A System for Nutritional Assessment and Diagnosis of Dietary Intakes through Internet (인터넷을 이용한 식사섭취의 진단 및 영양평가 시스템)

  • 본 연구는 영양분야의 전문 웹사이트로서 인터넷 상에서 이용할 수 있는 영양평가 시스템을 개발하기 위하여 수행되었다. 프로그램의 구성은 표준체중 및 열량필요량 자료, 식품 및 음식의 영양소 함량, 영양소별 20순위 식품, 식사섭취자료, 한국인 영양권장량 자료 등을 데이터베이스로 하여 사용자의 신체계측치와 활동정도에 따른 비만도 및 열량필요량의 분석과 섭취한 음식에 대하여 영양섭취상태를 평가받고 영양소별 함유량이 높은 식품과 음식에 대한 열량 및 영양소의 정보를 제공받을 수 있도록 웹 페이지 형식으로 만들어졌으며, 사용자는 인터넷 사이트로 들어가 자신의 이름, 성별, 나이, 키, 체중 및 활동정도를 입력함으로서 자신의 체중범위, 표준체중, 체격지수, 비만도, 기초대사량, 1일 열량필요량 및 이들의 섭취열량과의 과부족을 비교 평가한 자료를 얻을수 있으며, 만약 사용자가 비만이라면 체중을 감소하기 위한 1일 열량필요량도 얻을 수 있다. 또한 사용자는 음식입력 항목을 선택하여 날짜별, 식사별로 자신이 하루동안 섭취한 음식 및 섭취량 등을 입력하고 영양섭취상태를 클릭하면 식사섭취상태의 진단과 평가를 받을 수 있다. 즉 열량과 각 영양소에 대한 하루의 섭취량 및 아침, 점심, 저녁 및 간식의 섭취량이 분석되어지며 이들중 권장량이 정해진 영양소는 다시 사용자의 섭취량에 대한 권장량광의 비율을 그래프로 나타냄으로서 섭취된 영양소의 과부족을 쉽게 평가받을 수 있다. 지방질 역시 구성지방산과 콜레스테롤의 섭취 상태가 분석되어 이들의 표준섭취비율과 비교해 제시됨으로서 그릇된 섭취형태를 평가받을 수 있다. 이 밖에 식사별 열량영양소 구성비가 표준섭취비율과 비교 평가되어 그래프로 나타나며 급원식품에 대한 영양소 정보를 줄 수 있는 식품군별 영양소 섭취상태와 동식물성 식품 섭취상태도 평가되어진다. 따라서 본 프로그램은 인터넷을 이용하여 누구라도 자신의 영양섭취상태를 평가받고 과부족이 되기 쉬운 영양소의 급원 식품과 음식에 대한 영양정보도 제공받을 수 있도록 할 뿐 아니라 식이요법을 필요로 하는 질환의 영양상담을 위한 기본 프로그램으로 활용될 수 있도록 하는데 그 의의가 있다.

Safety Assessment of Estimated Daily Intakes of Antioxidants in Korean Using Dietary Survey Approach and Food Supply Survey Approach (식이를 통한 평가방법과 공급량 평가방법을 이용한 산화방지제 일일 추정 섭취량 안전성 평가)

  • This study evaluated daily intakes of BHT, BHA, and TBHQ in Korean. The daily intakes were estimated using both a dietary survey approach and food supply survey approach. In the dietary survey approach, individual dietary intake data from the Korea National Health and Nutrition Survey in 2005, as well as analytical results of BHT in 131 samples, BHA in 134 samples, and TBHQ in 104 samples, were used to assess daily intakes of the antioxidants. In the food supply survey approach, both total production amounts of BHT, BHA and TBHQ and maximum permitted levels of the antioxidants were used to calculate daily intakes. In the dietary survey results, the average daily intakes of BHT, BHA and TBHQ were 0.8, 0.5, and 0.3 ${\mu}g$/kg body weight/day, respectively, and below 0.2% of the ADI (acceptable daily intake) set by JECFA (Joint FAO/WHO Expert committee on Food Additives). In the food supply survey approach, the average daily intakes of BHT, BHA,and TBHQ were all 0.3 mg/kg body weight/day. The ratios of ADI were 97, 60, and 40%, respectively. According to these results, daily intakes of BHT, BHA, and TBHQ in Korean are lower than the ADI.

Assessment of Estimated Daily Intake for Preservatives by Maximum Permitted Level and National Food Disappearance Data (식품소비량과 최대허용량을 이용한 보존료의 추정섭취량 평가)

  • Daily intakes of 14 preservatives were evaluated by using their maximum permitted levels(MPL) and national food disappearance data in 1998. The estimated daily intake (EDI) of each preservatives were compared with corresponding acceptable daily intakes (ADIs). EDIs of dehydroacetic acid, sodium dehydroacetate, $\rho$-hydrobenzoic acid ester, propionic acid, sodium propionate and calcium propionate were less than 2% of ADI and judged to be safe. However, EDI of sorbic acid and potassium sorbate were 76.61 mg/person/day and it reached 5% of its ADI.. EDI of benzoic acid and sodium benzoate were 85.65 mg/person/day and it reached 31% of its ADI. The highest intake of benzoic acid came from carbonated drink.

  • The aims of this study were to estimate daily intake of macrominerals from beverages, liquid teas, and liquid coffees and to evaluate their potential health risks for Korean children and adolescents (1-to 19 years old). Assessment of dietary intake was conducted using the actual level of sodium, calcium, phosphorus, potassium, and magnesium in non-alcoholic beverages and (207 beverages, 19 liquid teas, and 24 liquid coffees) the food consumption amount drawn from "The Fourth Korea National Health and Nutrition Examination Survey (2007-2009)". To estimate the dietary intake of non-alcoholic beverages, 6,082 children and adolescents (Scenario I) were compared with 1,704 non-alcoholic beverage consumption subjects among them (Scenario II). Calculation of the estimated daily intake of macrominerals was based on point estimates and probabilistic estimates. The values of probabilistic macromineral intake, which is a Monte-Carlo approach considering probabilistic density functions of variables, were presented using the probabilistic model. The level of safety for macrominerals was evaluated by comparison with population nutrient intake goal (Goal, 2.0 g/day) for sodium, tolerable upper intake level (UL) for calcium (2,500 mg/day) and phosphorus (3,000-3,500 mg/day) set by the Korean Nutrition Society (Dietary Reference Intakes for Koreans, KDRI). For total children and adolescents (Scenario I), mean daily intake of sodium, calcium, phosphorus, potassium, and magnesium estimated by probabilistic estimates using Monte Carlo simulation was, respectively, 7.93, 10.92, 6.73, 23.41, and 1.11, and 95th percentile daily intake of those was, respectively, 28.02, 44.86, 27.43, 98.14, and 3.87 mg/day. For consumers-only (Scenario II), mean daily intake of sodium, calcium, phosphorus, potassium, and magnesium estimated by probabilistic estimates using Monte Carlo simulation was, respectively, 19.10, 25.77, 15.83, 56.56, and 2.86 mg/day, and 95th percentile daily intake of those was, respectively, 62.67, 101.95, 62.09, 227.92, and 8.67 mg/day. For Scenarios I II, sodium, calcium, and phosphorus did not have a mean an 95th percentile intake that met or exceeded the 5% of Goal and UL.

The Experience on Intake Estimation and Internal Dose Assessment by Inhalation of Iodine-131 at Korean Nuclear Power Plants (국내 원전에서 $^{131}I$ 내부 흡입 에 따른 섭취량 산정과 내부피폭 방사선량 평가 경험 몇 개선방향에 대한 연구)

  • During the maintenance period at Korean nuclear power plants, internal exposure of radiation workers occurred by the inhalation of $^{131}I$ released to the reactor building when primary system opened. The internal radioactivity of radiation workers contaminated by $^{131}I$ was measured using a whole body counter. Intake estimation and the calculation of committed effective dose were also conducted conforming to the guidance of internal dose assessments from publications of International Commission on Radiological Protection. Because the uptake and excretion of $^{131}I$ in a body occur quickly and $^{131}I$ is accumulated in the thyroid gland, the estimated intakes showed differences depending on the counting time after intake. In addition, since ICRP publications do not provide the intake retention fraction (IRF) for whole body of $^{131}I$, the IRF for thyroid was substitutionally used to calculate the intake and subsequently this caused more error in intake estimation. Thus, intake estimation and the calculation of committed effective dose were conducted by manual calculation. In this study, the IRF for whole body was also calculated newly and was verified. During this process, the estimated intake and committed effective dose were reviewed and compared using several computer codes for internal dosimetry.

  • The purpose of this study is to develop a software program to diagnose and assess status of dietary intake obtained using 24-hour dietary recall method. This program consist of four functions. The first function is assessing tile general status of the body such as ideal body weight, obesity measure, activity expenditure energy and total energy requirement by the analysis of age, height, weight and the degree of activity. The second one is calculating the intake number of food classified by the food group and evaluating the status of food intake by comparing them with the number of standardized intake in recommended dietary allowances for Koreans,6th revision. Third one is calculating energy and nutrient intakes contributed by the food group in dietary intake and evaluating the status of dietary intake by comparing the nutrient intake with the recommended dietary allowance for Korean, especially calculating and evaluating the status of dietary fat intake. The fourth function is calculating and evaluating the status of nutrient intake and nutrients energy Percent contributed by three regular meals and at least one in-between-meal snack. The results are displayed as tabular forms and graphical forms on the computer screen.

Development and Evaluation of a Brief Fruit and Vegetable Food Frequency Questionnaire for Higher-Grade Elementary School Children (초등 고학년 아동의 과일 및 채소 섭취 수준 평가를 위한 간단한 식품 섭취 빈도 조사지의 개발과 평가)

  • 본 연구는 초등 고학년 아동의 과일과 채소 섭취 수준 측정을 위한 간단한 식품 섭취 빈도 조사지를 개발 및 평가하고자 실시되었다. 2001년도 국민건강영양조사에 참가한 만 $9{\sim}11$세 아동의 24-시간 회상 자료에 기초하여 식품섭취빈도 조사지의 항목 선정을 위한 기초 조사를 하였다. 각 과일과 채소 식품의 일일 평균 섭취량을 전체 아동 및 섭취량의 4분위 집단 별로 산출한 후, 이 자료를 바탕으로 전체 인구 집단의 섭취량을 대변할 수 있을 뿐만 아내라 고 섭취군과 저 섭취군 간의 차이도 나타낼 수 있는 항목을 선정하였다. 총 9개의 과일 항목과 11개의 채소 항목이 식품 섭취 빈도지에 포함되도록 선정되었으며, 섭취 빈도에 대한 응답 항목은 지난 7일 간의 섭취에 대하여 1개의 선택 범주가 제시되었다. 개발된 조사지의 신뢰도와 상대적 타당도에 대한 평가는 서울에 위치한 한 공립 초등학교에 재학 중인 153명의 $4{\sim}6$학년 학생들을 대상으로 실시하였다. 한 달간의 간격을 두고 2회에 걸쳐 조사를 실시한 자료를 이용하여 검사-재검사 신뢰도를 평가하였으며, 두 번째 방문 시에 28개의 과일 및 채소 항목을 추가로 포함하고 있는 식품섭취빈도 조사지를 더불어 실시한 자료를 바탕으로 상대적 타당도를 평가하였다. 상관 분석과 항목 별 빈도 응답 및 섭취 수준에 따른 집단내 순위 분류의 일관성에 대한 분석을 실시한 결과, 본 연구에서 개발된 20개 항목의 과일 채소 식품 섭취빈도 조사지의 신뢰도와 상대적 타당도가 적정 수준 이상으로 나타났다. 신뢰도를 살펴보기 위한 개별 항목 및 과일, 채소, 과일+채소 섭취 빈도에 대한 상관계수는 $0.22{\sim}0.64$(p<0.001)이었으며, 항목별 빈도 응답의 일관도는 평균 69.3%(<=1 빈도 응답차이)와 81.3%(<=2 빈도 응답 차이)로 나타났다. 상대적 타당도를 살펴보기 위한 과일, 채소, 과일+채소 섭취 빈도에 대한 상관계수는 $0.91{\sim}0.93$(p<0.0001)로 높게 나타났다. 따라서 본 연구를 통하여 개발된 식품섭취빈도 조사지는 초등고학년 아동의 과일채소 섭취 수준을 간단히 심사하거나 평가하는 도구로서 유용하리라 사료된다.

Revision of an iodine database for Korean foods and evaluation of dietary iodine and urinary iodine in Korean adults using 2013-2015 Korea National Health and Nutrition Examination Survey (한국인 상용 식품의 요오드 데이터베이스 업데이트와 이를 활용한 한국 성인의 요오드 섭취량 및 배설량 평가: 2013-2015 국민건강영양조사자료를 이용하여)

  • Purpose: Variations in the iodine contents of foods is critical for estimating the iodine intake. This study aimed to update the iodine database of common Korean foods and evaluated the iodine intake in Korean adults. Methods: A list of 855 Korean foods was selected for the updated iodine database. The updated database was established with Version 1 and 2 by applying an average or minimum value for the imputed values. The iodine intake was estimated in 5,927 Korean adults using the data from the 2013-2015 Korea National Health and Nutrition Examination Survey. Results: The analytical values in the updated database were 166 (19.4%), followed in order by 318 (37.2%), 247 (28.9%), and 124 (14.5%) for the adapted, imputed, and missing values, respectively. The median of dietary iodine intake was 352.1 ㎍/day (± 2,166.1) and 343.4 ㎍/day (± 2,161.9) in Version 1 and 2 among the total population. The contribution rates of each food group to the iodine intake were 55.7% for seaweeds, which showed a similar trend in Version 2. When subjects were divided by consumption of seaweeds, the median iodine intake was 495.7 ㎍ in the consumer group, which was almost double (241.2 ㎍) that of the non-consumer group. The proportion of subjects who consumed below the Estimated Average Requirement of iodine was 11.0% in the non-consumer group. In contrast, 11.6% in the consumer group of seaweed consumed above the Upper Level of iodine. When the dietary iodine and urinary iodine were examined, the regression coefficient was 0.11718 in Version 1 and 0.11512 in Version 2 after adjusting for age and sex. Conclusion: This study presented the variation of iodine intake in Korean adults by applying different versions of the iodine database. As the iodine intake can vary due to the highly variable concentrations in the major food sources, an iodine database is necessary to be monitored, and caution should be taken when the database is used in research.

Assessment of Estimated Daily Intakes for Preservatives from Survey Data (설문조사법을 통한 보존료의 일일 추정섭취량 평가)

  • This study has been carried out to estimate the average daily intakes of preservatives commonly used in Korea. The survey method, which is specific approach that can be applicable for Koreans has been proposed to estimate the daily intakes. The ADIS of sorbates, benzoates, and esters of p-hydroxybenzoic acid were less than 1% and judged to be safe. The EDIs of sorbates, benzoates, and esters of p-hydroxybenzoic acid were 0.39, 0.29, and 0.04 mg/kg bw/day respectively. And these data were compared with existing data. In case with sorbates, the EDI of survey data was higher than that of analytical data but much lower than that of maximum permitted level data. Also, the EDIs of benzoates and esters of p-hydroxybenzoic acid showed to be the same results. In conclusion, the EDIs of survey data lie between maximum Permitted level data and analytical data of Preservatives. The survey method could be considered as the possible alternative of refined method.
