Arthroscopic Treatment of Lateral Epicondylitis (상완골 외 상과염의 관절경적 치료)

  • Purpose: The lateral epicondylitis is very common around elbow joint. We investigated the effectiveness of arthroscopic treatment. Materials and Methods: From Oct. 2002 to Feb. 2006, a total of 14 patients who were diagnosed with lateral epicondylitis and treated with arthroscopy were used in this study. The average symptom durations before receiving treatment was 27.3 months (9 to 47 months) and the average follow-up periods was 13.6 months (8 to 22 months). Results: The mean preoperative VAS scores was 8.3 (7 to 10 points) and the mean postoperative scores was 1.6 (0 to 4 points). The lesions of extensor carpi radialis brevis were classified using Baker's classification system. Type 1 lesion was found in 7 cases, type 2 in 4 cases, type 3 in 1 case Patients were able to return to their normal life style by 12.3 days (6 to 27 day). Conclusion: The Clinical outcomes of patients who received arthroscopic treatment were found to be good. Patients experienced lesser pain, and shorter recovery time. Therefore, when conservative treatment is ineffective in treating the lateral epicondylitis, arthroscopic treatment is considered af another option available for patients.

  • 외측 및 내측 상과염은 흔히 관찰되며 지속적인 치료 과정의 관찰이 요구되는 건의 질병이지만 현실적으로는 전체적인 질병의 발생과 치료의 과정에 대한 이해가 부족한 상태에서 모든 의사들이 치료 할 수 있는 질병처럼 취급되면서 초기 치료에서 수술적 치료까지의 결정이 전문적인 정형외과 전문의에 의하여 시행되지 못하여 적절한 시기에 적합한 치료가 선택되지 못하고, 각 치료 과정의 정보가 공유되지 않아 환자에게 꼭 필요한 치료 방법을 선택하는데 어려움이 발생하고 있는 실정이며, 치료 과정중의 불편함이 환자에게 전가되고 있는 상황이다. 따라서 전문적인 지식을 갖춘 정형외과 전문의의 지속적인 관심과 적절한 치료 방법의 선택으로 환자의 유병 기간을 최대한 단축 할 수 있다고 생각한다.

Arthroscopic Release of the Extensor Carpi Radialis Brevis Tendon for Chronic Recalcitrant Lateral Epicondylitis (만성 불응성 외 상과염에서 시행한 관절경적 단 요 수근 신건 유리술)

  • Purpose: The objective of this study was to assess the clinical outcome of arthroscopic release of the extensor carpi radialis brevis (ECRB) tendon in chronic recalcitrant lateral epicondylitis and tried to determine any prognostic factors. Materials and Methods: A retrospective review of 24 patients with lateral epicondylitis treated by arthroscopic ECRB release was performed. Outcome measures included a patient self rating and visual analog scale (VAS). Functional evaluation was made with Quick-disabilities of the arm, shoulder and hand (DASH) score system to identify preoperative factors that might be associated with the outcomes. Results: Twenty one (88%) elbows received benefit from the procedure. A mean preoperative VAS pain score and the mean Quick-DASH were significantly improved at final follow up. Age, sex, dominant arm and duration of symptom, presence of capsular tear or calcification did not correlate significantly with the clinical outcome. Three of four patients without T2-weighted high signal focus of the ECRB origin on preoperative magnetic resonance imaging (MRI) showed higher Quick-DASH score. Conclusion: Arthroscopic release of the ECRB is an effective option for chronic recalcitrant lateral epicondylitis. Lack of high signal focus on preoperative MRI is significantly associated with a poor surgical outcome and a considerable factor for the proper surgical indication.

The Results of Ultrasound Examination of the Elbow in Middle School Baseball Players (중학교 야구선수에서 시행한 주관절 초음파 검사의 결과)

  • Purpose: To evaluate the ultrasonographic findings of the elbows on group screening of middle school baseball players. Materials and Methods: Ninety-three players (age: 12-15, mean 13.5 years) of four middle school baseball team were evaluated with bilateral elbow ultrasonographies in the field regardless of elbow pain. Medial and anterolateral ultrasound examination of the both elbow were performed in the field to detect any abnormalities including medial epicondylar separation or fragmentation and capitellar osteochondritis dissecans respectively. We analyzed the relationship among elbow pain, physical findings and sonographic abnormalities and the differences of sonographic abnormalities between pitchers and fielders. Results: Thirty-six of 93 (39%) players had sonographic abnormalities of elbow in dominant arm, 30 with medial epicondylar apophyseal separation or fragmentation, 2 with osteochondritis dissecans, 4 with both lesions. Twenty-nine of 37 (78%) players with elbow pain had sonographic abnormalities. On physical examination, players with medial epicondylar abnormalities had medial epicondylar tenderness (59%) and pain on valgus stress test (52%), and 5 of 6 (83%) players with osteochondritis dissecans showed flexion contracture more than $5^{\circ}$. The incidence of medial epicondylar abnormalities between pitchers and fielders was statistically not significant but osteochondritis dissecans was more prevalent in pitchers (p<0.05). Conclusion: Elbow sonography is a simple and useful screening tool in the field and also effective for early detection of medial epicondylar abnormalities or osteochondritis dissecans that could be the main causes of elbow pain in adolescent baseball players.

  • Musculoskeletal ultrasound has unique advantages that may be free from exposure to radiation, low price compared to MRI, outpatient procedure that can be easily accessible, and better accuracy combined with physical examination. Dynamic ultrasound performed with stress tests are known to be useful for detecting the hidden lesions in the tendons, ligaments, nerves. Ultrasound in the elbow can be used easily in the outpatient for evaluation of the joint surface and synovial space; diagnosis for tendon diseases such as lateral epicondylitis, medial epicondylitis and morbidity of peripheral nerves; guide for anterior-posterior bursal and intra-articular injections.
