnesota Multiphasic Personality Inventory-2(MMPI-2). Methods : The participants were adult women who visited the Busan Smile Center within 6 months following sexual assault. This study categorized the participants according to types of sexual violence and then identified their social demographic features and characteristics. Student's t-tests were carried out to compare the MMPI-2 results between the sexually assaulted group and sexually molested group. Chi-square tests were also conducted to compare between participants who scored above 65T. Results : Among the 55 participants, 32 were sexually assaulted; 23 were sexually molested. Significant differences between the two groups were found in the following scales: F, F(B), F(P), Pt, Sc, Ma, RC6 and PSYC. Moreover, the number of participants with higher scores in the Pa, Pt, Sc, RC6, and PSYC scales in the sexually assaulted group was significantly higher than in the sexually molested group. Conclusions : Both groups of victims may commonly suffer beyond the moderate level of depression and anxiety in the post-case term. In addition, the sexually assaulted group seems to suffer relatively more from panic and psychological pain than the sexually molested group does.

Assessment of Isokinetic Muscle Function, Balance and Flexibility in Elite High School Kayakers (고등학교 엘리트 카약 선수들의 등속성 근기능·균형성·유연성 측정 및 평가)

  • The purpose of this study is to evaluate elite high school kayaker's iso-kinetic muscle function and provide physical training advices. Biodex system IV, Biodex stabilizer system, Inbody 3.0 and CNP-5403 are used to investigate 11 kayaker's muscle function(peak torque, average power and work fatigue), dynamic balance, body compositions, and flexibility. S4, S5, S8, S10, S11 are needed to train shoulder muscle strength and power. S5, S6, S10, S11 are needed to strengthen shoulder muscle endurance. S5, S7, S8, S10, S11 are needed to train knee muscle strength and power. S1, S4, S5, S10 are needed to strengthen knee muscle endurance. S3, S10, S11 are needed to train wrist muscle endurance. Based on these results we should establish an muscle function enhancing program with periodic inspection for elite kayakers. And standardized training program should be sublated.

Use of the Stomach as an Esophageal Substitute after Total Pharyngolaryngoesophagectomy for Treating Cervical Esophageal Cancer or Hypopharyngeal Cancer (경부식도암 및 하부인두암에서 근치적 전후두인두식도절제술 후 위를 이용한 재건술의 의의)

  • Purpose: The aim of this study was to analyze the post operative outcome of reconstruction with using the stomach after performing total pharyngolaryngoesophagectomy in patients with hypopharyngeal cancer or cervical esophageal cancer. Materials and Methods: We conducted a retrospective chart review of 23 patients who underwent gastric pull up for esophageal substitution at the Department of Surgery, Yonsei University College of Medicine, between January 1991 and December 2006. All the patients had transhiatal esophagectomy performed without thoracotomy. Results: There were seventeen males and six females with a median age of 58.1 years (range: 40-70 years). 19 cases were hypopharyngeal cancer, 13 cases had cancer in the pyriform sinus, 15 cases had cancer in the postcricoid area and one case had cancer in the glottic area. The rest were cervical esophageal cancers. The pathologic result was squamous cell carcinoma in all cases. The median total follow-up period was 33 months (range: 1-62 months) and there were two (8.6%) postoperative deaths: one was due to carotid rupture and the other was due to hepatic failure with liver metastasis. The complications were leakage in 1 patient (4.4%), pneumothorax in 1 patient (4.4%) and pneumonia in 1 patient (4.4%). Conclusion: The use of stomach for esophageal reconstruction has many benefits for treating hypopharyngeal cancer or cervical esophageal cancer, So, we made sure there was a sufficient length for the anastomosis after pharyngolaryngoesophagectomy and a rich blood supply from the stomach. There was a low incidence of the leakage at the anastomotic site, along with a low incidence of stenosis and bleeding.

Characteristics of Pain Threshold and Pain Experience in Elderly Patients with Dementia (노인 치매 환자의 통증 역치 및 통증 경험의 특성)

  • Objectives: We compared the characteristics of the pain threshold and pain experience between demented group and non-demented group. Methods: This study was part of Gangwon projects for early detection of dementia in 2010. We recruited 8302 local resident ages over 65 years old. Of theses, 1259 people who scored low MMSE were selected and 365 of them completed CERAD-K(Consortium to Establish a Registry for Alzheimer's disease). Finally, 90 in non-demented group and 57 in demented group(mild to moderate Alzheimer's disease) were analyzed. Pain threshold was experimentally measured by pressure algometer and we investigated the pain experience, by Brief pain inventory (BPI), a self-report test. Results: In the demographic characteristics, there are more female, higher ages, lower education in the demented group. There was no significant difference between the two groups in the pain threshold. On the BPI results, 'shoulder pain', 'the number of pain' and 'interference of working' were significantly more prevalent in non-demented group. However, there are no significant differences between the groups in the 'pain severity', 'prevalence of pain' and 'pain treatment'. Conclusions: Demented group report less pain experience but, still perceived pain. It support previous studies that patient with dementia have increased pain tolerance but preserved pain threshold. Thus, active pain assessment and treatment for patients with dementia is needed.

The Effects of the Walking Exercise on ST/HR Slope and QRS Vector in the Middle-Aged Men (운동부하 심전도를 이용한 중년 남성들의 걷기 운동이 ST/HR 경사 및 QRS 벡터에 미치는 영향)

  • The purpose of this study was to investigate the changes of long term ECG response in a company with middle-aged male employees. Subjects were 60 men who were 40~55 years old. We enrolled 30 exercise group subjects into a 3-year exercise program. In measurement index, body composition was measured by % body fat and BMI. Exercise stress test analyses were measured using ST/HR slope and QRS vector. Statistical analysis was performed using analysis of repeated ANOVA. Results of this study were as follows: In ST/HR slope, the control group showed symptoms of ischemia after nine minutes of exercise. In the rest frontal axis of the QRS vector, the control group had a tendency towards right axis deviation. In the rest horizontal amplitude of the QRS vector, the control group had a tendency to show a significant decrease, but it was increased significantly in the exercise group. These findings suggest that inactive company workers was showed a decrease of exercise capacity, early diagnosis exercise-induced ST depression, and prolonged deviation of QRS vector, but that cardiac function could be elevated in active middle aged men through regular exercise program participation.

Evaluation of a Tunneling Technique under the Latissimus Dorsi Muscle for Thoracostomy Tube Placement in Eleven Dogs (흉강 튜브 삽입을 위한 넓은 등근 아래 터널 만들기 방법에 대한 평가)

  • The present study evaluated the outcome of use of thoracostomy tube tunneling technique under the latissimus dorsi muscle for the evacuation of postoperative pneumothorax induced by thoracotomy in 11 dogs. A stab incision was made through the skin and the latissimus dorsi muscle over the rib in the fifth intercostal space caudal to a surgical window. The thoracostomy tube with a Kelly hemostat was advanced into the thoracic cavity in a cranioventral direction through the sublatissimal tunnel. After tube placement, a # 1 nylon horizontal mattress suture was placed around the skin incision. The thoracostomy tube was removed after creating a negative pressure in the thoracic cavity. Dogs were monitored after surgery for pneumothorax, subcutaneous emphysema, clinical signs including dyspnea, and tube kinking in a muscle tunnel using physical examination and postoperative radiography. There was no tube kinking in the sublatissimal tunnel in 11 dogs on introducing the tubes into the thoracic cavity. The mean (${\pm}SD$) follow-up period was $19{\pm}10$ months. On postoperative radiography, there was no evidence of pneumothorax in 11 dogs. Subcutaneous emphysema was identified around the stab incision in a dog postoperatively. The subcutaneous emphysema disappeared spontaneously within 3 days. On postoperative physical examination, there was no evidence of dyspnea in 11 dogs. Our results suggest that the sublatissimal tunneling technique for thoracostomy tube placement is effective to prevent air leakage around the thoracostomy tube while the tube remains in the thoracic cavity and along the thoracostomy tunnel after tube removal. Tunneling under the latissimus dorsi muscle should be considered the thoracostomy tube placement technique to prevent iatrogenic pneumothorax with first priority.

Surgical Repair of Rostral Hemimandibular Fractures with Canine Tooth Luxation in a Dog (개에서 견치 탈구를 동반한 앞쪽 하악골 골절의 수술적 정복 1례)

  • A sexually intact female Alaskan Malamute, 5-year-old weighing 25.2 kg presented to the Hangang Animal Hospital, following hit-by-car. On initial presentation, the dog had severe salivation and the right canine tooth luxation. Radiographs revealed fractures of the right rostral hemimandible with the right canine tooth luxation and symphyseal separation. Surgical repair of fractures was performed by use of an interdental wiring technique and external fixation technique that included pins, an acrylic connecting bar, and polymethyl methacrylate bone cement. Post-operative radiographs revealed that bridging callus was well formed over cortices of the fracture area. Pins and an acrylic connecting bar were removed 7 weeks post-operatively. The dog exhibited evidence of normal mastication.

A Study on Pathological Pattern Detection using Neural Network on X-Ray Chest Image (신경회로망을 이용한 X-선 흉부 영상의 병변 검출에 관한 연구)

  • In this study, we proposed pathological pattern detection system for X-ray chest image using artificial neural network. In a physical examination, radiologists have checked on the chest image projected the view box by a magnifying glass and found out what the disease is. Here, the detection of X-ray fluoroscopy is tedious and time-consuming for human doing. Lowering of efficiency for chest diagnosis is caused by lots mistakes of radiologist because of detecting the micro pathology from the film of small size. So, we proposed the method for disease detection using artificial neural network and digital image processing on a X-ray chest image. This method composes the function of image sampling, median filter, image equalizer used neural network and pattern recognition used neural network. We confirm this method has improved the problem of a conventional method.

Prepubic Urethrostomy for Surgical Correction of the Urethral Stricture and Rectourethral Fistula after Perineal Urethrostomy in a Cat (고양이에서 회음부 요도개구술 후 발생한 직장요도 루 및 요도 협착의 외과교정을 위한 치골 요도개구술 증례)

  • A 2-year-old, castrated male domestic short-haired cat was presented with dysuria. The physical and radiographic examinations were revealed the urethral stricture and rectourethral fistula after perineal urethrostomy (PU). A prepubic urethrostomy was performed to resolve these problems. After the treatment, the cat had normal urination and complete wound healing without any complications. This is the first case report on co-occurrence of the urethral stricture and rectourethral fistula after PU in the cat.
