Diagnostic Imaging of Lipoma in the Retroperitoneum and Pelvic Cavity in a Dog (개의 후복강과 골반강에 발생한 지방종의 진단영상 증례)

  • An 11-year-old, spayed female, weighing 10 kg miniature Schnauzer was admitted for abdominal distension, anorexia, pollakiuria, and constipation. A large, homogeneous fat opacity mass was identified in the retroperitoneum and pelvic cavity on the abdominal survey radiography. The mass displaced the descending colon peripherally and ventrally, and the urinary bladder ventrally. On the abdominal ultrasonographic examination, the mass was uniformly hyperechoic with a coarse internal echo texture and had outer hyperechoic capsule, and it showed homogeneously same attenuation (-180 ~ -110 HU) as adjacent fat on the computed tomography. There was no evidence of invasion into the surrounding structures or organs. Cytological findings from fine needle aspirates were numerous sheets and clusters of adipocytes. The dog showed complete resolution of clinical signs after surgical resection of the mass. The mass was confirmed as simple lipoma through histopathological examination.

  • Sono-guided fine needle aspiration (FNA) of the thyroid is widely used, but the aspirated samples are typically not well preserved and low cellularity makes diagnosis difficult in many cases. The object of the current study is to evaluate the adequacy and diagnostic accuracy of the use of $SurePath^{TM}$ liquid-based cytology (SP-LBC) in the sonoguided fine needle aspiration of the thyroid nodule and to compare its use with that of the use of a conventional smear (CS). A total of 172 sono-guided FNAs of thyroid nodules from April to June, 2006 were prepared by the use of the split method with either SP-LBC or CS; the samples were stained with the use of hematoxylin-eosin (H&E) and Papanicolaou (Pap) stains. A cyto-histological correlation was performed in 69 (30 SP and 39 CS) cases that had been histologically confirmed. The rate of producing unsatisfactory slides by the use of the SP-LBC method (9.3%) was less than that of the use of the CS method (20.9%). The diagnostic accuracy of the SP method (93.3%) was better than that of the CS method (85.3%). The sensitivity and specificity of the SP method (94.4% and 92.3%) was better than that of the CS method (83.3% and 70%), respectively (p < 0.05). The CS of sono-guided aspirated specimens had some unavoidable limitations related to inadequate sampling such as a bloody background, low cellularity and an indication that some clinicians smeared many useless slides (averaging four to ten slides), and that most slides showed only blood that included few follicular cells. The SP method resulted in more thinly smeared slides and showed cleaner background and greater cellularity than the use of the CS method. Each follicular cell shows superior nuclear detail, and more distinct cytoplasmic features than with the use of the CS method. SP-LBC appears to be an easy, highly accurate, and reliable cytological method for employ for a diagnostic approach of thyroid disease and thyroid nodules. The SP-LBC method is a suitable alternative to the CS method to overcome diagnostic difficulties.

Fine Needle Aspiration Cytology in the Diagnosis of Tuberculosis (결핵 진단의 세침 흡인 세포검사)

  • A total of 48 cases of tuberculous lesion in the lymph nodes (43 cases), lung (3 cases) and soft tissue (2 cases), was subjected to fine needle aspiration cytology (FNAC). The age of the patients ranged from 19 to 77 year-old (average 33.6 years in age) and the male to female ratio was 1 : 4. Thirty-four cases (70.8%) demonstrated distinct granulomatous reaction with or without caseation necrosis, nine cases (18.8%) showed no granulomas, but large amount of necrotic debris with numerous polymorphonuclear cells and histiocytes, and five cases (10.4%) revealed acellular material only. The overall AFB positivity in smears was 62.5%. In areas associated with granulomatous reaction and necrosis, AFB positivity was 55.8%, while it was 80.0% in cases with acellular necrotic material. There were 2 cases of parasitic infestation which could not be easily differentiated from tuberculosis based on aspiration smears only.

  • A 36-year-old man was studied because of signs and symptoms of superior vena caval syndrome. Chest computerized tomography showed $10{\times}8{\times}6$ cm sized lobulated anterior mediastinal mass, compressing superior vena cava. Fine needle aspiration cytology revealed seminoma. There was no detectable tumor mass in the testes. We report a case of primary mediastinal seminoma presenting with superior vena caval syndrome.

  • Fine needle aspiration is usually performed as a preoperative cytologic evaluation in salivary gland tumors, and complications of the lesion after fine needle aspiration are scarcely developed. The secondary change of tissue by fine needle aspiration can make the diagnostic and subsequent therapeutic difficulties for clinicians and require a careful approach. Fine needle aspiration can cause variety of changes in Warthin tumor, and it is presumed that those changes are mainly caused by the infarction of the mass due to vascular injury and following inflammatory reactions. We would introduce our experience of granulomatous and necrotic change after fine needle aspiration on parotid Warthin tumor with consequent difficulties in diagnostic and surgical approach to the patient.

Cytologic Aspects of Oral Squamous Cell Carcinoma in a Captive African Hedgehog (Altelerix albiventris) (고슴도치의 구강 편평상피암종의 세포학적 고찰 1증례)

  • A 3-year-old intact female hedgehog (Atelerix albiventris) was presented for evaluation of mandibular swelling. Fine needle aspiration of swollen mandibular was performed and smears were stained with Romanowsky type stain for cytological evaluation. Smears were highly cellular with predominance of variably shaped keratinized or non keratinized squamous cells with low numbers of spindloid to abnormally elongated cells. Cytologic impression was squamous cell carcinoma. The mass was surgically removed for histological examination. Microscopically tortuous and anastomosing delicate to broad pegs and nests of neoplastic squamous epithelial cells were supported by a moderate collagenous and spindloid fibroblast stroma. Tumor cells had moderate anisocytosis and mild anisokaryosis and range from moderately to well keratinized, with areas of intratumoral acantholysis accompanied by mixed stromal lymphoplasmacytic, neutrophilic inflammation. The mitotic index is 2-3 per high-power field. Tumor cells were expanding the subcutis subjacent to the layer of skeletal muscle and incorporating the osseous tissue fragments. The final diagnosis was squamous cell carcinoma. The patient survived three months after surgery without any further medical treatments.

  • Cryptococcosis is a systemic fungal infection caused by Cryptococcus neoformans. This mycosis is best known for meningeal involvement, yet, this disease rarely can be limited to the lungs. The usual portal of entry is respiratory tract. The recently rising incidence of the disease can be attributable to wide use of steroid, immunosuppressive agents and advent of AIDS. We experienced one case of pulmonary cryptococcosis in healthy person, who was admitted to the our hospital because of cough. Cryptococcus neoformans was isolated in the sputum, and lung tissue obtained by CT guide needle aspiration biopsy. Clinical improvement was noted after 6weeks of medical therapy with fluconazole.

  • Pleomorphic adenoma is the most common neoplasm of the parotid gland, generally presenting as a slowly growing, firm, well-circumscribed, painless nodule. It is often difficult to separate into benign and malignant categories because its bland histomorphologic and cytologic features. This tumor can be rarely associated with cystic change, hemorrhagic necrosis, or spontaneous infarction. Necrosis of lesional tissue may be associated with malignant transformation, particularly in a pleomorphic adenoma. We report a case of 50-year-old woman presented with a enlarging right parotid mass. Computed tomographic scan demonstrated a right superficial lobe mass with ill-defined border. The preoperative fine needle aspiration yielded necrotic debris and atypical squamous elements that were thought to be compatible with high degree of mucoepidermoid carcinoma. A total parotidectomy with intraoperative frozen section revealed extensive necrosis and diagnosed as malignant tumor. This tumor was finally diagnosed as a pleomorphic adenoma with necrosis on permanent sections. Caution should be exercised in evaluation of the parotid neoplasms with central necrosis to avoid misdiagnosis of such lesions as malignancy.

  • Thyroid nodular disease is the most frequently appeared in thyroid disease. Thyroid ultrasonography offers location of nodules, size, the number, information of internal echo characteristic. Thus, it makes possible to sort high-risk nodule containing high possibility about thyroid cancer and to induct precisely when take a Fine Needle Biopsy Aspiration. On thyroid nodule, the case which is diagnosed as malignant is less than 5% but screening test is very important on ultrasound and also must be reduced unnecessary procedure. Therefore, in this study an approach for describing a region is to quantity its texture content. We applied TFA algorithm on case which has been pathologically diagnosed as papillary thyroid cancer. we obtained experiment image which set the ROI on ultrasound and cut the $50{\times}50$ pixel size, histogram equalization. Consequently, Disease recognition detection efficiency of GLavg, SKEW, UN, ENT parameter were high as 91~100%. It is suggestion about possibility on CAD which distinguishes thyroid nodule. In addition, it will be helpful to differential diagnosis of thyroid nodule. If the study on additional parameter algorithm is continuously progressed from now on, it is able to arrange practical base on CAD and it is possible to apply various disease in the thyroid US.

Pathological Factors Affecting DNA Quality in BRAF, EGFR, and KRAS Gene Molecular Tests (BRAF, EGFR, KRAS 유전자 분자병리검사에서 DNA 품질에 영향을 미치는 병리학적인 인자에 관한 연구)

  • The quality control of pathological specimens is important for accurate molecular pathology testing. This study evaluated that specimen factors affecting the DNA quality during tissue processing and sample types for BRAF, EGFR, and KRAS mutations tests. One thousand seven hundred and seventy-two molecular pathology tests were investigated for the factors influencing the DNA quality, such as sample type, formalin fixation time, and reexamination status. Cytology samples stored in a saline solution had better DNA quality than commercial cytology preservation. Tissue samples fixed in formalin within 24 hours had better DNA quality than the samples fixed over 24 hours. Between the types of samples, fresh tissue samples and tissue samples with a high tumor cell density had relatively better DNA quality than the formalin-fixed paraffin-embedded (FFPE) tissues and cytology specimens. Of real-time PCR, the non-PNA Ct value increased proportionally with samples held for longer than 24 hours in formalin, and that the formalin-fixed time affects the sample DNA quality. In conclusion, the appropriate tumor cellularity and 10% neutral formalin fixation time are the most important factors for maintaining the DNA quality. These factors should be managed properly for an accurate pathological molecular test to ensure optimal DNA quality.
