Effects of Depression, Anxiety, Quality of Sleep on Excessive Daytime Sleepiness in nursing students (간호대학생의 우울, 불안, 수면의 질이 주간과다졸림에 미치는 영향)

  • This study identified the effects of Depression, Anxiety, and Quality of Sleep on Excessive Daytime Sleepiness (EDS) in nursing students. Totally, 213 students of a nursing college located in C do, Korea, were enrolled for the study, which was accomplished through a questionnaire. Data were collected from May 20 to 30, 2019. The collected data were compiled using the SPSS/WIN 25.0 statistic program by applying Chi-square test, Paired t-test, Pearson's Coefficient, and Multiple logistic regression. The following results were obtained. The mean score for EDS was 8.95(±4.56), and Prevalence of EDS was determined to be 40.8%. The mean scores obtained for the affecting factors were Depression 10.05(±7.85), STAI-S 46.09(±9.50), STAI-T 46.47(±9.93), and Quality of Sleep 6.51(±2.95). Depression, STAI-S, STAI-T, and Quality of Sleep were significantly different for EDS and Non-EDS students (t=1.955, p=0.024; t=5.446, p<0.001; t=1.716, p=0.007; t=12.168, p<0.001; respectively). Multiple logistic regression revealed that factors associated with EDS were STAI-S and Quality of Sleep (adjusted odds ratio=1.04, 95% Confidence Interval=1.01-1.08; adjusted odds ratio=1.16, 95% Confidence Interval=1.04-1.29; respectively). These findings indicate the necessity to improve the quality of sleep and manage anxiety alleviation of nursing students, to reduce excessive daytime sleepiness.

The Effect of Health Status on Sleep Quality and Quality of Life among Elderly patient in geriatric hospitals (요양병원 입원노인의 건강이 수면과 삶의 질에 미치는 영향)

  • The purpose of this study is to analyze the effect of health status on sleep quality and quality of life among elderly patients in geriatric hospitals. A survey was held to the elderly patient of 5 geriatric hospitals in Kyunggi-do and Busan from May 8 to May 28, 2013. As result of factor analysis, the health status divided to the 7 domain: Emotional function, Strength, Daily activity, social role, Change of body, Social function and pain. Pain and Emotional function revealed the negative effect on sleep quality and Change of body and Daily activity were positive effect on quality of life. In conclusion, it is need to improve sleep quality through finding pain and emotional problem in elderly patient, and in order to upgrade nursing in geriatric hospitals, it have to develop nursing strategies considering the geriatric hospital for health screening and health promotion to take care of daily activity and change of older's body.

Functional Magnetic Resonance Imaging of Brain Reactivity to Insomnia-Related vs. General Anxiety-Inducing Stimuli in Insomnia Patients with Subjective-Objective Discrepancy of Sleep (주관적-객관적 수면시간 차이를 보이는 불면증 환자에서 일반적 불안에 비해 불면증 관련 자극으로 인한 뇌활성에 관한 기능적 자기공명영상 연구)

  • Objectives: Subjective-objective discrepancy of sleep (SODS) is a common symptom and one of the major phenotypes of insomnia. A distorted perception of sleep deficit might be related to abnormal brain reactivity to insomnia-related stimuli. We aimed to investigate differences in brain activation to insomnia-related stimuli vs. general anxiety-inducing stimuli among insomnia patients with SODS, insomnia patients without SODS, and healthy controls (HCs). Methods: All participants were evaluated for subjective sleep status using a sleep diary and questionnaires; occult sleep disorders and objective sleep status were assessed using polysomnography and actigraphy. Task functional magnetic resonance imaging was performed during insomnia-related stimuli (Ins) and general anxiety-inducing stimuli (Gen). Brain reactivity to Ins versus Gen was compared among insomnia with SODS, insomnia without SODS, and HC groups, and a combined insomnia disorder group (ID, insomnia with and without SODS) was also compared with HCs. Results: In the insomnia with SODS group compared to the insomnia without SODS group, the right precuneus and right supplementary motor areas showed significantly increased BOLD signals in response to Ins versus Gen. In the ID group compared to the HC group, the left anterior cingulate cortex showed significantly increased BOLD signals in response to Ins versus Gen. Conclusion: The insomnia with SODS and ID groups showed higher brain activity in response to Ins versus Gen, while this was not observed in the insomnia without SODS and HC groups, respectively. These results suggest that insomnia patients with sleep misperception are more sensitive to sleep-related threats than general anxiety-inducing threats.

Changes in Sleep Patterns and Mood States of Shift Workers Following Nocturnal Light Exposure (교대근무자에서 야간 광 노출에 따른 수면양상 및 기분상태 변화)

  • Objectives: We intended to observe changes in sleep patterns and mood states of night-shift workers following light exposure. We also estimated the degree of tolerance of light exposure. By studying these, we investigated the possibility of applying light therapy to night-shift workers for improving their adaptation. Methods: Twelve night-shift nurses working at Yong-In Mental Hospital volunteered to participate in this study. The study consisted of 3 parts: 1) night-shift control study; 2) light exposure study; 3) day-shift control study. All the nurses accomplished 3 parts of the study, each of which continued for 3 days, except one nurse who did not participate in day-shift control study. During light exposure study, nurses were exposed to bright light for 4 hours from 1AM to 5AM. Sleep patterns were evaluated with wrist actigraphy and automatic sleep analysis program. Mood states and side effects of light exposure were assessed with self-report scales. Results: Sleep period time, total sleep time, and sleep efficiency were increased following light exposure compared with night-shift control study. Light exposure study showed no difference from day-shift control study in above-mentioned sleep parameters. Daily fluctuation of sleep efficiency was less prominent during light exposure study than during night-shift control study. During light exposure study, the subjects felt more elated and energetic in the evening after daytime sleep than during night-shift control study. None of the subjects complained of severe side effects related to light exposure on the third day of light exposure. Tolerance of side effects was noted to develop with the repetition of light exposure. Conclusion: Light exposure improved the daytime sleep of night-shift workers to the level of normal nighttime sleep, making the subjects more elated and energetic. Side effects of light exposure were found to be tolerable. Light exposure seems to be safely applicable to night-shift workers for their adaptation.

  • 본 논문에서는 일반적으로 뇌파, 안전도, 근전도, 심전도, 동맥혈, 산소포화도, 복부와 흉부의 호흡운동, 호흡기류, 코골이, 몸의 자세 등을 이용하여 측정 하는 수면 다원검사를 근간으로 하여 보다 간편하게 수면상태를 측정하고 피드백할 수 있는 시스템을 개발하여 제시한다. 이 코골이 경감을 위한 임베디드 시스템을 통해 코골이로 인해 어려움을 겪는 사용자가 숙면을 취하고, 자신의 상태를 쉽게 모니터링 할 수 있을 것으로 기대된다.

이중 모형 해석을 통한 선박의 최적 운항 자세 도출

  • 선박의 최적 운항 자세를 찾기 위해 자유 수면을 포함한 저항 해석을 수행할 경우 많은 격자를 이용한 비정상 해석으로 인해 계산 시간이 오래 소요되는 단점이 있다. 본 연구에서는 이를 개선하여 이중 모형을 이용한 정상 해석을 통해 보다 효율적으로 선박의 최적 운항 자세를 도출하는 방법을 연구하였다. KCS를 대상선으로 선정하여 설계속도(24knots)와 저속(15knots)인 경우에 대하여 평형 상태 및 선수/선미 트림 상태에서 이중 모형 수치해석을 수행하고 기존의 연구결과와 비교하였다. 그 결과 자유수면 유무와 속도에 상관없이 선체 자세에 따른 저항 값은 동일한 경향을 갖는 것을 확인하였다. 하지만 저속일 경우 트림조건에 따른 저항의 변화의 폭에서 차이를 보였으며, 이는 압력저항에 기인한 것이다. 따라서 이중 모형을 이용한 저항 해석은 압력저항의 변동이 큰 경우에는 적용되기 어렵다는 한계점이 있다. 하지만 마찰 저항이 주요한 경우에 적용되어 계산효율을 획기적으로 높일 수 있으며, 정성적인 경향을 제시할 수 있는 타당한 방법이라 사료된다.

  • 최근 현대인에게는 질좋은 숙면이 필수적이다. 건강한 숙면을 위해서는 편안한 수면환경이 중요하므로 IoT베개와 매트는 개개인에게 맞는 수면 자세를 잡아 효율적으로 현대인들의 수면의 질을 높여줄 것이다. IoT의 베개의 센서가 머리의 움직임과 방향을 실시간으로 모니터링하여 설치된 장치가 자세를 바로잡아주고 장치와 연동된 앱을 통하여 수면의 종류와 패턴, 수면 시간을 상세분석하는 체계적인 맞춤형 수면 관리를 통해 수면 건강정보를 제공할 것이다. 또한, 수면 건강정보를 서버에서 종합하여 각 가정의 환자의 상태를 모니터링하여 원격진료가 가능하다.

  • To assess the reliability of chronobiological models of sleep/wake regulation, it is necerssary that the models predict the data which has been studied in sleep research, and they should be generalized across all ages. To date, many adult human data on such models have accumulated, yet it is evident that a comprehensive theory of the biorhythmic aspects of sleep/wake states has not established. Circadian rhythms such as the time going to bed, sleep onset, slow wave sleep pressure, periodicity of REM sleep, daytime performance, and early evening alertness are resumed everyday. Even in adult humans, sleep is inherently polyphasic. In both the disentrained and entrained states, naps when allowed tend to recur in a temporally lawful manner. The monophasic sleep pattern of most industrial societies therefore appears to be purely of social origin. The endogenous biorhythmic nature of circasemidian sleep tendency is supported by the ubiquity of the phenomenon across all ages. The NREM/REM sleep cycle within sleep with its inherent physiological, endocrine, and neurochemical fluctuations represents the best-documented ultradian sleep rhythms. Also, a daytime ultradian variation in sleepiness with a periodicity similar to nocturnal NREM/REM cycle(BRAC hypothesis) is suggested. This review article provides a brief synoptic review of the evidences for circadian, circasemidian, and ultradian sleep/wake rhythms, and then the authour will suggest the issues which expedite fuller modeling of sleep/wake system, to be further discussed.

Numerical simulation of turbulent flow with Hydrodynamic Pressure Model in trench channel (트렌치 수로에서 동수압을 고려한 난류흐름해석)

  • 트렌치 수로에서 동수압을 고려한 자유수면 흐름을 해석하고 난류 모형의 적용성을 제시하였다. 본 연구에 사용된 지배방정식으로 비정상 상태의 비압축성 유체에 대한 연속방정식과 비점성 Navier-Stokes 방정식을 사용하였다. 난류완결문제를 해결하기 위해서 $\kappa-\varepsilon$방정식을 사용하여서 난류 와점성을 구할 수 있다. 자유수면과 동수압을 고려하기 위하여 3단계로 나누어서 해석하였다. 제 1단계에서는 운동량방정식을 연직방향에 대해 음해적으로 차분하였다. 제2단계에서는 유속과 동수압 보정항으로 이루어진 식을 연속방정식에 대입하여 타원형 방정식인 동수압-포와송 방정식을 해석하여 얻어지는 유속은 질량보존법칙을 만족하게 된다. 마지막으로 자유수면과 최종유속을 보정 및 계산하였다. 본 연구에서 제시한 수치모형을 검증하기 위해서, 트렌치 수로에서 난류의 흐름에 변화를 고려하기 위해 수치모의를 하였다. 전반적으로 수치모의에 의한 결과와 실험 자료가 일치하는 경향을 보였다.
