The Effects of Eating Training on Patients with Swallowing Disorder Caused by Stroke to Their Swallowing Functions (섭식훈련이 연하치료를 받는 뇌졸중 환자의 삼킴기능에 미치는 영향)

  • Purpose : This study was conducted for the purpose of finding out of effects Eating training on patients with swallowing disorder caused by stroke to their swallowing functions. Method : 29 subjects were selected and the divided into two groups. Group 1 is consist of 14 patients under eating training group and Group 2 is consist of 15 patients each group has five times per week. 30 minuted per time covering 12-week period. Selected patients can swallow them selves without aspiration and basically eating trainings involve Thermal Tactile Stimulation(TTS), Shaker exercise, tongue movements, laryngeal mobilization exercise. MASA(The Mann Assessment of Swallowing Ability)and VFSS(Videofluoroscopic Swallowing Study) was carried out to find out effects of training. And We use FDS(Functional Dysphagia Scale) for VFSS to more objective score. Result : Through the results of this study was increase in Eating performance skill actual eating training group than the practiceless group. Therefore, the actual eating training is to improve the swallowing function and It will be useful in clinical intervention for stroke patients. Conclusion : Through the results of this study was increase in Eating performance skill actual eating training group than the practice less group. Therefore, the actual eating training is to improve the swallowing function and It will be useful in clinical intervention for stroke patients.

  • In Korea, current medical technology is the highest level in the world. As a result, many doctors have specialized knowledge of various disorders or diseases, and are proceeding in a better way. With such high medical technology, it is possible to increase the probability of success of surgery to provide high reliability to patients. Rehabilitation is also a form of medical treatment that reduces the side effects that occur after surgery that is done for quick cure. However, the situation in this section is slightly different. There are moves to develop rehabilitation devices and operations, but most of them are now dependent on foreign technology. Rehabilitation therapy, which we commonly know, is dominated by behavior. However, it is also a kind of rehabilitation to find out how much the patient's symptoms are improved or recovered. In this paper, we have studied the development of a program by using the Deeplearning method in order to detect the problem of the food swallowing operation by the severity.

The Effect of SSP Electrical Stimulation Applied to Acupoints Impact on the Swallowing Function of Stroke Patients (경혈점에 적용한 은침전기자극이 뇌졸중 환자의 삼킴기능에 미치는 영향)

  • Purpose : The goal of this study is to estimate the effect of SSP electrical stimulation applied to acupoints as conducted in oriental medicine on the swallowing function of patients with swallowing difficulties caused by stroke. Methods : Twenty-two hemiplegic stroke patients with swallowing difficulties were divided into an intervention-only group (Group I; n=11) and an intervention-SSP electrical stimulation group (Group II; n=11). Therapeutic intervention was performed on participants in the intervention-only group and the intervention-SSP electrical stimulation group, for 20 minutes per session, three times per week, for four weeks. In this study, SSP electrical stimulation to the acupoints (Cheondol, Yomchon, Budol, Pungji, and Sanyinjian) of stroke patients with swallowing difficulties was applied, and to investigate the effects of SSP electrical stimulation, Clinical dysphagia scale (CDS) and ultrasonography assessment was used to measure pre and post-intervention scores. Results : In the clinical dysphagia scale assessment, the post-intervention score for group II had significant enhancement unlike group I. In the ultrasonography assessment, the hyolaryngeal movement in group II showed significant increase in the swallowing function unlike group I. Conclusion : From the above results, significant effects came from SSP electrical stimulation of 3Hz, biphasic wave, continuous current when applied to the acupoints as conducted in oriental medicine to improve the swallowing function of stroke patients.

Acoustic analysis of wet voice among patients with swallowing disorders (삼킴장애 환자의 wet voice 관련 음향학적 분석)

  • Wet voice quality (WVQ) is a characteristic that appears after swallowing. Although the concept is accepted by many clinicians worldwide, it is nevertheless ambiguous. In this study, we investigated WVQ in patients with swallowing disorders using acoustic analysis. A total of 106 patients diagnosed with penetration-aspiration by the videofluoroscopic swallowing study (VFSS) were recruited. A voice recording of vowel /a/ was conducted before and after the VFSS, and an acoustic analysis was then performed using PRAAT. Voice after VFSS was used for a perceptual judgment and divided into two groups: the Wet group (48 patients) and the Non-wet group (58 patients). At the post-VFSS stage, the two groups displayed significant differences in many acoustic parameters including F0_SD, Jitter, RAP, Shimmer, APQ, HNR, NHR, FUF, DVB, and CPP. The parameter affecting judging wetness resulted into Jitter and NHR by the logistic regression test. At the pre-VFSS stage, the two groups differed significantly in many acoustic parameters including Intensity, Jitter, RAP, Shimmer, NHR, FUF, DVB, and CPP. Both pre-and post-VFSS, the mean values of all significant parameters, except Intensity, HNR, and CPP, were higher in the Wet group. According to pre-and post-VFSS, the two groups displayed interactions in many parameters (Intensity, F0_SD, Jitter, RAP, Shimmer, APQ, HNR, NHR, FUF, DVB, and CPP). In particular, Intensity increased in both groups after the VFSS, although the increase in the Non-wet group was greater. Based on these results, it was conjectured that the WVQ after swallowing resulted from the secretion effect of the mucous membrane due to the dry laryngeal characteristic of elderly patients, rather than aspiration resulting in food on the vocal cords.

Effects of Biofeedback Training on the Suprahyoid Muscle Activity of Stroke with Swallowing Disorder (바이오피드백 훈련이 삼킴장애가 있는 뇌졸중환자의 설골상근 근활성도에 미치는 영향)

  • Purpose : The purpose of this study figures out how the biofeedback exercise combined with a Shaker exercise and a jaw-opening exercise affects the suprahyoid muscle activation of stroke with a swallowing disorder. Methods : The study period was from June, 2018 to September, 2018, to 45 patients who were suitable for selection criteria. Participants were divided into three groups: a visuoauditory biofeedback group (VABG), and a visual biofeedback group (VBG), and a self-exercise group (SG). The three groups were divided into the Shaker exercise and the jaw-opening exercise, and the biofeedback training by themselves. Three groups performed an intervention three times a day, five times a week, and four weeks long. Also, suprahyoid muscle activity was measured with a pre-test, a post-test and a follow-up test. Results : To know the suprahyoid muscle activity after the intervention, there were statistically significant differences between the pre-test and the post-intervention (p< .01). All three groups showed the improvement with the mean comparing followed by VABG, VBG and SG. Comparing between the post- test and the follow-up test, all three groups showed the reduction of suprahyoid muscle activity with the mean comparing, followed VABG, VBG and SG. Conclusion : It found that it was more effective when providing a double-sensory biofeedback than when training with a single-sensory biofeedback. Therefore, it is necessary to provide a multi-sensory input when applying the biofeedback in rehabilitation of the swallowing disorder.

  • Modern people have abundant food and various forms of life compared to the past, but they have come to form an unhealthy diet, such as skipping breakfast and not eating in time in a busy life. When these eating habits are maintained for a long time, it leads to digestive trouble. The most easily occurring symptoms are called reflux esophagitis and dysphagia. Among them, dysphagia requires quick and accurate diagnosis as they develop into various forms of complications or are also identified as presymptoms of gastric and laryngeal cancers. The result of the diagnosis is still passively judged by the doctor and each of results are different depending on the doctor. The result of the diagnosis here means the severity. When they identify treatment or complications following the results of the diagnosis, the wrong diagnosis may lead to excessive or insufficient treatment. In this paper, to figure out the severity of dysphagia in the diagnosis of dysphagia, we studied the development of a program using the K-means algorithm in the processing of X-ray images for identifying residual food in epiglottic vallecula and pyriform sinus in the section leading to esophagus.

Serial Magnetic Resonance Imaging Findings in Hyperglycemia-Related Osmotic Demyelination Syndrome: A Case Report (고혈당에 의한 삼투압성 탈수초화 증후군 환자의 연속 자기공명영상 소견: 증례 보고)

  • We report a rare case of hyperglycemia-related osmotic demyelination syndrome (ODS) with focus on the imaging findings. A 61-year-old man with diabetes was admitted for general weakness and severe thirst. A few days later, he complained of dysarthria, dysphasia, and dysmetria. Laboratory examinations showed significant hyperglycemia, but normal electrolyte levels. Brain MRI revealed T2-signal abnormalities that were symmetrical, non-space occupying, and located in the central pons with a peripheral sparing pattern, which were suggestive of ODS. In addition, subsequent MRI revealed progression of signal hyperintensity; however, the patient's symptoms improved.

Effect of Water Volume and Temperature on Muscle Activity of the Supra and Infrahyoid Muscles (물의 양과 온도가 목뿔위근육과 목뿔아래근육의 근활성도에 미치는 영향)

  • Objectives : This study was conducted to investigate the activity of the supra and infrahyoid muscles according to the temperature and the amount of water in healthy adults. Methods : The amount of water was set to 3 mL and 20 mL, and the temperature was subdivided into $4^{\circ}C$, $22^{\circ}C$, and $40^{\circ}C$ in order to compare the activity of the supra and infrahyoid muscles in twenty-seven healthy adults. We used the mean value of the activity of the supra and infrahyoid muscles while they were swallowing saliva in order to set the reference voluntary contraction (%RVC). Results : Muscle activity was significantly increased when they swallowed 20 mL of water rather than 3 mL in the left infrahyoid muscles at $4^{\circ}C$, $22^{\circ}C$ and $40^{\circ}C$ (p=.00; p=.00; p=.00), the right infrahyoid muscles at $22^{\circ}C$and $40^{\circ}C$ (p=.01; p=.01), the left suprahyoid muscles at $4^{\circ}C$ (p=.03). Muscle activity of the right suprahyoid muscles was significantly decreased at $40^{\circ}C$ compared to $4^{\circ}C$ and $22^{\circ}C$ when they swallowed 20 mL of water (p=.04). Conclusion : In the future, other variables such as viscosity and taste should be considered, and further studies on patients with impaired nervous system as well as healthy subjects will be needed.

Survey on Children Toothbrushing Teaching Guidance with Child Care Center Teachers (일부 보육교사의 유아 칫솔질 지도에 관한 실태조사)

  • This study aimed to investigate the current situation of toothbrushing guidance with child care teachers in day care facilities caring and managing infants and young children and to grasp problems neglected in using toothpaste and guiding toothbrushing to provide basic data for measures to cope with them. This study included 165 child care teachers or teachers of the kindergartens who agreed with its purpose. By excluding 10 questionnaires with omitted or conflicting answers, 156 ones were finally analyzed in this study. The correlation between education about oral health and toothbrushing guidance, the subjects with and without the experience of the education did not show any significant difference in time to encourage toothbrushing, behaviors of toothbrushing guidance, guidance on prevention of toothpaste intake and perception of an appropriate amount of toothpaste (p>0.05) while perception of the importance of oral health was significantly related with the education on oral health (p<0.05). Therefore, education system for specialists in oral health to offer a specific and systemic education and oral health education programs for child care teacher, are considered to be necessary.

An Evaluation of Usefulness of FEES Method using Full HD Video that Improves Diagnostic Value of VFSS (Full HD 동영상을 이용한 FEES법이 VFSS의 진단적 가치를 향상 시키는 유용성에 대한 평가)

  • Deglutition inspection has been the gold standard for inspection of dysphagia using a penetration device of the division of radiology to date. However, inspection using an endoscope has facilitated diagnosis of dysphagia in recent days. In this study, we compared the results of VFSS alone with those of VFSS combined with FEES to determine if the detection rate of penetration, aspiration, and pharyngeal residue increased. This study evaluated at 130 persons who underwent both VFSS and FEES from December 2013 through March 2014, and used VFSS (FELAVISION, SHIMADZU) and FESS (EVIU LUCERA CV-260SL, OLYMPUS) as devices. After preparing barium diluted solution, yogurt that diluted barium and rice that diluted barium in 2.5 ml, 5 ml, 10 ml by modifying Logemann's protocol in part, and having subjects take the solution, this study confirmed pharyngeal residue, penetration, aspiration etc. VFFS and FEES using full HD video revealed aspiration in 36 of the 130 patients in both inspections. Additionally, aspiration was discovered in 36 patients upon VFFS, but in 41 upon FESS. Although FEES was more sensitive at diagnosing dysphagia, the two inspection methods are complementary, so diagnostic value will be greatly improved when the endoscope operating method and accurate fluoroscopic inspection are combined.
