Intramural Bronchogenic Cyst of the Esophagus - A case report - (식도 근육층에 함몰한 기관지성 낭종 - 1예 보고 -)

  • Bronchogenic cysts are usually located in the pulmonary parenchyma or mediastinum. When they are located in the mediastinum, they are usually near the bronchus or esophagus, but they are rarely at an intramural location of the esophagus. It is very difficult to differentiate between bronchogenic cyst and esophageal cyst before the operation. We report here on a patient for whom the preoperative diagnosis was supposed to be an esophageal cyst. The esophageal cyst was excised through the longitudinal division of the esophageal muscular layer without injury of the mucosa after right thoracotomy. The pathologic report revealed intramural bronchogenic cyst. We report here of a case of bronchogenic cyst that was located in the muscle layer of the lower esophagus.

  • 성인의 선천성 식도 기관지루는 드문 질환이고 그 중 Braimbridge 제III형은 매우 드물다. 본 교실은 폐내형 기관지성 낭종을 동반한 선천성 식도 기관지루(제III형) 1례를 경험하였다. 환자는 38세 남자로 내원 3일전부터 시작된 기침과 복통을 주소로 내원하였다. 식도조영술상 식도와 우폐하엽의 낭성 병변사이에 누관이 발견되었다. 누공절제술과 우폐하엽과 우폐중엽 절제술을 시행하였고 수술후 경과는 별 문제가 없었다.

  • Bronchogenic cysts are anomalous cystic lesions of foregut and usually located in the lung or mediastinum. Generally intramuscular cysts of the esophagus are considered as enterogenous foregut malformations. We report a young adult with an intramural bronchogenic cyst causing dysphagia and heartburn. It was located in the muscular layer of the esophagus and was removed without any damage to the mucosa. Histopathologic findings revealed that it was a bronchogenic cyst.

  • 저자들은 1982년부터 1987년까지 5년간 서울대학교병원 이비인후과에서 경험한 21예의 갑상설관 낭종에 관한 임상적 관찰을 하여 다음과 같은 결과를 얻었다. 발현 연령은 출생시부터 43세로 다양하였으나 21예중 10례가 10세 이전에 발생하였다. 7예에서는 경부종괴가 주소였으며 4예에서는 누공이 주소이었다. 낭종은 대부분 정중선에 위치하였으며 설골 하부에 위치한 예가 17예이었다. 전례에서 설골 중앙부를 포함한 전 병소를 제거하였으며 현재까지 재발된 예는 없었다.

  • A case of foregut cyst communicated with esophagus and lined by bronchial mucosa is reviewed and its embryologic base of maldevelopment is discussed. It is not always easy to distinguish between digestive and respiratory cyst in mediastinum. There is whole range of intermediate between a cyst with ciliated and one with squamous or columnar mucosa. Origin of this dysembryoplasia is difficult to determine when one consider that the esophagus is covered with ciliated epithelium until the eleventh week of fetal life and that ciliated growth are found on its wall until the sixth month of the fetal life. And we concluded, general agreement is that cysts which have gastric epithelium in whole or in part, represent a distinct type and should be classified as (gastro) enteric cyst, mediastinal cyst containing cartilage were considered definitely as respiratory(bronchial or bronchogenic) cyst.

  • Persistance of developmental remnants during fetal life may be attributed to congenital cysts and fistulas in the neck, which are experienced rather rarely. Recently authors have experienced three cases of progressive increased tumor mass in the lateral side of the neck and four cases of tumor mass in the suprathyroid region. We have performed surgical removal under the diagnosis of branchial cysts and thyroglossal duct cysts, respectively. The biopsy specimens were confirmed by histopathological study.

Report of eight cases of thyroglossal duct cyst and fistula (갑상선 설관낭종 및 설관루)

  • The authors have experienced 8 cases of the thyroglossal duct cysts and fistula which were diagnosed and treated surgically in the Dept. of Otolaryngology, Taegu Presbyterian Hospital during the period of last 2 years. 1. Age distribution showed at age 2∼10 years old (4 cases), and 10∼30 years old (3 cases); The youngest age of 2, and the oldest age of 42. 2. The form of disease revealed as cystic form (7 cases) and fistula form (1 case). 3. Operating procedure were excision of thyroglossal duct cyst with partial removal of hyoid bone. 4. Sex distribution 7 in female and 1 in male. 5. Histologic examination revealed as stratified squamous epithelial lining were 7 cases, its cell composed with plasma cells lymphocytes. No epithelial lining is 1 case. 6. Post-operative complications were not developed in all cases.

  • Bronchogenic cysts are usually located in the pulmonary parenchyma or in the mediastinum. When bronchogenic cysts are located in the mediastinum, they are usually near the bronchus or esophagus, and rarely located in the retroperitoneal space. It is difficult to differentiate between bronchogenic cysts and benign cysts prior to surgert. We report here on a patient for who had a mass in the retroperitoneum, with the preoperative diagnosis being a benign neurogenic tumor. Via left open thoracotomy, pathologic reports revealed that the mass was a bronchogenic cyst. We report here on the case of a bronchogenic cyst that was located in the retroperitoneal space of the diaphragm.

A Case of Dentigerous Cyst of the Maxilla (상악골에 발생한 함치성낭종 1예)

  • The dentigerous cyst involving maxilla is rare disease in Otolaryngologic field. A case of 22-year-old male patient involving dentigerous cyst of the maxilla with chronic inflammatory signs has been presented. The occurrence of odontogenic epithelium in the wall of dentigerous cyst is a well known entity. This epithelium usually remains inactive and does not have clinical significance, but these small inactive islands of epithelium may be stimulated, resulting in chronic inflammation, ameloblastoma and squamous cell carcinoma. Therefore correct diagnosis and proper treatment are very important. The authors have recently experienced a case of dentigerous cyst causing a mild disfigulation of face. The cyst was surgically enucleated through sublabial approach.
