Retrospective Study of Characteristics and Treatment of Dentigerous Cysts Involving the Maxillary Anterior Tooth (상악 전치부에 발생한 함치성 낭종의 임상적 특징과 치료에 대한 후향적 연구)

  • The aim of the present study was to identify the clinical characteristics and treatment outcomes associated with maxillary anterior dentigerous cysts in children. Among 55 patients who had been diagnosed with maxillary anterior dentigerous cysts, 33 patients had cysts located in the incisor region and 22 had cysts located in the canine region. Cysts in the incisor region were 7.2-fold more prevalent in male patients, while cysts in the canine region were 1.75-fold more prevalent in female patients. For cysts in the incisor region, marsupialization was performed in 21 cases and enucleation in 12 cases. In the canine region, marsupialization was performed in 20 cases and enucleation in 2 cases. Spontaneous eruptions of the displaced teeth in the incisor and canine region were 90% and 54.5%, respectively. Among patients which spontaneous eruption occurred, patients with cysts in the incisor region underwent orthodontic treatment more often due to malocclusion. This study is expected to be used as fundamental data for establishing future treatment plans by providing the analyzed results of distribution and characteristics of dentigerous cysts involving the maxillary anterior tooth.

TRAUMATIC INJURIES TO THE UPPER INCISORS IN PRIMARY AND MIXED DENTITION (유치열과 혼합치열기 어린이의 상악 절치부 외상)

  • In children, change in lifestyles such as an increase in participation of physical activities has increased the chance of receiving an injury, harming the child and their parents. This study was carried out to give basic information of traumatic injuries to the upper anterior teeth which can be used in prevention of future complications that may arise. From June, 2002 to May, 2003, of the 8,799 children in their primary and mixed dentition visiting Department of Pediatric Dentistry, College of Dentistry, Yonsei University 955 upper anterior teeth(primary 543, permanent 412) of 487 children(male 316, female 171) with traumatic injures to the upper incisors were studied. The results were as follows: 1. Trauma to the primary teeth prevailed at the age of 1-3 and to the permanent teeth at the age of 7. The male to female ratio was 1.85:1. 2. Trauma to the upper incisors for both the primary and permanent teeth occurred in the afternoon. Primary teeth were injured most frequently in the home(44.1%) and the permanent teeth outdoors, in the street(17.5%). 3. Both the primary and permanent teeth were most injured by fall-down injuries(39.8%, 12.9%), and next were collisions(22.2%, 6.0%). For the primary teeth, high fall, traffic accident, violence and sports are next in order, and for the permanent teeth, violence, sorts, traffic accidents were next. 4. Average number of 1.6 primary teeth and 2.8 permanent teeth were injured with the right central incisor most prevailing. 5. Crown fracture without pulpal exposure and subluxation injures were highest in frequency in both the primary and permanent teeth.

  • Pediatric dentists often meet children with abnormal in number of tooth. Presence of supernumerary teeth is frequent cause of malocclusion. Etiology for supernumerary teeth is not yet clearly defined, but it is thought to be caused by excessive proliferation of dental lamina by hereditary and environmental factors. Supernumerary teeth occur in the maxilla nine times more frequently than in the mandible. Most common supernumerary tooth is the mesiodens in the maxilla, and some are observed in the maxillary molar and mandibular premolar. It occurs rarely in the mandibular incisor region with the incidence of 1-2% among all supernumerary teeth. A six-year old boy visited the department of the pediatric dentistry, Yonsei University Dental Hospital, with the chief complaint of crowded supernumerary teeth on the mandibular incisor region. Clinical and radiographic examinations revealed six permanent mandibular incisors similar in size, shape, and length. Further investigation using computed tomography(CT) was proceeded on the mandible to measure and compare morphologic features and positions of the six incisors. Then, we decided to remove two incisors which were already erupted. Periodic check-up was followed to monitor the dental development and spontaneous positional enhancement of the remaining four incisors in the mandible.

  • This study was performed, by Finite Element Method, to evaluate the stress distribution on the periodontal tissue according to activation of the various closing loops and to predict the pattern of movement of maxillary incisors. At the same time, bull loop, key-hole loop, T-loop, combination loop and asymmetrical T-loop which were used for retraction of maxillary incisors was analysed by Finite Element Method. The following results were obtained 1. Horizontal force was the greatest in bull loop, the followed by key-hole loop, combination loop, T-loop and initial tooth movement exhibited uncontrolled tipping. 2. Horizontal force in asymmetrical T-loop compared to other closing loops was remarkably decreased, and the intrusive force on the incisors occurred. 3. As torque was increased, the moment was increased as a linear increment. 4. As moment was increased, initial movement of tooth changed to root movement from uncontrolled tipping.

  • The purpose of this study was to investigate the stress distribution in the periodontal tissue and the displacement of teeth when active torque was applied to the maxillary incisors by three-dimensional finite element analysis A three-dimensional finite element model consisted of the maxillary teeth and surrounding periodontal membrane, $.022{\times}.028$ Roth prescription bracket and stainless steel, NiTi and TMA rectangular ideal arch wires which were modeled by hexahedron elements. Applied active torques were 2, 5 and 10 degrees ThHe findings of this study showed that the reaction force acting or the bracket was the extrusion force on the mesial side of the incisors and canine and the intrusion force on the distal side of the incisors and canine. The amount of force and moment was greatest at the lateral incisor. When active anterior labial crown torque was applied. labial crown and distal tipping and Intrusion of the incisors took place. and lingual crown distal tipping and extrusion of the canine occured. An excessive force was concentrated on the lateral incisor, when the stainless steel wire was used NiTi or TMA wire is desirable for torque control.

  • Multiple features of occlusion have been combined by epidemiologists in attempts to objectively to determine malocclusion severity and treatment priority. Jenny et al were developed DAI(Dental Aesthetic Index), which is an orthodontic index based on socially defined aesthetic standards. This study aimed to evaluate association between 10 occlusal components of DAI and individual measures of skeletal morphology by use of univariate and multivariate analysis. In addition the relationship between a Dental Aesthetic Index score and skeletal morphology was evaluated. The data for this study were obtained from cephalometric radiographs and dental casts of 182 Korean patients(79 men, 103 women) with permanent dentition who had been accepted for the orthodontic treatment at Department of Orthodontics, Chonbuk National University. The results are as follows: 1. Several occlusal components of DAI(missing, crowding and spacing in the incisal segment, diastema, largest anterior irregularity on upper and lower arch) were not associated with individual measures of craniofacial morphology. 2. Sagittal occlusal components of DAI were associated with anteroposterior skeletal measures. 3. The relation between vertical occlusal components of DAI and vertical skeletal measures was low magnitude. 4. A Dental Aesthetic Index, which is based on several aspects of occlusion, did not identify craniofacial morphology. We can conclude that combination of certain occlusal components of the DAI may be related with specific skeletal morphology; but, this concept could not be accepted generally.

The effect of labial inclination on intrusion of the upper and lower incisors by three-dimensional finite element analysis (분절호선법으로 상하악 절치부 압하 시 순측경사도가 미치는 영향에 관한 3차원 유한요소법적 연구)

  • This study was designed to investigate the position of anteroposterior center of resistance for genuine intrusion and the mode of change of the minimum distal force for simultanous intrusion and retraction of the upper and lower incisors according to the increase of labial inclination. For this purpose, we used the three-piece intrusion arch appliance and three-dimensional finite element models of upper and lower incisors. 1. Positions of the center of resistance in upper incisors according to the increase of the labial inclination were as follows; 1) In normal inclination situation, the center of resistance was located in 6m behind the distal surface of the lateral incisor bracket. 2) In $10^{\circ}$ increase of the labial inclination situation, the center of resistance was located in 9mm behind the distal surface of the lateral incisor bracket. 3) In $20^{\circ}$ increase of the labial inclination situation, the center of resistance was located in 12m behind the distal surface of the lateral incisor bracket. 4) In $30^{\circ}$ increase of the labial inclination situation, the center of resistance was located in 16m behind the distal surface of the lateral incisor bracket. 2. Positions of the center of resistance in lower incisors according to the increase of the labial inclination were as follows; 1) In normal inclination situation, the center of resistance was located in 10mm behind the distal surface of the lateral incisor bracket. 2) In $10^{\circ}$ increase of the labial inclination situation, the center of resistance was located in 13m behind the distal surface of the lateral incisor bracket. 3) In $20^{\circ}$ increase of the labial inclination situation, the center of resistance was located in 15m behind the distal surface of the lateral incisor bracket. 4) In $30^{\circ}$ increase of the labial inclination situation, the center of resistance was located in 18m behind the distal surface of the lateral incisor bracket. 3. The patterns of stress distribution were as follows; 1) There were even compressive stresses In and periodontal ligament when intrusion force was applied through determined center of resistance. 2) There were gradual increase of complexity in compressive stress distribution pattern with Increase of the labial inclination when intrusion and retraction force were applied simultaneously. 4. With increase of the labial inclination of the upper and lower incisors, the position of the center of resistance moved posteriorly. And the distal force for pure intrusion was increased until $20^{\circ}$increase of the labial inclination.

  • The anterior maxillary incisor is the most traumatized region in the mouth and trauma is frequent between the ages of 8-10. Traumatic loss of teeth, can lead to many complications in children. Thus, as possible to keep traumatic teeth, but if you need extractions There may be. Complications occur and early tooth loss is frequent. Complications of early loss of central incisors are esthetic compromise, loss of vertical and horizontal width, height, contour of alveolar bone, tilting of adjacent teeth, arch length loss. Alveolar bone loss may affect normal function and stability, and results in esthetic problem for future prosthesis restoration. The 9-year-old girl and 6-year-old boy got early loss of upper central incisor. The amount of alveolar bone resorption was measured using cone beam computed tomograph and cast analysis.

  • The purpose of this study was to investigate the histologic changes in mandibular periodontium during overbite closure for openbite treatment by continuous arch wires and anterior vertical elastics. Two female monkey(Macaca nemestrina) with permanent dentition were used. Posterior bite block was fixed to each of their maxillae, which made the animal temporary anterior openbite as well as stabilized the whole maxillary anchorage. In each mandible, all the teeth except the second molars which had been extracted, were prepared for cast crowns. 018 inch Standard brackets were welded on these crowns. After cementation, two types of the $016{\times}022$ inch continuous arch wires, the plain ideal arch to the control animal and the MEAW(multiloop edgewise archwire) to the other experimental one were inserted. Then anterior vertical elastics were applied for two weeks. The overbite depth changes in the monkeys and histologic examinations of the mandibular periodontiums suggested the following conclusions. 1. During two weeks of the experimental period, the overbite increased + 0.3 mm in the control and + 1.3 mm in the experimental one. 2. In both the control and the experimental animal, histologic examinations showed that incisors, canines and first premolars were subject to extrusive force and the rest of posteriors were subject to intrusive one. 3. In periodontiums of the extruded incisors of the experimental one, reorientation of the periodontal fiber structures reflected the direction of force and the alveolar bone surfaces including apical and crestal areas which had been subject to tension, were the front of new bone formation. 4. In periodontiums of the extruded incisors of the experimental one, neither excessive hyalinization nor gross root resorption was observed. 5. Alveolar bone remodeling of anteriors and posteriors was more remarkable in the experimental one than the control.

Miniscrew Installation Area and Condition on Maxillary Palatal Side (상악구개측 미니스크류 식립위치 및 조건)

  • Anchorage control is important in orthodontic treatment. Recently miniscrew is widly used as maximum anchorage in orthodontic treatment, and then it is important to install miniscrew safely without damaging adjacent anatomic structure. In a view of Miniscrew's stability, maxilla is unfavorable than mandible, and moreover maxillary soft buccal bone has disadvantage on stability. so palatal area comes into notice for installation area. We measured distance between palatal roots and bone thickness at midpalatal area using 3D computed tomography, and have found following results. 1. On the comparison of distance between palatal roots, the distance between 2nd premolar and 1st molar was significantly longest and the distance between premolars was significantly shortest. 2. Going toward lateral area from midpalatal suture and posterior area from zero point, bone thickness significantly became shorter and shorter. And 5.0mm palatal sagittal plane has more significance decrease of bone thickness than 2.5mm palatal sagittal plane. According to these results, we can conclude that the palatal installation of miniscrew between 2nd premolar and 1st molar is safest. And it is more safe that comes closer to midpalatal suture and to anterior area in regard to incisive canal.
