Effects of EMLA Cream Application on Pain Perception and Pain Response of Children with Cancer During Implanted Venous Access Port Needle Insertion (EMLA크림 도포가 소아암환자의 피하매몰 중심정맥포트 바늘삽입 시 통증인지와 통증반응에 미치는 영향)

  • Purpose: The purpose of this study was to identify effects of EMLA (Eutectic Mixture of Local Anesthetics) cream application on pain perception and pain response during insertion of implanted venous access port needle in children with cancer. Methods: From December 2010 to August 2011, at U university hospital, 20 patients scheduled for implanted venous access port needle insertion were recruited, and randomly assigned to receive either EMLA or a placebo cream 1 hour before the implanted venous access port needle insertion. While conducting needle insertion, changes in pulse and oxygen saturation on the pulse oxymeter monitor were measured and pain behavior reaction was also measured during needle insertion in the treatment room. After conducting needle insertion, self-reported pain reaction, and mothers' perception of the children's pain reaction were measured. Collected data were statistically processed using SPSS version 17.0 for Windows, and analyzed using descriptive statistics, t-test. Results: Children's self-reported degree of pain, degree of pain as perceived by mothers and pain behavior reaction decreased significantly in the EMLA application group compared with the placebo group. Conclusion: Findings indicate that application of EMLA cream is effective in relieving pain in these children during implanted venous access port needle insertion.

Concept Analysis of Stress in Siblings of Patients with Childhood Cancer (소아암 환자 형제자매의 스트레스 개념분석)

  • Purpose: This study was conducted to define the concept of stress in siblings of childhood cancer patients. Methods: The hybrid model was used to perform the concept analysis of stress in childhood cancer patients' siblings. Through reviews of 16 studies in the theoretical phase and interviews with 20 siblings in the field phase, the derived results were brought together in the integration phase. Results: The concept of stress in siblings of patients with childhood cancer was found to have 6 attributes and 28 indicators in 3 domains. Personal factors included 2 attributes (fear about childhood cancer and immature coping skills), and family factors had 2 attributes (changes in relationships with family and changes in family environment), social factors had 2 attributes (changes in relationships with friends and in the school experience, and insufficient social support). Conclusion: The stress of siblings of childhood cancer patients was defined as a state of tension associated with personal, family, social factors that can be related to their siblings' childhood cancer. The findings in this study provide the base for the development of a tool for measuring siblings' stress and/or the development of nursing programs for these siblings.

Fatigue in Pediatric Patients with Cancer (소아청소년 암환자의 피로)

  • Purpose: This study was done to identify factors associated with fatigue in pediatric patients with cancer in order to develop nursing interventions for this population. Methods: The participants were 95 pediatric cancer patients admitted to a university medical center in Seoul, Korea, and 95 parents. The $PedsQL^{TM}$, Multidimensional Fatigue Scale developed by Varni (2002) was used to measure fatigue. Data were analyzed with the SAS program and t-test, analysis of variance (ANOVA), Pearson correlation coefficients, and multiple regression were used to identify the association of factors with fatigue. Results: The mean score for fatigue was 30.42 in the pediatric cancer patients and 34.77 in the parents. Fatigue was higher in patients living with a single parent, in patients whose father had a lower education and those patients with a fever. Pain, frequency and intensity of nausea and vomiting, depression, anxiety, and disruption of usual activity were positively associated with fatigue. The predictive factors for fatigue were disruption of usual activity, depression and living with or without parents. Conclusion: Pediatric patients with cancer experienced fatigue during their diagnosis and treatment. Of the multiple factors associated with fatigue, the association between disruption of usual activity and fatigue was the highest indicating a need to be concerned with this factor when providing interventions to alleviate fatigue.

Nutritional Assessment and Dietary Management during Anti-Cancer Chemotherapy in Pediatric Oncology Patients (항암 화학요법 중인 소아암 환자의 영양 평가 및 식이 관리)

  • Purpose: We assessed the nutritional status and the alterations of oral diets during anti-cancer chemotherapy in pediatric oncology patients. Methods: Twenty children with malignancy were evaluated from day 0 until day 21 of post-chemotherapy. Nutritional status was assessed by body weight and biochemical parameters. The amount and calories of oral diets were assessed and food preference before and during chemotherapy were analysed by questionnelle. Results: 1) The underlying diseases of 20 patients were 11 acute lymphoblastic lekemia, 2 non-Hodgkin's lymphoma, 2 Langerhans cell histiocytosis, 2 Wilm's tumor, 2 brain tumor, 1 rhabdomyosarcoma. 2) There were weight loss during chemotherapy in 8 patients (40.0%), weigt gain in 5 patients (25.0%), and no significant changes in 7 patients (35.0%). 3) Biochemical parameters showed no significant interval changes during chemotherapy except elevation of serum ALT level. 4) The daily caloric intakes of oral diets during chemotherapy were 310~600 Kcal which was much lower than average of daily recommended calory for Korean children. 5) The most favorate food was altered by chemotherapy, from meats to carbonated beverages and unfavorate food was not altered as vegetables. Conclusion: The periodic assessment of nutritional status and dietary supplements according to preferred foods of patients will be required for the optimal nutrition care in cancer patients.

Oral Complications after Antineoplastic Treatment in Pediatric Patients (소아환자의 항암치료 후 나타나는 구강 내 합병증)

  • The purpose of this study was to examine dental complications and to evaluate the effects of initial treatment age, treatment modalities, and treatment duration on the disorder after radiation and chemotherapy in pediatric cancer patients. For 93 children with clinical and radiographic data, the number of teeth, the morphology of teeth, the shape of the roots, and development status of the dentition were evaluated. Dental development disorders were found in 61.3% of the children. The mostly found abnormality was root deformity with the prevalence of 31.2%. In children submitted to the therapy before the age of 6, the number of missing teeth (p = 0.029) and microdontia (p = 0.003) were greater compared to the children who started to receive the treatment after the age of 6. The combination of radiation and chemotherapy showed significantly greater incidences of missing teeth (p = 0.030), microdontia (p = 0.046), and root deformity (p = 0.009) when compared with the sole application of chemotherapy. When the children were submitted to anticancer therapy for 18 months or longer duration, greater number of missing teeth (p = 0.032), microdontia (p = 0.011), root deformity (p = 0.025), and total number of teeth affected (p = 0.036) were observed compared with duration less than 18 months. The number of dental abnormalities increased when the children were treated at earlier ages, with combination of radiation and chemotherapy, and for longer period of time.

Bilateral Nephromegaly as a Presenting Symptom of Acute Lymphoblastic Leukemia (양측 신장비대로 진단된 급성림프구성 백혈병 1례)

  • Bilateral renal enlargement is a very rare manifestation as the primary presenting feature of acute lymphoblastic leukemia. We are reporting an unusual clinical picture of a 9-month-old male patient diagnosed as precusor B-cell lymphoblastic leukemia, who showed bilateral nephromegaly without any hepatosplenomegaly at the time of initial presentation.

Content Analysis on Psychosocial Adjustment of Adolescent Survivors of Leukemia (백혈병 생존 청소년의 심리사회적 적응에 대한 내용분석)

  • Purpose: This study was done to describe psychosocial adjustment of adolescents who have survived childhood acute lymphocytic leukemia (ALL). Method: Out of a total of 16 adolescents ($11{\sim}20$ years old) registered at the Pediatric Oncology Clinic at one university affiliated hospital, 13 adolescents agreed to participate in this study. The data were collected through in-depth interviews using a semi-structured questionnaire. The contents of the interviews were analyzed using the inductive content analysis method. Result: Three main categories were identified 'personal characteristics', 'coping patterns', and 'interpersonal relationship'. Personal characteristics included self-praise, self-confidence, altruism, being worrisome, and being difficult. Coping patterns included positive thinking, activeness, and avoidance. Interpersonal relationship included appreciation, intimacy, burdened by over protectiveness, and feelings of regret and equality as peers. Conclusion: The results indicate that adolescents who have survived childhood cancer have both positive and negative experiences. It is suggested that care providers identify and support the strengths of the adolescents in order to help them to adjust more positively after the experience of childhood cancer.

  • 송전선로 주변의 전자계 노출은 소아들의 암 발생/사망과 성인의 암 등 여러 가지 질병 발생 가능성이 항상 있으므로 전자계 노출을 장기간 계속적으로 받고 있는 환경을 관찰하는 것이 가장 중요하다. 따라서 본 연구에서는 이에 대한 연구기반을 형성하기 위해 송전 선로의 모델과 현장 실측을 통해 송전선로 자계치를 계산하여 시뮬레이션 하고자한다. 시뮬레이션을 통해 산출된 자계치를 이용하여 최근 효과적인 자계 저감책으로 보고되고 있는 수동루프에 의한 자계저감효과를 검토하고자 한다. 입력된 데이터는 765kV 실 송전선로를 모델로 사용하였으며, 자계의 계산 및 수동 루프에 의한 자계 저감 효과를 알아보기 위해서 미국 EPRI에서 개발한 자계 계산 전용 프로그램인 EMF Modeler를 이용하였다. 먼저 송전선로에 의해 발생되는 자계의 원리를 서술하였으며, 수동 루프를 통한 자계 감소 정도를 수식으로 유도하였고, EMF Modeler를 통하여 송전선 주변의 자계 분포 및 수통루프에 의한 자계 저감 효과를 각각 확인하였다.

Papillary Thyroid Carcinoma Arising from a Thyroglossal Duct Cyst in a 9-Year-Old Child (갑상설관 낭종에서 기원한 9세 소아의 유두 갑상선암 1예)

  • Thyroglossal duct cyst(TGDC) is the most common midline congenital neck mass in children. However, carcinoma arising from TGDC is very rare and most of them are found in adults. In this report, we describe a 9-year-old child presenting with TGDC, which finally turned out to be papillary thyroid carcinoma. He underwent Sistrunk operation only. We review the literature and highlight the important points of the treatment.
