  • Fine needle aspiration cytology(FNAC) is a valuable method in preoperative diagnosis of thyroid nodule. Authors evaluated one hundred and ten thyroid nodules by FNAC compared with the postoperative histopathologic diagnosis during the period from Jan. 1, 1989 through Dec. 31, 1992. The results were as followed. The sensitivity was 86.5%, specificity 90.4%, false-negative rate 13.5%, false-positive rate 9.6%, positive predictability 91.5%, negative predictability 60.1% and overall diagnostic accuracy 87.3 %.

  • 랑게르한스 세포 조직구증은 골수에서 유래하는 랑게르한스 세포 조직구의 이상 증식에 의해 발병하는 희귀한 질병으로 알려져 있다. 비록 모든 장기에서 발생 할 수 있으나 갑상선을 침범하는 경우는 매우 드물다. 18세 남자가 5달전부터 점점 커지는 갑상선 종괴를 주소로 내원하여 세침흡인 세포검사, 총샘검, 경부 전산화단층촬영을 시행하였다. 세침흡인 세포검사에서 악성신생물이 의심되었고, 총생검에서 랑게르한스 세포 조직구증으로 나타났다. 경부 전산화단층촬영에서는 우측 갑상선에서 윤곽이 잘 구분되는 저음영의 종괴와 우측 기관 주위 림프절의 종대가 관찰되었다. 갑상선 전절제술과 우측 중앙 선택적 경부 림프절 청소술이 시행되었다. 랑게르한스 세포 조직구증이 갑상선을 침범하는 경우는 드물지만 갑상선 비대가 있는 환자가 뇌하수체 기능부전의 증상이나 뼈와 폐의 침범과 관련된 증상을 호소한다면 갑상선의 랑게르한스 세포 조직구증 침범을 고려해야 한다. 또한, 다른 장기의 랑게르한스 세포 조직구증을 치료한 과거력이 있는 경우는 갑상선 종괴를 감별 진단하는데 있어 랑게르한스 세포 조직구증을 고려해야 한다.

Clinical Value of fine Needle Aspiration Biopsy in Head and Neck Masses (두경부 종물에 대한 세침천자생검의 임상적 의의)

  • Fine needle aspiration biopsy(FNAB) is widely used in screening of head and neck(H&N) masses because it has high accuracy and few damage. The 235 cases of FNAB were performed on patient with H&N mass at the Department of Otolaryngology, Hanyang University Hospital during 1 years, from March 1985 to February 1992. The 188 of 235 were proven histologically, clinically. The results obtained were as follows ; 1) FNAB for malignant H&N mass shows sensitivity of 81.5% specificity of 96.8%. 2) FNAB for malignant LN shows sensitivity of 86.0%, specificity of 88.9%. 3) FNAB for salivary gland shows sensitivity of 66.7%, specificity 81.8%. 4) FNAB for soft tissues, bone, and intraoral lesions shows sensitivity of 90%, specificity 95.8%. 5) FNAB for tuberculosis shows false negative of 55.2%. 6) The result of FNAB was not closely related with location or size of neck nodes.

Performance of Artificial Intelligence and Elastography in Thyroid Nodule Diagnosis (갑상선 결절 진단에서 인공지능과 탄성초음파의 성능 분석)

  • The objective of this study is to analyze and evaluate the diagnostic performance and utility of elastography and artificial intelligence program in distinguishing between benign and malignant thyroid nodule. In a hospital outpatient clinic, we performed thyroid ultrasound from January 2023 to June 2024, and retrospectively analyzed 126 patients who performed elastography, S-detect, and fine needle aspiration cytology(FNAC) because nodules were found. The analysis of differences based on cytology results showed statistically significant differences in age, nodule size, echogenicity, nodule orientation, margins, shape, presence of calcification, posterior shadowing, K-TIRADS ultrasound interpretation, S-detect results and elasticity contrast index. The ROC curve analysis determined a cut off value for the elasticity contrast index at 2.32, with diagnostic concordance rates of 0.66 for expert interpretation, 0.49 for S-detect, and 0.67 for the elasticity contrast index, indicating superior diagnostic performance with elastography. Thus, elastography may ge used as an adjunct tool to minimize unnecessary repeat examinations and the frequency of tissue biopsies in the diagnosis of thyroid nodules.

Diagnostic effectiveness of fine needle aspiration cytology on pediatric cervical lymphadenopathy (소아 경부 림프절 종대의 세침 흡인 세포검사의 진단적 유용성)

  • Purpose : There are few reports in Korean literature on the diagnostic efficacy of fine needle aspiration cytology(FNAC) for pediatric cervical lymphadenopathy. This study examined the diagnostic value of FNAC on cervical lymphadenopathy in children. Methods : Data from 57 pediatric patients(aged 0.3 to 14 years) who underwent FNAC due to cervical lymphadenopathy between January 2001 and March 2005 was reviewed retrospectively. Results : Reactive cervical lymphadenitis was the most common result of the FNAC(52.5 percent). Malignant disease were revealed in 14 percent of all cases. The sensitivity, specificity was 86 percent and 96 percent, respectively. The positive and negative predictive values for malignant disease was 75 percent and 98 percent, respectively. There were two false-positive cases on FNAC; one case of Langerhans cell histiocytosis, and another with infectious mononucleosis. A false-negative case on FNAC was found to be acute lymphocytic leukemia. In seven cases(12 percent), the final diagnosis was confirmed by an open biopsy after the FNAC. There were four FNAC cases where the specimen was not satisfactory for making a diagnosis(7 percent). There were no serious complications of the FNAC procedure. Conclusion : FNAC had a high diagnostic efficacy for evaluating children with cervical lymphadenopathy.

Role of Cytologic Scoring System in Minimizing "Gray Zone" in Breast Aspiration Cytology (진단이 애매했던 유방 세침흡인 세포검사에서 등급 점수표의 역할)

  • Fine needle aspiration(FNA) has been quite successful in identifying benign and malignant breast lesions, but a "gray zone" exists. A total of 697 FNAs of breast were performed at Korea Cancer Center Hospital for a period of one year. One hundred and eleven of the 697 FNAs were diagnosed as atypical or suspicious for malignancy. Among them, we reviewed 74 FNAs, un which histologic diagnoses were made, and applied cytologic grading system proposed by Masood et al (1990) to evaluate the usefulness of this system in minimizing the size of gray zone. Technical problem was responsible for equivocal diagnoses in 19 FNAs Of the remaining 55 FNAs, 18 were benign and 37 were malignant. Among benign conditions, fibroadenoma(5 cases) and fibrocystic disease with fibroadenomatous feature(3 cases) constituted the largest groups. The majority of malignant conditions were infiltrating ductal carcinoma(29 cases), however, those low grade carcinomas Including tubular carcinoma(3 oases), cribriform carcinoma(2 cases), and mucinous carcinoma(2 cases) occupied a relatively large proportion Cytologic grading system was quite useful in minimizing the size of gray lone. The scores of 27 out of 29 usual infiltrating ductal carcinomas belonged to the group of cytologic malignancy, how-ever, only 2 out of 7 low grade carcinomas got scores of malignancy FNA from fibroadenoma or fibrocystic disease with fibroadenomatous features showed a tendency toward high scores. Experience of the cytopathologist and familiarity with cytologic alteration in breast disease cannot be overemphasized.

Evaluation of Diagnostic Usefulness of Thyroid Lesions of Deep Learning-based CAD System (딥러닝을 기반으로 한 CAD 시스템의 갑상샘 질환의 진단 유용성)

  • This study aims to evaluate the diagnostic concordance and accuracy by comparing thyroid lesions diagnosed with the artificial intelligence-based computer-aided diagnosis (CAD) system, S-DetectTM, to the results of fine-needle aspiration biopsy(FNAB). A retrospective study was conducted involving 60 patients at N Hospital in Gyeongnam from May 2023 to September 2023. The study used S-DetectTM to analyze ultrasound findings and malignancy risk of thyroid nodules and compared these findings with FNAB results to determine accuracy. The study assessed the sensitivity, specificity, accuracy, positive predictive value (PPV), and negative predictive value (NPV) of S-DetectTM and evaluated the diagnostic concordance between the two methods using Kappa analysis. S-DetectTM demonstrated a sensitivity of 90.5%, specificity of 83.2%, accuracy of 88.3%, PPV of 80.7%, and NPV of 92.7%. The Kappa value for diagnostic agreement between S-DetectTM and FN AB was 0.719 (p<0.05), indicating a high level of agreement between the methods. Therefore, the CAD system S-DetectTM proves valuable in distinguishing between malignant and benign thyroid lesions and could reduce unnecessary tissue examinations when used appropriately before thyroid fine-needle aspiration.

Clinical Evaluation of Tissue Biopsy for Children with Neck Mass; A Single Center Study (조직검사를 시행한 소아 경부 종류의 임상적 고찰 : 단일 기관 연구)

  • Purpose : Neck masses, in pediatric population, derive from a multitude of congenital, inflammatory, or neoplastic diseases. The majority of these masses represent benign conditions. However, thorough clinical evaluation is required to rule out malignant diseases. We evaluated the causes, clinical characteristics and outcomes of children with neck masses who underwent tissue biopsy. Methods : A total of 28 medical records of children with neck mass who underwent tissue biopsy at Chungnam National University Hospital, from January 2000 to March 2004 were retrospectively analyzed. The methods of biopsy were ultrasonography guided core biopsy(CB), fine needle aspiration biopsy(FNAB) and excisional biopsy. Results : Out of 28 patients, half were boys. The most common location of the mass was the posterior cervical area(N=19, 67.9%). Laboratory findings of peripheral blood and serologic studies were nonspecific. In 25(89.3%) cases, CB or FNAB was initially performed for neck masses. Among them 10 cases(40%) were reactive hyperplasia, 8(32%) inflammatory granulation tissues, 4(16%) necrotizing lymphadenitis, and 3(12%) acute suppurative inflammations. Initially, excisional biopsy was performed for diagnosis in 3(10.7%) cases. Diagnosis of these cases was thyroglossal duct cyst, dermoid cyst and lymphoblastic lymphoma, respectively. Conclusion : Most neck masses in children were benign. CB and FNAB were safe methods for tissue sampling, without need for general anesthesia.

Peripheral Neuroblastoma of the Ulnar Nerve : Diagnosis by Fine Needle Aspiration Cytology (척골신경에 발생한 말초성 신경아세포종 -세침흡인 세포검사로 진단된 1례 보고-)

  • A 30-year-old woman who was diagnosed as peripheral neuroblastoma by fine needle aspiration of a soft mass of the right upper arm is described. She presented a slowly growing, soft mass of the right upper arm for 1 month. The right humerus revealed no abnormal finding on X-ray. Ultrasonogram of the right upper arm revealed a well demarcated, smooth marginated solid mass without invasion of adjacent structures. Fine needle aspiration was done under the impression of soft tissue tumor with undetermined biologic behavior. The aspirates were highly cellular and the tumor cells were dispersed both singly and in clusters of varying size. The clusters occasionally showed a central capillary core and rosette-like structures. The tumor cells were small in size and had a small to medium amount of cytoplasm. Some of them revealed slender cytoplasmic processes. The nuclei showed distinct nuclear membranes, finely clumped chromatin and small conspicuous nucleoli. Cellular pleomorphism or mitotic figure was not definite. These cytologic findings were interpreted as a malignant, non-lymphomatous small round cell tumor, most likely representing peripheral neuroblastoma or Ewing's sarcoma. Final diagnosis was confirmed by simple excision as peripheral neuroblastoma.
