  • Esophageal perforation, regardless of the etiology, is a catastrophic event. The importances of early diagnosis and an aggressive surgical approach in the management of such a potentially lethal situation are stressed, in fact the mortality rate is directly related to the interval between perforation and initiation of treatment. We experienced a rare case of esophageal rupture caused by compressed air which produce a rupture of the colon not infrequently, which was treated successfully by an aggressive surgical approach consisting of closure of the perforation and adequate drainage.

압력 상해에 의한 식도파열 -1례보고- (Barotraumatic Rupture of The Esophagus -A Case Report-)

  • Pneumatic rupture is a rare cause of esophageal injury, as evidenced by only 19 cases reported in the literature. We experienced one case of esophageal rupture due to bursting of a truck inner tube. The patient, who was a 45-year old male, had severe chest pain, respiratory distress, flushing in the face and neck, and subcutaneous emphysema after tire explosion. Three days after the incident, a diagnosis of rupture of the thoracic esophagus was established by esophagogram using water soluble contrast media, and then emergency operation was done. The operation involved mediastinal and thoracic drainage and resection of the esophagus combined with cervical esophagostomy and feeding gastrostomy. On the 105th day after the operation, cervical esophagogastrostomy via substernal route was performed. The patient was successfully treated with the staged operations. As in the other reported cases, the injury was located in the lower one third of the esophagus. Four main characteristics of the clinical signs of pneumatic rupture are 1] wounds or burns to the face or mouth, 2] chest pain or epigastric pain, 3] subcutaneous emphysema, and 4] respiratory distress. We emphasize that the high index of suspicion of esophageal rupture is very important in diagnosis and that diagnosis should be based on the same findings common to other forms of esophageal injury.

흉부둔상에 의한 식도파열의 성공적인 보존적 치료 (A successful conservative management of traumatic thoracic esophageal rupture)

  • Thoracic esophageal rupture caused by blunt trauma is often not recognized until late because of the vague symptoms in the initial state as well as its rare incidence, which can easily lead to fulminant mediastinitis with frequent fatal outcome. Once extensive mediastinitis occurs, the primary surgical repair of the esophageal tear is considered to be practically impossible. Various methods have been proposed for the management of these desperately ill patients, but no one provides an acceptable good result yet. The purpose of this article is to report the successful result obtained in the treatment of a patient with fulminant mediastinitis from traumatic esophageal rupture by continuous transesophageal irrigation. A 27 year-old male patient was brought to the emergency room of our hospital complaining of dyspnea and chest pain after blunt trauma. The diagnosis of esophageal rupture in the thorax was made late, about 46 hours after the initial injury, when mediastinitis had already progressed. The transesophageal irrigation method was immediately instituted which consisted of profuse transesophageal irrigation of the mediastinum with orally ingested fluid and/or by Levin tube, positioned proximal to the site of the rupture, and drainage of the irrigation fluid by thoracoscopically accurately positioned chest tubes connected to a well suctioning system. With subsiding inflammatory signs and symptoms, the esophagogram, obtained 54 days after the treatment, showed no evidence of the mediastinal leakage of contrast material which contrasted previous esophagograms with definitive dye collections in the mediastinum. Additional endoscopic finding confirmed complete healing of the esophageal mucosa, previously ruptured. He has been followed up without any problem until recently, 6 months after discharge.

흉부식도 파열에 대한 외과적 치험 - 4례 보고 - (Surgical managements of Thoracic Esophageal Rupture - 4 Cases Reports -)

  • Perforating injury of the thoracic esophagus leads to an almost Immediate exposure of the mediastinum to both oral secretions and refluxed gastric contents. And necrotizing inflammatory response developed inevitably. So, without proper managements, mediastinitis results in severe dehydration or septic conditions, and consequently the life of the patient can be threaten. We experienced recently 4 cases of esophageal perforation. Three cases were Boerhaave's syndromes and the other one was instrumental perforation in patient with corrosive esophageal stricture. Curative surgical procedures were done in 3 cases and the drainage procedure was only possible in the other one due to hepatic failure. And the patient with the drainage management was dead.

  • 저자들은 발열과 끙끙거림을 주소로 내원한 2개월 된 소아에서 빈도가 드문 선천성 식도 중복의 감염, 파열로 인하여 농흉, 유미흉과 종격동염을 초래하고 이로 인하여 대동맥류를 초래한 1례를 경험하였기에 보고하고자 한다. 본 증례의 희귀함과 종격동염, 농흉, 대동맥류와 같은 다양한 합병증의 발생은 식도 중복을 진단하는데 있어서 어려운 점이었다.

  • Even in the hands of the expert endoscopists, an occasional instrumental perforation of the esophagus occurs. But instrumental perforation of the esophagus should not be difficult to diagnose if the possibility is borne in mind. Occasionally patient with esophageal perforations show little reaction at first, but usually they develop systemic manifestation if surgical management is delayed. Early surgical drainage of esophageal perforation is very important & effective therapeutic method. The delayed surgical treatment of esophageal perforation would have increased the morbidity & mortality by allowing mediastinitis & empyema thoracis. We have experienced 6 cases of delayed surgical management of instrumental perforation of esophagus from May 1974 to April 1986 in the department of thoracic and cardiovascular surgery, Yonsei University, college of the medicine. The ages ranged from 4 years to 57 years. The underlying esophageal diseases consisted of esophageal stricture in 3 cases, foreign bodies in the esophagus in 2 cases and esophageal ca. in one case. Most clinical manifestations on admission were high fever, chest discomfort, chest pain, dysphagia and subcutaneous emphysema. Most complications due to esophageal rupture were acute mediastinitis with or without empyema thoracis. Failure to diagnose promptly and failure to promptly institute adequate treatment undoubtedly were largely responsible for this patients death. All 6 patients had been taken delayed surgical drainage more than 24 hours following esophageal perforation. One patient had been in the open drainage state for long time and the another patient has been in the tracheostomy with postintubation vocal cord ulceration. The third patient died due to respiratory failure and sepsis due to fulminant mediastinitis & empyema thoracis. Even if the patients with esophageal perforation have been taken delayed surgical management, the patients should be survived with aggressive & effective surgical drainage with intensive post-operative care.

식도 파열의 수술적 치료 (The Surgical Treatment of Esophageal Perforation)

  • Background : It is well-known that esophageal perforation (EP) is difficult in diagnosis and has high mortality rate despite proper management. There are disputes in regarding the reatment in cases of delayed diagnosis although in the early diagnosed cases, operation is recommended without arguments. Methods: From April, 2001 to December, 2004, nine patients who were diagnosed as EP in our hospital were analyzed retrospectively about the causes, the interval between the cause and the treatment, and operation methods. Results: There were 8 male and one female with men age of 49.3 years (range: 25-67 years). The causes of EP included perforations following operations of corvical spine in three cases, spontaneous perforation(Boehaave syndrome) in two cases, foreign bodies in two cases, operation of esophageal diverticulum in one case and blunt trauma bytraffic accident in one case. Mean interval between the first treatments and the causes was 11.6 days (range: 2-30 days). The sites of perforation were upper third of esophagus in three cases, middle third in three cases and lower third in three cases. All except two cervical cases presented as mediastinitis or empyema at the time of diagnosis. Primary repair and irrigation had been performed in 7 cases but five cases out of them required more than two procedures. Conclusions : More than one procedure wasrequired in the treatment of EP because of contaminations and infections which had been spread at the time of initial manifestatios, howeverprimary closure and massive irrigation is the best method in order to preserve esophagus unless the remaining esophagus is extensively damaged.

둔상에 의한 경부 식도의 파열 (Rupture of the Cervical Esophagus from Blunt Trauma)

  • Rupture of the esophagus after blunt trauma is a rare event. But any type of esophageal rupture has the high morbidity and mortality rate. In these situations, the sign and symptom of the esophageal rupture is subtle and nonspecific; therefore, the physicians are usually not suspicious. Delaying in diagnosis prevents proper treatment (surgical or non-surgical) before significant complications occurred. We report a case of a cervical esophageal perforation with primary repair and drainage after blunt trauma.

  • 1923년 Pierre-Robin이 소하악증, 설하수증 및 고궁구개(high-arched palate)혹은 구개 파열의 3주징을 처음으로 기술하였다. 기도장애가 자주 발생하고, 신속하며 적절한 치료를 필요로 한다. 이 증후군은 출생아 5만명 중 1명 정도로 드물게 발생한다. 이 증후군의 원인은 불명확하지만 하악골발달의 일차적 부전 때문으로 알려져 있다. 소하악증으로 인해 혀의 기저부가 후방으로 이동하여 인두후벽에 접근하여 기도폐쇄의 증상이 발생하며 이는 영아가 앙와위를 취할 때 악화된다. 환자를 복와위로 유지시킴으로서 경한 기도폐쇄의 문제들은 해결되지만, 섭식장애가 있을 경우에는 기도폐쇄가 악화되므로 빨리 설고정술을 실시하는 것이 좋으나 기관절개술은 최대한 피하는 것이 좋다. 설고정술에는 Douglas 수술법, Kirschner wire 삽입법 및 Duhamel 봉합법 등이 있다고 알려져 있다. 저자들은 최근 호흡곤란 및 섭식장애를 주소로 내원한 출생 2개월 된 남아에서 소하악증, 설하수증 및 고궁구개 (high-arched palate)를 확인하였으며 Duhamel 봉합법으로 기도폐쇄의 증상이 호전되지 않아 기관절개술을 시행한 후 관찰 중인 이 환자를 Pierre-Robin Syndrome으로 생각하기에 보고하는 바이다.

Blunt Trauma 에 의한 기관-식도 파열의 치험 1례 (Rupture of the Trachea and the Esophagus Following Blunt Trauma: A Report of a Case)

  • This is a case report of the rupture of the trachea and the esophagus following external blunt trauma without any associated injury. A 7 year old male patient was brought to the emergency room. Hanyang University Hospital, on 23 Jul, 1974 while he played under the pile of pingpong table A table was fall down over the patient accidentally. The patient was injured by the table and found lying in unconsciousness on the ground.There was subcutaneous emphysema on his anterior chest and neck. A contusion on the upper part of the sternum was noted. The breathing sound were diminished over the left side of the chest. Ronchi were heard over the right lung field. This patient was suffered from vomiting, dypsnea, and irritable mental state after this accident. On the chest roentgenogram in A-P view, hyperlucency at the mediastinum and parapericardiac area suggested the pneumomediastinum. On the next day, a diagnosis of the tracheal and esophageal rupture was confirmed by the esophagogram with Lipiodol swallowing. A right thoracotomy was performed and ruptured orifice of the trachea and the esophagus were closed with interrupted sutures. Postoperative course was uneventful and primary closure of the rupture of the trachea and the esophagus were succeeded.
