Pediatric Radiation Examination by Development of Bismuth Shield Research on Radiation Exposure (비스무스 차폐체 개발을 통한 소아 방사선검사의 피폭에 관한 연구)

  • Currently, with the development of technologies, X-ray examinations for medical examinations at hospital is increasing. This study was conducted to help reduce radiation exposure by measuring the exposure dose received by pediatric patients and the spatial dose of the X-ray room. Dosimeters were installed in the eyeball, thyroid gland, breast, gonads and 4 directions at a distance of 30 cm, 40 cm, 50 cm from the phantom. The dose was measured ten times each, before, and after the application of the bismuth shield under the examination conditions of the head, chest, and abdomen of pediatric patients. Under the condition of head examination, when a shielding was applied, the dose reduction rate was 68.58% for the eyeball, 72.88% for the thyroid, 84.2% for the breast, and 72.36% for the gonad. The chest examination showed reductions of 19.56% eyeball, 56.98% thyroid, 1.21% breast, and 0.68% gonad. The abdominal examination showed reduction rates of 2.6% eyeball, 10.67% thyroid, 19.85% breast, and 82.02% gonad. Spatial dose decreased by 62.25% at 30 cm, 61.16% at 40 cm, and 68.68% at 50 cm. When the bismuth shield was applied, there was a decrease in dose across all examinations, as well as a reduction in spatial dose. Continued research on the use of bismuth shields will help radiological technologists achieve their goal of dose reduction.

  • The purpose of this study is to determine the developmental relationship between the maxillary primary central incisors and their permanent successor. The auther took 315 cases of lateral cephalogram of the children (males were 171, females were 141) Angular change of the teeth and horizontal and vertical linear change were observed. The obtained results were as follows: 1. The inclination of the long axes of both incisor teeth was relatively stable and labio-version of both incisore was significant at 7 years of age. 2. The distance between the incisal edge of the permanent central insisor and the resorbing apex of the primary maxillary central incisor remained within 2mm of each other, 3. Vertical growth of the maxillary anterior portion was greater than horizontal growth from 6 to 7 years of age. 4. There was not a significant sexual difference.

  • The purpose of this study was to investigate the range of condylar movement of children with young permanent dentition. Materials included 33 roentgenograms of 4 serial projections of TMJ by modified transcranial projection with Accurad 100. Four serial projections were taken in the centric occlusion, rest position, 1 inch open, and maximum open position. The results obtained from this study were as follows. 1. The condyle was located anteriorly in articular fossa at centric occlusion. 2. The condyle was moved forwardly and downwardly during mandibular movement from centric occlusion to maximum open position. 3. The pattern of condylar movement was similar in both sexes, but there were no sex differences in distance of condylar movement.

  • 단순 흉부 X-선 사진은 흉곽내에 공기를 함유하고 있는 폐실질, 심장과 대혈관을 포함한 연부조직, 그리고 흉곽을 이루는 골격 등의 다양한 조직으로 이루어진 3차원적인 해부학적 구조물을 X-선 film이라는 2차원적 평면에 흑백의 음영으로 표현하는 검사이다. 뿐만아니라 이들 구조물들이 전후, 좌우로 겹쳐져 있어 병변이 쉽게 가려질 수 있고, 폐실질 질환 뿐만 아니라 심장 질환 또한 폐실질의 변화를 유발시킬 수 있으므로, 단순히 각 질환의 X-선 소견에 대한 지식을 습득하는 것보다는 병인과 해부학적인 지식에 대한 이해를 기본으로 X-선 판독에 임하는 것이 중요하다. 또한 환자들의 임상 증상과 부합되지 않는 X-선 소견이 관찰되거나, 치료 도중 임상 경과와 판독소견이 일치되지 않는다면 방사선과 전문의와의 긴밀한 의견교환이 진단과 치료에 중요하리라 생각된다.

  • 150 "K" primary school children, from 6 to 10 years old, have been examined both clinically and radiographically for proximal caries in deciduous canine and deciduous posterior teeth and permanent first molar teeth. 1. 598 proximal cavities were diagnosed 154 (25.75%) were revealed by both examinations. 39 (6.52%) appeared on clinical examination only, but 405 (67.73%) were revealed by radiographic examination only. 2. The dental caries occurance on distal surface of first deciduous molar and mesial surface of second deciduous molar were revealed much higher values in all ages. 3. There is no significant differences observed between right and left arch. 4. Proximal cavities were revealed 1.7 times more in mandible than in maxilla.

  • Psychogenic cough is barky, croupy, and explosive and disappears during sleep, and is not affected by antitussive drugs. Physical and radiological examinations of the respiratory tract and other routine laboratory test are normal. It is helpful to recognize this condition early in its course so that appropriate treatment can be instituted before iatrogenic problems resulting in multiple hospitalizations and chronic steroid use occur. We experienced two child and adolescent cases that showing typical psychogenic cough so reported it with the literatures review.

Sleep Disordered Breathing in Children (어린이의 수면호흡장애)

  • Sleep disordered breathing (SDB) is a disease characterized by repeated hypopnea and apnea during sleep due to complete or partial obstruction of upper airway. The prevalence of pediatric SDB is approximately 12 - 15%, and the most common age group is preschool children aged 3 - 5 years. Children show more varied presentations, from snoring and frequent arousals to enuresis and hyperactivity. The main cause of pediatric SDB is obstruction of the upper airway related to enlarged tonsils and adenoids. If SDB is left untreated, it can cause complications such as learning difficulties, cognitive impairment, behavioral problems, cardiovascular disease, metabolic syndrome, and poor growth. Pediatric dentists are in a special position to identify children at risk for SDB. Pediatric dentists recognize clinical features related to SDB, and they should screen for SDB by using the pediatric sleep questionnaire (PSQ), lateral cephalometry radiograph, and portable sleep monitoring test and refer to sleep specialists. As a therapeutic approach, maxillary arch expansion treatment, mandible advancement device, and lingual frenectomy can be performed. Pediatric dentists should recognize that prolonged mouth breathing, lower tongue posture, and ankyloglossia can cause abnormal facial skeletal growth patterns and sleep problems. Pediatric dentists should be able to prevent these problems through early intervention.

Primary leiomyosarcoma of the chest wall in a child : a case report (소아의 흉벽에서 진단된 평활근육종 1례)

  • Leiomyosarcoma is an uncommon soft tissue sarcoma of mesenchymal cell origin, which shows smooth muscle differentiation. Leiomyosarcoma is seldom found in the pediatric population, and accounts for fewer than 2% of all soft tissue sarcomas. Leiomyosarcoma of the chest wall is extremely rare in children. We report here a case of an 8-year-old boy with a primary leiomyosarcoma that was incidentally found as a rib mass. The patient underwent a complete resection for a suspected osteochondroma diagnosed by a three-dimensional chest computed tomography examination. Pathological findings of the mass revealed intersecting fascicles of spindle cells showing cigar-shaped nuclei, inconspicuous nuclear pleomorphism and occasional mitotic figures in the background of a suspected osteochondroma of the rib. This report documents the first description of a leiomyosarcoma possibly arising in an osteochondroma of the rib in a child.

  • Despite preventative efforts, aspiration and ingestion of foreign bodies during clinical dental procedures occasionally occur. Careful clinical and radiographic examination and prompt emergency management can prevent potentially serious consequences. In this report, we describe a case of accidental foreign body ingestion and discuss prevention and management of dental instrument aspiration and ingestion.

A Case of Autoimmune Chronic Pancreatitis in a Child (자가면역성 만성 췌장염으로 진단된 소아 1예)

  • We present a case of autoimmune chronic pancreatitis in a previously healthy child without any history of autoimmune disease. A 12-year-old boy was admitted to the hospital with abdominal pain. The serum amylase, lipase, and IgG levels were elevated and autoantibodies (antinuclear antibody, antineutrophil antibody) were detected. An abdominal CT (computed tomographic) scan revealed diffuse enlargement of the pancreas. ERCP (endoscopic retrograde cholangiopancreaticography) demonstrated an irregular stricture of the main pancreatic duct in the pancreas tail. After two years of oral steroid and immunosuppressive drug therapy, the clinical, laboratory and radiological findings were improved. The patient has been symptom-free for 18 months after the discontinuation of medication.
