Sciatica from Retroperitoneal Lymphoma (후복막강 임파종에 의한 좌골신경통)

  • Back pain with sciatica is a common clinical syndrome that may be the expression of an uncommon pathological process. We describe a case of retroperitoneal lymphoma mimicking herniated lumbar disc. 62 year old male patient with a clinical picture of sciatica as a first manifestation of retroperitoneal lymphoma. His pain was not response to conventional therapy, and progressively worsened. We reevaluated him and found huge retroperitoneal lymphoma in his pelvic cavity. So we discuss how to identify the few who had extra attention. It is important to rule out occult pathology in patient with an atypical presentation. In assessing a patient with a low back pain, accurate evaluation require a logical, step by step method.

Neurolytic Splanchnic Nerve Block for the Treatment of Upper Abdominal Pain (상복부 암성통증 환자관리에 있어서 내장신경통 차단술의 임상적 고찰)

  • Neurolytic splanchnic nerve block is an effective method for the relief of pain of upper abdominal cancer. Nine cases of intractable upper abdominal cancer pain were treated by splanchnic nerve block with absolute alcohol (25 ml) at the pain clinic of Kangdong Sacred Heart Hospital, Hallym University, during a 19 month period from March, 1988 to September, 1989. The group included six patients with stomach cancer, two patients with pancreatic cancer, and one patient with hepatobiliary cancer. We used fluoroscopy in all cases of alcohol spianchnic nerve block to determine both, the position of the needle tip, and the spread of the neurolytic solution. Of the 9 patients, 6 patients had excellent pain relief, and 3 patients who had combined upper abdominal and lower back pain had relieved upper abdominal pain only, but remaining lower back pain. Of the 9 patients', 5 patients had excellent pain relief through the patients remaining life (1-2 months) in which follow up was possible.

Pain Management of Post-herpetic Neuralgia (대상포진 후 신경통의 통증관리)

  • Postherpetic neuralgia, the most feared complication of acute herpes zoster, may agonize the pain clinician because the appropriate management of intractable pain can fail inspite of various and prolonged therapeutic techniques. Of all patients with herpes zoster, approximately 5~10% will develop postherpetic neuralgia. Postherpetic neuralgia is very rare in young patient but very common in patients over 60. In other words, the older, the higher incidence. In our pain clinics, 13 postherpetic neuralgic patients were treated with sympathetic blocks, local infiltration with local anesthetics and steroids, TENS, Laser and various drugs including antiderpressant. The results of management of pain in 13 patents were as follows. 1. Excellent pain control :4(30.8%) 2. Good pain control :3(23.0%) 3. Fair pain control :2(15.4%) 4. No effect :4(30.8%).

  • This study was compared with each 2 groups (Intermittent traction therapy alone group1: Control group and Intermittent traction therapy plus Auriculotherapy group 2: Experimental group) on 20 HIVLD(Hemiated Intervertebral Lumbar Disk) patients with sciatics Each subjects were divided to control and experimental. Each group were applied Intermittent traction therapy (HOld:25, Rest:15) for 15 minute and control group were not received auricular stimulation. However experimental groups were applied auricular stimulation simultaneosly. Auricular acupuncture points(max-17, min=13) were stimulated with low frequency. high intensity Electro-Acupunture stimulator for 20 seconds per each point. Treatment and measurements(R.P and VAS) was administered to each patient during the 10 times therapy per 2 weeks. The results were significant differences between the two groups. Experimental group appeared significantly greater than before in pain, and pain relief, and significant improvement in centralization phenomenon of radiating pain.

  • Trigeminal neuralgia(tic douloureux) is a condition characterized by excruciating paroxysms of pain in lips, gums, cheek or chin. There are many treatments of trigeminal neuralgia, such as carbamazepine, electrical stimulation, radiation therapy and ablative procedures. We blocked the mandibular nerve via lateral extra-oral route with pure ethyl alcohol in patient with localized gingival pain. The results were as follows; 1) The mandibular nerve block with pure ethyl alcohol was effective. 2) No complications were noted.

  • Objective; This study was designed to investigate the effect of Sa-am acupuncture and Arctii Fructus water extract on Post-herpetic Neuralgia. Method: We have treated them with acupuncture treatment of Sa-am acupuncture and Arctii Fructus water extract and evaluated the effect by Pain Rating Score(P.R.S.) and Visual Analogue Scale(V.A.S.) scale. Result: Before theraphy, the grades of Song's scale were P.R.S. 80 and V.A.S. 100, Kim's sclae's P.R.S. 77 and V.A.S. 100. After theraphy, the grades of song's scale were P.R.S. 5 and V.A.S. 10, Kim's scales's P.R.S. 10 and V.A.S. 10 Conclusion: The treatment of Sa-am-acupuncture and Arctii Fructus water extract on Post-herpetic Neuralgia was effective and will be attempted to the patients with it.

  • Fifteen patients was analyzed on effect of the management of postherpetic neuralgia by local anesthesia on the special region at pain clinic in Youngnam University Hospital. The results were on follows : 1) The frequency of occurrence of sex and the lesion side were similar in all patients. 2) The age of incidence was between 50 and 70 years old. 3) The most frequent site of lesions was the neck. 4) There was no relationship between age and treatment time. 5) Whole patients was done average 7-10 time local injection.

  • Objectives : This is a clinical report about the 47-year-old female patient with Trigeminal Neuralgia(TN) treated by oriental medicine treatment including Bee Venom Pharmacopuncture therapy(BVP), without any western medical treatment. Methods : The patient was treated by acupuncture, herb medication, and BVP. The improvement of the patient was judged by Visual Analog Scale(VAS). Results : The pain was gradually reduced and VAS became 0 after 8 days of treatment. Since then the pain didn't reappeared for 6 months. Conclusions : Oriental medicine treatment including BVP was very effective to improve the TN patient's symptoms. It is necessary to have more observation and many cases of patients with TN.

A Case Report of Post-herpetic Neuralgia Improved by Oriental Medicine with Depletion and Cupping Therapy (자락발관법(刺絡拔罐法)을 병행한 한방치료로 호전된 대상포진 후 신경통환자 치험 1례)

  • Objective : This study reports the effect of oriental medicine with depletion and cupping therapy on post-herpetic neuralgia(PHN) 1 case. Methods : We experienced one case of PHN treated by oriental medical treatment with depletion and cupping therapy. To evaluate the effectiveness of this treatment, we used the Visual Analogue Scale(VAS). Result : After the treatment the grade of VAS was decreased and clinical symptoms were improved. Conclusion : Oriental medicine with depletion and cupping therapy can be available for relieving symptoms related with PHN.

Nerve Block for Treatment of Tienchu Syndrome and Occipital Neuralgia (천주증후군 및 후두신경통의 치료를 위한 신경차단)

  • A myofascial syndrome due to continuous muscle contraction with the trigger point at the upper lateral edge of the nuchal muscles where they attach to the occipital bone is frequently seen in daily pain clinic practice. The Tienchu syndrome is a myofascial condition of the posterior neck region with a trigger point at the Tienchu acupoint(B10). When advanced, occipital neuralgia and muscle contraction headache follow. Therefore, a Tienchu block and/or occipital nerve block with local anesthetic combined with a small dose of steroid is a most effective therapeutic method for many patients who complain of posterior headache or posterior neck pain.
