  • Malignant lymphoma of the sinus is very rare but potentially radiocurable neoplasm. The disease is tend to be localized to the sinus and spread to adjacent local structures on initial presentation, and change to generalized lymphoma is uncommon. Most of sinus lymphoma is diffuse histiocytic type. Radiotherapy to the primary and neck nodes is the treatment of choice, and a survival rate of $50-70\%$ can be expected.

  • Malignant fibrous histiocytoma (MFH) is a sarcoma that occurs principally in soft tissue and typically involved the skeletal muscle and deep fascia. Although it is one of the most common types of soft tissue sarcoma in late adult life, the involvement of the head and neck area is relatively rare. MFH shows variable histologic appearance, and may be classified into several subtypes(storiform-pleomorphic, myxoid, giant cell, inflammatory, angiomatoid) and the storiform-pleomorphic type is the most common type. The authors have recently experienced a case of MFH in the right maxillary sinus, and report it with a brief review of the related literature.

  • 부비동에 발생하는 악성종양은 자각증상이 비교적 늦게 나타나 병변이 많이 진행된 후에 발견되는 경우가 대부분이다. 진행된 부비동압의 치료는 수술, 화학요법, 방사선요법의 병용치료법이 종래부터 시도되어왔으며 대부분의 경우 광범위한 수술적 절제가 필요하여 이에 따른 후유증이 크고 치료결과는 아직 많은 논란이 있다. 저자들은 진행된 부비동암 (T4)환자 10명에서 유도화학요법 (Cisplatin, 5-FU)후 수술 (상악절제술 4예, 두개안면 절제술 1예 )과 방사선 치료 (약 7000 cGy)를 받은 5명과 유도화학요법후 방사선치료만 받은 5명을 비교하여 다음과 같은 결과를 얻었다. 1. 수술을 포함한 병용요법을 받은 환자 6명중 3명은 평균추적기간 28개월 동안 재발의 증거가 없었으나 2명에서 국소재발이 있었고 이들은 국소 절제술 및 방사선치료후 현재 1년 이상 무병상태이다. 2. 수술을 포함한 병용요법을 받은 환자 5명중 4명에서 초진시 안와내 종양침윤의 소견이 있었으나 수술시 안와를 보존하였고 그 후 안와 부위의 국소 재발은 없었다. 3. 유도화학요법과 방사선치료만을 받은 5명 모두 6개월 이내에 국소재발하였다. 4. 진행된 악성 부비동암은 수술을 포함한 병용요법으로 좀 더 좋은 치료성적을 얻을 수 있을 것으로 기대된다.

  • Inverted papilloma arising from mucous membrane of the nasal cavity and paranasal sinuses is very rare benign neoplasm. Ward first described nasal papilloma in 1854, but its infrequent occurrence has delayed accurate understanding. This tumor was histologically benign neoplasm and clinically malignant, because it is locally invasive with extensive bone erosion at times and it shows a high incidence of local recurrence, and change of squamous cell carinoma was sometimes found. Recently, the authors have experienced a case of inverted pailloma with focal squamous cell carcinoma change which occupied the right side of the nsal cavity and maxillary sinus in a 48-year-old male. The tumor mass was removed surgically through intranasal and Caldwell-Luc's approach, and then was treated with systemic administration of Bleomycin, local spray of 5-FU and radiotherapy ($Co^{60}$). We report our case with review of current literatures.

Treatment Outcomes of Maxillectomy for Squamous Cell Carcinomas of the Maxillary Sinus (상악동 편평세포암종에 대한 상악절제술의 치료 결과)

  • Background and Objectives: Maxillectomy is the mainstay of treatment for malignant tumors of the maxillary sinus(MS). Nevertheless, few have been reported on the surgical outcomes of maxillectomy for malignant tumors of MS in Korean literature. Based on our clinical experience, the authors aimed to present the treatment outcomes of maxillectomy for squamous cell carcinomas(SCC) of MS. Subjects and Methods: We reviewed the medical records of 26 cases of maxillectomies with see of MS, who were treated from 1995 to 2004 at Samsung Medical Center. Most patients(73.1%) were locally advanced stage(T3 or T4a) at initial presentation. Total maxillectomy was performed in 18 cases, which is the most frequent procedure(69.2%). We analyzed the treatment outcomes of see of MS and several variables includeing tumor stage and resection margin to identify predictors for treatment failure after maxillectomy. Follow-up duration ranged from 7 to 89 months with a mean of 33 months. Results: Treatment failure occurred in 7 cases(26.9%), among which 3 were salvaged. Three of 26 maxillectomies(11.5%) showed the positive or close(less than 5mm) resection margin in their posterior resection sites; however it did not coincide with the site of recurrence after radiation therapy. Among patients who had been followed up for more than 6 months, disease-free 3 year survival rate was 100.0% in T1 and T2, 76.2% in T3, 60% in T4a, and 69.6% in total. Conclusion: Even though most of see of MS were detected at locally advanced stage, maxillectomy with or without postoperative radiation therapy for resectable MS see(T1-T4a) provided the acceptable treatment outcome(70%, 3Y disease-free survival rate).

Non Hodgkin lymphoma in the maxillary sinus mimicking dental abscess: a case report (치성 농양과 유사한 상악동에 발생한 비호지킨 림프종의 증례 보고)

  • Malignant lymphomas are neoplasms with diffuse proliferation of neoplastic lymphocytes and their precursor cells. Diffuse large B-cell lymphoma, which is a subtype of non-Hodgkin's lymphomas, rarely occurs in the head and neck area and is especially rare in the maxillary sinus. We report a case of a 76-year-old female patient who was referred to the oral and maxillofacial surgery office for evaluation of a dental abscess as a clinical diagnosis. Laboratory tests revealed no signs of inflammation or infection; therefore, incisional biopsy was performed. The final diagnosis was diffuse large B-cell lymphoma in the maxillary sinus. Here we describe this case with a review of relevant literature.

THE STUDY OF THE MALIGNANT TUMORS OF THE MAXILLARY SINUS BY COMPUTED TOMOGRAPHY (전산화 단층 촬영상에 의한 상악동 악성종양에 관한 연구)

  • CT findings of proven 25 malignant tumors of the maxillary sinus were retrospectively analyzed to be of help in the diagnosis and treatment. The results were follows: 1. Average age was 54 years old, and eighteen were males and seven were females with a ratio of 2.6:1 2. The most common histopathologic feature was squamous cell carcinoma (19 cases) and others were two cases of adenoid cystic carcinoma, one case of malignant fibrous histiocytoma, mucoepidermoid tumor, histiocytic lymphoma, unidentified malignant tumor. 3. CT findings were sinus opacificaqtion (4%), soft tissue mass (92%), low densities within soft tissue mass (44.%), air densities within soft tissue mass (24%), osteosclerosis (4%), bone destruction (92%), bone displacement (32%), fat plane obliteration (76%). 4. CT in the malignant maxillary sinus tumors approved the value in evaluation of tumor extension to nasal cavity, ethmoid sinus, orbit, infratemporal fossa, pterygopalatine fossa, pterygoid fossa, pterygoid muscle, cheek skin and intracranial cavity. 5. Twenty four cases (96%) were stage Ⅲ, stage Ⅳ according to AJCC TNM classification. 6. Bone findings were destruction, displacement, sclerosis and most frequent site of bone destruction was the medial wall of the antrum(92%). 7. Tumor growth pattern showed destructive pattern in 18 cases(72%), and squamous cell carcinoma showed destructive pattern. (P<0.05)

Multidisciplinary Treatment for Maxillary Cancer (상악암에 대한 집합적 치료)

  • Even though maxillary cancer is usually discovered in advanced stage, its regional lymph node or distant metastases are not common comparing to other head and neck cancer. So the result of treatment depends upon local control of the tumor. Because maxillary sinus is anatomically located adjacent to orbit and skull base, it is difficult to remove the tumor completely with sufficient safety margin like in other malignant tumor. Traditionally, surgery including aggressive resection, radiotherapy or both combined therapy have been widely accepted in many institutes, but their results are not still satisfactory. Sixteen cases of maxillary cancer( all squamous cancer, T2 1 case, T3 6 cases, T4 9 cases, mean age 57.2 years) were treated by intraarterial chemotherapy, raditherapy and surgery and followed up retrospectively. 5 year survival rate by Kaplan-Meier method was 51.95%, and orbit, palate or cheek skin could be preserved in many cases and their functional result was good.

  • Inverted papilloma of the nose and paranasal sinuses is a rare and benign neoplasm, which has been classified as a true neoplasm by most authors. The most characteristic microscopic feature of inverted papilloma is an increase in thickness and proliferation of the covering epithelium with extensive finger-like inversion into the underlying stroma. Radical aggressive treatment and careful follow up have been advocated in view of the high recurrence rate after inadequate removal and the possibility of malignant transformation. The authors have recently experienced a case of huge inverted papilloma which occupied the left side of nasal cavity, maxillary sinus and ethmoid sinus. A 64 year old female patient who had been suffering from nasal obstruction, hyposmia, headache and frequent nasal bleeding since about 3 years. The patient had had removal surgery as Caldwell-Luc approach with good postoperative effect.
