Hoarse Speech Analysis Using Dissymmetric Four-Mass Model of Vocal Cords (비대칭 4 질량 성대 모델에 의한 쉰목소리 분석)

  • In this paper, a new vocal cords model, called a four-mass model, is proposed for a hoarse speech mechanism. Pathological changes of vocal cords cause hoarse speech and glottal waveform reflects motion states of vocal cords. From these facts, we assumed that the morbid vocal cords be dissymmetric and take the four-mass type. The glottal waveforms and the model parameters of normal and hoarse speech signals are analyzed, and some relations bet ween the model parameters and the hoarse pathology are discussed. Experimental results show that the new research method of hoarse speech can reveal relations between the acoustic features of hoarse speech and the hoarse pathology, and be used to diagnose laryngeal diseases and to improve tone quality of hoarse speech.

The Effect of Anticholinergic Agents on Postoperative Sore Throat in Patients Undergoing Endoscopic Sinus Surgery (내시경적 부비동 수술 시 항콜린성제제 사용이 수술 후 인후통 발생에 미치는 영향)

  • Background : After sinus surgery, airway dryness is made worse because patients breathe by mouth. Anticholinergic agents have antisialogogue effects. The airway dryness that occurs after surgery is severe and can increase the incidence of postoperative sore throat. Materials and Methods : One-hundered patients who were scheduled for endoscopic sinus surgery(ESS) were divided into four groups. Each group was premedicated with glycopyrrolate or had no premedication and reversed with glycopyrrolate or atropine. After the surgery the patients were asked about a sore throat and hoarseness postoperatively after 1h, 6h, 12h, 24h, and 48hours, respectively. Results : The incidence of postoperative sore throat and hoarseness was not significantly different among the four groups at any time. Conclusion : The severity of complications after ESS was low and subsided within 24 hours. Therefore, the use of anticholinergic agents does not need to be limited.

Finding Report of Unilateral Vocal Cord Paralysis Using Computed Tomography (컴퓨터단층촬영술을 이용한 수술 후 편측 성대마비의 진단보고)

  • VCP (Vocal Cord Paralysis) is rare but one of most serious complications related to endotracheal intubation. This report is a clinical experience of radiography and laryngeal EMG (Electromyography) assessment for the VCP. A 50-year-old woman with hoarseness, which was occurred after urethral diverticulum excision was examined by laryngoscopy. As a result of laryngoscopy, VCP was observed in left side of her vocal cord, and then recurrent laryngeal nerve damage was detected with additional CT (Computed tomography) scan and laryngeal EMG. After that, the vocal cord movement was recovered as normal state with regular conservative treatment for the 6 months.

  • Smith-Magenis syndrome (SMS) is a clinically recognizable multiple congenital anomaly and mental retardation syndrome caused by an interstitial deletion of chromosome 17 p11.2. Physical features include short stature, characteristic facial appearance: flattened mid-face, down-turned mouth, prominent and often rosy cheeks; prominent jaw in older children and adults, chronic ear infections, hearing impairment, eye problems, including: strabismus (an eye which turns in or out) and myopia (nearsightedness), hoarse voice, short fingers and toes, heart defects or murmurs, problems related to the urinary system, scoliosis (curvature of the spine), an unusual gait (walking pattern), and decreased sensitivity to pain. Behavioral and developmental characteristics include speech delay and articulation problems, developmental delay, learning disability, mental retardation, hyperactivity, self-injury, including: head banging; hand biting; picking at skin, sores and nails; pulling off finger- and toenails; inserting foreign objects into ears, nose, or other body orifices, explosive outbursts, prolonged tantrums, destructive and aggressive behavior, excitability, arm hugging or hand squeezing when excited. This report is the case of a Korean 3-year-3-month old male with Smith-Magenis syndrome referred from local clinic for the treatment of dental caries. The patient was treated by physical restraint after prophylatic administration of antibiotic(Amoxacillin 50mg/kg).
