Review of Chuna Manual Therapy for Adult Digestive System Disease (추나 치료가 성인 소화기계 질환에 미치는 영향에 대한 문헌 고찰)

  • Objectives : To determine the effectiveness of Chuna manual therapy(CMT) for Adult Visceral Diseases. Methods : To find evidence of CMT for adult digestive system disease, we used 9 electronic databases(Pubmed, CAJ, EMBASE, Cochrane Library, Web of Science, KISS, KISTI, NDSL, RISS) up to April 2018. We reviewed randomized control trials(RCTs) using Chuna for Digestive system disease. To assess the methodological quality of each RCT, we used the Cochrane risk of bias tool. Results : We identified 11 RCTs about irritable bowel syndrome, chronic ulcerative colitis, gastritis, gastroptosis, spleen entropathy, gastrointestinal dysfunction after stroke in 356 studies. All studies used total efficiency rate for main outcome. In all studies, CMT was effective for digestive system disease. Conclusions : CMT was effective for Digestive system disease. Risk of bias was high and the difference between the experimental group and the control group was not great. Considering that there were no reports of side effects, it seems possible to apply CMT to the treatment of adult digestive system disease.

Medical Convergence Analysis of Complaint about Medical Service in an Affiliated Hospital (일 상급종합병원 의료서비스 불만족 내용의 의료 융합적 분석)

  • Implementation of patient feedback is considered as a critical part of effective and efficient management. This study is a retrospective survey after to ensure the medical convergence analysis of contents on customer dissatisfaction using medical services. The data were collected from dissatisfaction 319 case at a affiliated hospital. The result indicate that a repeat visit than the initial visit, outpatients were more dissatisfied than the inpatients. The type of dissatisfaction were more dissatisfied of the in communication and explanation of medical service, nursing service and procedure of administrative services. In the classification of disease the percentages of dissatisfaction in the digestive system and neoplasm were high in both groups(p<0.05). Based on the study to improve the quality of medical service. Prevent the recurrence of dissatisfaction and to establish a customer-oriented business strategy. Characterized by dissatisfaction factors and contents of each patient and should be structured to specific disease types of services through continuous medical convergence research.

Assessment of Relapsing Urolithiasis from K43 with Erosive Gastritis (미란성 위염 환자 K43에서 재발성 요로 결석에 관한 연구)

  • Nephrolithiasis is the most common disorder of the urinary tract in hospitalized patients, more frequently increased in 30~50 years of age, more common in males than in females, prior right stone to left side, and than upper ureteral stone is found in cultural country, while lower ureteral stone is increased in uncultural country. Stone components are classified as calcium oxalate, calcium phosphate, magnesium ammonium phosphate, uric acid, cystine, and their mixed stone, respectively. According to the pathophysiology of urinary stones, supersaturation/crystalization of inorganic salt concentration in urine, organic matrix, inhibitor deficiency, and epitaxy theory could be based on the stone formation. Not only hypercalciuria, hyperparathyroidism, hyperoxaluria, hyperuricosuria, and cystinuria, but also renal tubular acidosis, hypervitaminosis D, and peptic ulcer, are significantly associated with nephrolithiasis. In this study upper ureteral stone component were analyzed with chemical analysis, infrared spectrum, and image analyzer from K43 patient wit erosive gastritis. As the results, mixed stone of calcium oxalate dihydrate and calcium phosphate apatite was identified, the values of clinical test in blood and urine maintained normal revels. The relapsing urinary stone from K43 have no correlation between factors for stone formation reported early, also have no evidence for risk from erosive gastritis.

  • Recently, much attention has been paid to the development of a value-added food category containing probiotics so as to improve human health and prevent diseases. Among various foods, the health benefits of milk and dairy products are known to humanity, and could be attributed to the bioactive components present in milk. In fermented milk products, the health benefits could be due to suitable modulation activities produced by the action of probiotic bacteria. Besides the modification of various milk components, probiotics might also act directly as preventive and therapeutic agents against some severe diseases. Probiotics promote health via their positive effects on the immune response, stimulation of natural immunity, and modulation of the production of antimicrobial peptides, cytokines, and so on. Whey proteins, a byproduct of cheese production could also have anticarcinogenic, immunostimulatory, antimicrobial, and health-promoting activities such as improving insulin sensitivity and reducing fat deposition. Therefore, milk and dairy products containing probiotics could provide various opportunities in the field of functional foods. Additionally, these functional foods may be important in the human diet and may help improve human health and prevent diseases.

A Study on Psychological and Physical Health of Families of Victims One Year after the Sewol Ferry Disaster (세월호 재난 1년 후 유가족의 심리적, 신체적 건강에 대한 연구)

  • Objectives : The purpose of this study was to assess physical illnesses, psychological difficulties, and daily life function of victims' family members one year after the Sewol ferry disaster, in order to establish a basis of strategies to promote their mental health and resilience. Methods : We statistically analyzed self-reported data collected from a survey conducted between March 22 and 28, 2015 with 139 family members of victims. Results : All respondents complained about one or more psychological difficulties, and 95.4% of them had newly diagnosed or exacerbated physical illnesses. An increased number of psychological difficulties was associated with a greater tendency to present physical illnesses including chronic headache, gastrointestinal, musculoskeletal, obstetrico-gynecological, genitourinary, and cardiovascular disease. In addition, the majority of respondents reported a decrease in life functioning. Specifically, 71.9% reported interpersonal avoidance and 63.9% reported difficulty in returning to work. Conclusions : Not only psychological trauma but also physical illness and daily functioning should be evaluated in detail and actively addressed in the bereaved family members after a catastrophe.

  • Coronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) mainly causes respiratory symptoms such as fever, cough, sputum, and rhinorrhea, as well as digestive symptoms such as diarrhea, vomiting, and abdominal pain in children. In this report, we describe a case of a child with a SARS-CoV-2 infection who presented with epigastric pain and was subsequently diagnosed with acute pancreatitis without any concomitant infections in other organs. The epigastric pain was relieved with goal-directed vigorous fluid therapy for acute pancreatitis for 24 hours, and the serological and radiological findings normalized after two months. Acute pancreatitis should be considered as a differential diagnosis when a child with a history of COVID-19 visits the hospital with epigastric pain.

  • Dermatomyositis (DM) is an idiopathic inflammatory connective tissue disorder and a systemic autonomic immune disease which shows a progressive muscle weakness and characteristic rash. It is identified by a characteristic rash accompanying, or more often preceding muscle weakness. Pathognomonic skin lesions are a blue-purple discoloration on the upper eyelids with edema (heliotropic rash), a flat red rash on the face and upper trunk, and erythema of the knuckles with a raised violaceous scaly eruption (Gottron's papule). The myopathy represents inflammatory and degenerative changes primarily affecting proximal muscles. DM often involves GI tract and respiratory system with as risk of 15-25% internal malignancy. It's managed with sun protection since muscle weakness as well as a rash could be aggravated by sun exposure. Systemic corticosteroid is an initial therapy and other immunosuppressive agent has been used as alternatives. Facial muscles are unaffected and masticatory muscles are rarely affected in DM. We present trismus close to muscle contracture in a patient with DM. Therefore, it needs continuous mouth-opening exercise to prevent progressive muscle contracture and to ensure normal mouth opening.

Contents Analysis on the Health Information of Major Daily Newspaper and TV in Korea (우리나라 주요(主要) 일간지(日刊紙) 및 TV 건강정보(健康情報)의 내용분석(內容分析))

  • 주요 일간지와 KBS 1 9시 뉴스에서 다루고 있는 건강관련 기사의 내용을 분석하여, 건강관련 정보의 경향을 파악하여, 건강 예보제 실시를 위한 현황 분석을 위해 본 연구를 실시하였다. 1. 일간지 건강정보 내용분석 결과요약: 기사 보도 분야에서는 생활건강(47.8%), 사회(14.6%), 종합(7.7%), 국제(4.7%), 경제(1.5%) 순이었고, 3/4분기에서만 국제와 경제 분야가 종합분야 보다 많이 보도 되었으며, 기타 분야가 22.9%로 많은 비율을 차지하였다. 건강정보를 대상 성별별로 분류 하였을 시 남녀 모두에 해당되는 자료가 대다수였고(87.9%), 여성이 대상인 정보(8.0%)가 남성(4.1%)보다 많았으며, 생애 주기별 대상으로 분류 하였을 시에 역시 특별히 구분되지 않은 모든 연령층에 해당 정보가 가장 많았고(60.8%), 성인(19.8%), 학동기 어린이(7.3%), 청소년 (4.9%), 노인(4.9%), 영유아(3.3%) 순이었다. 일간지별 기사의 주제를 조사한 결과 두 일간지의 총합은 병의 원인 및 위험인자(15.5%)가 가장 많이 보도 되었는데, 조선일보는 건강증진, 치료 및 술기, 투약, 기타의 순이었고, 한겨레신문은 건강증진, 행정, 치료 및 술기, 투약 순이었으며 각 주제별로 분기별로 약간의 차이를 나타내었다. 예측성에 관한 내용분석을 보면 예보성은 있는 경우(76.2%)가 없는 경우보다 많았고, 예보성의 정보는 알림성, 예측성, 행사성의 순이었고 예측성의 경우 건강형태가 가장 많았으나 한겨레 신문의 경우 기타에 속하는 경우가 가장 많았다. 사 ICD-10 체계, 21대 분류로 질병에 관한 기사를 분류한 결과 신생물(14.5%), 특정 감염성 및 기생충성 질환(13.6%), 정신 및 행동장애(9.5%)의 순이었으며 두 일간지간의 차이를 보였다. 2. TV 뉴스 건강정보 내용분석 결과요약: 건강정보의 대상 특징 성별은 모두 해당되는 경우가 265회 중 238회 (89.8%)로 가장 많았고, 생애주기별 대상으로 보면 모든 연령층에 해당 되는 것이 154회(58.1%)로 가장 많았다. 건강정보의 주제에 대해서 조사한 결과 병의 원인 및 위험인자가 73회(27.5%), 역학(역학조사 및 보도성)이 64회 (24.2%), 행정이 30회(11.3%), 증상 및 호소가 27회(10.2%) 등의 빈도순으로 조사 되었다. 건강정보의 내용의 분야를 보면 대분류로는 질병관리 분야가 102회(38.5%), 보건의료제도 및 행정 분야가 52회(19.6%), 보건행태 및 기타 42회(15.8%), 생활환경분야 39회(14.7%)의 빈도순으로 나타났다. 건강정보 내용을 질병의 분류 ICD-10 체계로 분석한 결과는 특정감염성 및 기생충성 질환 48회(26.8%), 소화기계의 질환, 손산, 중독 및 외인에 의한 특정기타 결과, 건강상태 및 보건서비스 접촉에 주는 요인 등의 빈도순으로 조사되었다.

  • An elevated serum CA19-9 level is an indication of pancreatic and biliary tract cancer. However, it has recently become known that nonmalignant gastrointestinal diseases and a variety of nonmalignant respiratory diseases, such as idiopathic interstial pneumonia, collagen vascular disease associated lung diseases, diffuse panbronchiolitis and bronchiectasis, can also show an elevated serum CA19-9 level. We recently encountered a case of bronchiectasis with persistently elevated serum CA19-9, but without any evidence of malignant disease in endoscopic retrograde pancreatocholangiography, abdominal computed tomography, and positron emission tomography. After serial follow-up of 3 years and 10 months, there was still no evidence of cancer. It is believed that the elevated serum CA19-9 level was due to bronchiectasis. An elevated serum CA19-9 level should be interpreted carefully with the patients' clinical condition.

Clinical Analysis of Marine Telemedicine Cases for Ocean-Going Vessel Crew (원양선박 선원들의 해양원격의료 실태를 통한 임상분석)

  • The purpose of this study is to contribute to the study of the prevention of diseases and promotion of health of ocean going vessel crew members, through the medical diagnosis and disease classification efforts of this study. From the second half of 2016 to 2017, the diagnosis and health characteristics of 195 crew members were collected through counseling, treatment, and emergency care for about 1 year and 2 months. As a result, it is noted that the incidence of diseases was in the order of urticaria (5.6%), lumbar sprain (4.1%), acute gastroenteritis (3.1%) and anxiety (3.1%). In categorical review, the incidence of musculo-skeletal disease was the most common (25.1%) which was followed by skin disease (17.9%) and digestive disease (11.3%). In addition, the disease that was noted as was the most common in the under 30 years old category, and the incidence of the disease was high in the crew group. Finally, there was a difference between the pathogenesis (trauma vs disease, etc.) (p <.001) and the type of vessel (merchant ship and fishing vessel) (p <.005) as noted in this case.
