Forced-air Warming System을 이용한 수술 전 가온이 복부 수술 환자의 체온, 불안, 통증 및 온도 편안감에 미치는 효과 (The Effect of Pre-warming for Patients under Abdominal Surgery on Body Temperature, Anxiety, Pain, and Thermal Comfort)

  • Purpose: The purpose of this study was to examine the effect of pre-warming on body temperature, anxiety, pain, and thermal comfort. Methods: Forty patients who were scheduled for abdominal surgery were recruited as study participants and were assigned to the experimental or control group. For the experimental group, a forced air warmer was applied for 45-90 min (M=68.25, SD=15.50) before surgery. Body temperature and anxiety were measured before and after the experiment, but pain and thermal comfort were assessed only after the surgery. Hypotheses were tested using t-test and repeated measured ANOVA. Results: The experimental group showed higher body temperature than the control group from right before induction to two hours after surgery. Post-operative anxiety and pain in the experimental group were less than those of the control group. In addition, the score of thermal comfort was significantly higher in the experiment group. Conclusion: Pre-warming is effective in maintaining body temperature, lowering sensitivity to pain and anxiety, and promoting thermal comfort. Therefore, pre-warming can be recommended as a preoperative nursing intervention.

수술전 가온과 수술중 가온이 수술환자의 체온과 전율에 미치는 영향 (The Effect of Warming Patients Before or During the Surgical Operations on the Patients' Body Temperature and Shivering)

  • Purpose: The purpose of this study is to examine the changes of surgical patients' body temperature in applying warming to patients. The study of an effective nursing intervention, which aims to prevent hypothermia during surgical operations, use of anesthesia, and to remove dermal discomforts. The nonequivalent control group pre-test/post-test design was used for this quasi-experimental study. Method: The study subjects were adult patients who would take a surgical operation under general anesthesia in C Hospital; the surgical operations done were, total abdominal hysterectomy or Myomectomy; 20 patients were included in experimental group I, 20 patients were included in experimental group II, and 20 patients were in the comparative group. The total number of study subjects was 60. The data was collected from September the 1st, 2001 to October the 20th, 2001. The data was analyzed by SPSS program, F-test and Repeated measures of ANOVA. Multi-comparison method of DUNCAN was used for the sections that show the significant differences at the level of p<.05, which was a posterior examination. Result: 1) "The body temperatures of the three groups of patients will be respectively different at the end of the operations; experimental group I to which warming was applied before the operations, experimental group II to which warming was applied during the operations, and the comparative group with no warming being given," showed (F=12.609, p=.000). 2) "Degrees of shivering symptoms for the three groups will be respectively different at the end of the operations; experimental group I which applied warming before operations, experimental group II which applied warming during operations and the comparative group with no warming." Showed assumed (F=6.626, p=.000). Conclusion: Summing up the above study, the warming assumed during operations was a more effective nursing intervention for preventing patients' hypothermia than the warming assumed before operations.

척추마취하 고관절 전치환술 환자의 수술 전 가온이 수술 중 저체온 예방에 미치는 효과 (Effect of Preoperative Warming on Prevention of Hypothermia during Surgery in Patients with Total Hip Replacement Arthroplasty under Spinal Anesthesia)

  • Purpose: The purpose of this study was to evaluate the effect of preoperative warming to prevent hypothermia in surgery for patients undergoing total hip replacement arthroplasty under spinal anesthesia. Methods: A randomized experimental study was conducted. Data were collected at an S University hospital in Gyeonggido from December 3, 2019 to March 31, 2020. A random allocation program was used to randomize participants into intervention and control groups. A total of 90 participants were assigned to the study: 30 people were randomized to a pre-warming group using Bair Hugger forced-air warming blankets(Model 505) 30 minutes before surgery, 30 to a pre-warming group 15 minutes before surgery, or 30 to a control group. The findings from 88 participants were analyzed. For data analysis, χ2 test and ANOVA were used utilizing the SPSS 21.0 program. Results: The pre-warming group 30 minutes before surgery had significantly higher body temperature than the control group, from 30 minutes after inducing anesthesia to the end of anesthesia. Body temperature over anesthesia time showed significant differences among the three groups, but there were no statistically significant differences in interactions between time and groups. Conclusion: Warming patients' body for 30 minutes before surgery was effective in maintaining normal body temperature while preventing intraoperative hypothermia.

수술전과 수술중 피부가온요법 적응이 고관절 전치환술 환자의 수술 주기 고막체온 변화에 미치는 영향 (The Effect of Pre and Intra-Operative Warming Therapy on Tympanic Temperature Changes during Perioperative Phase in Receiving Patients with Total Hip Replacement)

  • The purpose of the study was to investigate the effects of Warming Therapy used with patients consistantly before and during surgery to on changes in their body temperatures. The data were collected from patients in a university hospital in Taegu between December 1, 1998 and May 31, 1999. The subjects were selected from patients who were hospitalized for total hip replacement surgery. Thirty participants were assigned to two groups : experimental(Warming Therapy) group and control group. Each group consisted of 15 patients. The research design was a repeated measurement design, using a nonequivalent control group. The Warming Therapy, using a forced-air warming blanket, that is a, 'Bair Hugger' was applied to subjects in the experimental group. The subjects in the group were treated with the 'Bair Hugger' to warm up the whole body for 40 minutes before surgery and upper body and face during the operation. The core temperature was measured using a tympanic thermometer. The body temperature of the patients was measured 13 times every 15 minutes during the surgery. After the operation the body temperature of the patients was measured 4 times every 15 minutes, from the time of arrivial in the recovery room to the time of leaving the recovery room. The SPSS Win 9.0 program was used for data analysis. Specific methods tested were done using ${\chi}^2-test$, t-test, repeated measures ANOVA. The findings of the study are as follows. 1. The first hypothesis, 'The level of tympanic temperature for the experimental group which received Warming Therapy will be higher than that of the control group during the operation', was supported (F=32.16, p=.000). 2. The second hypothesis, 'The level of tympanic temperature for the experimental group which received Warming Therapy will be higher than that of the control group after the operation', was supported.(F=33.36, p=.000) 3. During recovery, shivering was observed one patient in the experimental group and seven patients in the control group. In summary, the findings of the study suggest that the 'Warming Therapy' applied before and during the surgery was a very effective treatment for surgical patients in maintaining the core temperature during surgery

30분의 수술전 가온이 고관절 전치환술 노인 환자의 수술중 심부체온, 수축기압, 심박동수, 수술후 전율 및 염증반응에 미치는 효과 (The Effects of 30-Minutes of Pre-Warming on Core Body Temperature, Systolic Blood Pressure, Heart Rate, Postoperative Shivering, and Inflammation Response in Elderly Patients with Total Hip Replacement under Spinal Anesthesia: A Randomized Double-blind Controlled Trial)

  • Purpose: This study was designed to determine the effects of pre-warming on core body temperature (CBT) and hemodynamics from the induction of spinal anesthesia until 30 min postoperatively in surgical patients who undergo total hip replacement under spinal anesthesia. Our goal was to assess postoperative shivering and inflammatory response. Methods: Sixty-two surgical patients were recruited by informed notice. Data for this study were collected at a 1,300-bed university hospital in Incheon, South Korea from January 15 through November 15, 2013. Data on CBT, systemic blood pressure (SBP), and heart rate were measured from arrival in the pre-anesthesia room to 3 hours after the induction of spinal anesthesia. Shivering was measured for 30 minutes post-operatively. C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) were measured pre-operatively, and 1 and 2 days postoperatively. The 62 patients were randomly allocated to an experimental group (EG), which underwent pre-warming for 30 minutes, or a control group (CG), which did not undergo pre-warming. Results: Analysis of CBT from induction of spinal anesthesia to 3 hours after induction revealed significant interaction between group and time (F=3.85, p=.008). In addition, the incidence of shivering in the EG was lower than that in the CG ($x^2=6.15$, p=.013). However, analyses of SBP, heart rate, CRP, and ESR did not reveal significant interaction between time and group. Conclusion: Pre-warming for 30 minutes is effective in increasing CBT 2 and 3 hours after induction of spinal anesthesia. In addition, pre-warming is effective in decreasing post-operative shivering.

초기 자궁 경부암에서 치료전 예후 인자 (Pretreatment prognostic Factors in Early Stage Caricinoma of the Uterine Cervix)

  • 1979년 3월 부터 1986년 12월까지 초기 자궁 경부암으로 서울대학교병원 치료방사선과에서 방사선 치료를 받은 124명의 환자를 대상으로 치료 결과 및 예후에 영향을 미치는 인자에 대하여 분석하였다. 병기 IB(35명)에서 5년 국소 치료율, 5년 무병 생존율 및 5년 생존율은 각각 $79.0\%$, $76.4\%$, $81.8\%$였다. 병기 IIA(89명)에서는 $78.0\%$, $66.8\%$, $72.1\%$였다. 나이, ECOG수행상태, 임신 횟수, 당뇨또는 고혈압의 여부, 병리조직학적 소견, 종양의 크기, 종양의 침투 양상, 전산화단층 촬영 소견 및 혈액 검사소견에 대한 후향적 분석을 단변수 분석과 다변수분석을 이용하여 5년 국소 치료율, 5년 무병생존율, 5년 생존율에 따라 실시하였다. 단변수 분석상 종양의 크기와 전산하단층 촬영상 직장 침윤이 5년 국소 치료율, 5년 무병 생존율 및 5년 생존율에 의미 있는 예후 인자이었다. 전산화 단층촬영상 자궁강 결합 조직 침범이 5년 국소치료율, 5년 무병 생존율에 의미가 있었다. 혈색소치가 5년 무병 생존율 및 5년 생존율에 의미가 있었다. 병리 소견과 환자의 나이가 5년 국소치료율에 의미가 있었다. 전산화 단층촬영결과를 제외한 다변수 분석결과에서 종양의 크기가 5년 국소치료율과 5년 생존율에 의미가 있었다. 혈색소치가 5년 무병생존율에 의미가 있었다. 전산화 단층촬영결과를 포함한 다변수 분석결과에설 병리소견이 5년 국소치료율과 5년 무병생존율에 의미가 있었다. 혈색소치와 전산화단층촬영상 직장침윤이 5년 국소치료율에 의미가 있었다.를 나타냈으나 통계학적으로 유의한 차이는 없었다. 결론적으로 선양 낭포암의 수술후 방사선치료에 있어서 60 Gy이상의 방사선양과 $64cm^2$이상의 field size로 치료함으로써 좀 더 좋은 국소치유율을 얻을 수 있었으며 환자의 생존율을 높이기 위해서는 좀 더 효과적인 항암제의 개발이 시급하다 하겠다.시에 비하여 심하지 않았으며 치료에 잘 적응하였다. 본 연구에서 DDP가 비교적 적은 합병증을 동반한 의미있는 방사선 민감제임을 확인하였으며 치료효과를 증대시키기 위하여 DDP와 방사선치료의 적절한 투여 계획을 결정하는 전향적 연구가 필요하다고 생각한다.C$-$43^{\circ}C$까지 잘 가온될 수 있음을 입증 하였다. 또한 폐의 온열요법시 종격동은 보다 높은 온도에 도달함으로 종격동의 열손상에 대한 고려가 필요함을 시사한다.r=0.990)로 각각 표시되었으며 각 간의 기울기에 대한 유의차는 없었다.18. 혈청중 LH와 total protein과의 상관계수는 +0.947이다. 19. 혈청중 FSH와 total protein과의 상관계수는 +0.709이다. 20. 혈청중 FSH와 triglycerides와의 상관계수는 +0.549이다. 21. 혈청중 estradiol-$17{\beta}$와 triglycerides와의 상관계수는 +0.673이다. 22. positive feedback mechanism에 의해서 LH, FSH와 estradiol-$17{\beta}$는 간을 자극시켜 albumin, total protein 및 triglycerides를 분필시킴으로서 난황형성(vitellosenesis)에 관여하는 것으로 나타났다.
