소절개 봉합술은 관혈적 봉합술에 비해 동반된 관절와 상완 관절(glenohumeral joint) 내의 병변을 확인하고 이에 대한 처치를 시행할 수 있으며, 견봉하 감압술과 함께 동반된 견봉 쇄골 관절의 병변을 치료할 수 있다. 또한 삼각근 기시부에 대한 손상을 최소화하기 때문에 빠른 재활과 조기 퇴원을 시행할 수 있는 등의 장점이 있다 관절경적 봉합술에 비해서는 수술 술기가 어렵지 않기 때문에 항상 좋은 결과를 기대할 수 있다 회전근 개 파열에 대한 수술을 시행할 때 관절경적 봉합술이 모든 경우에서 관혈적 또는 소절개 봉합술에 비해 좋은 결과를 나타내는 것은 아니다. 환자가 심한 골다공증이 있는 경우, 기술적 문제가 있는 경우, 봉합 후 회전근 개에 지나친 긴장이 염려될 때는 항상 소절개 봉합술로의 전환을 시도하여야 한다. 소절개 봉합술을 시행한 경우에도 관절경적 봉합술시와 비슷한 좋은 결과를 얻을 수 있고, 치료의 결과는 수술의 방법보다는 정확한 술기 및 수술 전 환자의 상태에 좌우된다고 하겠다.

The purpose of this study was to determine the effects of scapular taping on muscle activities of the scapular rotators and anterior deltoid, ROM, shoulder pain, proprioception in subacute stroke patients. Twenty-eight patients were randomly assigned to an experimental and control groups of fourteen patients respectively. Muscle activity of upper and lower trapezius, serratus anterior, anterior deltoid was measured using surface electromyography. Visual analog scale was used for shoulder pain. Electro-Goniometry was used for shoulder elevation ROM. Assessment board was used for shoulder elevation proprioception. The muscle activity of the lower trapezius and serratus anterior increased significantly after scapular tape application (p<0.05). The AROM and PROM in the shoulder elevation significantly increased after scapular tape application (p<0.05). The VAS in the shoulder no significantly decreased after scapular tape application (p>0.05). The proprioception in the shoulder no significantly increased after scapular tape application (p>0.05). The results of this study suggest that scapular taping can be used an additional therapy for increasing muscle activity of lower trapezius and serratus anterior and ROM during shoulder elevation in subacute stroke patients.

Most patients experience pain relief and functional improvement after arthroscopic rotator cuff repair. In some patients, however, symptoms still remain after surgery. Failed rotator cuff repair is a complex outcome of biological, technical, and traumatic factors. Moreover, re-tears might or might not be the main cause for patients with persistent pain after rotator cuff repair. Therefore, a thorough understanding of the patient's history, physical examination, and appropriate imaging studies will be needed to evaluate and manage these patients. The patient's age, functional requirement, quality of the rotator cuff, preoperative range of motion, quality of the deltoid, and glenohumeral arthritis are factors to consider before performing revisional rotator cuff repair. Preoperative patient education is as important as the surgical technique for successful revisional rotator cuff repair.

The aim of this study was to investigate difference of the muscle activation patterns of the upper body during wheelchair cycle ramps ascent of different slopes for disabled with spinal cord injury. Three subjects who is disabled with spinal cord injury participated in this study. Surface electromyography (EMG) data (reaction time [RT], onset-offset time, and peak value of muscle activation) were collected biceps, triceps, upper trapezius, anterior deltoid, latissimus dorsi, and upper rectus abdominal muscles during wheelchair cycle ramps ascent ($0^{\circ}$, $3^{\circ}$, and $6^{\circ}$). For latissimus dorsi muscle, RT and peak value of muscle activation was were increased and offset time was delayed as the slope increased (p < 0.05). These results indicate that wheelchair cycle ramps ascent might cause excessive overuse of latissimus dorsi muscle.

Rotator cuff disease is the most common cause of shoulder pain. Recent understanding of structure and mechanics enables scientific approach to the treatment of the cuff teat. It is an integral component in the normal movement and function of the shoulder. This article addresses current knowledge of the structure and biomechanics of the rotator cuff, and reviews various kind of operative treatment.

We experienced a rare case of deltoid compartment syndrome by the limb compression during altered consciousness including crush syndrome with myoglobinuric renal failure and report the reviews of those syndromes.

견관절의 다방향 불안정성은 생역학 및 생화학적 이상의 복합적 요인에 의해 발생되는 것으로 알려져 있으나 이에 대한 보다 많은 연구가 이루어져야 될 것으로 생각된다. 최근 다방향 불안정성과 관련된 요소에 따라 분류가 세분화되어 치료가 이루어지고 있다. 따라서 진단시 세심한 병력과 진찰이 매우 중요하며 이에 따라 치료 방침을 결정해야 할 것이다. 많은 환자에서 삼각근, 회전근 개, 견갑 안정화 근육을 강화시킴으로써 증상이 호전되나 비수술적 치료에 호전이 없는 경우는 관절 낭 이동술을 이용하여 비교적 만족스러운 결과를 얻을 수 있다. 일반적으로 개방적 술식이 다방향 불안정성을 위한 수술적 치료의 표준적 술식으로 알려져 있으나. 최근에는 여러 관절경적 술식이 개발되고 있고 이에 따른 중 단기 추시 결과도 비교적 좋은 것으로 보고되고 있다. 그러나 이 술식에 대한 정확한 평가를 위해서는 많은 경험과 장기 추시 결과를 요한다.

Objective: The objective of this study is to observe the effect of anti-inflammatory herbal acupuncture on the Supraspinatus Tendinosis and subdeltoid bursitis Methods : Anti-inflammatory herbal acupuncture, A-Shi Point, Sa-am acupunture were used to treat shoulder pain. We evaluated the patient through VAS(Visual Analog Scale) daily and Physical Examinations Results & Conclusions : After 12 days of treatment, shoulder pain was decreased from VAS9 to VAS1 and the patient showed nearly full ROM(range of movement). In shoulder pain, oriental treatment is good method for pain relief and better movement.
