Smith-Magenis Syndrome (SMS) 환아의 증례 보고 (SMITH-MAGENS SYNDROME (SMS) : A CASE REPORT)

  • Smith-Magenis syndrome(이하 SMS)은 제 17번 상염색체의 부분적 결손으로 발생하는 다발성 선천적 기형 및 정신지체를 특징으로 하는 증후군으로 1 : 25,000의 빈도로 발생하는 것으로 알려져 있다. 이 증후군은 작은 키와 단두증(brachycephaly)을 동반한 편평한 중안면, 처진 입, 종종 두드러지게 붉은 뺨, 성인에서 돌출된 턱 등의 특징적인 얼굴 생김새, 만성이염, 청각 손상, 사시와 근시를 포함한 눈의 이상, 목 쉰 소리, 짧은 손가락과 발가락, 심장 질환, 비뇨기 질환, 척추 만곡, 비정상적 걸음걸이, 통각에 둔감함 등의 신체적 특징을 지닌다. 특히 유아기에는 토실토실한 아기 얼굴, 행복한 기분, 드문 울음소리, 낮은 근 긴장도, 섭식 장애 등의 특징이 있다. 수면 장애를 포함하여 언어 지체 및 발음 장애, 발달지체, 학습 장애, 정신지체, 활동항진과 자해, 폭발적 분노, 지속되는 짜증, 파괴적이고 공격적인 행동, 흥분성, 흥분시에 팔로 안거나 손을 끼우는 행동 등의 행동 발달적 특징을 나타낸다. 본 증례는 3세 3개월된 남아로 생후 10개월에 SMS를 진단받은 환자로 상악 좌측 제 1유구치의 통증을 주소로 본과에 내원하였으며, 임상 및 방사선학적으로 다발성 우식증을 보였으며, 소아과 주치의에게 의뢰한 결과 심내막염의 예방을 위한 항생제의 처치를 처방 받고, 환아의 행동 발달적 문제로 인해 물리적 속박의 방법 하에 외래에서 치료되었다. 이 증례를 통해 SMS라는 희귀한 증후군에 관하여 보고하고자 한다.

  • 저자는 후기 영아형 이염성 백질 이영양증으로 인해 신경학적인 퇴행이 관찰되고 섭식 장애로 인한 다발성 우식을 보이는 환아를 전신마취 하 치료하였기에 문헌 고찰과 함께 보고하는 바이다. 저작기능 장애로 인한 영양 결핍으로 환아는 신체 발육이 매우 저하되었으며 긴 식사 시간으로 다발성 치아 우식에 이환된 상태였다. 협조도 부족 및 치료 범위 등을 고려하여 전신마취 하에 치과 치료를 시행하였고 정기적인 검진을 시행 중이다. 진행성, 신경성 퇴행질환의 특징을 이해하고 환아의 상태를 고려한 구강 건강 관리 및 보호자 교육을 제공하는 것이 필요할 것으로 사료된다.

섭식장애 위험군 여중생의 체중, 식습관과 식이자아효능감 (Weight, Eating Habits and Dietary Self-efficacy of Middle School Girls with Eating Disorder)

  • The slimness favored trend made students shape up body image by weight control using restrained eating. Many students especially female ones tend to be in eating disorder status. The aim of this study was to find the relation between weight, eating habits and dietary self efficacy in the selected middle school girl students group with high risk eating disorder (7.9%) and the one with low risk eating disorder (24.1%). This study was conducted by EAT-26 questionnaire method and all the data was analyzed by SAS (Statistical Analysis System) program. The results were as follows; The physical condition of eating disorder students (159.5cm height, 50.7kg weight and 97.4% PIBW) was higher and bigger than that of normal students(158.2 cm, 47.2 kg, and 92.6% PIBW). Weight control experience in the high risk group (69.4%) was significantly more frequent than normal group (p<0.001). The gap between actual body weight and desired weight was higher in high risk eating disorder group than in normal group (p<0.001). Dietary self-efficacy score of middle school female students in the high risk eating disorder group was high when they were in temper, in confusion, and after argument. However, when they were in cooking (p<0.01), with friends (p<0.05), in assembling dishes (p<0.01), and with family (p<0.05) the dietary self-efficacy score of high risk group was lower than that of normal group. In the high risk eating disorder group, eating speed was often faster (p<0.05) and overeating rate (p<0.01) was higher than in normal group. In general, EAT-26 score was correlated positively with gap weight, but negatively correlated with dietary self efficacy score(p<0.01). Gap weight and dietary self efficacy were significantly different in normal group. however, there was no relation in high risk eating disorder group. Under the circumstance of high risk eating disorder, as weight and dietary self efficacy did not affect the relation with eating disorder score, when it is determined as eating disorder some other factors besides weight and diet self efficacy seem to affect the eating disorder score. In conclusion, the factors related with eating disorder were gap weight and some items of dietary self efficacy. Thus, correct understanding of healthy weight and dietary self efficacy enhancement require the development of nutrition education contents and the practice of nutrition education.

베체트병의 조기 진단 (EARLY DETECTION OF BEHCET'S DISEASE)

  • 베체트병은 전신적 혈관염으로 여러 장기에 걸쳐 나타날 수 있으며, 현재로선 확진을 위한 검사 방법이 없으므로 병의 진단은 임상적 증상에 기초하여 내려진다. 구강 및 외음부의 재발성 궤양, 피부 병소, 안구 병소, 및 이상초과민검사(pathergy test)에 대한 반응을 기초로 진단을 내리며, 이 외에도 위장관과 중추신경계에 증상이 나타날 수 있다. 이 중 재발성 구강 궤양은 대부분의 베체트병 환자에서 나타나며, 많은 경우 베체트병의 첫번째 임상 증상으로 발현되기 때문에 치과 영역은 베체트병의 조기 진단에 있어서 중요한 위치에 있다. 본 증례에서 6세 여환은 반복되어 나타나는 재발성 구강 궤양으로 인한 동통 및 섭식 장애를 주소로 본원 소아치과에 내원하였다. 문진 및 병력 채취, 그리고 임상 검사를 통해 치과 영역에서 베체트병을 조기 진단할 수 있었고, 추후 환자는 소아과, 피부과, 소화기내과, 안과와의 협의 진료 하에 베체트병으로 확진되었다. 치과에선 환자의 동통을 완화시키고 섭식을 돕기 위해 도포마취제를 처방하였으며, 추후 베체트병의 확진 하에 그에 따른 적절한 의과적 치료를 시행할 수 있었기에 보고하는 바이다.

경기 오산지역 중.고등학생의 체중과 식품섭취 및 섭식장애 경향 조사 (The Study on the Weight, Food Group Intake and Tendency of Eating Disorder of Adolescents in Osan GyeongGi Province)

  • This study was carried out to investigate the effect of eating behavior, physical status and tendency of eating disorder on the right eating habit. The subjects of this study were 324 middle school students and 340 high school students in Osan city GyeongGi province. The average of weight and height in the middle and high school students were 48.7kg, 160.0cm and 56.8kg, 164.8cm respectively. In PIBW values, 92.9% of middle school students and 99.1% of high school students were almost close to the average. The 28.5% of high school and 21.5% of middle school students were aware of themselves as overweight. The risk of eating disorders in high school students(16.4%) were higher than that in middle school students(4.9%). Moreover, the risk of eating disorders in girls high school students(19.3%) was significantly higher than those of middle school students(5.2%) (p<0.001). The thought of food problems were realized in 57.4% of in high school students and in 39.9% of in middle school students. The intake of food group frequency, middle school students ate more fruit than high school students(p<0.01). Boys ate more frequently soy beans (p<0.05) and algae(p<0.1) than girls in high school students. The meat intake was more frequent in high school students, but the eggs and milk were more in middle school students. The boys' intakes of fish and milk were more often than the girls' in high school students. Eating disorders and food intake frequency scores were closely correlated with weights(PIBW and the gap of ideal. weight with real weight). In conclusion, the education about realizing exact healthy body shape and how to modify behavior to prevent eating disorders should be planned since junior high school. Especially in high school girls, the variety food intake education would be needed. Proper recognition of the weight is required for good food intake and for prevent eating disorders.

인터넷 Web 강의를 수강하는 남녀 대학생에서의 체중조절 경험, 섭식장애 및 식이섭취 실태에 관한 연구 (A Study on Weight-Control Experience, Eating Disorder and Nutrient Intake of College Students Attending Web Class via the Internet)

  • This study was to investigate weight-control experience disorder and nutrient intake of college student attending web class via the internet. The subject were 1200 college students(392 male, 808 female) attending web class concerned with nutrition and weight control. This cross-sectional survey was conducted by self-administered questionnaire and data were analyzed by Students t-test and $\chi$$^2$-test. Nutrient intake collected from 3 day-recalls were analyzed by the Computer Aided Nutritional Analysis Program. Mean height, weight and RBW(relative body weight) of the male college students were 174.7 cm, 68.0 kg and 101.2%, respectively. Mean height, weight and RBW of the female college students were 162.3% cm, 52.7% kg and 95.3% respectively. Among the male college students, low weight were 1.0%, underweight were 12.8%, overweight were 12.5%, obese were 6.9% by RBW standard. Among the female college students, low weight were 5.1%, underweight 30.2%, overweight were 5.2%, obese were 1.5% by RBW standard. Percentages of weight control experience were 21.7% and 67.0% in the male and female college students, respectively. According to RBW, 22.4% of the weight control experience group were low weight or underweight in the female college students but none of weight control experience group were low weight or underweight in the male college students. Percentage of eating disorder were 6.8%(2.8%, male, 8.7% female) in the college students. Among the female college students, most nutrient intakes were significantly lower in the weight-control experienced group compared to the weight-control nonexperienced group. Among the male college students, there was a tendency that the intake of nutrients, except carbohydrate, calcium and vitamin A was higher in the eating disorder group compared to the non-eating disorder group. However, among the female college students, there was a tendency that the intake of nutrients, except calcium and thiamin was lower in the eating disorder group compared to the non-eating disorder group. Therefore, nutrition education via the internet is required for college students to know proper weight loss methods, desirable eating habits and eating disorder problem well.

암 악액질의 주요 병인 기전 및 최근 연구 동향 (Recent Studies on Anorexia and Tissue Wasting Induced by Cancer Cachexia)

  • 암 악액질은 암으로 인한 다기관 대사성 질환으로 식욕부진과 체중 감소가 주요 증상이다. 일반적으로 암 환자의 식욕부진과 체중감소는 항암화학요법 치료와 암 환자의 생존율에 악영향을 미치는 심각한 문제이다. 암 악액질은 일반적으로 췌장암, 폐암, 결장암 등 소화기관 암 환자의 약 80%에서 동반되는 것으로 알려져 있으며, 림프종이나 유방암 환자에서는 비교적 드물다. 암 악액질에 의한 식욕부진은 화학요법에 의해서도 일어나지만, 화학요법과는 독립적으로 발생하는 것으로 알려져 있다. 암 악액질의 발병기전으로는 종양 조직에 의해 과도하게 증가되는 염증성 사이토카인에 의한 정상 조직 기능의 저하가 주요 원인이다. 암 악액질의 메커니즘은 아직 완전히 이해되지 않았기 때문에 현재 악액질을 치료할 치료제나 진단 바이오마커가 없는 실정이다. 최근 발표된 연구에서는 암세포에서 분비되는 물질이 악액질에 의한 식욕억제를 일으키는 것이 확인되었고 그 분자생물학적 기전이 밝혀졌다. 이 물질의 발현 및 분비 증가는 암 환자의 악액질 증상과 통계적으로 상관관계가 있는 것으로 밝혀져 암악액질 진단 및 치료제 개발에 활용될 것으로 기대된다. 이 논문에서는 암 악액질의 주요증상인 섭식장애와 체중감소의 이해를 돕고자 알려진 원인과 분자 기전들의 내용을 소개하겠다.

인천지역 일부 여중생의 비만도에 따른 섭식장애경향과 식습관의 비교 (Comparisons of Dietary Habits and Eating Disorder by Obesity Index on One of Middle School Girls in Incheon)

  • On the general trends of weight control for slimness, this study surveyed and compared eating attitude influenced by eating disorder, eating habits, and nutrition knowledge by obesity index in middle school girls. The purpose of this study was to investigate the eating disorders and eating habits, and analyzed these data by degree of obesity. This will provide basic information of nutrition education for adolescents who need to have proper body shape and dieting habits. This survey was conducted to one of girls' middle school in Incheon and all the data was analysed by SAS(Statistical Analysis System) program. The average height, weight and PIBW were 158.7cm, 48.2kg, and 93.9% in students. However, 50.8% of students were underweight or severely underweight. Most of female students perceived that they were normal or overweight even though they had underweight or normal. There was a significant difference in experience and intention of weight control by degree of obesity. In the overweight group, EAT-26 score tendency was high, especially preoccupation with losing weight(factor 1). It shows that obesity index had meaningful correlation with EAT-26 score and factor1. In dietary habit, overweight group showed more irregular meals tendency, and overeat frequently. On the other hand, this group had lower frequency and of smaller amount snacks. The intake frequency of meals and snacks showed in the affirmative way regardless of degree of obesity. The average score for nutrition knowledge about calorie and weight control was low. Correct answer rates were increased in higher overweight group. In conclusion, teenage girls who concerned too much about their slim body image need to be educated about accuracy concept of the body image. Also, practical and systematic nutrition education should be done for the correct nutrition knowledge and its application ability of individuals.

  • Objective: The purpose of this study is to find out the effect of acupuncture at HT7 (Shinmun) on the feeding behavior and hypothalamic neuropeptide Y(NPY) expression in the maternally separated rat pups. Methods: To show the effect of acupuncture, we performed maternal separation(MS) for 7 days beginning on postnatal day 14, and observed body weight, food intake, and NPY immunoreactivity in the paraventricular nucleus (PVN) of hypothalamus after acupuncturing at HT7, the end of the transverse crease of the ulnar wrist of the forepaw. Results: MS induced a significant decreases in body weight and food intake, while acupuncture treatment at acupoint HT7 showed much more improvement in those evaluations. NPY-immunoreactivity in area PVN were decreased in the MS group, but significantly increased in the HT7 group. Conclusions: These findings suggest that acupuncture has an effect on the feeding disorders caused by MS, possibly by modulating NPY expression in the PVN.

중학생의 섭식장애행동과 학업적응도의 관계 연구 (A Study on the Correlation between Middle School Students' Eating Disorder Behavior and Scholastic Adjustment)

  • The purpose of this study was to verify the difference of middle school students' eating disorder behavior and scholastic adjustment according to the gender and weight group and evaluate correlations between their eating disorder behavior and scholastic adjustment. A survey was conducted in middle school second graders attending five schools located in Seoul. Results from 355 questionnaire sheets were utilized for statistical analysis, and the results were gained as below: With regard to eating disorder behavior by gender, female students showed more eating disorder behavior than male students (p < 0.01); however, scholastic adjustment did not indicate significant differences by gender. According to the result of analyzing scholastic adjustment according to the weight group, the obese group showed lower scholastic adjustment (p < 0.05) and adjustment to studying (p < 0.01) than other groups; however, the eating disorder behavior, attachment to school, and adjustment to school did not indicate significant differences. According to the results of analyzing scholastic adjustment by eating disorder group, the normal group showed higher adjustment to studying than the risky group (p < 0.01). And scholastic adjustment, attachment to school, and adjustment to school did not indicate significant differences. The eating disorder behavior showed negative relationship with adjustment to studying. They showed more eating disorder behavior, their adjustment to studying became lower. Therefore, it is necessary to provide nutritional education at home and at school for them to have appropriate dietary habits as well as health education for them to maintain normal weight.
