Correlation between Self-evaluation Factor and Academic Achievement of Medical Students according to Introduction of Explanation Meeting in Cadaveric Dissection (해부설명회의 도입에 따른 의학전문대학원생들의 자기 평가 요인과 학업성취도 상관관계 분석)

  • This study aimed to evaluate correlation between self-evaluation factors(satisfaction, linkage with major, suitability of management, verbal and non-verbal communication) and academic achievement of medical students according to introduction of explanation meeting in cadaveric dissection. The study was carried out that medical students(n=57) explained cadaveric structure to health allied sciences students and discussed with each other. Just after the meeting,, medical students filled out a questionnaire on impact of self-evaluation factors and communication. We analyzed these factors and their scores using frequency analysis, T-test and analysis of variance. Regardless of their gender, age, previous experience, the majority of the students gave high scores in all of self-evaluation factors. Among them, only verbal communication factor was closely related to their academic achievement(p<0.05). The verbal and non-verbal communication also had a high correlation of 0.673(p<0.01). The explanation meeting provided chance to learn further with positive attitude to medical students and motivated them academically. Additionally, they realized that communication skill played a key role in transmitting medical knowledge to others. Therefore, introduction of communication-based explanation meeting would be very useful tool in improving educational efficiency.

Anatomic Study for Hamstring Tendon Harvest (슬괵건 채취를 위한 해부학적인 고찰)

  • Purpose: The harvest of hamstring tendon is technically demanding because of the inadequate identification of hamstring tendon separation and accessory tendon of semitendinosus tendon. We conducted therefore conducted an anatomic study, aiming at the anatomic knowledge for graft harvest. Materials and Methods: 20 human cadaveric knees (10 cadavers) were used for the study. The location of tendon separation in conjoined tendon and accessory tendon of semitendinosus tendon were described and recorded. Results: The location of tendon separation of conjoined tendon was average $39.68{\pm}9.97mm$ vertically and $18.57{\pm}2.91mm$ horizontally from the tibial spine. We found that the accessory structure of the semitendinosus tendon was mostly fascia-like structure(17 knees), the tendinous structure, 3 cases which was straightly located 15cm from the tibial crest. Conclusion: We propose that the expected incision for hamstring tendon harvest is centered on the inferior 40mm, medial 20mm from the tibial spine. The accessory structure of the semitendinosus tendon was mostly found of fascia-like structure.

  • Various nerves from many areas of body can be used as a donor of autogenous nerve graft in the microneurosurgical repair of the oral and maxillofacial region. In the grafting procedure of nerve repair, the best results will be achieved with a maximum approximation of fascicular surface at both the porximal and the distal stumps. Therefore, appropriate selection of donor nerve will bring out the best results. The sural nerve has been used as one of the most popular donor nerve in the microneurosurgical repair of the oral and maxillofacial region. The authors examined the fascicular characteristics of the human sural nerve microscopically and compare this results with our previous report of the greater auricular nerve and that of the inferior alveolar nerve.

Surgical Anatomy of Lateral Extracavitary Approach to the Thoracolumar Spine - Cadaveric Study - (흉요추부 외측 강외 접근법(Lateral Extracavitary Approach)의 수술해부학적 구조 - 사체해부실험 -)

  • Objective : The lateral extracavitary approach(LECA) to the thoracolumbar spine is known as one of procedure which allows not only direct vision of pathologic lesion, but also ventral decompression, and dorsal fixation of the spine through the same incision. However, some drawbacks of LECA, including the technically- demanding, time-consuming, unfamiliar surgical anatomy and excessive blood loss, make surgeons to hesitate to use this approach. This study is to provide the surgical anatomy of LECA using cadavers, for detailed informations when LECA is considered for the surgery. Methods : We performed the 10 cadaveric studies, 7 male and 3 female, and careful dissection was carried out on right side of thoracolumbar region, except one for thoracic region. The photographs with micro-lens were taken to depict the close-up findings and for demonstrating detailed anatomy. Results : The photographs and hand-drawings demonstrated the relationships among the musculature, segmental vessels and nerve roots seen during each dissection plane. The lateral branches of dorsal rami of spinal nerve and the transverse process were confirmed to be the most important landmark of this approach. Conclusion : We concluded that detailed anatomical findings for LECA through this step-by-step dissection would be useful during operative intervention to reduce the intraoperative complications in LECA.

  • The computational human phantom including major radiation sensitive organs at risk (OARs) can be used in the field of radiotherapy, such as the variation of secondary cancer risks caused by the radiation therapy and the effective dose evaluation in diagnostic radiology. The present study developed a Korean adult female voxel phantom, VKH-Woman, based on serially sectioned color slice images of Korean female cadaver. The height and weight of the developed female voxel phantom are 160 cm and 52.72 kg, respectively that are virtually close to those of reference Korean female (161 cm and 54 kg). The female phantom consists of a total of 39 organs, including 27 organs recommended in the ICRP 103 publication for the effective dose calculations. The female phantom composes of $261{\times}109{\times}825$ voxels (=23,470,425 voxels) and the voxel resolution is $1.976{\times}1.976{\times}2.0619mm^3$ in the x, y, and z directions. The VHK-Woman is provided as both ASCII and Binary data formats to be conveniently implemented in Monte Carlo codes.

Study on accuracy of panorama and CBCT through length measurement of wisdom teeth (사랑니 길이 측정을 통한 파노라마와 CBCT의 정확도 연구)

  • Panorama of dental radiation generators can observe the wide anatomical structures of oral and maxillofacial areas but there can be distortion of lengths, angles, or shapes. CBCT can diagnose 3D images and get the ones whose errors by superposition and interference are remarkably smaller between anatomical structures. But the quality of the images by movement of subjects can be lowered as it takes long to diagnose them. And if there are impermeable radiation objects like metal in mouths, impermeable radiation lines can radially appear with the objects as center. This study tries to analyze accuracy of panorama and CBCT and get useful anatomical information in dental treatment by comparing the length of wisdom teeth which were measured by Panorama and CBCT with the teeth which were actually extracted and analyzing distortion of the teeth. The test result could be found that Panorama is expanded by average 7.3% as the errors of Panorama and Digital Vernier Caliper range from 110.7% to 103.9%. The length of wisdom teeth which were measured in CBCT and Digital Vernier Caliper could be found that the error range is 1.3%. And the length of wisdom teeth which were measured in Panorama and Digital Vernier Caliper has found that the error range shows 7.3%. So it could be found that the images of CBCT is about 6% more exact than those of Panorama. It could be found that CBCT shows the more exact images than those of Panorama. But because the examination expenses of CBCT are higher than those of Panorama and exposure dose of CBCT is much more than that of Panorama, it is thought to find proper ways in examination.

Microvasculature of Human Menisci (정상 반월상 연골의 혈관 분포)

  • Purpose : To evaluate the morphologic measurement and microvasculature of the normal human meniscus in Korean and to compare the difference of microvasculature according to the age and anatomical location. Materials and methods : Menisci of 21 cadavers divided into 3 groups according to the age. Each meniscus was divided into 10 parts of which the width and area of microvasculature were measured. Results : The average width of the medial and lateral menisci were $10.49{\pm}02.92mm$ and $10.55{\pm}2.05mm$, respectively. The average area of the microvasculature measured $20.29{\pm}8.44\%$ in the medial meniscus and $18.99{\pm}7.03\%$ in the lateral meniscus. The microvasculature of the medial meniscus was most abundant in the anterior horn and least abundant in the posterior horn (p<0.05). The lateral meniscus showed the most vessels in the posterior horn (p<0.05). The microvasculature of popliteal hiatus was not avascular but hypovascular. The area of microvasculature decreased with increasing age in all parts of the medial meniscus and body and posterior horn of the lateral meniscus (p<0.05). Conclusion : Differences in the anatomical distribution of the vessels were noted of both the medial and lateral menisci. The area of microvasculature generally decreased proportionally with increasing age in both medial and lateral meniscus.

  • Nowadays, digital Radiography (DR) systems are widely used in clinical sites and substitute the analog-film x-ray imaging systems. The resolution of DR images depends on several factors such as characteristic contrast and motion of the object, the focal spot size and the quality of x-ray beam, x-ray scattering, the performance of the DR detector (x-ray conversion efficiency, the intrinsic resolution). The DR detector is composed of an x-ray capturing element, a coupling element and a collecting element, which systematically affect the system resolution. Generally speaking, the resolution of a medical imaging system is the discrimination ability of anatomical structures. Modulation transfer function (MTF) is widely used for the quantification of the resolution performance for an imaging system. MTF is defined as the frequency response of the imaging system to the input of a point spread function and can be obtained by doing Fourier transform of a line spread function, which is extracted from a test image. In clinic, radiologic technologists, who are in charge of system maintenance and quality control, have to evaluate or make routine check on their imaging system. However, it is not an easy task for the radiologic technologists to measure MTF accurately due to lack of their engineering and mathematical backgrounds. The objective of this study is to develop and provide for radiologic technologists a medical system imaging evaluation tool, so that they can measure and quantify system performance easily.

Study on Oneself Developed to Apparatus Position of Measurement of BMD in the Distal Radius (자체 개발한 보조기구로 원위 요골의 골밀도 측정 자세 연구)

  • Purpose : The aim of this study was to evaluate the difference of bone mineral density according to distal radius rotation and to develop the supporting tool to measure rotation angles. Materials and Methods : CT scanning and the measurement of BMD by DXA of the appropriate position of the forearm were performed on 20 males. Twenty healthy volunteers without any history of operations, anomalies, or trauma were enrolled. The CT scan was used to evaluate the cross sectional structure and the rotation angle on the horizontal plane of the distal radius. The rotational angle was measured by the m-view program on the PACS monitor. The DXA was used in 20 dried radii of cadaveric specimens in pronation and supination with five and ten degrees, respectively, including a neutral position (zero degrees) to evaluate the changes of BMD according to the rotation. Results : The mean rotation angle of the distal radius on CT was 7.4 degrees of supination in 16 cases (80%), 3.3 degrees of pronation in three cases (15%), and zero degree of neutral in one case (9%), respectively. The total average rotation angle in 20 people was 5.4 degrees of supination. In the cadaveric study, the BMD of the distal radius was different according to the rotational angles. The lowest BMD was obtained at 3.3 degrees of supination. Conclusion : In the case of the measurement of BMD in the distal radius with a neutral position, the rotational angle of the distal radius is close to supination. Pronation is needed for the constant measurement of BMD in the distal radius with the rotation angle measuring at the lowest BMD and about five degrees of pronation of the distal radius is recommended.

Biological Characteristics and Current Status of Nutria (Myocastor coypus) Introduced in Jeju Island (제주도에 도입된 뉴트리아(Myocastor coypus)의 생물학적 특성 및 서식 현황)

  • The study was carried out to understand the inhabiting status and biological features of nutria (Myocastor coypus) in the Jeju Island from May, 2013 to August, 2016. Five individuals were captured by traps and two individuals were collected in dead. Of the 7 individuals, those whose bio-information could not be known were excluded to perform autopsy of the rest of them. The results from morphological, cranial and anatomical analyses, it was found that the females achieved sexual maturation but had no breeding experience. An adult male showed similar sizes to those found in the Korean Peninsula. After collecting the 7 individuals in 2013, 1 nutria individual was filmed by the scouting camera in May, 2014 but not captured. The long-term investigation of the research area found no inhibiting individual. It is deemed the nutria was either died of natural death or migrated. In order to establish a proper nutria control, the environmental characteristics of Jeju Island should be considered where indigenous and introduced species coexist in a limited space. Our findings will provide helpful information for understanding biological features of nutria and planning protection program of natural ecosystems of Jeju Island.
