Biomechanical Analysis of Elderly Fall Related Risk Factors using Downhill Walking on Treadmill (트레드밀 내리막 보행을 이용한 노인 낙상관련 위험요인의 운동역학적 분석)

  • The purpose of this study was to investigate biomechanical differences between young and old adults during downhill walking on a treadmill in order to understand the mechanisms of elderly falls. Eighteen healthy young females(YG: yrs: 21.17±1.5) and eighteen healthy old females(OG: yrs: 66.67±1.33) participated in this study. They were asked to walk at their preferred speed on a treadmill at level, 7.5° and 15° decline. OG walked more wobbly in the medial and lateral directions than YG(p<.05). As slope got steeper, OG had smaller ROM(range of motion) of ankle and knee joints compared with YG. However, there was no difference in ROM of the hip between OG and YG, but maximum extension angle of OG was smaller compared with YG(p<.05). Smaller extensor moment was generated on OG during downhill walking(p<.05). It was hypothesized that more risk factors would be found on older people compared to young people during downhill. However, older people actually walks with a safer strategy compared to young people during downhill. Finally, current findings about biomechanical characteristics of elderly walking would provide useful fundamental information for a follow-up study regarding the prevention of elderly fall during their daily life.

Effect of resistance training on joint flexibility and muscle strength of upper extremities of elderly with impaired cognition (탄력저항성운동 프로그램이 인지기능저하 노인의 상지유연성 및 근력에 미치는 효과)

  • The purpose of the study was to examine the effect of resistance training on joint flexibility and muscle strength of upper extremities of institutionalized elderly with impaired cognition. The study design was pretest-posttest control group study and inclusion criteria were elderly aged 65-year older, MMSE score 23 or less, ones who had no serious physical and/or mental problem except impaired cognition, and were capable to carry out resistance training. After consents were obtained participants were randomly assigned. Pre-post evaluation was performed by staff nurses trained beforehand. Among those 4-week study period, experiment was carried out during 5 consecutive days a week for 3 weeks. ROM and extension range of shoulder joints and muscle strength of shoulders and hands for both sides were measured. Flexion, extension, abduction range of right shoulder joint was significantly improved. Flexion and extension muscle strength of left side shoulder and abduction muscle strength of both sides of shoulder were significantly improved. With the study result, it could be concluded that resistance training has therapeutic effects on joint flexibility and muscle strength. More studies adopted longer experimental period to evaluate timing of effect and extinction to refine the protocol are called for.

Effect of Heat Stress of Extreme Heat Lever on Muscle functionand Muscle Injury Markers in Elderly Women (열 스트레스가 여성노인들의 근기능 및 근손상에 미치는 영향)

  • The purpose of this study was to examine the influence of extreme heat on muscle function and muscle injury marker in elderly women. The subjects of this study were eight post-menopausal elderly women without any metabolic disease. All eight subjects were asked to perform the knee joint isokinetic exercise using isokinetic equipment (cybex) in the laboratory and experimental temperature within laboratory was adjusted to two conditions: extreme heat temperature(33±0.5℃) and normal temperature(20±0.5℃) maintained in 50±3% humidity conditions. Each experimental exercise was monitored and analyzed the change of HSP70, LDH and CK. Muscular functions (peak torque, total work, percentage of peak torque body weight, fatigue index, average power and total work) were significant differences at exercise between temperatural conditions (p<.05). In extreme heat temperature, muscular injury markers (HSP70, LDH and CK) were increased, threfore resulted in significantly higher than normal temperature(p<.05). These results show that extreme heat temperature can decrease muscle function in elderly women.

The Effects of Lumbar Stabilizing Exercise on Increasing of the Strength of Lumbar Extensor Muscle in Normal Subjects (요부안정화 운동이 정상인의 요부 신전력 증진에 미치는 영향)

  • The purpose of this study was to find the effects of the lumbar stabilizing exercise on the isometric lumbar extension strength and the range of motion of lumbar extension. 16 healthy subjects were recruited (9 males, 7 females, mean age 25${\pm}$3.2). They performed 5 types of exercises 5 times per week for 4 weeks. We evaluated isometric trunk extension strength (0, 12, 24, 36, 48, 60, $72^{\circ}$) by MedX(Ocala, FL) and measured range of motion of lumbar extension. All measurements were measured at pre-exercise and 4 weeks post-exercise. The results of this study were summarized as follows; After 4 weeks of exercise, isometric lumbar extension strength was generally increased but there were only significant differences at $12^{\circ},\;24^{\circ},\;48^{\circ}$. The range of motion of lumbar extension was increased (p<.05). The isometric lumbar extension strength correlated with the range of motion of lumbar extension. In conclusion, our results have shown that lumbar stabilizing exercise influenced both lumbar extension strength and range of motion of lumbar extension.

Effects of Intentional Abdominal Muscle Contraction on Lumbar Muscle Activities andLumbar Extension During Lifting Above the Shoulders (어깨위로 들어올리기 작업시 의도적인 복근수축이 요부근 활성도와요추신전각도에 미치는 영향)

  • This study was conducted to evaluate the effect intentional contraction of abdominal muscles during lifting above shoulder on the muscles activities of the lower trunk and on the degree of lumbar extension. Fifteen healthy adult males were selected as test subjects. A 5kg weight was raised to the shoulder level, 20cm, and 40cm above the shoulder level. EMG activities of all muscles except the rectus abdominis were significantly greater when subjects were asked to contract their abdominal muscles intentionally during lifting(p<0.05). Degree of lumbar extension also significantly decreased with intentional abdominal contraction during the lifting. Also, degree of lumbar extension significantly increased with increase in lifting height(p<0.05). The results of this study show that intentional contraction of abdominal muscles during lifting above shoulder increases the EMG activities of external abdominis oblique, internal abdominis oblique, and erector spinae.

Comparative Study of Biomechanical Left and Right Elbow Joint Extension Movements After Wheel Axle Application (윤축을 적용한 좌·우 주관절 신전 동작의 운동역학적 비교 연구)

  • In this study, we have experimented with 9 players at the national delegate level. Although there were some differences in the average effects of 3 types of one-two straight movements after the application of wheel axle, there were no statistical differences in the case of surface reacting forces, electromyograms, and impact forces. When the right fist was impacted using the one-two straight movements and the wheel axle was applied with 3 segmentations, high impact forces were obtained for the pronation in the following order-72.01 $m/s^2$ (type 2), 70.93 $m/s^2$ (type 3), and 58.19 $m/s^2$ (type 1). Higher values of the surface reacting force were found for type 1 that did not exhibit pronation in the left foot, whereas in the case of the vertical direction of the right foot, type 2 with pronation exhibited higher values and impact forces. In the right electromyogram, high impact forces due to the activation of the muscular electric potential were obtained for lumbar erector (LE) spinae and triceps brachii (TB) with type 1; LE spina, latissimus dosi (LD), and upper trapezius (UT) with type 2; and brachioradialis (BR), UT, and rectus abdominal (RA) with type 3. Due to pronation and complex motions of the 3 pronation segmentations, the efficiency was higher for impacts due to one-two straight movements.

Fatigue Analysis of Knee Extensor Using sEMG and Dynamometer (sEMG와 Dynamometer를 이용한 슬관절 신전근의 피로 분석)

  • Fatigue is a common experience in our daily life and in therapeutic exercises. Human muscle fatigue has been studied using a wide variety of exercise models, protocols and assessment methods. This study conducts quantitative evaluation of fatigue of knee extensor with surface electromyography and dynamometer and is to provide basic materials in physical therapy diagnosis and assessment of skeletal muscle. Measurement of two stages was conducted with 88 normal adults between their twenties and seventies, correlations and differences using collected data according to age was compared and correlations among measured items were analyzed and then the following conclusions were obtained. Analysis of sEMG and dynamometry of knee extensor showed that MDF, FI, MVIC, ET had the closest relations with age and MDF, FI, MVIC were significantly increase with aging. In addition, it was found that there were high correlation among items of analysis.

Effect of Wrist Extensor Strength and Pain on Extracorporeal Shock Wave Therapy of the Lateral Epicondylitis (체외충격파치료가 주관절 외측상과염 환자의 손목 신전근의 근력과 통증에 미치는 영향)

  • Purpose : The purpose of this study was to assess the effectiveness of Extracorporea Shock-wave therapy(ESWT) on the pain and improvement of wrist extensor strength which is one of the available medical treatment in patients with lateral epicondylitis. Object and Methods : The subjects of this study were consisted of 15 patients with lateral epicondylitis, These medical treatments was conducted total 6 time for 3 weeks. In the ESWT application group. To investigate the effectiveness of the treatment after ESWT treatment, Pain was measured by visual analogue scale(VAS) which is divided into 10 ranks and wrist extensor strength was measured by Aneroid phygmomanometer Results : 1. It was showed that VAS and wrist extensor strength before treatment and after two treatment(1 week) were no significant differences(p<0.05). 2. It was showed that VAS and wrist extensor strength before treatment and after four treatment(2 week) were significant differences(p<0.05). 3. It was showed that VAS and wrist extensor strength before treatment and after six treatment(3 week) were significant differences(p<0.05). Conclusion : We think that ESWT for the patients with lateral epicondylitis. is effective to wrist extensor strength and decrease of pain.

Analysis of Isometry of the Anterior Cruciate Ligament for Optimal Ligament Reconstruction (전방십자인대의 최적 재건을 위한 등장성 해석)

  • The anterior cruciate ligament (ACL) is liable to a major injury that often results in a functional impairment requiring surgical reconstruction. The success of reconstruction depends on such factors as attachment positions, initial tension of ligament and surgical methods of fixation. The purpose of this study is to find isometric positions of the substitute during flexion/extension. The distance between selected attachments on the femur and tibia was computed from a set of measurements using a 6 degree-of-freedom magnetic sensor system. A three-dimensional knee model was constructed from CT images and was used to simulate length change during knee flexion/extension. This model was scaled for each subject. Twenty seven points on the tibia model and forty two points on the femur model were selected to calculate length change. This study determined the maximum and minimum distances to the tibial attachment during flexion/extension. The results showed that minimum length changes were $1.9{\sim}5.8mm$ (average $3.6{\pm}1.4mm$). The most isometric region was both the posterosuperior and anterior-diagonal areas from the over-the-top. The proposed method can be utilized and applied to an optimal reconstruction of ACL deficient knees.

Gait Study on the Normal and ACL Deficient Patients After Ligament Reconstruction Surgery Using Chaos Analysis Method (전방십자인대 재건수술 환자와 정상인의 보행 연구)

  • The anterior cruciate ligament(ACL) is an important stabilizer of knee joint. The ACL injury of knee is common and a serious ACL injury leads to ligament reconstruction surgery. Gait analysis is essential to identify knee condition of patients who display abnormal gait. The purpose of this study is to evaluate and classify knee condition of ACL deficient patients using a nonlinear dynamic method. The nonlinear method focuses on understanding how variations in the gait pattern change over time. The experiments were carried out for 17 subjects(l2 healthy subjects and five subjects with unilateral deficiency) walking on a motorized treadmill for 100 seconds. Three dimensional kinematics of the lower extremity were collected by using four cameras and KWON 3D motion analysis system. The largest Lyapunov exponent calculated from knee joint flexion-extension time series was used to quantify knee stability. The results revealed the difference between healthy subjects and patients. The deficient knee was significantly unstable compared with the contralateral knee. This study suggests an evaluation scheme of the severity of injury and the level of recovery. The proposed Lyapunov exponent can be used in rehabilitation and diagnosis of recoverable patients.
