Hourglass-Like Constrictions of Peripheral Nerve in the Upper Extremity (상지에서 발생한 말초 신경의 모래시계형 협착)

  • Nerve torsion is a rare condition that causes sensory abnormalities and decreased muscle strength due to a nerve-twisting phenomenon in the peripheral nerves. To date, the progression of the condition from its cause to diagnosis and treatment has not been established. The authors report three cases of good results from the treatment of nerve torsion in the anterior interosseous and radial nerves with epineurotomy and neurolysis.

  • Schwannomas are benign neoplasms with a Schwann cell origin. A plexiform schwannoma is a rare variant of a schwannoma with a plexiform or multinodular growth pattern. The condition occurs mostly as a solitary lesion in the skin or subcutaneous tissue, or uncommonly located in the deep soft tissue. We report a rare case of recurred multiple plexiform schwannomas arising from the posterior tibial nerve and its branch, which was located in a deep anatomic location and accompanied by a bony deformity.

  • The temporal branch of the facial nerve is particularly vulnerable to traumatic injuries due to its anatomic location, which often causes severe aesthetic and functional loss in the patient. Moreover, a chronic injury with nerve defect is more difficult to treat compared to acute injury, because it usually needs an additional procedure such as a nerve graft surgical procedure. This case shows a male patient who had a divided temporal branch of the facial nerve one month after an injury. We successfully grafted the split sural nerve and showed a good aesthetic, functional recovery for the patient.

  • Recurrent motor branch entrapment syndrome is a compressive mononeuropathy of recurrent motor branch of median nerve. It is a rare condition as a cause of thenar muscle wasting and may have different pathogenesis. If such an anatomical variation is the cause, there is a possibility that thenar muscle atrophy remains if only the transcarpal ligament release is performed. We report a 25-year-old male patient with carpal tunnel syndrome with thenar muscle wasting 1 month ago.

  • Perineural cysts are usually discovered incidentally; only approximately one percent of such cysts are symptomatic. Symptomatic cervical perineural cysts, of which only five case reports have been found in the literature, are extremely rare and there is no consensus regarding treatment. When a cervical perineural cyst is discovered in a patient with cervical radiculopathy without the cause of other apparent compression, the cyst can be considered as a possible cause of cervical radiculopathy. In such cases, non-surgical conservative treatment can be considered before any surgical intervention. We reported the successful treatment of case of a 40-year-old woman with a symptomatic cervical perineural cyst by oral steroids and transforaminal epidural steroid injection.

  • Recent advances in the understanding of the pathophysiology of spinal cord injury (SCI) and new therapeutic approaches have provided promising results for this incurable and debilitating central nervous system injury. Various neuro-protective and neuro-regenerative trials have been attempted to overcome SCIs. This review summarizes the reported experimental and clinical data regarding neuro-regenerative trials with the proven pathophysiology of SCI. In addition, the prerequisites for safe and effective clinical trials are discussed.

  • Spinal hematomas are a rare but serious complication that is typically observed in the epidural space. Spinal subdural hematomas are a dangerous clinical situation because of their potential to cause significant compression of the neural elements and can be mistaken easily for spinal epidural hematomas. This paper reports a case of a severe whole lumbar subdural hematoma after a simple epidural injection that was treated with surgical decompression with excellent clinical results.
