  • Background and Objectives : Recently, many people suffering from voice change visit otolaryngologist. So, it is important to study vocal function in patients with glottic or laryngeal disease. Aerodynamic investigation is valuable information about the efficiency of the larynx in translating air pressure to acoustic signal. The purpose of this study was to investigate the aerodynamic data in patients with vocal polyp, compare this data with that of the normal Korean. Materials and Methods : In aerodynamic study, maximum phonation time, mean air flow rate, phonatory flow volume and subglottal pressure were tested by using Aerophone II voice function analyzer in 157 normal korean and 143 patients with vocal polyp, aged from 20 to 69 years randomly selected. Results and Conclusion : In vocal polyp, maximum phonation time was significantly decreased and mean air How rate was increased. Phonatory flow volume was significantly decreased and subglottic pressure was increased only in female with vocal polyp. These data will be served as basic data of evaluation after treatment and postoperative assessment of the patients with vocal polyp.

  • Background and Objectives: Vocal polyp is the most common disease that causes hoarseness and its incidence is increased currently. The purposes of this study are to investigate the correlation between severity of vocal polyp and acoustic parameters and compare these data with those of the normal Korean. Materials and Methods: We analyzed the acoustic parameters of sustained vowel for 70 vocal polyp patients and 20 normal controls. A CSL(computerized speech lab) was used to carry out the analysis of the voice sample and statistical analysis was used Spearman correlation coefficient & t-test. Results: According to the conclusion of correlation analysis, 21 parameters of all the 34 parameters are significant. Conclusion: These data will be served as basic data for the evaluation of postoperative assessment of the patients with vocal polyp.

  • Background and Objectives : Aerodynamic analysis is an examination which provides information regarding various vocalization measures indicating laryngeal efficiency. Voice evaluation using such examination must be capable of distinguishing between normal to abnormal voice. It also observes variables on aerodynamic characteristics by gender in regards to patients of vocal disorders, especially of vocal cord paralysis and vocal polyp, and compares the conditions before and after surgery. This paper therefore, seeks to build a framework for establishing standard levels of aerodynamical characteristic on vocal disorders. Subjects and Methods : The study was intended for a total number of 20 patients with vocal polyp or unilateral vocal cord paralysis. Those with the vocal polyp underwent laryngomycroscopy surgery and the vocal cord paralysis, vocal fold injection using Restylane. Aerodynamic analysis fulfilled the Maximum sustained Phonation (MXPH) and Voicing Efficiency (VOEF) by using PAS Model 6600 (KayPENTAX, USA). Results : In MXPH, increase in PHOT were evident with vocal polyp after surgery. As for patients with vocal cord paralysis, MAXDB, MEADB, DHODB, PHOT all have increased and MEAP, PEF, MEAF decreased after surgery. In VOEF, patients with vocal cord paralysis who underwent surgery showed increase in MAXDB, MEADB, DHODB, FET100, ARES, but decreases in PEF, TARF. Conclusion : Overall, it can be concluded that patients with the vocal polyp and vocal cord paralysis seemed to get closer to the normal values after than before surgery in majority of measures. This confirms that the function of their vocal cord has improved nearly to normality through operations.

  • Objectives:This study aims to investigate the correlation between the measurements of Praat as an acoustic evaluation and those of GRBAS and CAPE-V as perceptual rating tool respectively. Through this, it also tries to find out parameters to which attention should be paid when an evaluator, who is untrained in auditory-perceptual voice evaluation, conducts voice evaluation with objective tool. Materials and Methods:Voice samples of this study were 33 vocal polyp patients(23 males and 10 females) who visited our Department of Otorhinolaryngology. They sustained vowel voices of 'e' were recorded and acoustically analyzed. Results:As the results of correlation analysis between GRBAS and Praat measurements, G scale and R scale showed statistically significant correlation with Jitt, Shim and NHR. And it is found that B scale represented significant correlation with Jitt, S scale with Shim. As the results of analysis on correlation with CAPE-V and Praat measurements, OS scale and R scale showed statistically significant correlation with Jitt, Shim and NHR. B scale represented significant correlation with Jitt, S scale with Shim. Conclusion:Although, both GRBAS and CAPE-V were highly reliable, in comparison between CAPE-V and Pratt, more parameters that showed statistically significant correlation are observed, which implies that VAS has more potential to make detailed evaluation than ORD.

  • Background and Objectives : Vocal polyps and nodules are representative chronic benign laryngeal disease. Treatment options for vocal polyp and nodule in general include voice therapy or laryngeal microsurgery. The purpose of this study was to analyze voice results before and after laryngeal microsurgery. Materials and Method: Vocal polyp and vocal nodule patients were treated by laryngeal microsurgery from March 2004 to December 2006 at Kosin University Hospital. All were women. Voice analysis studies were done before and after laryngeal microsurgery. Five measurements were performed: MPT, Fo, jitter, shimmer and NHR. Results: There was significant improvement in the vocal polyp patients regarding MPT, jitter, shimmer and NHR. Also there was significant improvement in the vocal nodule patients regarding MPT, jitter and shimmer. Conclusion: MPT, jitter, shimmer and NHR will be effective acoustic parameters in documenting the quantitative changes in the vocal polyp patients. MPT, jitter and shimmer will be effective acoustic parameters in documenting the quantitative changes in the vocal nodule patients before and after laryngeal microsurgery.

  • The present study examined the change of aerodynamic features after laryngomicrosurgery in patients with vocal polyps. Aerodynamic evaluation was performed in thirty-nine patients (15 males and 24 females) one week before surgery and four weeks after surgery. Evaluation protocols of vital capacity, maximum sustained phonation(MXPH), and voicing efficiency(VOFT) were used to collect 29 phonatory aerodynamic measures, requiring voice with a comfortable pitch and loudness. Statistically significant changes were found for phonation time and airflow values in the MXPH protocol, while changes were also found for airflow values, subglottal pressure values and acoustic resistance values in the VOFT protocol. Although phonation time was increased in both male and female patients, gender-dependent changes were found in airflow measurements. Men's phonation time increased with no difference in airflow rate, but women's phonation time increased with decreased airflow rate and lower subglottal pressure. The changes of aerodynamic features may be affected by women's self-perceived change for vocal attitude, which was reducing sense of vocal effort after surgery.

  • Vocal folds injury from vocal abuse is important topics of phonosurgery. Recent advances in diagnostic equipment, phonosurgery and speech analysis equipment have provided a lot of Information about fine movement of the vocal folds. However, predicting the reaction of the vocal folds to phonatory trauma remains difficult. The vocal folds need to withstand great vibratory and shearing stress and anchoring fibers of basement membrane Bone play a role in maintaining structural integrity of histologically different epidermis and superficial layer of lamina propria(cover of vocal folds). The purpose of this study is to demonstrate the changes of anchoring fibers in vocal polyp using transmission electron microscope. Various defects were observed : a irregular thickening of basement membrane, a near absence of normal anchoring fiber, a lot of electron dense material in superficial layer of lamina propria, a destruction of hemidesmosome and many vesicles carrying electron dense material In basal keratinocyte. These observations were suggestive of a hyperactivity of basal keratinocyte of vocal folds epithelium in response to vibratory stress.
