Ecdysteroid Titer during Metamorphosis and the Effect of Ecdysteroid on Oocyte Develoment on Phormia regina (검정금파리의 변태기에 따른 엑디스테로이드와 난세포성숙에 미치는 엑디스테로이드의 효과)

  • The ecdysteroid titers of representive developmental stages of the blackblow fly, Phormia regina, were determined by radioimmunoassay and the effect of ecdysteroid on the oocyte maturation was investigated. Prior to every molts ecdysteroid levels began to increase sharply, suggesting ecdysteroid was the major component for egg-larval, larval-larval, and larval-pupal transformation. A difference in the levels of ecdysteroid between male and female was ob¬served during adult life span. Following the protein meal, ecdysteroid in the females increased rapidly to a maximum at 96 hr of age when terminal oocyte fully matured. Effect of ecdysteroid on oocyte development was determined for control and ecdysone-treated female flies after the liver-feeding. The growth of oocyte in the flies treated by $\mu$g of ecdysone, along with the control flies, was not facilitated. When the flies treated by 5 $\mu$g of ecdysone, however, duration of oocyte maturation was shorter than those of other two groups. This can be suggested that oocyte development in P. regina is due to the critical level of ecdysone.

  • 1980년대 초반에 'Neurosteroid'라는 개념이 소개 되면서, 지난 20년간 이 분야에 많은 연구결과들이 쌓이게 되었다. 그리고, neurosteroid는 과거에 알고 있었던 스테로이드들과는 다른 방법으로 생물체에 생리적 작용을 하는 것으로 알려졌으며, 특히 신경계 내에서는 신경전달물질과 같은 작용을 한다는 것이 알려지면서 신경계 내에서의 자체적인 스테로이드 호르몬 합성에 관한 연구들이 본격화되기 시작했다. 지난 10년 사이에 많은 review 논문들이 발표되고 있지만, neurosteroid와 신경계의 특성상스테로이드 호르몬의 신경계에 미치는 기능에 대한 연구가 쉽지 않으며, 정밀하게 조절되는 신경계의 스테로이드 함량변화를 조절하는 합성 효소에 대한 연구는 아직도 초보적인 단계에 머물러 있다. 이에 본 논단에서는 neurosteroid에 대한 소개와 함께 최근 우리실험실의 연구결과를 소개하고자 한다.

The Effect of Steroid on Renal Involvement in $Henoch-Sch\ddot{o}nlein$ Purpura ($Henoch-Sch\ddot{o}nlein$ 자반증에서 스테로이드 사용이 신장 침범에 미치는 영향)

  • Purpose : $Henoch-Sch\ddot{o}nlein$ Purpura(HSP) is a self-limited systemic small vessel vasculitis, however, renal involvement is considered to contribute to the outcome of this disease. Therefore, identifying the renal risk factors in HSP and prevention of renal involvement are important. The aim of this study is to investigate whether early steroid administration in HSP could reduce the rate of renal involvement. Methods : We retrospectively studied two hundred children with HSP. We had administrated steroids orally to resolve of severe abdominal pain, joint and scrotal symptoms. We analyzed the relationship between the steroid therapy to relieve systemic symptoms and the subsequent renal involvement in HSP. Results : There were no significant differences in the incidence and duration of renal involvement according to steroid administration and its duration. In HSP patients with renal manifestations, steroid administration group showed a tendency of hematuria and steroid non-administration group showed a tendency of proteinuria, however, we could not find statistically significant differences in each group. There was no significant difference in the duration of purpura presence according to steroid administration. However, persistent purpura increased the incidence and the duration of renal involvement. Conclusion : Early steroid administration did not reduce the risk of renal involvement, there-fore, steroid could not prevent delayed nephritis in children with HSP On the other side, Persistent Purpura, known to be not related to steroid therapy, was associated with renal involvement. We suggest that early steroid administration could not be useful in preventing the renal involvement in HSP. (J Korean Soc Pediatr Nephrol 2007;11:185-194)

  • Toxic epidermal necrolysis (TEN) and Stevens Johnson syndrome (SJS) are rare, life-threatening mucocutaneous -diseases, usually attributable to drugs and infections. Corticosteroids have been used in the management of TEN for the last 30 years. This remains controversial and is still much debated. TEN can occur despite administration of high doses of systemic corticosteroids. The psychiatric side effects of corticosteroids can include headache, insomnia, depression, and mood disorders with or without psychotic episodes. Steroid-induced psychosis is dealt with by tapering or discontinuing the steroid; antipsychotics are also sometimes used. We report a case of an 11-year-old boy who was admitted with TEN. He had also been diagnosed as having nephrotic syndrome in the past. Remission was achieved through induction therapy and by maintaining the use of steroids. After a full-dose intravenous dexamethasone for TEN, he showed psychotic symptoms. We diagnosed him as having steroid-induced psychosis. We tapered the steroid use and initiated an atypical antipsychotic medication, olazapine and intravenous immunoglobulin (IV-IG). His symptoms dramatically improved and he was discharged.

Evaluation of Steroid Therapy in Tuberculous Pleurisy - A prospective, randomized study - (결핵성 흉막염에서 스테로이드의 치료 효과 - 전향적, 임의추출법에 의한 비교 -)

  • Background : Tuberculous pleurisy has been treated With a combined regimen of corticosteroids- antimicrobial therapy. But whether such combination therapy add to benefits is unknown. We evaluate the effects of corticosteroid and its routine application in relief of clinical symptoms, absorption of pleural effusion, and pleural adhesions. Methods : A prospective, randomized study of the role of corticosteroid in the treatment of tuberculous pleurisy was performed in 83 patients(nonsteroid group: 50 patients, steroid group: 33 patients) from June, 1991 to September, 1994. Results : 1) The mean duration from symptoms(fever, chest pain, dyspnea) to relief was 3.8 days in the steroid group, and 7.4 days in the nonsteroid group(P<0.05). Clinical symptoms including fever, chest pain, sputum and weight loss were relieved more rapidly in the steroid group than other symptoms(weakness, night sweating and dyspnea). 2) Pleural effusion was taken an averge of 88 days in the steroid group and 101 days in the nonsteroid group 10 be absorbed completely(p>0.05) 3) The incidence of pleural adhesions was 17/33(5l.5%) in the steroid group and 32/50(64%) in the nonsteroid group(p>0.05) 4) Side effects of corticosteroids were observed in only one patient causing epigastric pain and discontinuation of drug. Conclusion : Corticosteroid exert benefitial role in the more rapid relief of clinical symptoms to patients with tuberculous pleurisy, but absortion of pleural effusion and occurrence of pleural adhesions was not influenced significantly Therefore, its routine application should be reevaluated.

  • 아나보릭 스테로이드인 nandrolone phenylpropionate(NPP)가 인슐린에 의한 지방축적의 증가 및 절식시 혈중 유리지방산 농도에 미치는 영향을 암컷쥐에서 조사하였다. 인슐린 투여는 식이 섭취량을 유의적으로 증가시켰는데 이는 인슐린에 의한 혈당 저하에 의한 것으로 보인다. 지방 축적은 인슐린 투여에 의해 유의적으로 증가하였으나 체단백질이나 근육단백질 함량은 변화하지 않았다. 인슐린에 NPP를 동시에 투여하였을 때 NPP는 인슐린에 의한 저혈당증과 지방축적의 증가를 부분적으로 완화시켰으며 체단백질과 근육단백질 함량을 유의적으로 증가시켰다. 반면 NPP는 절식시킨 쥐의 혈중 유리 지방산 농도에는 영향을 미치지 않았다. 이러한 결과를 통해 아나보릭 스테로이드가 단백질 대사에 미치는 영향은 인슐린과 무관하며, 아나보릭 스테로이드의 지방축적 억제효과는 아나보릭 스테로이드가 지방분해에는 영향을 미치지 않고 인슐린에 의한 지방함성으로의 당이용도를 저해함으로 발휘된다는 것을 알 수 있다.

Comparison of Efficacy of Steroid Oint with Different Potency in Phimosis (생리적 포경 치료를 위한 역가가 다른 스테로이드 연고의 효과 비교)

  • Purpose : Phimosis is the inability to retract the foreskin of the penis over the glans of the penis. Even though phimosis is not pathogenic, the presence of phimosis is known to increase the risk of urinary tract infection in infancy. The use of topical steroids has been advocated as a safe and economical alternative to surgical intervention. The purpose of the study was to assess the effectiveness of topical steroids for the treatment of phimosis in young boys. Methods : A total of 53 boys with phimosis were divided into two groups to distinguish between the efficacy of low-potency topical steroid(group 1, 27 cases) and of high-potency topical steroid (group 2, 26 cases). Results : The success rate increased in the process of time. After 6 weeks, the success rate was 21 cases in group 1 and 22 cases in group 2. There was no significant differences in treatment outcomes between the two groups. The age of the boys, the appearance of prepuce prior to treatment, and the presence of previous symptoms, including symptoms of UTI, and ballooning of the prepuce at micturation had no significant difference on treatment outcomes. Conclusion : Topical steroid was effective in treatment of phimosis in boys younger than 4 years old. Although treatment outcomes between low- and high-potency topical steroids were not significantly different, those treated with a high-potency steroid cream showed more fast improvement. Further studies are necessary to assess systemic side effects of steroid treatment, duration of treatment and prevention of genitourinary tract infection in male infants.

Topical Steroid Therapy using Stent on Chronic Ulcerative Gingival Lesions (스텐트를 이용한 만성 궤양성 치은 병소의 국소 스테로이드 치료)

  • The majority of chronic gingival ulcerative lesions are known to be due to autoimmune disease such as oral lichen planus, benign mucous membrane pemphigoid or pemphigus vulgaris. Topical or systemic corticosteroids are mainly used and adjuvant drugs like immunosuppressant, anti-inflammatory drugs, antimalarials or antimetabolites can also be prescribed. Because systemic corticosteroids causes various side effects, such as gastrointestinal disturbance, osteoporosis, diabetes or adrenal suppression. So, topical steroid therapy is main treatment for chronic gingival ulcerative lesion confined to small area. However, there's also limitation of topical corticosteroids. The effect of the corticosteroids decreases due to salivary flow and the movement of the tongue, lips, or buccal mucosa. When the lesions are widely distributed or positioned deeply in oral cavity, it is hard to apply the medication on patients' own. Moreover, it can be applied to unaffected mucosa. Although occlusive steroid therapy using stent was reported to minimize taking steroid and overcome the faults of applying topical steroids, it has been used less frequently in the clinical field. Therefore, this report is going to find out the usefulness and the way to utilize clinically through the case which acted satisfactorily by performing topical steroid therapy using stent on chronic ulcerative gingival lesions.

Therapeutic Effect of Plasmapheresis in Relapsed Focal Segmental Glomerulosclerosis after Steroid Pulse Therapy (스테로이드 충격요법 후 재발된 국소성 분절성 사구체 경화증에서 혈장반출법의 치료 효과)

  • Purpose : Focal segmental glomerulosclerosis(FSGS) is known to have a poor prognosis and a poor response to treatment. We performed a combination therapy of plasmapheresis, steroid pulse and immunoglobulin in 4 patients with relapsed steroid dependent(SD) or steroid resistant(SR) FSGS after steroid pulse therapy. Materials and Methods : 8 cycles of plasmapheresis were performed in 4 patients with biopsy proven FSGS who had relapsed after steroid pulse therapy from March 1988 to July 2002. Clinical findings and treatment courses were reviewed retrospectively. Results : Among the 4 patients, there were 3 males and 1 female. After 8 cycles of plasmapheresis, clinical remissions were obtained. Two of the four patients had two relapses and received 2 more cycles of plasmapheresis which resulted in remissions. One of these patients had two further relapses and was treated with oral steroid resulting in clinical remission. Three patients have maintained normal serum creatinine level and glomerular filtration rates during the follow-up period of 10 years, and the other 1 patient for 5 months. Conclusion : A combination therapy of plasmapheresis, steroid pulse and immunoglobulin led to a complete remission in patients with FSGS who were SD or SR and was effective in maintaining normal renal function.
