  • Plummer-Vinson syndrome(Paterson-Kelly syndrome) is characterized by dysphagia due to upper esophageal or hypopharyngeal web, iron deficiency anemia, and atropic oral and glossal mucosa. This syndrome is usually known as precancerous lesion that develops into postcricoid carcinoma. Universally, the clinical manifestations of this syndrome were markedly improved after oral iron replacement therapy or endoscopic balloon or electrocautery treatment. 63 year-old woman was received a short segment, free jejunal transfer to be released from esophageal stricture. After the operation, the stenotic lesion proved to be Plummer-vinson syndrome with carcinoma in situ by pathologic study.

Surgical treatment of esophageal submucosal tumor (식도의 양성 점막하 종양의 수술적 치료)

  • Background: Benign tumors of the esophagus are rare. They include leiomyomas, gastrointestinal stromal tumors, neurofibromas and lipomas. In this study we present our experience with enucleation of these 13 tumors for 10 years. Material and Method: A retrospective review of patients who underwent enucleation of benign esophageal tumors between 1995 and 2005 was conducted. Symptom, tumor location and size, operative approach and outcomes after surgery were recorded. Result Thirteen patients were identified(leiomyoma: n=12; GIST n=l). Eight patients were men, five patients were women. Most of them were 4rd and 5th decade. The tumors arose in the lower(7 patients) and middle(6 patients) thirds of the esophagus. Eleven patients underwent a thoracotomy; the remainder were resected using VATS. All of patients underwent simple enucleation. There were no complications and recurrences after surgery. Conclusion: We present our experience with enucleation of these 12 leiomyomas and one GIST for 10 years.

Preliminary Report of the $1998{\sim}1999$ Patterns of Care Study of Radiation Therapy for Esophageal Cancer in Korea (식도암 방사선 치료에 대한 Patterns of Care Study ($1998{\sim}1999$)의 예비적 결과 분석)

  • [ $\underline{Purpose}$ ]: For the first time, a nationwide survey in the Republic of Korea was conducted to determine the basic parameters for the treatment of esophageal cancer and to offer a solid cooperative system for the Korean Pattern of Care Study database. $\underline{Materials\;and\;Methods}$: During $1998{\sim}1999$, biopsy-confirmed 246 esophageal cancer patients that received radiotherapy were enrolled from 23 different institutions in South Korea. Random sampling was based on power allocation method. Patient parameters and specific information regarding tumor characteristics and treatment methods were collected and registered through the web based PCS system. The data was analyzed by the use of the Chi-squared test. $\underline{Results}$: The median age of the collected patients was 62 years. The male to female ratio was about 91 to 9 with an absolute male predominance. The performance status ranged from ECOG 0 to 1 in 82.5% of the patients. Diagnostic procedures included an esophagogram (228 patients, 92.7%), endoscopy (226 patients, 91.9%), and a chest CT scan (238 patients, 96.7%). Squamous cell carcinoma was diagnosed in 96.3% of the patients; mid-thoracic esophageal cancer was most prevalent (110 patients, 44.7%) and 135 patients presented with clinical stage III disease. Fifty seven patients received radiotherapy alone and 37 patients received surgery with adjuvant postoperative radiotherapy. Half of the patients (123 patients) received chemotherapy together with RT and 70 patients (56.9%) received it as concurrent chemoradiotherapy. The most frequently used chemotherapeutic agent was a combination of cisplatin and 5-FU. Most patients received radiotherapy either with 6 MV (116 patients, 47.2%) or with 10 MV photons (87 patients, 35.4%). Radiotherapy was delivered through a conventional AP-PA field for 206 patients (83.7%) without using a CT plan and the median delivered dose was 3,600 cGy. The median total dose of postoperative radiotherapy was 5,040 cGy while for the non-operative patients the median total dose was 5,970 cGy. Thirty-four patients received intraluminal brachytherapy with high dose rate Iridium-192. Brachytherapy was delivered with a median dose of 300 cGy in each fraction and was typically delivered $3{\sim}4\;times$. The most frequently encountered complication during the radiotherapy treatment was esophagitis in 155 patients (63.0%). $\underline{Conclusion}$: For the evaluation and treatment of esophageal cancer patients at radiation facilities in Korea, this study will provide guidelines and benchmark data for the solid cooperative systems of the Korean PCS. Although some differences were noted between institutions, there was no major difference in the treatment modalities and RT techniques.

DOUBLE PULMONARY BLASTOMAS - A CASE REPORT - (다발성폐아세포종 -수술치험 1례 -)

  • Pulmonary blastoma is rare malignant neoplasm usually situated peripherally in lung and composed of epithelial and mesenchymal components that resemble the lung at about 3 months gestation. Fortysix year-old man had no symptoms and signs, but solitary pulmonary nodule was found accidentally in a routine simple chest X-ray occupying right upper lung field. Preoperative chest CT-guided aspiration cytology revealed adenocarcinoma. We had done right upper lobectomy with Iymph node dissecton therapeutically. Final postoperative pathology reported this tumor as double pulmonary blastomas(well differentiated fetal adenocarcinoma). We report a case of pulmonary blastoma with review of literature.

상부기도 협착의 나선식 CT를 이용한 3차원 영상의 진단적 의의

  • 상부기도 협착의 원인은 장기간의 기관 삽관, 외상, 감염, 종양, 및 선천적인 결함등에 의해 존재하며, 이에 대한 진단은 단순 촬영이나 전산화 단층 촬영술 등으로 쉽게 진단은 가능하나, 적절한 치료 계획을 세우기 위해서는 그 범위 및 정도를 정확히 파악하는 일이 중요하다 최근 방사선 촬영기술의 발달로 현재의 2차원적 단면 영상에서 3차원적 영상으로 발전해왔으며 상부기도 협착 또한 3차원적 영상으로 진단하려는 노력이 시도되고 있다. 이에 저자들은 최근에 경험한 기관 협착증 7례에서 3D CT를 시행하여 협착 부위의 상태를 기존의 단순 촬영 영상 및 2D CT 영상과 비교하였고, 7례의 기관 협착증 중 4례에서 기관 단단 문합술 및 후두 기관 문합술을 시행한 바 수술시 확인된 협착 상태를 3D CT 영상과 비교하였다.

  • Occult bronchial foreign body is that long-standing foreign body lodge in bronchial tree. Occult bronchial foreign bodies arc rare in adults, whereas tracheobronchial aspiration of foreign bodies occurs commonly in children. A 65-year-old man with chronic cough, sputum production, and fever was transferred for treatment of right middle and lower lobc collapse and obstructive pneumonitis as evidenced by imaging studies. The patient was treated with right middle-lower bilobectomy because fiberoptic bronchoscopic removal of the foreign body failed. We report this case with review of literatures.

  • Subcutaneous cervicofacial, mediastinal emphysemas are complications associated with head and neck surgery, trauma, infectious processes, tooth extraction. Drill cooling stream and dental syringe air ject are the sources of high pressure air that may enter exposed soft tissue. Since the introduction of the high-speed air turbine drill in the 1960s, The incidence of iatrogenic subcutaneous emphysema has increased. Most cases begin to resolve after 2 to 3 days and residual swelling is usually minimal at the end of 7 to 10 days. Surgical approach is not advised because it is likely to be ineffective. The differential diagnosis of neck swelling after dental procedure includes hematoma, cellulitis, angioedema, allergic reaction, subcutaneous emphysema. We report a rare case of patient with subcutaneous cervicofacial emphysema and mediastinal emphysema secondary to third molar extraction.

  • Foreign body in the air passage is not uncommon in the field of otolaryngology. The majority of bronchial foreign bodies can be removed by bronchoscopy, but some cases may require surgical procedure. Recently, we experienced an impacted foreign body (ball-pointed pen cap) in the left main bronchus with pneumothorax on the right side. Authors tried bronchoscopic removal under the general anesthesia but failed. Lateral thoracotomy was performed by chest surgeon and foreign body was successfully removed.

  • We managed surgically a case of local recurrence in esophageal cancer Twenty month after transthoracic subtotal esophagectomy and csophago-gastrostomy, he su(fared from dysphagia. Chest CT and percutaneous needle aspiration biopsy showed . Local recurrence involving residual esophagus, thyroid gland, posterior membraneous portion of trachea. We did cervical esophagectomy, laryngectomy thyroidectomy, partial resection of trachea and reconstruction with free jejunal antograft successfully.

Surgical Management of Esophageal Carcinoma after Esophageal Prosthesis (인공 식도를 삽입한 식도암 환자의 수술치험 4례)

  • Effective relief of dysphagia in unresectable esophageal cancer patients not only prolong survival but also is prerequisite for other treatment modalities.We tried surgical resection in 4 patients who had unresectable esophageal cancer,managed initially with esophageal prosthesis then followed by chemotherapy with good response. Curative resection was possible in 2 patients and palliative resection in one and exploration only in one case.Curatively resected patients are alive without recurrence for 57 months,8 months,each.Other non-curatively resected patients died after 10 months,7 months. Above results suggest that initial esophageal prosthesis to relieve dysphagia followed by chemotherapy in unresectable esophageal cancer may provide another chances for curative resection that promise better survival and need more clinical experience and trial.
