경부기도에 원발한 선양낭성암종

  • 경부기도의 종양은 편평상피암과 함께 이비인후과의에게 있어서 호흡곤란 환자의 감별진단을 위해 매우 중요한 임상적 의미를 갖는다. 종양은 암종에 의한 사망률의 0.1%이하를 차지하는 드문 질환이며 선양낭성암종은 기도의 원발성 종양중 두 번째로 많은 질환이다. 갑상선종양의 기도의 직접적인 침범이 흔히 발견되는 상태이며, 그다음으로 편평상피암, 선양낭성암종이 기도의 원발성 종양으로 흔한 질환이다. 갑상선의 악성종양이 기도의 벽이나 내강을 침입하는 것과 마찬가지로, 기도의 원발성 악성종양도 흔히 갑상선을 침범하여 갑상선의 종괴처럼 발현할 수 있다. 본 연구는 이와 같이 갑상선의 악성종양과 유사한 임상경과를 보이는 기도의 선양낭성암종의 향후 감별진단을 위해 4명의 조직학적으로 증명된 갑상선을 침범하는 기도의 선양낭성암종환자의 임상기록과 전산화 단층촬영소견을 후향적으로 관찰하였다. 전산화 단층촬영에서 이들은 기도에 넓은 기저부를 가지고 갑상선을 밀고있는 균일한 음영의 일반적으로 부드러운 경계를 가지는 종괴로 보였으며, 횡단면과 두정면에서 모두 기도 벽의 비후소견을 보였다. 이러한 소견은 기도의 원발성 선양낭성암종의 감별에 도움이 될 것으로 생각된다.

$^{18}F$-FDG PET/CT in Patients with Initially Diagnosed Adenoid Cystic Carcinoma of the Head and Neck: Clinicoplathologic Correlation (처음 진단된 두경부 선양낭성암종에서 $^{18}F$-FDG PET/CT: 임상상 및 병리소견과의 상관성)

  • Purpose: We evaluated $^{18}F$-FDG PET/CT findings in initially diagnosed adenoid cystic carcinoma (ACC) of the head and neck in association with pathological subtype, staging, uptake comparison with squamous cell carcinoma (SqCC) and prognosis. Materials and Methods: The subjects were 16 patients with initially diagnosed ACC of head and neck who underwent pretreatment $^{18}F$-FDG PET/CT. Histological subtype (solid pattern vs. tubular/cribriform pattern), $SUV_{max}$ of size-matched SqCC of the head and neck as control group, disease-free survival (DFS) were compared with the $SUV_{max}$ of ACC of the head and neck. Results: Of total 16 patients, 6 had solid pattern and the remaining 10 had tubular/cribriform pattern. The $SUV_{max}$ were significantly higher in solid pattern group than in tubular/cribriform pattern group ($6.7{\pm}3.2$ vs. $4.2{\pm}0.9$, p=0.03). PET/CT found unexpected distant metastasis in 18.7% of patients (3/16) and changed the therapeutic plan in those patients. The $SUV_{max}$ of ACC was significantly lower than that of size-matched SqCC ($5.1{\pm}2.4$ vs. $13.6{\pm}6.0$, p<0.001). DFS was not significantly different according to the histological subtype. In contrast, patients with high $^{18}F$-FDG uptake ($SUV_{max}$ ${\geq}$6.0) had significantly shorter DFS than those with low $^{18}F$-FDG uptake ($SUV_{max}$ <6.0). Conclusion: $^{18}F$-FDG uptake of ACC of the head and neck is significantly associated with histological subtype and DFS. $^{18}F$-FDG PET/CT may be useful for detecting unexpected metastasis. Since $^{18}F$-FDG uptake of tubular/cribriform ACC compared with SqCC is relatively low, it is necessary to interpret PET images carefully in patients without alleged ACC.

  • Malignant tumors of the human salivary glands may arise from major or minor salivary glands. Adenoid cystic carcinoma (ACC) is the second most common malignant neoplasm in the salivary glands. ACC is occasionally highly aggressive tumor that readily invades adjacent tissues and metastasize to distant organs at early stages of the disease. Although ACC tends to grow slowly, treatment outcome may be poor due to wide local infiltration, perineural or intraneural spread and a propensity for hematogenous metastasis. Therefore, knowledge of cellular and molecular characteristics that influence the growth, survival and metastasis of tumor cells, is important for new treatment strategies of salivary ACC. I determined expressions of epiderma growth factor (EGF)-signaling molecules using surgical specimens of human ACCs. Protein expressions of EGF, transforming growth $factor(TGF)-{\alpha}$, EGF receptor (EGFR), phosphorylated EGFR (pEGFR), and human EGF receptor (HER)-2 were assessed in 18 cases of salivary ACC by immunohistochemical staining. Adjacent normal salivary tissues and mucosal tissues, uninvolved by the malignant tumor, served as internal controls. Most of the tumors, especially ACC with a tubulocribriform pattern, were positive for EGF signaling molecules. The overall percentages of the 18 specimens expressing EGF, $TGF-{\alpha}$, EGFR, pEGFR, and HER2 were 50, 89, 61, 61 and 83% respectively. Moreover, tumor-associated endothelial cells and infiltrating immune-related cells in the stroma of ACC, also expressed these biomarkers. Taken together, EGF-signaling molecules are actively expressed in salivary ACC. Therefore, we suggest that these biomarkers can be molecular targets for new treatment strategies of salivary tumors.

Predictors of Distant Metastasis in Adenoid Cystic Cancer of Salivary Gland (타액선 선양낭성암종의 원격 전이 예측인자에 관한 연구)

  • 배경 및 목적 타액선 선양낭성암종은 느린 성장 속도와 늦은 원격 전이를 특징으로 하는 드문 상피성종양이다. 본 연구는 다양한 임상 병리학적 변수를 통해 선양낭성암종의 원격 전이에 영향을 주는 인자를 조사하고자 하였다. 대상 및 방법 1999년 10월부터 2011년 12월까지 본원 이비인후과에서 타액선 선양낭성암종으로 진단되어 치료 받은 44명(남자 19명, 여자 25명)을 대상으로 원격전이를 유발하는 위험 인자를 조사하였다. 8명의 환자는 배제 기준에 따라 제외하였다. 환자의 평균 연령은 54세였다. 환자의 병리 보고서, 종양의 크기, T 병기, 수술 절제연의 종양 존재 유무, 신경 조직 침습, 림프절 전이가 조사되었다. 결 과 15명의 환자가 원격전이가 있었으며 21명은 원격 전이가 없었다. 원격 전이를 유발할 수 있는 여러 인자들을 비교하였을 때, 원격 전이는 수술 절제연의 종양 세포 잔존(p=0.014), 종양의 크기(p=0.038), 진행된 T 병기(p=0.024)가 통계적으로 유의하게 연관성이 있었다. 림프절 전이와 신경 조직 침습은 원격 전이와 연관이 없었다. 결 론 종양의 크기, 진행된 T 병기, 수술 후 절제연의 종양 세포 잔존은 원격 전이의 예측 인자로 생각된다. 따라서 이에 해당하는 환자의 경우 더욱 철저한 관리 및 경과 관찰을 요한다.

A Case of Adenoid Cystic Carcinoma of Sublingual Gland Forming a Major Sublingual Duct, which Empties into Wharton's Duct (악하선관으로 연결되는 주설하관을 가진 설하선과 이에 발생한 선양낭성암종 1례)

  • Tumors rarely arise in the sublingual salivary glands. They should be considered malignant until proved otherwise. Adenoid cystic carcinoma is most commonly encountered malignant tumor of the sublingual gland. We report a case of adenoid cystic carcinoma arising from the sublingual gland. Moreover, the sublingual gland had anatomic variation of main duct(Bartholin's duct) which is connected to Wharton's duct.

  • Adenoid cystic carcinoma(ACC) is one of the common malignant tumor of the major and minor salivary glands. ACC arising from the larynx is relatively rare(less than 1% of laryngeal malignant tumors) and only about eighty cases have been reported in the English literature. Definite diagnosis of these lesions is made only from a histological analysis, because findings and symptoms are non-specific. The diagnosis progresses very slowly, therefore it often presents at an advanced stage with regional and distant metastasis. Here, we present one case of ACC of the supraglottic region with a review of literature.
