The Human body is subject to two patterns of senescence, both of which are represented in the eye and vision. This article discusses the anatomical and physiological changes in anterior and posterior segments which occur with age. The areas covered include the globe, lids, tears, conjunctiva, cornea, sclera, anterior chamber angle, iris, lens, choroid, vitreous and retina. A sample of 219 individuals between the ages of 50 and 79 years was tested with case history, slit lamp examination, palpebral fissure, horizontal visible iris diameter(HVID) and pupil size. The palpebral fissure decreases with age while HVID remains constant in all age classes. This study suggests that an understanding of anatomical and physiological changes which occur with age is helpful to all eye clinicians because the aging process affects all of the ocular tissues.
본 실험은 pseudorabies 바이러스 (PRV) 의 Bartha strain 을 안면신경의 측두지, 하지를 지배하는 신경 (좌골신경) 및 상지를 지배하는 신경 (요골, 척골, 정중신경) 에 주입한 후 4 일간의 생존시간이 경과한 후 척수와 뇌를 적출하여 동결절편을 제작한 후 면역조직화학적 염색기법과 X-gal 조직화학 염색법을 시행하여 염색된 신경세포체를 척수와 뇌에 투사된 공통영역을 관찰하고 두침의 영역중 하나인 운동구와 사지와의 관계에 대한 실험적 증거를 제시하고자 시행하였다. 위의 실험에서 얻어진 결과는 아래와 같다. 1. 안면신경의 측두지, 하지를 지배하는 신경 (좌골신경) 및 상지를 지배하는 신경 (요골, 측골, 정중신경) 에서 투사된 공통된 영역은 척수에서 경수의 층판 1-IV, 흉수의 intermediolateral nucleus(IML), dorsal nucleus(D) 및 층판 X, 요수의 층판 IV, V, 천수의 층판 IV, V, IX, X 등의 영역에서 관찰되었고, 뇌줄기에서는 caudoventrolateral reticular nucleus(CVL), nucleus solitary tract(Sol), rostroventrolateral nucleus(RVL), area postrema(AP), raphe nuclei(raphe pallidus, raphe obscurus, raphe magnus), inferior olivary nucleus 의 등쪽부분 (gigantocellular reticular nucleus, Gi), Kolliker-Fuse nucleus(KF), central gray(CG), dorsal raphe nucleus (DR) and A5 영역에 표지된다. 또한 paraventricular hypothalamic nucleus(PRV) 와 lateral hypothalamic reticular nucleus(LH)에서도 관찰되고 locus coeruleus(LC) 와 subcoeruleus nuc!eus(SubCA) 에서도 관찰된다.
Sarcopenia is a leading cause of increased medical and nursing care costs among the elderly. In Korea, preventive measures for sarcopenia are mostly targeted toward the general elderly population without specific diseases. However, it is also necessary to implement measures for elderly individuals living in nursing homes and hospitals, where the prevalence of sarcopenia is high. Currently, computed tomography and/or magnetic resonance imaging are considered standard diagnostic tools. However, their complexity and time-consuming nature make them unsuitable for clinical use. The exact pathophysiological mechanisms of sarcopenia are unclear, as they involve various molecular biological pathways, including decreased exercise, protein and nutrient intake, changes in testosterone and growth hormone, and inflammation. Sarcopenia symptoms can lead to several diseases, such as osteoporosis, fractures, dementia, diabetes, and cardiovascular disease. Vitamin B deficiency is a significant factor in sarcopenia induction, with B vitamins being directly involved in energy and protein metabolism and nerve function. Vitamin B deficiency can lead to neuromuscular and neurogenic disorders, which often overlap with sarcopenia. Suboptimal intake of B vitamins, malabsorption, and anorexia are common among the elderly. This study aims to provide information on the role of water-soluble B vitamins in preventing and controlling muscle mass loss and deterioration among the elderly with sarcopenia. In addition, we discuss the potential of myokines from the B vitamin family in modulating sarcopenia.
Identification and protection of the facial nerve is very important in the proper operation of the parotid tumor. Posterior approach which finds main trunk of the facial nerve by surgical landmark such as tragal pointer, tympanomastoid suture, and posterior belly of digastric muscle is most commonly used. In case of posterior located tumor, inferior approach may be used, in which the retromandibular vein is followed from the neck and inferior branch of the facial nerve is located. In general, the facial nerve lies superficial to the retromandibular vein. But we experienced the anomalous relationship of the facial nerve and the retromandibular vien. We report this case with a literature review.
This study examined the anatomical relationships along with the variability of the facial nerve trunk and its branches with an emphasis on the intraparotid connections between the divisions. And histomorphometric observations of the facial nerve branches and fascicles were performed on 40 Korean half-heads. The facial nerve trunk was bifurcated into two main divisions(35/40, 87.5%) and the other five cases were divided into a trifurcation pattern. According to the origin of the buccal branch, the branching patterns of the facia l nerve were classified into four categories. Communications between the facial and auriculotemporal nerve branches were observed in 37 out of 40 cases(92.5%). In the histological observation, the buccal branch had the greatest number of branches(3.47), however the zygomatic branch had the largest diameters(0.93mm). This detailed description of the facial nerve anatomy wi ll provide useful information for surgical procedures such as a tumor resection. a facial nerve reconstruction, autonerve graft. and facelift.
Objectives : The neural substrate of fear is thought to be highly conserved among species including human. The purpose of this review was to address the neural substrates of fear based on recent findings obtained from animal and human studies. Methods : Recent studies on brain regions related to fear, particularly fear conditioning in rodents and humans, were extensively reviewed. Results : This paper suggests high consistency in anatomical structure and physiological mechanisms for fear perception, response, learning and modulation in animals and humans. Conclusions : Fear is manifested and modulated by well conserved neural circuits among species interconnected with the amygdala, such as the hippocampus and the ventromedial prefrontal cortex. Further research is required to incorporate findings from animal studies into a better understanding of neural circuitry of fear in human in a translational approach.

Peripheral nerve injuries in the oral and maxillofacial regions require nerve repairs for the recovery of sensory and/or motor functions. Primary indications for the peripheral nerve grafts are injuries or continuity defects due to trauma, pathologic conditions, ablation surgery, or other diseases, that cannot regain normal functions without surgical interventions, including microneurosurgery. For the autogenous nerve graft, sural nerve and greater auricular nerve are the most common donor nerves in the oral and maxillofacial regions. The sural nerve has been widely used for this purpose, due to the ease of harvest, available nerve graft up to 30 to 40 cm in length, high fascicular density, a width of 1.5 to 3.0 mm, which is similar to that of the trigeminal nerve, and minimal branching and donor sity morbidity. Many different surgical techniques have been designed for the sural nerve harvesting, such as a single longitudinal incision, multiple stair-step incisions, use of nerve extractor or tendon stripper, and endoscopic approach. For a better understanding of the sural nerve graft and in avoiding of uneventful complications during these procedures as an oral and maxillofacial surgeon, the related surgical anatomies with their harvesting tips are summarized in this review article.

Musculoskeletal ultrasonography has been used as a convenient and simple tool for diagnosis of various conditions of orthopaedic diseases for many years. Generally magnetic resonance imaging (MRI) is thought to be the best method to search for anatomical structures or variations. However, for dynamic conditions such as dislocation or subluxation of tendons and nerves, MRI is not superior to ultrasonography, especially dynamic ultrasonography. So we present such a patient with an ulnar nerve subluxation at the elbow who has symtoms mimicking cubital tunnel syndrome diagnosed by dynamic ultrasonography and treated successfully by ulnar nerve anterior transposition and think that dynamic ultrasonography is a useful method for diagnosing dynamic condition such as ulnar nerve subluxation mimicking cubital tunnel syndrome.

Cognitive reserve (CR) is a concept that can explain the discrepancies between the pathologic burden of the disease and clinical manifestations. It refers to the individual susceptibility to age-related brain changes and pathologies related to Alzheimer's disease, thus recognized as a factor affecting the trajectories of the disease. The purpose of this study was to explore the current states of clinical studies on neural substrates of CR in Alzheimer's disease using functional magnetic resonance imaging. We searched for clinical studies on CR using fMRI in the Pubmed, Cochrane library, RISS, KISS and ScienceON on August 14, 2023. Once the online search was finished, studies were selected manually by the inclusion criteria. Finally, we analyzed the characteristics of selected articles and reviewed the neural substrates of CR. Total thirty-four studies were included in this study. As surrogate markers of CR, not only education and occupational complexity, but also composite score and questionnaire-based method, which cover various areas of life, were mainly used. The most utilized methods in resting-state fMRI were independent component analysis, seed-based analysis, and graph theory analysis. Through the analysis, we demonstrated that neuroimaging techniques could capture the neural substrates associated with cognitive reserve. Moreover, functional connectivity of brain regions centered on prefrontal and parietal cortex and network areas such as default mode network showed a significant correlation with CR, which indicated a significant association with cognitive performance. CR may induce differential effects according to the disease status. We hope that this perspective on cognitive reserve would be helpful when conducting clinical researches on the mechanisms of traditional Korean medicine for Alzheimer's disease in the future.
Introduction : One of the important domains in OT is performance skills which include sensory perceptual skills, motor and praxis skills, emotional regulation skills, cognitive skills, and communication/social skills. Body : Stereology robust tool when employed to investigate morphological changes in neurons, cortex area, and specific parts of brain involved in special brain function. Stereology is an interdisciplinary field focused or analyzing biological tissue with the three-dimensional interpretation of planer sections by using estimating method and mathematically unbiased sampling. With the unbiased stereological method based on probability theory, researchers can estimate morphological and anatomical changes in biological reference areas accurately and efficiently. Changes in anatomical and cytoarchitectural parameters, such as volume, number, and length, affect specific brain function related to the brain area. Occupational therapists provide treatment to improve functions for participation of occupation in neurological disorder. The functional improvements in neurological disorder reflect neurobiological changes because functional difficulties, such as motor cognitive disorder, are due to neurological disturbances. Thus, combination of two kinds of evidence, neurological changes and functional improvement, provide fundamental evidence for OT intervention in neurological disorder. Even though most of stereological studies are in animal model and in postmortem human because of practical and ethical issues, stereology provides fundamental knowledge to support OT theory and practice. Conclusion : Therefore, stereology informs translation from neuroscience to OT based on structure-function relationship in performance skills and experience-dependent neural plasticity.
