이상에서 고찰한 위식도역류의 병태를 간략히 요약하면 다음과 같다. 위액에는 염산, 펩신, 담즙산과 췌효소등을 포함하고 있어 역류된 위액은 상부 기관식도관에 자극을 주거나 손상을 줄 수 있으며 개개인의 점막 상피의 저항도와 적절한 타액의 분비는 위액에 의한 손상의 정도를 결정하는 중요한 요소로 알려져 있다. 위식도역류로 인한 많은 증상이나 소견은 두가지의 기전으로 나타나서 첫째, 역류된 위액이 직접 조직에 영향을 주거나 둘째, 간접적으로 식도 하부에 분포된 미주신경에 의한 연관자극에 의해 일어나는데 위식도역류의 증상 중 두부, 경부, 심장 및 폐장의 증상은 이들 장기에 미주신경의 일부가 같이 분포하여 일어나는 간접적인 기전이다. 하부식도괄약근(lower esophageal sphincter)의 기능은 역류를 막는 가장 중요한 요소로 알려져 있어 간헐적으로 하부식도 괄약근의 압력이 떨어지면 역류가 일어나게 된다. 정상적인 식도의 연동운동은 식도에 역류된 위내용물을 위로 제거되는데 중요하며 위식도역류 환자에서 흔히 연동운동이 저하되거나 비정상적인 연동운동을 관찰할 수 있다. 또한 위내용물이 소장으로 비워지는 시간이 연장되어 위내용물이 위내에 축적되는 것도 위식도역류의 원인의 하나로 생각된다. 이러한 위식도역류의 병태를 연구하는 것은 이 질환의 이해에 도움을 줄 것으로 생각된다.
Introduction: It is frequently occur in sports injury of clavicular fractures in Hapkido players. Weevaluated Hapkido related clavicular fractures and preventive measure. Methods: We experienced 12 cases of Hapkido related clavicle fracture. The age ranged from 13 to18-years-old. All cases were treated by conservative managements. We examined the relationship between the clavicle fractures and Hapkido related pathomechanics.Results: All cases were injured during the practice for the falling-technique. Results were evaluatedby Weitzman and all cases were revealed more than good. Conclusions: In most cases, the correction of the wrong way of falling-technique will reduce theclavicle fractures in the Hapkido players.
Musculoskeletal injury is the most common cause of children visiting the department of emergency medicine. Since the bone is still developing, pediatric patients have characteristic radiological manifestations, including plastic deformation, greenstick fractures, and buckle (or torus) fractures. Furthermore, growth arrest can occur in those with physeal fractures. Various mechanisms are responsible for pediatric musculoskeletal injury since children have different ranges of activities, depending on their age, such as birth injury and fall and traffic accidents. Some fractures have characteristic locations and radiological manifestations. In this review, we will discuss various radiological manifestations of fractures involving both upper and lower limbs in pediatric patients.

The purpose of this study was to determine if a geriatric injury would have a statistically significant relationship with the spatial safety within a specific region. To achieve the objectives of this study, an independent sample t-test and One-way ANOVA were performed to verify difference in mean value of regional safety grade, depending on the injury intentionality, injury location, activities at the time of injury, and injury mechanism, in 6,572 geriatric injury patients by combining the National Hospital Discharge Patients Injury Survey data and regional safety index data. The results of statistical validation suggested that there was no difference in the mean value of regional safety grade by sub-group with respect to the injury intentionality. However, a statistically significant difference was observed in terms of the mean value of regional safety grade in each sub-group with regard to the injury location, activities at the time of injury, and injury mechanism in connection with some areas of the regional safety index. Based on the results of such analysis, we derived and presented 3 implications related to policies and practice for the prevention of geriatric injury and a reduction of its occurrence rate in light of spatial safety after a discussion of the results through a comparison with previous studies that examined individual areas.
Protective effect of Corydalis ternata against the carbon tetrachloride-induced toxicity was investigated. Carbon tetrachloride($CCl_4$) induces hepatotoxicity due to the reactive free radical(CCl$_3$ . ) generated by cytochrome P-450 enzyme. We examined effects of hexane, chloroform, butanol and water fractions prepared from the Corydalis ternata methanol extract. Rats were treated with those for 3 days, and liver microsomes and cytosols were prepared at 24 hour after last treatment. Hepatoprotective activity of the water fraction was higher than that of other fractions. To examine mechanism of the hepatoprotective effect of Corydalis ternuta, we measured contents of malondialdehyde(MDA), cytochrome P46O(CYP), glutathione, calcium as well as the activities of NADPH-CYP reductase, glutathione S-transferase(GST), superoxide dismutase(SOD), glutathione peroxidase(GPX) and catalase. The fraction inhibited production of MDA, content of CYP and calcium in liver of water fractions - treated rats as compared with those of CCl4-treated rats. The GST activity was increased. We speculate that the O2 radical scavenging activities of the water fraction might play a key role in the mechanism opposing the progression of $CCl_4$-induced hepatotoxicity, but the activities of SOD, GPX, CAT were decreased. These results suggest that the mechanism might be mainly due to the decrease of CYP contents, act as calcium channel blocker and increase of GST activity rather than $O_2$ radical scavenging activities.
FUN14 domain-containing protein 1 (FUNDC1), an outer mitochondrial membrane protein, contributes to removal of damaged mitochondria through mitophagy. In this study, to elucidate the role of the FUNDC1 in the amyloid beta peptide (Aβ)-induced neuropathy, changes in the degree of mitochondrial dysfunction and cell injury caused by Aβ treatment were examined in the HT-22 neuronal cells in which the FUNDC1 expression was transiently silenced or overexpressed. We found that Aβ treatment causes a time-dependent decrease of the FUNDC1 expression. In the Aβ-treated cells, there were a drop in MTT reduction ability, depletion of cellular ATP, disruption of mitochondrial membrane potential, stimulation of cellular ROS production, and increased mitochondrial Ca2+ load. Activation of caspase-3 and induction of apoptotic cell death were also observed. Transient silencing of the FUNDC1 expression by transfection with the FUNDC1 small interfering RNA per se caused mitochondrial dysfunction and apoptotic cell death like the effect of Aβ treatment. Conversely, in cells in which the FUNDC1 was transiently overexpressed by FUNDC1-Myc transfection, overexpression itself had no effect on the mitochondrial functional integrity and cell survival but showed a significant prevention effect against mitochondrial and cell injury caused by Aβ treatment. Overall, these results suggest that the FUNDC1 is importantly involved in the Aβ-induced mitochondrial dysfunction and cell injury in the HT-22 neuronal cells.
The present work was undertaken to investigate the protective effect of diallyl disulfide on the bromobenzene toxicity in mice. It was observed that the aniline hydroxylase and epoxide hydrolase activities were not changed by the treatment of diallyl disulfide for 5 days. But glutathione S-transferase activity was significantly increased. A striking enhancement of serum alanine aminotransferase activity and hepatic lipid peroxide content after bromobenzene administration was markedly decreased by diallyl disulfide pretreatment. These results indicate that the inducing effects of diallyl disulfide on the bromobenzene intermediate detoxifying enzyme such as glutathione S-transferase are believed to be a possible protective mechanism for the bromobenzene toxicity in mice.
A 64-year-old male visited our emergency department due to severe right flank pain after falling from a 2-meter height. Contrast-enhanced CT revealed a right hemothorax with multiple fractures in the right ribs and iliac bone. A small hematoma in the right perihepatic space was noted, but there was no hepatic laceration on CT. Initial surgical management led to continuous uncontrolled bleeding around the porta hepatis, and subsequent arterial angiography could not demonstrate a bleeding focus. However, immediate follow-up CT showed contrast extravasation on the left side of abdomen, and a percutaneous transhepatic portal venogram revealed active bleeding from the left portal vein. Although the wound was embolized with a glue, the patient suffered from a cardiac arrest and finally expired. In conclusion, during evaluation of abdominal trauma patients, portal vein bleeding and contrecoup injuries should be considered when hepatic arteriography findings are unremarkable.
