A Case of Parahilar Lymph Node Enlargement (폐문부의 림프절 종대 1예)

  • Sarcoidosis, a multisystemic granulomatous disorder of unknown causes, which presents with bilateral hilar adenopathy, pulmonary infiltration, and cutaneous, ocular, bones, and nervous and reticuloendothelial systems involvement, commonly involves young adults of both sexes. Herein, the case of a 70-year-old male, with progressive hoarseness of two weeks' duration and mild dyspnea, is reported. A fiberoptic bronchoscopy, performed to investigate the hoarseness, revealed paralysis of the left vocal cord, but with no other local abnormality. Two nodules, as pathologic findings, showed noncaseating epithelioid cell granulomas. We note a rare case of sarcoidosis, with vocal cord palsy, in Korea.

The Effect of Intracuff Pressure Adjustment on Postoperative Sore Throat and Hoarseness after Nitrous Oxide and Air Anesthesia (아산화질소(N2O)와 공기(Air)를 사용한 마취 수술의 기낭압 조정이 수술 후 인후통과 쉰목소리에 미치는 영향)

  • Purpose: To investigate the differences in postoperative sore throat and hoarseness by adjustment of endotracheal tube cuff pressure (CP) during nitrous oxide ($N_2O$) and air anesthesia. Methods: A one-equivalent control group pretest-posttest design was used. Data were collected from August 8 to October 19, 2017 and analyzed using the independent t-test and repeated measures ANOVA. Eighty-four participants were enrolled and divided into three groups: 28 in the Control Group (CP adjusted every 30 minutes using $N_2O$), 28 in Experimental Group 1 (CP adjusted every 10 minutes using $N_2O$), and 28 in Experimental Group 2 (non-adjusted CP using air), all of whom underwent urologic, gynecologic, and orthopedic surgeries at the G University hospital. Sore throat was assessed using a numeric rating scale; hoarseness was evaluate using the Stout classification at 1, 6, and 24 hours after surgery. Results: Scores for sore throat and hoarseness were significantly different between the groups at each measurement time, and scores were consistently higher in the control group. During subsequent measurements, sore throat and hoarseness scores were significantly lower at 6 hours. Cuff pressure changed significantly using air anesthesia (${\chi}^2=10.41$, p=.015) up to 2 hours after induction. Severe sore throat and hoarseness was observed for up to 6 hours after surgery. Conclusion: Cuff pressure adjustment at short time intervals would be helpful in reducing postoperative sore throat and hoarseness. Nursing intervention focused on prevention of sore throat and hoarseness should be required up to 6 hours postoperatively in patients undergoing endotracheal intubation.

A case of congenital vallecular cyst associated with gastroesophageal reflux presenting with stridor, feeding cyanosis, and failure to thrive (협착음과 수유시 청색증 및 성장 장애를 보인선천성 후두개 낭종(vallecular cyst) 1예)

  • Vallecular cyst is an uncommon but potentially dangerous condition causing stridor and has been associated with sudden airway obstruction resulting in death due to its anatomical location in neonates. It may also present with various degrees of feeding problems resulting in failure to thrive. When a vallecular cyst is suspected clinically, endoscopic laryngoscopy is necessary to visualize it. Other conditions leading to neonatal stridor such as laryngomalacia and other laryngotracheal abnormalities should be ruled out. Marsupialization with a $CO_2$ laser is the most effective and safest treatment to prevent recurrence. We report a case of a 1-month-old male infant with a vallecular cyst synchronous with gastroesophageal reflux, and failure to thrive. He was referred to our hospital because of hoarseness, inspiratory stridor, feeding-cyanosis, chest retraction and failure to thrive. Diagnostic workup revealed a cyst at the tongue base, suggesting a vallecular cyst. The cyst was removed by laryngomicrosurgery with $CO_2$ laser. After the surgery, the symptoms improved and the body weight increased steadily. We report a successfully treated case of neonatal vallecular cyst with symptoms of upper respiratory obstruction, gastroesophageal reflux, and failure to thrive.

Voice Boosting Filter Design in Frequency Domain for Relief of Husky Voice (쉰목소리 완화를 위한 주파수 영역 음성 강조 필터 설계)

  • The people who complain of pain due to voice causes such as vocal cord nodules is increasing year by year. If the voice is changed, it is possible to give to colleagues discomfort or inconvenience during conversation. In this paper, we propose a way to reduce discomfort by improving the husky voice during the conversation. A VBF (voice boosting filter) is firstly designed to improve the husky voices. This filter may further emphasize the formant frequency components than the frequency components around the formant frequency, because the value is relatively greater than the other frequency. And a fixed-point type DSP chipset, TMS320F2812 is applied to the system, the operating frequency is 150MHz. The system was implemented as a compact for use as a portable, its size is $2.5cm{\times}10cm$. Through the test using three husky voices with some type of statement, it was satisfactory in processing speed and sound quality improvement.

Hoarse Speech Analysis Using Dissymmetric Four-Mass Model of Vocal Cords (비대칭 4 질량 성대 모델에 의한 쉰목소리 분석)

  • In this paper, a new vocal cords model, called a four-mass model, is proposed for a hoarse speech mechanism. Pathological changes of vocal cords cause hoarse speech and glottal waveform reflects motion states of vocal cords. From these facts, we assumed that the morbid vocal cords be dissymmetric and take the four-mass type. The glottal waveforms and the model parameters of normal and hoarse speech signals are analyzed, and some relations bet ween the model parameters and the hoarse pathology are discussed. Experimental results show that the new research method of hoarse speech can reveal relations between the acoustic features of hoarse speech and the hoarse pathology, and be used to diagnose laryngeal diseases and to improve tone quality of hoarse speech.

The Result of Radiotherapy in Glottic Cancer (성문암의 방사선치료)

  • A retrospective analysis of 29 patients with glottic cancer, treated at the Department of Therapeutic Radiology, Seoul National University Hospital. $97\%$ of the patients was male. Of the 29 patients, stage $T_1N_0M_0$ comprised $31\%$, $T_2N_0M_0\;52\%$, and stage $T_3N_0M_0\;14\%$. Local control rate with radical readiotherapy was $78\%$ for stage $T_1N_0M_0,\;60\%$, for stage $T_2N_0M_0$, and $50\%$ for stage $T_3N_0M_),\;57\%$ of the patients with the radiation failure was salvaged by surgery. The overall 3 year survival rate was $89\%$ for the $T_1N_0M_0,\;80\%$ for stage $T_2N_0M_0$, and $50\%$ for stage $T_3N_0M_0$. Among the survivors: $88\%$ of $T_1N_0M_0\;75\%$ of $T_2N_0M_0,\;and\;50\%$ $T_3N_0M_0$ had an intact larynx and natural voice. It is concluded that radiotherapy is a highly effective method as the primary treatment of the early glottic cancer, emphasized on preserving of the larynx and natural voice.
