  • Management of the child with Attention-Deficit Hyperativity Disorder(ADHD) reguires a comprehensive approach of cognitive-behavioral, educational, and pharmacological interventions. Establishing the valid diagnosis is the first step of management. After the diagnosis is made, the clinician must then interpret the diagnosis and its impliations to the child, parents, and teachers. The pharmacotherapy is most effeceive, and the CNS stimulants (methylphenidate) is drug of choice. Although generally not as effective as stimulants, triacyclic antidepressants, clonidine, antipsychotics offer the alternatives to stimulants therapy. Additional treatments, including psychotherapy, cogntive-behavioral approach, educational infervention, parental counseling are also essential in managing the child with ADHD. Finally, controversial approaches-diet therapy, mineral therapy, hypoglycemia, megavitamin therapy, refined sugars, neurophysiological retraining approaches are reviewed.

  • Management of obesity needs good rapport among pediatrician, parents and children. Through motivational interview, pediatrician should explore practical ways to modify the eating behaviors conducive to obesity. Imbalance between energy intake and consumption contributes to weight gain. Therefore decrease of sedentary behavior (screen time <2 hrs) and increase of physical activity (>60 minutes) is also important in management of obesity. The goal of management is the long-term healthy life though healthy behaviors. Creating a home environment that supports healthy habits and behaviors is an important key to modifying lifestyle behaviors. For example, increasing intake of vegetables and fruits, restriction of sweetened beverage, education of healthy selection of foods, and frequent family dinner may be advisable. Family members should change their own behaviors in order to help their children. Parents should be positive role model and monitor their children authoritatively but not restrictively.

  • Purpose : Short attacks of headache are present in the pediatric age group. Short-lasting headaches (SLH) have been infrequently reported in children and adolescents. This study aims to assess the prevalence, clinical characteristics, treatment, and prognosis of short-lasting headaches in children and adolescents. Methods : One hundred twenty-three consecutive patients attended the Chosun University Hospital Headache Center between March 2006 and July 2007. Among 123 headache patients, 44 patients with one key criterion, namely, recurrent headaches episodes of a few seconds to less than 30 minutes, were selected. Results : Of the 123 headache patients, 44 (35.8%, males 18, females 26) had SLH. The mean age was 9.24 years (range 2.3-14.2). The mean headache frequency was $11.50{\pm}10.30$ per month, mean severity was $4.86{\pm}1.36$ out of 10, and the mean PedMIDAS score was $7.97{\pm}17.51$. SLH was significantly more common in children <6 years old (10/22, 45.5%) than in ${\geq}6$ years old (34/101, 33.7%) (P=0.015). The nature of pain was pressure in 30.2%, stabbing in 27.9%, throbbing in 18.6%, and dull in 11.7%. In 52.4% patients, the pain occurred in the front, one side of the temple in 28.5%, both sides of the temple in 7.1%, and back in 4.8% patients. The duration of attacks was <6 minutes in 38% and ${\geq}6$ minutes in 61.4%. The intensity of the pain was 4-6 out of 10 in 75.0%, in 13.6% 1-3, and in 11.4% 7-10. Treatment was significantly effective in reducing the frequency, duration, and severity of headaches and the PedMIDAS score after 3 months (P<0.05). Conclusion : Short attacks of headache are common (35.8%) in children and adolescents. These are difficult to classify according to International Headache Society criteria. The outcome, after a period of 3 months to 2 years, was favorable in most patients. Further studies are needed to gather more data and to obtain a better description of these forms of headache in the pediatric group.

  • The purpose of this study was to improve efficacy of oral health education in children and adolescents based on the experience and perception of oral health education in young adults who graduated from high school within 10 years. Questionnaires were given to 412 people, and among them, 388 completed survey forms were selected. The questionnaire included categories which examined one's experience and perception of oral health education, oral health knowledge, dental history, and oral health behavior. The results showed that the respondents perceived elementary school as the most effective period of oral health education. Oral examination with direct explanation was the most preferred method of oral health education, but web search was the main source of oral health information. And knowledge of fluoride and sealant about caries preventive effect was still lacking. Considering these results, oral examination with direct explanation and web search would be useful to improve efficacy of oral health education in children and adolescents.

  • This case study aimed to investigate changes in self-expression following participation in a rap making program with adolescent survivors of childhood cancer. The rap making program was constructed based on the contextual support music therapy model. Three adolescent survivors of childhood cancer participated in six 80-minute individual sessions. During the sessions, each participant engaged in the following tasks: song discussion, lyric creation, and rapping over a selected beat. At pre and posttest, the Self-Expression Scale was completed by participants. Their verbal expressions lyrics were observed during the sessions, and individual interviews with the participants were conducted at the completion of the program. The results demonstrated that the mean rating of the Self-Expression Scale increased after the rap making intervention. Analysis of the participants' verbal expressions and lyrics demonstrated that participants were experiencing difficulties adjusting to school that they wanted to resolve. Furthermore, the analysis of the interviews at posttest found that participants experienced positive changes in self-perception, self-expression, and expectations for their future, compared to the pretest when the participants expressed negative self-perceptions due to difficulties in interpersonal relationships at school and physical limitations. This indicates that rap making can be an effective resource for providing this population with the means to recognize positive attributes about themselves and improve self-expression.

  • To achieve the dental health of children and adolescents which is the objective of the pediatric dentistry, the role of the public service should be increased. The basis of the public service is established by many laws of which the Dental Health Law is most important. The percentages of primary schools that had the school dental health clinic, that implemented the fluoride rinsing program, and that implemented the group toothbrushing after lunch were 7.2%, 57.5%, and 46.9%, respectively, and the percentages of primary school children that received the dental health education and that received the fissure sealing were 48.0% and 12.4%, respectively(2006). About 42% of infants and preschool children from 0 to 6 years received preventive dental care in the last one year, at the nursery or kindergarten(18%) or at the health center(1%)(2005). The percentage of the health centers that implemented water fluoridation was 11.3%, and the percentage of the population who drank the fluoridated water was 5.7%(2006). It was suggested that the school dental health administration should be unified, that dental health teachers should be employed, and that the comprehensive dental health care should be supplied to all the children and adolescents through the circuit school dentists and the school dental hospitals in the long term. Also, the dentist in charge system for the children and adolescent was suggested.

  • Purpose: Recently, the conception and cognition that enuresis was resolved spontaneously, have changed. We reviewed the attitudes of the primary care physicians who make diagnose and treat nocturnal enuresis. Methods: From January 2006 to February 2007, a total of 293 primary care physicians in Daegu city participated in this survey. Questionnaires included questions about physicians' opinions on the appropriated age for diagnosis of enuresis, the likely causes of enuresis, etc. Physicians are grouped in two according to whether enuresis is major field of their subspecialty; the pediatrician & urologist group and the other physician group. Results: 59.2% of pediatricians and urologists thought that enuresis is defined as the nightly involuntary release of urine by children of the age of 5 to 6, while 49.6% of other physicians did. For the causes of enuresis, most of clinicians checked "yes" to the question that "Under-developed bladder and nerve" and "Emotional problems". In the patient's behavioral reactions related to enuresis, "Lack of concentration in home and school" and "Frequent urination" were most responded. Attendance to the education program of enuresis in last five years and willing to participate in education program was statistically different among pediatricians-urologists and other physicians. Regarding the treatment of enuresis, most physicians used imiprarnin widely, but pediatricians and urologists preferred desmopressin. Alarm was the last one in treatment modality. Conclusion: This study revealed that pediatricians and urologists are attending more to the educational places and knowing much about the recent information on enuresis when compared to other primary care physicians, regarding the diagnostic age and treatment modality of enuresis. The education of enuresis for primary physicians is more needed.
