A Comparative Study on Physical Examination Systems for Seafarers (선원에 대한 신체검사 제도 비교연구)

  • The purpose of this study is to provide some ideas for the improvement of the seafarers physical examination systems(PES) in Korea. This paper presents basic data about the items and detailed standards of the seafarers PES which are currently carried out worldwide. In this paper, a comparative analysis of the domestic and the foreign seafarers PES is conducted. In addition, rule of judgment in the railroad workers PES in Korea has been examined for reference. The paper proposes the amended and supplemented 16 items and detailed standards of the seafarers PES. Expert opinions of Occupational and Environmental Medicine specialist and the recommendations of ILO/WHO are also considered to draw an improvement scheme for the seafarers PES in Korea.

  • 필수 영양성분인 비타민. 비타민은 소량으로 신체기능을 조절한다는 점에서 호르몬과 비슷하지만 신체의 내분비기관에서 합성되는 호르몬과 달리 외부로부터 섭취되어야 한다. 비타민은 체내에서 전혀 합성되지 않거나, 합성되더라도 충분하지 못하기 때문이다. 외부에서 얻어야하는 꼭 필요한 영양성분이기에 중요한 비타민이지만 잘못된 상식들이 난무하고 있어 일반인들은 물론 전문가들조차 옥석을 구분하기 어려울 정도. 그리하여 주요 비타민에 대해 제대로 알아보는 시간을 갖고자 지난 호에 이은 두 번째 시간으로 비타민A와 비타민B군에 대해 알아본다.

  • Opioid aberrant behavior is an emerging problem as strong opioid is increasingly used to alleviate cancer pain in patients with cancer. Although the treatment of opioid addiction and physical dependence for non-cancer pain is well known, few studies have been conducted with cancer patients, particularly in the Korean population. Presented here are ten cases of cancer patients who were physically dependent on strong opioid and successfully treated with a partial mu-opioid receptor agonist, buprenorphine. This is the first report showing the efficacy of transdermal buprenorphine as a treatment for physical dependence on opioid medication in cancer patients.

Pharmacotherapy for Patients Complaining With Somatic Symptoms (신체증상을 호소하는 환자의 약물치료)

  • Patients complaining with somatic symptoms are very common in clinical practice, and are often consulted to medical department. But it is difficult to treat well. The treatment of somatic symptom disorder is multi-modal as none of the methods on their own provide a satisfactory outcome. The treatment of somatic symptoms disorders is complicated by lack of boundary, conceptual clarity, and overemphasis on psychosocial causation and effectiveness of psychological treatments. In clinical practice all classes of psychotropics are used to treat somatic symptoms disorder. Drugs such as tricyclic antidepressants, serotonin reuptake inibitors(SSRI), serotonin and noradrenalin reuptake inhibitors (SNRI), atypical antipsychotics are studied. The evidence indicates that these drugs are effective in somatic symptom disorders. All classes of antidepressants seem to be effective against somatic symptom disorders. SSRIs are more effective against hypochondriasis and body dysmorphic disorder, and SNRIs appear to be more effective than other antidepressants when pain is predominant. The author suggest that psychiatrists should know how to treat patients complaining with somatic symptoms by using not only psychotherapeutic approach but also pharmacological treatment. It will be helpful to reduce suffering and increase quality of life of these patients.

A Study on the Factors Influencing Stress, Sadness and Despair of Body Image Distortion Adolescents (신체인지 왜곡 청소년의 스트레스, 슬픔과 절망감 영향 요인에 관한 연구)

  • This study aimed to identify the factors associated with distorted body image in adolescents according to sex. The data obtained from the Korea Youth Risk Behavior Web-Based Survey (2019). The subjects included 9,635 adolescents who perceived their body type as overweight, despite having a normal body mass index. The main finding indicated that high levels of fast food consumption were associated with significantly higher feelings of stress, sadness, and despair in both males and females. In addition, both males and females experienced elevated levels of stress, sadness, and despair when they perceived that they were not sufficiently physically active. The results of this study can be used for the development of interventions for adolescents with distorted body image.

  • The purpose of this study is to provide basic data for the improvement for physical examination system. In this paper, we perform a comparative analysis of domestic and foreign physical examination systems for seafare's. In order to Reinforce the objectivity We also compare rule of judgement about physical examination system of railroad workers and foreign seaman. Then we consult expert opinions of Occupational and Environmental Medicine. Through the comparative analysis, we provide useful information that is needed to amend the physical examination system.

Comparison of Effect of SSRIs and SNRIs on Depression, Pain and Somatic Symptoms in Elderly Patients with Major Depressive Disorder (노인 우울증 환자에서 선택적 세로토닌 재흡수 억제제, 세로토닌 노르에피네프린 재흡수 억제제가 우울증상, 통증 그리고 신체증상에 미치는 효과 비교)

  • Objectives : The aim of this study was to compare the effect of Selective Serotonin Reuptake Inhibitor (SSRI) and Serotonin Norepinephrine Reuptake Inhibitor (SNRI) for mood symptoms, pain, and somatic symptoms in elderly depression patients with pain and somatic symptoms. Methods : This study is a prospective open-label study conducted by a single institution. A total of 43 subjects diagnosed with major depressive disorder under the DSM-5 diagnostic criteria participated in this study (average age: 72.53, 58.1% women). The subjects were classified as SSRI and SNRI groups. Depressive symptoms, pain, and somatic symptoms were evaluated by Korean version of the Hamilton Depression Rating Scale (K-HDRS), visual analogue scale (VAS) and Patient Health Questionnare-15 (PHQ-15) respectively at baseline and six weeks later. Two-way repeated-measure ANOVA was performed to analyze changes in the KHDRS, VAS, and PHQ-15 scores. Results : In the SSRI and SNRI groups, K-HDRS, VAS, and PHQ-15 all showed significant improvement after 6 weeks compared to each baseline values. There were no differences in therapeutic effect between the two groups. Conclusions : We found that SSRI and SNRI both improved somatic symptoms and pain in elderly depression patients. The results of this study are thought to help select antidepressants when administering medication to elderly depression patients who complain pain and somatic symptoms. Further research is needed on the longterm effects of the SSRI and SNRI.

  • Creativity is the thinking ability and the expression of new image by imagination as a problem recognition and way of solution. This study aims to search for the creativity theory of body movement and to analyze the creativity factor. According to the study, the creativity of body movement needs four steps: movement awareness, movement design, movement discovery and movement use. The use of new image through self-perception and self concept brings about a creative improvement in the problem recognition and its resolution function. In conclusion, the creativity of body movement means the infinity of body movement as 'the third energy' and 'the flexibility of flow' by interaction.

  • Purpose: The purpose of the study was to analyze the status of dietary behavior and nutrition intakes according to the gender and the level of physical activity of adolescents. Methods: Data from 659 adolescents aged 13 to 18 years who participated in the Korea National Health and Nutrition Examination Survey (2017-2018) were analyzed. Collected data were analyzed using complex sampling data analysis with SPSS v. 27.0. Results: The adolescents were categorized into three groups based on their physical activity as follows: The first group with high physical activity (8.9%), a second group with normal physical activity (29.7%), and a third group without physical activity (61.4%). Boys showed significantly higher physical activity than the girls. Adolescents with high physical activity had a significantly lower body mass index than adolescents without physical activity, but their meal regularity, subjective health status, and recognition of body image were significantly higher. In terms of biochemical factors, adolescents with high physical activity had lower triglycerides and cholesterol than adolescents without physical activity, while hemoglobin and hematocrit were significantly higher. In addition, the intakes of energy, carbohydrates, proteins, fat, vitamin B1, niacin, calcium, phosphorus, sodium, potassium, iron increased significantly as adolescents' physical activity increased. Conclusion: The physical activity of adolescents had a positive effect on dietary behavior, lifestyle, and nutritional factors. Therefore, it is necessary to increase participation in physical classes in schools and develop various programs considering the level of physical activity among adolescents.
