Background: Vascular ring is a rare anomaly of the aortic arch. We did surgical repair procedures on 16 cases of vascular ring over the past 12 years. This article reviews our results. Material and Method: Between 1995 and 2007, 16 patients (5 with double aortic arch, 7 with right aortic arch-left ligamentum, 4 with pulmonary artery sling) underwent surgical repair. Mean age at the time of the operation were as follows: double aortic arch, $5.7{\pm}5.5$ years; right aortic arch-left ligamentum, $6.1{\pm}13.4$ years; pulmonary artery sling, $2.9{\pm}2.6$ years. Five patients (71%) with right aortic arch-left ligamentum had an associated Kommerell's diverticulum. Two patients (40%) with double aortic arch, 2 patients (28.6%) with right aortic arch-left ligament and 4 patients (100%) with pulmonary artery sling had associated airway stenosis. Cardiac anomalies were present in 8 of 16 patients. Result: There was no peri-operative or post-operative mortality. The mean hospital stay was $27.1{\pm}38.2$ days. None of our patients underwent reoperation. Conclusion: Vascular ring is rare, but, it needs surgical correction. It is important to suspect the diagnosis and to validate with echocardiography. Preoperative and postoperative computed tomography and bronchoscopy are useful to evaluate the airway and surrounding structures.
Background: Many video-assisted thoracic surgery (VATS) lobectomies are performed as a potential alternative to thoracotomy despite the controversy about the safety and the associated morbidity/mortality rates. Material and Method: Between November 2006 and August 2008, we performed 87 lobectomies (VATS 36, Thoracotomy 51) and we retrospectively reviewed the surgical treatment results. A VATS lobectomy was performed by a 4~5 cm thoracotomy without rib spreading and this included anatomic hilar dissection, individual vessel and bronchus stapling and lymph node dissection. Result: We studied 52 male and 35 female patients whose age ranged from 6 to 79 (average age: $59.8{\pm}15.0$ years). The cases were diagnosed with lung cancer (66) (SQC 24, ADC 38, others 4), pulmonary metastasis (2), carcinoid (2) and benign diseases (17). There was no intraoperative death. Postoperative complications were seen in 5 (15.6%) VATS and 33 (64.7%) thoracotomies, and perioperative death caused by adult respiratory distress syndrome occurred in 1 (2.8%) VATS and 3 (5.9%) thoracotomies. Three patients Underwent conversion to thoracotomy (8.3%). The mean time to chest tube removal was 6 days for VATS and 9.4 days for thoracotomy (p<0.001), and the mean length of the hospital stay was 8 days for VATS and 12.8 days for thoracotomy (p<0.001). Conclusion: VATS lobectomy can be performed safely with low morbidity/mortality rates. Furthermore, all the patients benefited from earlier postoperative rehabilitation and less pain and they were candidates for an earlier return to normal activities.
Background: There is no standardized tool and parameter that can accurately assess the sympathetic function before and after performing sympathectomy in patients with primary palmar hyperhidrosis. We examined the effective-ness of the sympathetic skin response (SSR) study for documenting the change of sympathetic denervation before and after performing selective thoracic sympathicotomy. Material and Method: We prospectively investigated the SSR from 12 healthy subjects who were diagnosed with primary hyperhidrosis. Each SSR was recorded on the right palm or sole with electrical stimuli applied to the skin at the left wrist and foot and vice versa for the controlateral side. This test was performed before, 2 weeks and 1 year after selective thoracic sympathicotomy. The data was corrected for the onset latency and the amplitude of the SSR (n=24). Result: The mean age of the 12 patients was $24.6{\pm}0.4$ years (range: 19~36) and the gender ratio was 1 : 0.7. The mean values of the preoperative, postoperative 2 weeks and postoperative 1 year onset latency and amplitude of the palmar side (n=24) were $1.46{\pm}0.24$ msec and $6,043{\pm}2,339{\mu}V$, $1.63{\pm}0.42$ msec and $823{\pm}638{\mu}V$, and $1.44{\pm}0.39$ msec and $2,412{\pm}1,546{\mu}V$, respectively. The mean values of the plantar side (n=38) were $1.83{\pm}0.42$ msec and $2,816{\pm}1,694{\mu}V$, $2.16{\pm}0.39$ msec and $1,445{\pm}1,281{\mu}V$ and $1.95{\pm}0.25$ msec and $1,622{\pm}865{\mu}V$, respectively. Among the documented parameters, only the palmar amplitude (p=0.002) showed statistical significance in recording the change of the sympathetic system within the same individual for the pre and postoperative period. Conclusion: The SSR amplitude ratio may be a useful parameter for documenting the efficacy of sympathetic denervation after selective sympathicotomy.

Background: Lobectomy and more extended anatomic resection are regarded as standard treatment for stage Ia non-small cell lung cancer, but approximately 15~40% of patients suffer from treatment failures such as cancer recurrence or death. The authors analyzed types and causes of treatment failures in surgically treated cases of stage Ia non small cell lung cancer. Material and Method: We retrospectively reviewed the medical records of 156 patients who had undergone complete resection for stage Ia NSCLC between Jan 1992 and Aug 2005. Patients were divided into two different treatment failure groups: cancer-related deaths and non-cancer-related deaths. Risk factors were analyzed in each group by the Kaplan-Meyer survival method and the Cox proportional hazard model. Result: Among the 156 patients, 93 were males; the mean age was 61. The median follow-up period was 33.8 months. The 5 year survival rate was 87.6%. Microscopic lympho-vascular permeation was reported in 10 patients. Recurrence was reported in 19 patients and 12 patients died due to recurrent lung cancer. Noncancer related deaths occurred in 16 patients. Risk factors for cancer recurrence and cancer related death were microscopic lympho-vascular permeation (HR=6.81, p=0.007, HR=7.81, p<0.001); for non-cancer related death, risk factors were pneumonectomy (HR=25.92, p=0.001) and postoperative cardiopulmonary complications (HR=29.67, p=0.002). Conclusion: After complete resection of stage Ia non small cell lung cancer patients, mortality includes not only cancer related deaths but also cancer unrelated deaths. Adjuvant chemotherapy is advised for patients who show microscopic lympho-vascular permeation, which is a risk factor for recurrence and for cancer related death. Patients who had pneumonectomy or who suffered from cardiac or respiratory complications need meticulous care in order to reduce comorbidity-induced death.
This study aims at listing up those manufacturing activities sensitive to regional characteristics by analyzing locational hierarchy designed on the urban rank-size rule. This locational hierarchy by manufacturing activities is expected to provide a ground for the proper supply of an industrial complex. The analysis of the locational hierarchy by manufacturing activities can work as a method of observing the characteristics of the distribution of location for each economic activity by analyzing the trend in the change of manufacturing location. Consequently, it can be used to determine the appropriate manufacturing activities for the industrial complex of a particular region. Here, the locational hierarchy is analyzed depending on the base of the basic local government such as Gun(district level) and Si(city level), and manufacturing activities are categorized by Korea Standard Industry Code. Those activities demonstrating growth pattern are Manufacture of Electronic Equipment(KSIC 26), Manufacture of Medical Precision Optical Instruments Watch(KSIC 27), Manufacture of Motor Vehicles (KSIC 30, 31), etc. With proper infrastructures, these activities can be located everywhere. Those sectors on the decline pattern in the locational hierarchy can be summarized as Manufacture of Tobacco Products(KSIC 12), Manufacture of wearing apparel Fur Articles(KSIC 14), etc. Those sectors scattered widely in the locational hierarchy are Manufacture of Food Products(KSIC 10), Manufacture of Coke Petroleum Products(KSIC 19), Manufacture of Chemical Products(KSIC 20), Manufacture of Electronic Equipment(KSIC 26). These particular manufacturing activities can be operated in those regions in a sufficient supply of unskilled workers regardless of proper infrastructures. Those activities that have a tendency to reconcentrate on larger cities are Manufacture of Textiles(KSIC 13), Manufacture of Wearing Apparel Clothing Fur Articles(KSIC 14), Manufacture of Other Transport Equiptmen(KSIC 31). In most cases, these sectors tend to favor their existing agglomerated areas and concentrate around large cities. Therefore, it is inefficient to promote these sectors in small or medium-sized cities or underdeveloped regions. The establishment of developmental strategies of an industrial complex can gain greater competitiveness by observing such characteristics of the locational hierarchy.
The climate change impacts on hydrological components and water balance in Jeju Island were evaluated using multiple climate models and watershed model, SWAT-K. To take into account the uncertainty of the future forecast data according to climate models, climate data of 9 GCMs were utilized as weather data of SWAT-K for future period (2010-2099). Using the modeling results of the past (1992-2013) and the future period, the hydrological changes of each year were analyzed and the precipitation, runoff, evapotranspiration and recharge were increasing. Compared with the past, the change in the runoff was the largest (up to 50% increase) and the evapotranspiration was relatively small (up to 11% increase). Monthly results show that the amount of evapotranspiration and the amount of recharge are greatly increased as the amount of precipitation increases in August and September, while the amount of evapotranspiration decreases in the same period. January and December showed the opposite tendency. As a result of analyzing future water balance changes, the ratio of runoff, evapotranspiration, and recharge to rainfall did not change much, but compared to the past, the runoff rate increased up to 4.3% in the RCP 8.5 scenario, while the evapotranspiration rate decreased by up to 3.5%. Based on the results of other researchers and this study, it is expected that rainfall and runoff will increase gradually in the future under the assumption of present climate change scenarios. Especially summer precipitation and runoff are expected to increase. As a result, the amount of groundwater recharge in Jeju Island will increase.
Extracellular Polymeric Substances (EPS) in the water environment assemble fine, colloidal particles, such as clays, microorganisms and biomass, in large flocs, which are eventually subject to sedimentation and deposition and determine water/sediment quality and quantity. This study hence aimed to investigate the way that water and colloidal chemistry affects EPS-mediated flocculation of colloidal particles, using a jar-test experiment. Especially, ionic strength, divalent cation and humic substances concentrations were selected as experimental variables in the jar-test experiments, to elucidate their effects on EPS-mediated flocculation. A higher ionic strength increased flocculation capability, reducing electrostatic repulsion between EPS-attached colloidal particles and enhancing particle aggregation. 0.1 M NaCl ionic strength had higher flocculation capability, with 3 times larger floc size and 2.5 times lower suspended solid concentration, than 0.001 M NaCl. Divalent cations, such as $Ca^{2+}$, built divalent cationic bridges between colloidal particles and EPS (i.e., $colloid-Ca^{2+}-EPS$ or $EPS-Ca^{2+}-EPS$) and hence made colloidal particles to build into large, settelable flocs. A small $Ca^{2+}$ concentration enhanced flocculation capability, reducing suspended solid concentration 20 times lower than the initial dosed concentration. However, humic substances, adsorbed on colloidal particles, reduced flocculation, because they blocked EPS adsorption on colloidal particles and increased negative charges and electrostatic repulsion of colloidal particles. Suspended solid concentration in the tests with humic substances remained as high as the initial dosed concentration, indicating stabilization rather than flocculation. Findings about EPS-mediated flocculation in this research will be used for better understanding the fate and transport of colloidal particles in the water environment and for developing the best management practices for water/sediment quality.
The purpose of this study is to evaluate the climate change impact on watershed hydrology and flow duration in Geum River basin ($9,645.5km^2$) especially by extreme scenarios. The rainfall related extreme index, STARDEX (STAtistical and Regional dynamical Downscaling of EXtremes) was adopted to select the future extreme scenario from the 10 GCMs with RCP 8.5 scenarios by four projection periods (Historical: 1975~2005, 2020s: 2011~2040, 2050s: 2041~2070, 2080s: 2071~2100). As a result, the 5 scenarios of wet (CESM1-BGC and HadGEM2-ES), normal (MPI-ESM-MR), and dry (INM-CM4 and FGOALS-s2) were selected and applied to SWAT (Soil and Water Assessment Tool) hydrological model. The wet scenarios showed big differences comparing with the normal scenario in 2080s period. The 2080s evapotranspiration (ET) of wet scenarios varied from -3.2 to +3.1 mm, the 2080s total runoff (TR) varied from +5.5 to +128.4 mm. The dry scenarios showed big differences comparing with the normal scenario in 2020s period. The 2020s ET for dry scenarios varied from -16.8 to -13.3 mm and the TR varied from -264.0 to -132.3 mm respectively. For the flow duration change, the CFR (coefficient of flow regime, Q10/Q355) was altered from +4.2 to +10.5 for 2080s wet scenarios and from +1.7 to +2.6 for 2020s dry scenarios. As a result of the flow duration analysis according to the change of the hydrological factors of the Geum River basin applying the extreme climate change scenario, INM-CM4 showed suitable scenario to show extreme dry condition and FGOALS-s2 showed suitable scenario for the analysis of the drought condition with large flow duration variability. HadGEM2-ES was evaluated as a scenario that can be used for maximum flow analysis because the flow duration variability was small and CESM1-BGC was evaluated as a scenario that can be applied to the case of extreme flood analysis with large flow duration variability.

Debris flow disasters primarily occur in mountainous terrains far from cities. As such, they have been underestimated to cause relatively less damage compared with other natural disasters. However, owing to urbanization, several residential areas and major facilities have been built in mountainous regions, and the frequency of debris flow disasters is steadily increasing owing to the increase in rainfall with environmental and climate changes. Thus, the risk of debris flow is on the rise. However, only a few studies have explored the characteristics of flooding and reduction measures for debris flow in areas designated as steep slopes. In this regard, it is necessary to conduct research on securing independent disaster prevention technology, suitable for the environment in South Korea and reflective of the topographical characteristics thereof, and update and improve disaster prevention information. Accordingly, this study aimed to calculate the amount of debris flow, depending on disaster prevention performance targets for regions designated as steep slopes in South Korea, and develop an independent model to not only evaluate the impact of debris flow but also identify debris barriers that are optimal for mitigating damage. To validate the reliability of the two-dimensional debris flow model developed for the evaluation of debris barriers, the model's performance was compared with that of the hydraulic model. Furthermore, a 2-D debris model was constructed in consideration of the regional characteristics around the steep slopes to analyze the flow characteristics of the debris that directly reaches the damaged area. The flow characteristics of the debris delivered downstream were further analyzed, depending on the specifications (height) and installation locations of the debris barriers employed to reduce the damage. The experimental results showed that the reliability of the developed model is satisfactory; further, this study confirmed significant performance degradation of debris barriers in areas where the barriers were installed at a slope of 20° or more, which is the slope at which debris flows occur.
The effects of growth regulators, sucrose and gelling agents were investigated to increase the efficiency of the callus growth and plant regenerarion in tissue culture of Angelica koreana Max. The fresh weight and dry weight of subcultured callus was highest in MS medium supplemented with 1 mg/l 2,4-D. Callus growth was excellent in 2% sucrose, but it was inhibited in propotion to sucrose content. Effect of gelling agents on callus growth was highest on 1.2% agar and 0.4% Gelrite medium, respectively. The browning of callus was protected on the media supplemented with 10 mg/l ABA and 5 or 10 mg/l $AgNO_3$. In the callus induction and growth from the peduncle of immature inflorescence, 2,4-D was more effective than NAA, and the frequency of callus induction was highest as 81.7% in 2 mg/l 2,4-D. Plant was not regenerated from the callus derived from young leaf. Somatic embryos were developed from the surface of callus drived from the peduncle of immature inflorescence in the medium containing 0.5 mg/l 2,4-D, 1 mg/l kinetin, 5 mg/l ABA and 5 mg/l $AgNO_3$. Plants were developed from the matured somatic embryos in the medium supplemented with 0.2 mg/l 2,4-D and 1 mg/l kinetin.
